노인의 수분섭취정도와 구강건조증, 구취, 구강미생물과의 관계에 대한 융합적 연구 (지역사회 거주 노인중심으로) (A Convergence study of Water intake on relationship between Xerostomia, Halitosis, Oral microorganisms in the Elderly)

  • 본 연구는 지역사회에 거주하는 노인 50명을 대상으로 수분섭취 정도와 구강건조증, 구취, 구강미생물과의 관계를 확인하고 자발적인 수분섭취강화 프로그램개발의 기초자료로 활용하기 위하여 진행되었다. 자료수집은 2018년 7월 1일부터 8월 15일까지 진행되었으며 50명의 자료를 분석하였다. 대상자의 일반적 특성과 변수와의 관계를 확인한 결과 S. mutans와 P. intermedia 검출에 공통으로 영향을 주는 변인은 칫솔질 횟수와 치과 방문 경험이었으며, 수분섭취량이 다른 세 그룹을 비교한 결과 P. intermedia에서 통계적으로 유의한 차이가 있는 것으로 분석되었다. 본 연구결과를 토대로 수분섭취강화프로그램을 개발 적용 후 구강건강 개선을 확인하는 연구가 필요하다.

  • The antibacterial efficacy of a mouthrinse(Denta Gargle) containing CPC(cetylpyridinium chloride), NaF and UDCA(ursodeoxycholic acid), on major periodontopathogens, was in vitro examined and compared with that of Listerine by a broth dilution method. The bacteria tested were Actinobacillus actinomycetemcomitans, Bacteroides forsythus, Fusobacterium nucleatum subsp. vincentii, Prevotella intermedia, Porphyromonas gingivalis and Treponema denticola. The growth of all the bacteria were completely inhibited by a 1-min exposure to the both mouthrinses. When diluted at 1:5 or more, all bacteria analyzed but P. intermedia were not inhibited by Listerine. In contrast, Denta Gargle showed highly increased maximum inhibitory dilutions(MID) against all periodontopathogens included in this study, with MIDs ranging from 5-fold(F. nucleatum) to 160-fold dilutions(P. intermedia). The MIDs against A. actinomycetemcomitans, B. forsythus, P. gingivalis and T. denticola. were 1:40, 1:80, 1:80 and 1:80, respectively.

Expression of Inflammasome Complex Following Various Oral Bacterial Infection in THP-1 Cells

  • Interleukin-1b ($IL-1{\beta}$), a proinflammatory cytokine, regulates the innate immune responses against bacterial infection. Mature $IL-1{\beta}$ is produced from $pro-IL-1{\beta}$ by activated caspase-1, which in turn is activated by the inflammasome complex formation. In this study, we compared the inflammasome mRNA expression induced by S. sanguinis, S. oralis, F. nucleatum and P. intermedia. Among the tested bacteria, S. sanguinis induced the highest $IL-1{\beta}$ secretion. S. oralis, F. nucleatum and P. intermedia induced very weak $IL-1{\beta}$ secretion. S. sanguinis mostly induced the NLRP3 mRNA expressions. Although F. nucleatum did not induce high $IL-1{\beta}$ secretion, it induced high expression levels of AIM2, NLRP2, and NLRP3. No specific inflammasomes were induced by S. oralis and P.intermedia. Studying the inflammasome complex activation induced by oral bacteria may thus enhance our understanding of the pathogenesis of oral diseases.

치아 근관 감염 검체에서 검은 색소 형성 Prevotella species와 Porphyromonas species의 동정 (Identification of the Black-pigmented Prevotella Species and Porphyromonas Species from Infected Dental Root Canals)

  • Anaerobic black-pigmented bacteria have been implicated in the endodontic infections. This group of microorganisms includes Porphyromonas endodontalis, P. gingivalis, Prevotella intermedia, and Prevotella nigrescens. The organisms display a wide variety of virulence factors that may be pertinent to acute endodontic infections. The aim of this study was to identify P. endodontalis, P. gingivalis, P. intermedia, and P. nigrescens by using the special potency disk test, filter paper spot test, 16S rRNA gene-directed PCR, and API 32A system. Microbial samples were collected from root canals of 33 intact teeth with necrotic pulp and apical periodontitis. Conventional laboratory methods were used to identify the strains of anaerobic black pigmented bacteria. Eighteen out of 33 samples were positive for the growth of black-pigmented bacrteria. Five colonies were cultured from each pure cultured colony from Brucella agar plates. Seventy seven colonies were positive for the growth of black-pigmented bacteria. Thirty three out of 77(42.8%) were identifed as P. nigrescens, 10 out of 77(13%)were P. gingivalis, 6 out of 77(7.8%) were P. endodontalis, 10 out of 77(13%) were P. intermedia. On the contrary the reference strains of P. nigrescens, experimental strains of P. nigrescens were susceptible to kanamycin in the special potency disk test. We concluded that after rapid presumptive identification methods, such as the special potency disk test and filter paper spot test were done, 16S rRNA gene PCR and API 32A test would be accurate detection methods for black-pigemented bacteria.

Antimicrobial Activity of Oleanolic Acid, Ursolic Acid, and Sophoraflavanone G against Periodontopathogens

  • In general, oleanolic acid (OA) and ursolic acid (UA) have antimicrobial effect against Gram-positive bacteria but not Gram-negative bacteria whereas sophoraflavanone G has antimicrobial activity against both bacterial types. However, the antimicrobial effects of OA, UA, and sophoraflavanone G against periodontopathogens have not been studied to any great extent. The aim of this study was to investigate antimicrobial effect of OA, UA, and sophoraflavanone G against 15 strains (5 species) of oral Gram-negative bacteria, which are the major causative bacteria of periodontal disease. The antimicrobial activity was evaluated by minimal inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) determinations. OA and UA showed antimicrobial effects against all of the Porphyromonas gingivalis strains tested and also Prevotella intermedia ATCC $25611^T$. Interestingly, P. intermedia ATCC 49046 showed greater resistance to OA and UA than P. intermedia ATCC $25611^T$. In contrast, sophoraflavanone G had antimicrobial activity against all strains, with MIC and MBC values below $32{\mu}g/ml$, except Aggregatibacter actinomycetemcomitans. These results indicate that sophoraflavanone G may have potential for use in future oral hygiene products such as dentifrices and gargling solution to prevent periodontitis.

성인의 타액 내 구강세균 검출과 위험요인에 관한 연구 (Study on Detection of Oral Bacteria in the Saliva and Risk Factors of Adults)

  • 구강질환은 단독 진행만이 아닌 혼합감염증으로 발생하므로 치아우식증 및 치주질환에 관여하는 원인 균주들의 명확한 분석이 필요하다. 이에, 본 연구는 치아우식증 및 치주질환의 원인균으로 잘 알려져 있는 세균을 검출하기 위해 타액을 채취하였다. 성인의 연령, 흡연 및 음주, 질병유무에 따른 구강세균분포 차이를 보고 구강세균의 위험요소를 확인하고자 한다. 본 연구 대상은 20대 이상 65세 미만 성인 120명을 대상으로 2014년 3월 15일 - 5월 10일까지 조사하였다. 타액 내 gDNA를 추출 후 PCR 방법을 이용하여 구강질환의 균 분포도를 확인하였다. 그 결과, S. mutans는 72명, P. intermedia 88명, S. mutans와 P. intermedia 균이 모두 검출된 성인은 54명, 구강세균이 검출되지 않은 성인은 14명으로 나타났다. 구강세균의 위험 요소 결과, 흡연자는 비흡연자에 비해 S. mutans 2.8배, P. intermedia 3.5배 더 높게 나타났다. 음주자는 비음주자에 비해 S. mutans 검출 위험도가 3.3배 더 높은 것으로 나타났으며, 전신질환자는 정상인에 비해 P. intermedia 검출된 위험도가 4.1배 더 높은 것으로 나타났다. 흡연, 음주, 전신질환은 구강 내 세균 검출위험도가 높은 인자임을 확인할 수 있었다. 또한 연령이 증가할수록 치주질환 세균이 더 많이 검출되었으며, 20대는 치아우식증과 치주질환이 공존하는 연령대인 만큼 구강세균의 분포가 더 두드러지게 나타남을 알 수 있었다. 구강세균이 검출된 성인은 치아우식증과 치주질환에 이환될 위험도가 그만큼 높다고 볼 수 있으므로, 구강환경을 청결히하고 정기적인 치과방문을 통해 구강질환을 예방해야 할 것이다.

Cloning of a Potentially Strain-Specific DNA Probe of prevotella intermedia ATCC 25611 by Inverted Dot Blot Hybridization Screening Method

  • The purpose of this study was to isolate a specific DNA probe for the strain ATTC 25611 of the species Prevotella intermedia by using a new rapid screening mothod. The whole-genomic DNA of P. intermedia ATCC 25611 was isolated and purified. The HindIII-digested genomic DNAs from the strain were cloned by the random cloning method. To screen the strain-specific DNA probe, inverted dot blot hybridization tests were performed. In this assay, 20 ng of recombinant plasmids containing the HindIII-digested genomic DNA fragment were boiled and blotted onto a nylon membrane, and hybridized with digoxigenin-dUTP labeled genomic DNAs in a concentration of 100 ng/ml. Southern blot analysis was performed in order to confirm the results of the inverted dot blot hybridization tests. The data showed that a Pi34 probe (2.1 kbp; 1 out of 32 probes) was specific for P. intermedia strain ATCC 25611 and could be useful for the detection and identification of the strain, particularly in epidemiological studies of periodontal disease.

Quantitative analysis of periodontal pathogens present in the saliva of geriatric subjects

  • Purpose: At present, information regarding periodontal disease in geriatric patients is scarce. The purpose of this study was to quantify the periodontal pathogens present in the saliva of Korean geriatric patients and assess the relationship between the bacterial levels and the periodontal condition. Methods: Six putative periodontal pathogens were quantified by using a real-time polymerase chain reaction assay in geriatric patient groups (>60 years) with mild chronic periodontitis (MCP), moderate chronic periodontitis (MoCP), and severe chronic periodontitis (SCP). The copy numbers of Porphyromonas gingivalis, Tannerella forsythia, Treponema denticola, Aggregatibacter actinomycetemcomitans, Fusobacterium nucleatum, and Prevotella intermedia were measured. Results: It was found that the bacterial copy numbers increased as the severity of the disease increased from MCP to SCP, except for P. intermedia. For P. intermedia, it was found that samples in the MCP group yielded the largest amount. It was also found that the quantities of P. gingivalis, T. forsythia, and T. denticola, the so-called "red complex" bacteria, were lower than those of F. nucleatum, A. actinomycetemcomitans, and P. intermedia in all of the samples. Conclusions: Collectively, the results of this study suggest that the levels of P. gingivalis, T. forsythia, F. nucleatum, and T. denticola present in saliva are associated with the severity of periodontal disease in geriatric patients.
