Value of Bronchoalveolar Lavage Fluid Cytology in the Diagnosis of Pneumocystis jirovecii Pneumonia: A Review of 30 Cases

  • Background: Pneumocystis jirovecii is a fungus that has become an important cause of opportunistic infections. We present a summary of the clinical status and findings from bronchoalveolar lavage (BAL) of patients with Pneumocystis jirovecii pneumonia (PJP). Methods: We selected 30 cases of PJP that were proven through a surgical specimen evaluation. BAL fluid cytology was reviewed, and agreement with the initial diagnosis was evaluated. Results: All 30 cases of PJP occurred in immunocompromised patients. Only 15 of the 30 cases were initially diagnosed as PJP. We found PJP in 13 of the 15 cases that were negative at the initial diagnosis. The most characteristic finding of PJP was frothy exudates, and BAL fluid tended to show rare neutrophils. Two of seven patients with PJP and diffuse alveolar damage (DAD) revealed no frothy exudates in BAL fluid. Conclusion: BAL fluid cytology was reconfirmed as a sensitive and rapid method to diagnose PJP. We must be aware of the possibility of PJP to maintain high diagnostic sensitivity. We cannot exclude PJP in cases of PJP with DAD, even if frothy exudates are not observed in the BAL fluid.

Proposal of a Selective Prophylaxis Strategy Based on Risk Factors to Prevent Early and Late Pneumocystis jirovecii Pneumonia after Renal Transplantation

  • Background: Currently, trimethoprim-sulfamethoxazole is used for Pneumocystis jirovecii pneumonia (PJP) prophylaxis, but it is associated with frequent adverse effects. This study evaluated the efficacy and safety of the current protocol and proposes an individualized risk-based prophylaxis protocol. Methods: The PJP incidence and risk factors during the first 6 months (early PJP) and afterwards (late PJP) was assessed in renal transplant recipients with (prophylaxis group) and without (no-prophylaxis group) 6-month PJP prophylaxis. Results: In 578 patients, there were 39 cases of PJP during a median follow-up of 51 months. Renal adverse events were encountered frequently during trimethoprim-sulfamethoxazole prophylaxis, leading to premature discontinuation. Patients without the prophylaxis had a significantly higher incidence of early PJP (n=27, 6.6%) compared to patients with the prophylaxis (n=0). The incidence of late PJP was 2.2%, without between-group differences. The factors associated with early PJP were preoperative desensitization and acute rejection within 1 month, whereas late PJP was associated with age, deceased donor transplant, and acute rejection requiring antithymocyte globulin treatment. Conclusions: Based on the simulation results of several risk-based scenarios, the authors recommend universal prophylaxis up to 6 months post-transplant and extended selective prophylaxis in patients aged ${\geq}57$ years and those with a transplant from deceased donors.

  • The purpose of this study is to identify the direct effects of PSM on job satisfaction (JS) and perceived job performance (PJP) in public institutions and to identify indirect effects of the mediator. In particular, we define the person-organization fit (POF) used as the mediator as the fitness of the personnel system and identify the influence of POF in the relationship between PSM, JS, and PJP. The questionnaires used in this study were conducted for the members of Changwon Public Health Center and empirical analysis using structural equation modeling was conducted and the following results were obtained. First, PSM in the integrated public service organization has a positive effect on JS and PJP. Second, PSM shows that JS and PJP of organization members are improved through mediating effect of POF. It can be concluded that PSM can improve JS and PJP through institutional fit between members and organizations.

  • Design of a Pump Jet Propulsor (PJP) was undertaken for an underwater body with axisymmetric configuration using axial/low compressor design techniques supported by Computational Fluid Dynamics (CFD) analysis for performance prediction. Experimental evaluation of the PJP was earned out through experiments in a Wind Tunnel Facility (WTF) using momentum defect principle for propulsive performance prior to proceeding with extensive experimental evaluation in towing tank and cavitation tunnel. Experiments were particularly conducted with respect to Self Propulsion Point (SPP), residual torque and thrust characteristics over a range of vehicle advance ratio in order to ascertain whether sufficient thrust is developed at the design condition with least possible imbalance torque left out due to residual swirl in the slip stream. Pumpjet and body models were developed for the propulsion tests using Aluminum alloy forged material. Tests were conducted from 0 m/s to 30 m/s at four rotational speeds of the PJP. SPP was determined confirming the thrust development capability of PJP. Estimation of residual torque was carried out at SPP corresponding to speeds of 15, 20 and 25 m/s to examine the effectiveness of the stator. Estimation of thrust and residual torque was also carried out at wind speeds 0 and 6 m/s for PJP RPMs corresponding to self propulsion tests to study the propulsion characteristics during the launch of the vehicle m water where advance ratios are close to Zero. These results are essential to assess the thrust performance at very low advance ratios to accelerate the body and to control the body during initial stages. This technique has turned out to be very useful and economical method for quick assessment of overall performance of the propulsor and generation of exhaustive fluid dynamic data to validate CFD techniques employed.

Growth Performance, Carcass and Meat Characteristics of Black Goat Kids Fed Sesame Hulls and Prosopis juliflora Pods

  • Finding alternative feeds, such as sesame hulls and Prosopis juliflora species can attenuate difficulties of feed shortage and reduce the cost of animal feed in arid and semi-arid countries. Thirty-two Black male kids with similar initial weights (BW = $16.7{\pm}0.80\;kg$) and $120{\pm}5\;d$ of age, were used to evaluate the effect of replacing barley grains and soybean meal with Prosopis juliflora pods (PJP) and sesame hulls (SH) on growth performance, digestibility and carcass and meat characteristics. Kids were equally divided into four dietary treatment groups for an 84-d fattening period. Treatment diets had similar crude protein (CP) and metabolizable energy (ME). The treatment groups were: (T1) no PJP nor SH, (T2) 10% PJP and 20% SH, (T3) 15% PJP and 15% SH, and (T4) 20% PJP and 10% SH. A tendency was detected (p<0.08) for dry matter (DM), crude protein (CP), organic matter (OM) and neutral detergent fiber (NDF) intakes that were greater for T2 than T3 and T4 while T1 was not different from all other treatment groups. Ether extract (EE) intake was the greatest (p<0.05) for T2 and the lowest for T1. Acid detergent fiber (ADF) intake was greater (p<0.05) for T2 than T1 while T3 and T4 were intermediate. Final live weight, average daily gain (ADG) and feed conversion ratio (FCR) were comparable among different treatment groups. Digestibility of DM, OM, CP, NDF and ADF were similar among all treatment groups, however, EE digestibility was the lowest (p<0.05) for T1 when compared to other treatments. In addition, nitrogen intake, nitrogen in urine and retained and retention percentages were similar among all treatment groups. However, N loss in feces was higher (p<0.05) for T2 than T3 and T4 while T1 was intermediate. No differences were observed among treatment groups with respect to fasting live weight, hot and cold carcass weights, dressing-out percentages, mesenteric fat, visceral organs, carcass cuts percentages and carcass linear dimensions. No differences were also observed for dissected loin, leg, rack and shoulder tissues except in the total bone % for loin cuts and in the meat to bone ratio for rack cuts. T3 has the greatest total bone % and the lowest meat to bone ratio when compared to all other treatment groups. No differences were observed between treatment groups in all quality characteristics of the longissimus muscle. The present study demonstrates the potential of using PJP and SH for growing kids without adverse effects on growth performance, carcass characteristics and meat quality.

Experimental Evaluation of Seismic Column Splice with Partial Joint Penetration Welds (부분용입용접 내진기둥 이음부의 강도평가)

  • The seismic performance of a column splice fabricated with PJP (partial joint penetration) welds for special moment frames was experimentally evaluated in this study. The steel materials that were used for the specimens included SHN490 and SN490 steel, or the newly developed structural steel for seismic application. Fabricating the column splice with PJP welds is highly attractive from the perspective of reducing the welding cost and the construction time. PJP welds in column splices are viewed apprehensively, however, because several tests have shown that PJP welds in thick members tend to become brittle under tensile loads. The column splices in this testing program were designed for the expected plastic moment of the column that current seismic codes typically require. The design strength of partial-penetration welded joints was determined according to the 2005 AISC-LRFD Specification. Three-point loading was applied monotonically, using a universal testing machine, such thatthe column splice joints were subjected to pure tension. The test results showed that the PJP welded splices, if designed properly, can develop a strength exceeding that of the actual plastic moment of the column. The specimen made of the SM490 rolled section, however, showed a brittle fracture at the splice soon after achieving the actual plastic moment of the column. The tensile coupon test results also showed that the material properties of SM490 steel are more unpredictable. Overall, although the test data are limited, the SHN490 and SN490 steel specimens showed a superior and reliable performance.

CT Findings of Granulomatous Pneumocystis jiroveci Pneumonia in a Patient with Multiple Myeloma (다발성 골수종 환자에서 발생한 육아종성 폐포자충 폐렴의 컴퓨터단층촬영 소견)

  • Although the typical CT findings of Pneumocystis jiroveci pneumonia (PJP) include diffuse or multifocal areas of ground-glass opacities in both lungs, it can also rarely manifest as multiple pulmonary nodules. We report a rare case of atypical PJP in an immunocompromised patient with multiple myeloma, presenting as widespread ground-glass opacities and multiple necrotic subpleural nodules in both lungs on CT, which proved to be granulomatous PJP on percutaneous transthoracic needle biopsy.

  • " Analyzing on an object in the sphere of domestic law with the method of international law" has been already on the horizon in international law. For the lawyers of international law and space law, it is quite natural that they utilize the method of international law, whatever objects they may choice. The reason is that the characteristic of international law do not depend upon object in itself, but depend upon method in itself. The object of this paper is the idea of Peaceful Use(IPU or PU) in Japanese Space Policy. The method to be applied to this analysis is the international law's interpretation theory on legal principles, i.e., the method of international law. One of the aims of this paper is to explain critically the need of review on IPU in Japanese Space Policy; in particular with respect to the positive reconstruction of IPU through historical analyzing on the transfiguration and the mere shell of the Post-War Japanese Pacifism(PJP) as the starting point of IPU The historical process of the transfiguration and the mere shell is as followed .i.e.," from the ultranationalism in the pre-war Japan to PJP in the post-war Japan, from PJP to IPU, and from IPU to IPU's regression. In particular with respect to the interpretation theory, the meaning of the teleological, aims and objects school's approach on the interpretation of legal principles(P) has been emphasized. The reason is that the promising development of IPU will be realized by cooperating with the interpretation theory on P in international law. At the end of the beginning, I'd like to quote K. Marx's thesis in order to make the positions and missions the lawyers of international law and space clear. It is as follows, i.e.," THE PHILOSOPHERS HAVE ONLY INTERPRETED THE WORLD IN VARIOUS WAYS - THE POINT, HOWEVER IS TO CHANGE IT.

Cavitation studies on axi-symmetric underwater body with pumpjet propulsor in cavitation tunnel

  • A pumpjet propulsor (PJP) was designed for an underwater body (UWB) with axi-symmetric configuration. Its performance was predicted through CFD study and models were manufactured. The propulsor design was evaluated for its propulsion characteristics through model tests conducted in a Wind Tunnel (WT). In the concluding part of the study, evaluation of the cavitation performance of the pumpjet was undertaken in a cavitation tunnel (CT). In order to assess the cavitation free operation speeds and depths of the body, cavitation tests of the PJP were carried out in behind condition to determine the inception cavitation numbers for rotor, stator and cowl. The model test results obtained were corrected for full scale Reynolds number and subsequently analyzed for cavitation inception speeds at different operating depths. From model tests it was also found that the cavitation inception of the rotor takes place on the tip face side at higher advance ratios and cavitation shifts towards the suction side as the RPS increases whereas the stator and cowl are free from cavitation.
