Background: Gall bladder cancer (GBC) usually presents as unresectable or metastatic disease. We conducted a feasibility study to evaluate the effect of neoadjuvant therapy (NAT) on radiologic downstaging and resectability in unresectable GBC cases. Materials and Methods: Patients with locally advanced disease were treated with chemoradiotherapy [CTRT] ( external radiotherapy (45Gy) along with weekly concurrent cisplatin $35mg/m^2$ and 5-FU 500 mg) and those with positive paraaortic nodes were treated with neoadjuvant chemotherapy [NACT (cisplatin $25mg/m^2$ and gemcitabine $1gm/m^2$ day 1 and 8, 3 weekly for 3 cycles). Radiological assessment was according to RECIST criteria by evaluating downstaging of liver involvement and lymphadenopathy into complete response (CR), partial response (PR), stable disease (SD) and progressive disease (PD). Results: A total of 40 patients were evaluated from January 2012 to December 2014 (CTRT=25, NACT=15). Pretreatment CT scans revealed involvement of hilum (19), liver infiltration (38), duodenum involvement (n=22), colon involvement (n=11), N1 involvement (n=11), N2 disease (n=8), paraaortic LN (n=15), and no lymphadenopathy (n=6). After neoadjuvant therapy, liver involvement showed CR in 11(30%), PR in 4 (10.5%), SD in 15 (39.4%) and lymph node involvement showed CR in 17 (50%), PR in 6 (17.6%), SD in 4 (11.7 %). Six patients (CTRT=2, NACT=4) with 66.6 % and 83% downstaging of liver and lymphnodes respectively underwent extended cholecystectomy. There was 16.6 % and 83.3% rates of histopathological CR of liver and lymph nodes. All resections were R0. Conclusions: Neoadjuvant therapy in unresectable gall bladder cancer results in a 15% resectability rate. This approach has a strong potential in achieving R0 and node negative disease. Radiologic downstaging (CR+PR) of liver involvement is 40.5% and lymphadenopathy is 67.5%. Nodal regression could serve as a predictor of response to neoadjuvant therapy.
This research was conducted to investigate the changes in soil chemical properties of root media as influenced by incorporation rate of a polyacryl amide hydrogel, Stocksorb C. The pH at 5 weeks after treatment in four root media such as peatmoss + vermiculite (1:1, v/v; PV), peatmoss + composted rice hull (1:1; PR), peatmoss + composted saw dust (1:1; PD) and peatmoss + composted pine bark (1:1; PB) containing STSB were in the range from 7.04 to 7.30, which was too high. Elevated incorporation rate of STSB resulted in increase of EC in soil solution of four root media with linear and quadratic response. The concentrations of $NH_4^+-N,\;NO_3^--N,\;PO_4-P^{3-},\;K^+,\;Ca^{2+}and\;Mg^{2+}$ in four kinds of root media increased as incorporation rates of STSB were elevated. But the $NO_3^-$-N concentrations in PS media were lower than those in other there root media tested. The Fe concentrations in PV, PR and PS media increased as incorporation rates of STSB were elevated, but those in PB medium did not show significant different. The concentrations of $Fe^{2+},\;Mn^{2+},\;Zn^{2+}and\;Cu^{2+}$ in PS media were higher than those in other three root media.

Franz Keldysh Oscillation (FKO)은 p-n 접합 구조의 Photoreflectance (PR) spectra에서 표면 및 계면의 전기장(electric field) 특성을 반영한다. InAs/GaAs 양자점 태양전지(Quantum Dot Solar Cell, QDSC) 구조에서 InAs 양자점 층 전후에 AlGaAs 층을 삽입하여 퍼텐셜 장벽(potential barrier) 두께에 따른 PR spectra 및 GaAs-matrix에서 FKO 주파수 특성을 비교 분석하였다. InAs/GaAs 양자점 태양전지는 p-i-n 구조의 i-GaAs에 2.0 monolayer (ML), 8주기의 InAs 양자점 층을 삽입하여 Molecular Beam Epitaxy (MBE) 방법으로 성장하였다. 각 양자점 층 전후에 두께가 각각 0.0, 1.6, 2.8, 6.0 nm인 AlGaAs 층을 삽입하여 퍼텐셜 장벽 두께에 따른 FKO 주파수 변화를 관측하였다. 또한 태양전지 구조의 전기장 분포를 좀 더 용이하게 관측하기 위해 여기 광의 세기(power intensity)를 충분히 낮추어 Photovoltaic effect에 의한 내부 전기장의 변화를 최소화하여 비교 분석하였다. InAs/GaAs 양자점 태양전지 구조에서 AlGaAs 장벽층이 없는 경우, PR spectra의 Fast Fourier Transform 결과에 반영되는 FKO 주파수 특성은 p-i-n 구조 계면에서 공핍층(depletion region)의 space charge field보다 양자점 층의 내부 전기장에 의한 FKO 주파수가 더 큰 진폭(amplitude)을 보였다. 반면에, AlGaAs 장벽층이 삽입되면 두께가 커짐에 따라 p-i-n 구조 계면의 space charge field에 의해 더 큰 진폭의 FKO 주파수가 관측되었다. 이는 AlGaAs 장벽층이 삽입됨으로써 양자점 층 내 양자 상태 수 및 여기광에 의한 캐리어의 수와 관련이 있음을 확인하였으며, 결과적으로 GaAs-matrix에서 p-i-n 구조 계면의 space charge field에 영향을 미치게 됨을 알 수 있다. 이러한 PR 특성 결과들을 InAs/GaAs 양자점 태양전지의 설계 및 제조에 반영함으로써 양자효율 증대에 기여할 것으로 기대된다.
Asymmetric inducing effects of substituents attached at nitrogen, the 5-position and boron in oxazaborolidine rings on asymmetric borane reduction of ketones were investigated. Thus, the effect of N-substituents examined with the oxazaborolidines prepared from (lR,2S)-N-alkyl norephedrine derivatives showed the remarkable decrease of enantioselectivities of the product alcohols by the variation of the steric size of alkyl groups on nitrogen from Me${\leftrightarro}$n-Bu(${\simeq}$Bn)${\leftrightarro}$ neopentyl${\leftrightarro}$i-Pr, such as 83${\%}$ ee with 5b, 22${\%}$ ee with 5c, 23${\%}$ ee with 5f, 16${\%}$ ee with 5e, and 3${\%}$ ee with 5d for the reduction of acetophenone. The presence of diphenyl groups at the 5-position enhanced the enantioselectivities dramatically. The effect of B-alkyl substituents in the oxazaborolidines derived from (lR,2S)-ephedrine showed that the enantioselectivities of product alcohols decreased gradually when the substituents were changed from hydrogen to steric bulky groups such as methyl, n-butyl, thexyl and phenyl.

An oscillatory motion of the natural convection in a two dimensional, partially divided square enclosure heated from below, and fitted with a partition is investigated numerically. The enclosure was composed of the lower hot and the upper cold horizontal walls and the adiabatic vertical walls, and a partition was situated perpendicularly at the mid-height of the one vertical insulated wall. The governing equations are solved by using the finite element method with Galerkin method. The computations were performed with the variation of the length and the thermal conductivity of the partition, and Rayleigh number based on the temperature difference between horizontal walls and the enclosure height with water(Pr=4.95). also, the effect of the inclination angles was studied for the transition to the oscillating flow. As the results, it was found that the intensity and frequency of oscillatory motion were affected significantly by the Rayleigh number and the length of partition. The effect of oscillatory motion was weaken with the increase of the thermal conductivity of partition. The inclination angle for the transition was raised with the increase of Rayleigh number and the length of partition.
Background : Endotracheal intubation is one of the methods most securely establishing airway. But accompanying hemodynamic responses are harmful to coronary or cerebral vascular disease patients. These hemodynamic responses are regarded as the results of sympathetic stimulation due to pharyngolaryngeal stimulation, and sympathetic blocking method-stellate ganglion block- may be obtundate these hemodynamic responses. Methods : 75 patients of ASA physical status I-II were selected. There were 40 patients normotensive (Group I), 35 patients hypertensive (Group II) Group I, steliate ganglion block was performed on 20 patients (Group I-S) the remainder had no procedure (Group I-O). Group II, 18 patients received SG3 (Group II-S), 17 patients had no procedure (Group II-O). SGB was performed with 1% lidocaine 8 ml on right stellate genglion after patient's consent. Blood pressure (IIP) and pulse rate(PR) were first measured in the pre-anesthesia room. Follow up BP and PR are checked immediately following SGB and every 5 minutes for subsequent 20 minutes, then after arrival at operatig room, then immediately after intubation and at 3, 5, 10, 15 and 20 minutes after incubation. Results : All group experienced significantly increased blood pressure and pulse rate upon arrival at the pre-anesthesetic and opeating rooms, as compared to when patients rates in the ward. After intubation and for subsequent 5 minutes, significant changes were measured. Patients then recovered to preblock value. In Group I, no statistical significance was recorded between subgroup I-S and I-O. However in Group II, there were significant differences between sub-group II-S and II-O. In evaluating pulse rate changes, there were no significant differences between Group I-S and I-O; nor II-S and II-O. Conclusion : The proper diagnosis of Stellate Ganglion Block had some measure of protective effect on hemodynamics following endotracheal intubation, especially in hypertensive patients.
Ketene-forming elimination from 2-X-4-nitrophenyl furylacetates (1a-d) promoted by $R_2NH-R_2NH_2{^+}$ in 70 mol % MeCN(aq) has been studied kinetically. When X = Cl and $NO_2$, the reactions exhibited second-order kinetics as well as Br$\ddot{o}$nsted ${\beta}$ = 0.37-0.54 and $|{\beta}_{lg}|$ = 0.31-0.45. The Br$\ddot{o}$nsted ${\beta}$ decreased with a poorer leaving group and $|{\beta}_{lg}|$ increased with a weaker base. The results are consistent with an E2 mechanism. When the leaving group was changed to a poorer one [X= H (1a) and $OCH_3$ (1b)], the reaction mechanism changed to the competing E2 and E1cb mechanisms. A further change to the E1cb mechanism was realized for the reaction of 1a with $i-Pr_2NH/i-Pr_2NH_2{^+}$ in 70 mol % MeCN-30 mol % $D_2O$. By comparing the kinetic results in this study with the existing data for $ArCH_2C(O)OC_6H_3-2-X-4-NO_2$, the effect of the ${\beta}$-aryl group on the ketene-forming elimination was assessed.
The effect of the thermal conductivity of a centered, square, heat-conducting body on natural convection In a square enclosure was examined numerically. Numerical simulations was carried out for Pr=0.17, $Ra=1.0{\times}10^4,\;1.0{\times}10^5,\;1.0{\times}10^6$, $K^*$=1.0, 6.6, 34.0 and t=0.5, 1.0, 2.0. The results were reported in terms of streamlines, isotherms, Nusselt number. As the results, the mean Nusselt number increases with the increasing of ${\zeta}$ at a constant Ra and $K^*$. In the case of ${\zeta}=1.0$(obstruction shape ratio), the mean Nusselt numbers were decreased as increasing of $K^*$(obstruction thermal conductivity ratio) with regardless of the Rayleigh number. When the constant obstruction size and thermal conductivity ratio, convective heat transfer effect was more enhanced at ${\zeta}=2.0$ than ${\zeta}=0.5$.
Purpose: This experiment is designed to find out anti-oxidant effects of Samultang-Gami which was composed of Rehmanniae Radix(RR), Angelicae Gigantis Radix, Cnidii Rhizoma(CR), Paeoniae Radix(PR), Cortex Moutan Radicis, Hedyotis Diffusa(HD) and Caesalpinia Sappan on MEF cells. Methods: In vitro antioxidant effects were measured by MTT assay, DPPH assay, cell cycle analysis, AnnexinV-FITC/PI assay and DAPI staining using MEF cells treated with various concentrations of 70% ethanol extract of Samultang-Gami. Results: 1. In the scavenging for DPPH radical, the each treated groups of PR, CR and HD showed positive effects. RR and CR increased the viability of oxidative damaged MEF cells in a dose-dependent manner. 2. 70% ethanol extract of Samultang-Gami was shown best antioxidative effect in the concentration of $0.5mg/m{\ell}$. 3. The treatment of Samultang-Gami in oxidative damaged MEF cells didn't have any effect on cell cycle restoration. but it could lower late apoptosis rate a little and be observed the protection of nucleus. Conclusion: It can be concluded that Samultang-Gami, RR and CR have antioxidant effects on MEF cells.
This study used Q methodology for analogizing culinary arts major students' subjectivity through their participation of one of restaurant start-up experience program, called pop-up restaurant. The study tried to figure out particular structure of among students' responses and noticed five distinctive types. There were Increase learning effect type(Type 1, N=4), Collaboration of members importance type(Type2, N=8), Marketing PR need type(Type3, N=6), Restaurant business plan type(Type4, N=4), Industry work experience required type(Type5, N=3). The study also revealed that each type contained diverse characteristics figures in their own. The research finding could be used as fundamental source of future similar research but in other research methodology in the format of difference among students or diverse measuring point of time frame.
