Background: A significant proportion of cardiac surgery intensive care unit (CSICU) patients require long-term ventilation, necessitating tracheostomy placement. The goal of this study was to evaluate the long-term postoperative outcomes and complications associated with percutaneous dilatational tracheostomy (PDT) in CSICU patients. Methods: All patients undergoing PDT after cardiac, thoracic, or vascular operations in the CSICU between January 1, 2013 and January 1, 2021 were identified. They were evaluated for mortality, decannulation time, and complications including bleeding, infection, and need for surgical intervention. Multivariable regression models were used to identify predictors of early decannulation and the complication rate. Results: Ninety-three patients were identified for this study (70 [75.3%] male and 23 [24.7%] female). Furthermore, 18.3% of patients had chronic obstructive pulmonary disease (COPD), 21.5% had history of stroke, 7.5% had end-stage renal disease, 33.3% had diabetes, and 59.1% were current smokers. The mean time from PDT to decannulation was 39 days. Roughly one-fifth (20.4%) of patients were on dual antiplatelet therapy and 81.7% had anticoagulation restarted 8 hours post-tracheostomy. Eight complications were noted, including 5 instances of bleeding requiring packing and 1 case of mediastinitis. There were no significant predictors of decannulation prior to discharge. Only COPD was identified as a negative predictor of decannulation at any point in time (hazard ratio, 0.28; 95% confidence interval, 0.08-0.95; p=0.04). Conclusion: Percutaneous tracheostomy is a safe and viable alternative to surgical tracheostomy in cardiac surgery ICU patients. Patients who undergo PDT have a relatively short duration of tracheostomy and do not have major post-procedural complications.
Experimental investigation was conducted to evaluate the seismic ductility of earthquake-experienced concrete columns with an aspect ratio of 2.5. Eight circular concrete columns with a diameter of 600 mm were constructed with three test parameters: confinement ratio, lap-splice of longitudinal bars, and retrofitting with Fiber Reinforced Polymer (FRP) materials. The objective of this research is to examine the seismic performance of RC bridge piers subjected to a Quasi static test (QST), which were preliminary tested under a series of artificial earthquake motions referred to as a Pseudo dynamic test (PDT). The seismic enhancement effect of FRP wrap was also investigated on these RC bridge piers. Six specimens were loaded to induce probable damage by four series of artificial earthquakes, which were developed to be compatible with earthquakes in the Korean peninsula by the Korea Highway Corporation (KHC). Directly after the PDT, six earthquake-experienced columns were subjected to inelastic cyclic loading under a constant axial load of $0.1{f_c}^{\prime}A_g$. Two other reference specimens without the PDT were also subjected to similar quasi-static loads. Test results showed that specimens pre-damaged by moderate artificial earthquakes generally demonstrated good residual seismic performance, which was similar to the corresponding reference specimen. Moreover, RC bridge specimens retrofitted with wrapping fiber composites in the potential plastic hinge region exhibited enhanced flexural ductility.
Percutaneous dilatational tracheostomy (PDT) has become an increasingly popular method of establishing an airway for patients in need of chronic ventilator assistance. We report a rare case of a 42-year-old female who developed extensive subcutaneous emphysema, bilateral pneumothoraces, pneumomediastinum, and pneumoperitoneum after percutaneous dilatational tracheostomy. The patient suffered from amyotrophic lateral sclerosis, and underwent PDT after a period of mechanical ventilation. During PDT, tracheostomy tube was inserted into the paratracheal space. Follow-up chest radiography and computed tomography of chest and abdomen revealed extensive subcutaneous emphysema, bilateral pneumothoraces, pneumomediastinum, and pneumoperitoneum. The patient was treated successfully with insertion of the thoracostomy tube and conservative care.
Selective accumulation of a photosensitizer and the subsequent response in only the light-irradiated target are advantages of photodynamic diagnosis and therapy. The limited depth of the therapeutic effect is a positive characteristic when treating surface malignancies, such as peritoneal carcinomatosis. For photodynamic diagnosis (PDD), adjunctive use of aminolevulinic acid- protoporphyrin IX-guided fluorescence imaging detects cancer nodules, which would have been missed during assessment using white light visualization only. Furthermore, since few side effects have been reported, this has the potential to become a vital component of diagnostic laparoscopy. A variety of photosensitizers have been examined for photodynamic therapy (PDT), and treatment protocols are heterogeneous in terms of photosensitizer type and dose, photosensitizer-light time interval, and light source wavelength, dose, and dose rate. Although several studies have suggested that PDT has favorable effects in peritoneal carcinomatosis, clinical trials in more homogenous patient groups are required to identify the true benefits. In addition, major complications, such as bowel perforation and capillary leak syndrome, need to be reduced. In the long term, PDD and PDT are likely to be successful therapeutic options for patients with peritoneal carcinomatosis, with several options to optimize the photosensitizer and light delivery parameters to improve safety and efficacy.
This study aimed to systematically review the effectiveness of methylene blue (MB) photosensitizers in the management of symptomatic oral lichen planus (OLP). Electronic online databases and manual searches were performed for randomized controlled trials (RCTs) published in English between January 2010 and February 2022. RCTs comparing photodynamic therapy (PDT) and corticosteroid therapy at baseline and follow-up period were identified. The Cochrane risk of bias tool was used to assess the quality of the included studies. A meta-analysis was performed regarding visual analog scale (VAS) scores, Thongprasom sign scores, lesion size, response to treatment, and exacerbation of lesions after therapy. The clinical severity was analyzed qualitatively. Five RCTs consisting of 180 samples fulfilled the inclusion and exclusion criteria. All parameters of VAS score, Thongprasom sign score, lesion size, and response to treatment were statistically non-significant. Our results indicate that both MB-PDT and corticosteroid therapy are effective for the management of OLP. Moreover, MB-PDT is an effective alternative treatment option for OLP when corticosteroids are contraindicated. However, conclusive evidence cannot be ascertained owing to the heterogeneity among the studies.
광역동 치료는 광감각제가 빛에 의해 활성화되면서 발생하는 화학 반응을 이용한 것으로, 치료 원리는 광화학 반응으로 자유 라디칼 및 반응성 산소가 생성되고 이 산물들에 의한 세포 독성으로 항균 효과를 가지게 되는 것이다. 이 연구의 목적은 치과 임상에서 널리 사용되는 광원(할로겐, LED)과 광감각제(erythrosine)를 이용하여, 치아 우식증과 연관된 세균인 Streptococcus mutans에 대한 광역동 치료의 항균 효과를 알아보고, 광감각제의 농도에 따른 광역동 치료의 효과를 평가하기 위함이다. 또한 임상 분리 균주와 표준 균주에 대한 광역동 치료의 효과를 비교해 보았다. 연구 결과, 표준 및 임상 분리 균주 모두 광감각제 처리 후 광조사를 시행한 군에서만 대조군에 비해 S. mutans의 유의한 감소가 나타났다. 또한 광조사를 시행한 군에서 첨가한 광감각제의 농도가 높을수록 S. mutans의 감소가 증가하는 것으로 나타났다. 표준 균주와 비교 시 임상 분리 균주에서는 표준 균주와 비슷하거나 약간 낮은 S. mutans의 감소가 나타났고, 이는 통계적으로 유의한 차이는 없었다(p < 0.05). 이상의 결과들로 보아 광감각제로 에리스로신의 사용과 광원으로 치과용 광중합기를 사용한 광역동 치료는 S. mutans 연관 질병에 대한 효과적인 치료 방법이 될 수 있을 것으로 사료된다.

광역동치료(PDT)는 갖가지 고형 종양의 치료를 위한 임상적시도로 시작되어 화학요법 및 방사선 요법에 내성이 있는 종양의 대체 치료법의 하나로 제시되고 있다. PDT는 전신적 또는 국소적으로 투여하여 유해한 조직에 선택적으로 농축되도록 한 광증감제와 이를 활성화하는 적절한 파장과 에너지를 가진 레이저광의 조합에 기초한다. 이 연구에서는, 인체 골육종 세포(HOS)에 미치는 레이저 EIT 21의 광독성 효과에 대해 알아보았으며, 그런 광독성 효과가 세포고사를 유발하는가를 규명하고자 하였다. 본 연구는 레이저 EIT 21이 HOS 세포에 대해 광독성을 효과를 가진다는 것을 증명했다. 세포 죽음이 세포괴사에 의해 유발되는지 아니면 세포고사에 의해 유발되는지를 알아보기 위해 세포 고사를 평가 하는 여러 실험 기법을 이용하였다. TUNEL 분석은 극소수만이 응축된 핵의 양성반응을 보여주었다. Hemacolor와 AO/EB 염색 또한 대부분의 세포가 괴사로 죽는 것을 보여주었다. 레이저 EIT로 조사된 HOS 세포에서 응축되거나 분절된 핵을 발견하는 것은 어려웠다. DNA 전기영동에서, 세포고사에서 보여지는 DNA 분절의 전형적인 특징인 사다리형 절편 형태(ladder fragmentation pattern)가 나타나지 않았다. Western blotting에 의한 분석에서 p53의 발현은 일정하게 나타났고 레이저로 조사된 세포는 caspase-3과 PARP의 분열을 나타내지 않는 것으로 보아 레이저 유도 세포 죽음(laser-induced cell death)은 p53과는 관련이 없는 것 같다.
본 논문에서는 HM-Net (Hidden Markov Network)을 다양한 태스크에의 적용과 화자의 특성을 효과적으로 나타내기 위해 HM-Net 음성인식 시스템에 MLLR (Maximum Likelihood Linear Regression) 적응방법을 도입하였으며, HM-Net 학습 알고리즘을 개량하여 회귀클래스 생성방법을 제안한다. 제안방법은 PDT-SSS (Phonetic Decision Tree-based Successive State Splitting)알고리즘의 문맥방향 상태분할에 의한 상태레벨 공유를 이용한 방법이다. 즉, 문맥방향의 각 상태에 적응화자 음성데이터에 포함된 문맥정보를 분할하여 적응화될 음소환경을 결정하는 것이다. 따라서 제안방법은 새로운 화자로부터 문맥정보와 적응화 데이터의 발성 양에 의존하여 결정된 많은 적응 파라미터들을 (평균, 분산) 자유롭게 제어할 수 있게 된다. 제안방법의 유효성을 확인하기 위해 국어공학센터 (KLE) 452 데이터와 항공편 예약관련 (YNU200) 연속음성을 대상으로 인식실험을 수행한 결과, 음소인식, 단어인식, 연속음성인식에 대해서, 평균 34∼37%, 평균 9%, 평균 20%의 성능 향상을 각각 보였다. 또한 적응화 데이터의 양에 따른 인식성능 비교에서 제안방법을 적용한 인식 시스템이 적응 데이터의 양이 적은 경우에도 향상된 인식률을 보여 MLLR 적응방법의 특성을 만족하였다. 따라서 MLLR 적응방법을 도입한 HM-Net 음성인식 시스템에 제안한 회귀클래스 생성방법이 유효함을 확인할 수 있었다.
중공사형 정밀여과(MF)와 한외여과(UF)를 포함한 가압식 막여과 시스템은 높은 수준의 병원균 제거 효율로 인해 음용수 처리에 점점 더 많이 사용되고 있다. 그러나 가압식 막여과 시스템에서 분리막의 손상으로 인한 막완결성의 문제가 발생시 병원체가 유출될 수 있으므로, 효과적인 온라인 또는 오프라인 방식의 막 완결성 모니터링 방법을 통해 수질의 안정성을 보장하여야 한다. 최근 막 완결성 시험 중 하나 인 압력손실시험(pressure decay test, PDT)이 가압식 분리막 모듈을 이용한 음용수 처리 시설에 적용되고 있다. 이 논문에서는 PDT의 기체 주입방법에 따른 세 가지 조건으로 원수 측, 여과수 측, 원수 여과수 양방향 측 세 가지 조건을 선정 후 이에 따른 감지감도를 이론값과 실험값을 적용하여 비교하였다. 실험결과 가압되는 측의 부피와 감도는 반비례함을 확인하였고, 주입부의 반대방향에 부압을 형성 시에도 감도가 증가함을 확인할 수 있다. 즉 막 손상의 감지감도는 주입되는 부분의 체적이 작아질수록, 분리막을 경계로 유입과 여과 측의 압력차가 클수록 높아짐을 알 수 있었다.
이 연구의 목적은 악궁 전반적으로 조사가 가능한 미백 LED를 이용하여 우식원성 Streptococcus mutans에 대한 광역동 치료의 효과를 알아보기 위한 in vitro 연구이다. S. mutans를 각각 두가지 다른 단계의 상태로 배양하였다. Planktonic 상태와 Hydroxyapatite disk와 CDC biofilm reactor를 5일간 사용하여 동적인 바이오필름 상태를 형성하였다. 20 μM 로 희석한 erythrosine을 5분간 전처리한후 가정용 미백기와 진료실 미백기를 사용하여 각 15분씩 광조사를 시행하였다. 실험 종료 후 각 실험군의 Colony Forming Unit를 측정하였으며 공초점 레이저 현미경를 이용하여 각 조건에 따른 광역동 치료 효과를 비교하였다. 실험 결과 S. mutans에 대한 aPDT효과는 control군과 비교시에 각각 플랑크토닉상태와 동적인 바이오필름 상태에서 모두 통계적으로 유의한 광역동 치료 효과가 나타났다(각각 p = 0.001, p = 0.002). 하지만 동적인 바이오필름 상태에서는 플랑크토닉 상태보다 광역동치료에 내성을 지녀 항미생물 효과가 떨어지는 양상이 관찰되었다. 진료실용 미백 LED와 가정용 미백LED는 유사한 항미생물 효과를 나타냈다. 구강악궁 전반적 조사가 가능한 미백용 LED는 우식의 원인이 되는 S. mutans 바이오필름에 유의한 광역동 치료의 효과를 나타냈으며, 이러한 결과는 치아 우식 예방으로의 광역동 치료의 임상적 적용 가능성을 제시한다.
