Up to the present, finger replantation patients have complained about various symptoms because of blood circulatory failure. Objective evaluation methods for the level of the blood circulatory failures in fingers were needed other than patient's subjective symptoms and complaints. Photoplethysmogram(PPG) measured from fingers is very useful for evaluation of vascular aging and sclerosis level since the PPG signal represents characteristics of peripheral vascular. Several researchers have reported that second derivative of the finger PPG waveform was useful to evaluate vascular compliance and developed various analysis methods for vascular compliance. However, vascular compliance study for finger replantation patient was never evaluated by using second derivative of PPG. Therefore, we aimed to objectively compare and to assess normal and replanted finger vascular compliance using the second derivative of PPG waveform in this study. The evaluated factors of second derivative of PPG are 'a', 'b', 'c', 'd', 'e' and b/a represents vascular compliance. Study found out that when vascular compliance is decreased, b/a is increased, the absolute value of b/a is decreased. For 43 finger replantation patients, the average vascular compliance of replanted finger is lower than the normal finger statistically(p=0.001). we suggested an objective evaluation of finger vascular compliance for finger replantation patients and effect of surgical operation.

Recently, the prevalence rate of diabetes in Korea has been increasing rapidly due to high growth of economy and changes in dietary lifestyle. Vascular complication is one of diabetic complications which have frequently occurred by obesity, hyperglycemia and impaired glucose metabolism. Photoplethysmogram(PPG) measured from finger and toe is very useful for evaluation of vascular aging and sclerosis level since the PPG signal represents characteristics of peripheral vascular. Several researchers have reported that second derivative of the finger PPG waveform was useful to evaluate vascular compliance and developed various analysis methods for vascular compliance. However, peripheral vascular compliance study for diabetic patient was never evaluated by using second derivative of PPG. Therefore, we aimed to objectively compare and to assess normal and diabetic group vascular compliance using the second derivative of PPG waveform in this study. The evaluated factors of second derivative of PPG are 'a', 'b', 'c', 'd', 'e' and b/a represents vascular compliance. Study found out that when vascular compliance is decreased, b/a is increased, the absolute value of b/a is decreased. The average vascular compliance of 50 diabetic patients with neuropathy is statistically lower than the normal group(p<0.05). We suggest an objective evaluation of peripheral vascular compliance for diabetic patients and prevention of vascular complication.

This study found out that when vascular compliance is decreased, the absolute value of b/a is decreased. The average vascular compliance of 50 diabetic patients with neuropathy, $-1.09{\pm}0.14$ is statistically lower than the normal group, $-0.81{\pm}0.09$ (p<0.05). In conclusion, we suggest an objective evaluation of peripheral vascular compliance for diabetic patients and prevention of vascular complication.

There are many people who suffer from simulation sickness when immersing in virtual reality. In this study, we analyzed two photoplethysmogram(PPG) parameters - a second derivative parameter and power spectral density ratios - in order to relate PPG parameters with simulation sickness. 36 young, healthy subjects were participated in the experiment, and each subject was equipped with a PPG electrode during his or her immersion. Simulation sickness section was defined as a 7 - second section which starts from the point where a subject reported simulation sickness, and normal section as a same-length section where no physical stimuli was presented to him or her. We compared the PPG parameters of the simulation sickness sections with the normal sections, - d/a ratio is believed to have lower value during vasodilation and higher value during vasoconstriction, however, we could not find much difference in the parameter between normal and simulation sickness sections. We also compared 1 to 10Hz power spectral density ratios in normal sections with in simulation sickness section, and found that 6 density ratios among them have different value. Therefore, the density ratios might be utilized as parameters to detect simulation sickness of subjects.

In this study, we developed novel indicators to assess postoperative pain based on PPG derivative waveform. As the candidate indicator of postoperative pain assessment, the time from the start of beating to the n-th peak($T_n$) and the n-th peak amplitude($A_n$) of the PPG derivative were selected. In order to verify derived indicators, each candidate indicator was derived from the PPG of 78 subjects before and after surgery, and it was confirmed whether significant changes were observed after surgery. Logistic classification was performed with each proposed indicator to calculate the pain classification accuracy, then the classification performance was compared with SPI(Surgical Pleth Index, GE Healthcare, Chicago, US). The results showed that there were significant differences(p < 0.01) in all indicators except for $T_3$ and $A_3$. The coefficient of variation(CV) of every time-related indicators were lower than the CV of SPI(30.43%), however, the CV in amplitude-related parameters were higher than that of SPI. Among the candidate indicators, amplitude of the first peak, $A_1$, showed that highest accuracy in post-operative pain classification, 68.72%, and it is 15.53% higher than SPI.

Photoplethysmography (PPG) is a simple and low-cost optical technique that can be used to detect blood volume changes in the microvascular bed of tissue. It is often used non-invasively to make measurements at the skin surface. In this paper, we described the implementation heart rate monitoring system using PPG sensor. Proposed system is collected data using PPG sensor and heart rate was detected using second derivative method. In order to verify the effectiveness of the proposed method, we performed the measurements of RR-intervals with a commercial medical device.
우리나라의 당뇨병 유병률은 경제발전과 식생활의 급속한 서구화로 빠른 속도로 증가하고 있다. 특히 당뇨병은 유병기간이 길수록 여러 합병증을 초래하는데 그 중 하나가 혈관경화증이다. 혈관 경화증은 신체 부위별로 그 진행 속도가 다르다. 광혈류측정법(PPG, photoplethysmogram)은 간편한 혈류량 측정방법으로 PPG 2차 미분 방법을 이용해서 혈관의 경화도를 진단할 수 있다. 이러한 PPG 2차 미분방법을 이용하여 당뇨병환자의 왼손과 오른손의 말초혈관 탄성도 차이를 정상인과 비교해 보았다. 실험결과 당뇨병환자와 정상인의 양 손가자 말초혈관 탄성도의 유의한 차이를 확인 할 수 있었다.
In this study, photoplethysmography(PPG) was suggested as a way to replace the ankle-brachial index(ABI) in diagnosing PAD. The method using the PPG was presented for the simplification of the PAD diagnosis method which was used before. And the index related to the health condition of the artery from the PPG measured in both big toes of the subjects through the experiment was drawn. The indexes showing the significant relativeness in the Pearson correlation analysis with the ABI were the stiffness index(SI), reflection index(RI); it was confirmed each of them had the correlation coefficient of 0.688, and 0.637 at p < 0.05. The explanation ability of the linear regression equation derived using ABI, SI and RI was 52.5%. The explanation ability of the secondary curve regression equation derived using ABI, squared SI was 54.7%. It is expected to provide patients with significant results and draw the index associated with PAD by measuring PPG easily in the real life instead of the ambulatory care field.

Palpation of the pulse has been used in Korean traditional medicine since ancient times to assess physical health. Pulse wave contour may be obtained by measuring arterial pressure or blood volume change of skin. The latter is called as Photoplethysmography(PPG) or digital volume pulse(DVP). The PPG signal is measured by a device comprising an infrared light sourece and a photodetector. Although less widely used, this technique deserves further consideration because of its simplicity and ease of use. The contour of the PPG is formed as a result of a complex interaction between the left ventricle and the systemic circulation. It usually exhibits an early systolic peak and an early diastolic peak. the first peak is formed mainly by pressure trasmitted along a direct path from the left ventricle to the finger. The second peak is formed in part by pressure transmitted along the aorta and large arteries to sites of impedance mismatch in the lower body. The contour of the PPG is sensitive to changes in arterial tone and is influenced by ageing and large artery stiffness. Measurements taken directly from the PPG or from its second derivative can be used to assess these properties. In some mathematical approaches, the extraction of periodic components using frequency analysis was tried to analysis of the PPG. But we don't understand yet what kind of factor in the cardiovascular system or human body is related with the respective specific Fourier components of PPG. This review describes the background to measurement principles, representative contour, contour analysis and frequency domain analysis of PPG, and current and future.

This study explored the effect of pulsed magnetic field (PMF) stimulus on the improvement of blood stagnation by means of photoplethysmography (PPG). Our stimulus system was designed to generate PMF with a maximum intensity variation of 0.20 T at a transition time of $160{\mu}s$, with pulse intervals of 1 Hz. In order to quantitatively estimate vascular condition, indices such as blood vessel tension (BVT), stress power (SP), differential pulse wave index (DPI) and remained blood volume (RBV) were calculated from the second derivative of the PPG signal and power density spectrum (PDS). Our results showed that non-invasive PMF stimulus was effective in improving blood stagnation. Therefore, it may be concluded that appropriate PMF stimulus affects the blood circulatory system.
