청소년의 주관적 건강 상태의 변화 궤적과 영향 요인: 잠재성장모형을 적용하여 (Analyzing Changes and Determinants of Self-rated Health during Adolescence: A Latent Growth Analysis)

  • Purpose: The purpose of this study was to examine changes in the self-rated health of adolescents and to identify its predictors using longitudinal data from the KCYPS. Methods: A sample of 2,351 adolescents who were in the first grade of middle school in 2010 was analyzed. The study employed latent growth analysis using data from 2010 to 2016. Results: Results indicated that self-rated health of adolescents increased, following the form of a linear function. The analyses revealed that adolescent self-perception of health were conceptualized not only by their health-related behaviors, but also by personal, socioeconomic and psychological factors. Specifically, physical activity, passive leisure time activities, gender (initial: b=-.060, slope: b=.030), place of residence (initial: b=-.079), self-rated economic condition (b=.098), working status of mother (b=.016), monthly family income (b=-.001), aggression (b=.061), depression (initial: b=-.104, slope: b=.012), stress (initial: b=-.172, slope: b=.014, ego-resiliency (initial: b=.197, slope: b=-.021), and self-esteem (initial: b=.106, slope: b=-.017) had significant effects on the overall linear change of self-rated health (p<.05 for all estimators above). Conclusion: The findings of this study suggest that adolescents' self evaluation of their health is shaped by their total sense of functioning, which includes individual, health-related behavioral, socioeconomic, and psychological factors.

국민건강영양조사 제6기 자료를 활용한 구강건강관련요소와 악력과의 연관성 (Association between oral health-related factors and grip strength using the 6th Korea National Health and Nutrition Examination Survey data)

  • Objectives: Grip strength is a measure for assessing overall muscle strength, muscle mass, and nutritional status, and is a useful tool for early examination of a person's general health. Thus, this study analyzed the association between oral health-related factors and grip strength, using the 6th Korea National Health and Nutrition Examination Survey (KNHANES) data. Methods: Data were analyzed using the IBM SPSS version 25.0 (IBM Co., Armonk, NY, USA) software with a complex sampling analysis of stratified and clustered variables, all with weighted values, applied for every analysis conducted. From the 6th KNHANES data, a total of 26,101 people were selected as the study population. A complex sample generalized linear model analysis was performed for participants' sociodemographic characteristics, health status, use of oral care products, and factors such as adult mastication and speaking habits. Results: Grip strength was found to be higher among men compared to women. Grip strength was the highest in the 20-39 year old group. Furthermore, grip strength was statistically significantly higher among married and employed individuals, and in the right hand (p<0.001). Grip strength was statistically significantly higher among people who perceived themselves to have good health and among those who did not have health-related problems in the past two weeks (p<0.001). It was also higher among those who used interdental brushes and electronic toothbrushes (p<0.001), but decreased with greater discomfort in mastication and speaking among adults (p<0.05). Conclusions: Grip strength was found to be associated with oral health-related factors and therefore, these could be helpful tools in evaluating both general and subjective health statuses. In general, oral care products are small in size with oral care involving the use of hands and certain precise motions and actions to remove foreign substances in the oral cavity. As shown in the findings, grip strength declines with increased age and as a result, the use of oral care products should be strongly recommended in order to promote better general health.

유해인자 노출감시를 통한 산업보건서비스 강화 (Strengthening Occupational Health Services through Monitoring Exposure to Health Hazards)

  • Objective: The purpose of this study was to find ways for strengthening occupational health services through monitoring exposure to health hazards. Methods: About 70,000 workplaces that have conducted the work environment measurement(WEM) over the last three years(2017~2019) were classified according to the Korean Standard Industry Classification(KSIC), and the current status of WEM by industry was analyzed. The authors considered ways to monitor exposure to health hazards in order to strengthen occupational health services and protect workers' health. Results: Based on the KSIC, 934 of the 1,196 total sub-classified industries have conducted WEM in at least one workplace over the last three year(2017~2019). In the case of manufacturing, out of a total of 477 sub-classified industries, 474 have conducted WEM at more than one workplace. However, in some industries, WEM was not conducted or the implementation rate was low, so it was necessary to examine whether WEM should be expanded based on a detailed analysis of the WEM database. To this end, it is necessary to form an exposure monitoring committee in which various experts from different fields can participate. The committee needs to discuss the overall matters necessary for selecting industries that require detailed investigation or research, review the results, and prepare a final recommendation. Conclusion: In conclusion, the government needs to come up with a plan to strengthen occupational health services through surveys and research on the current status of WEM and work environment management models by industry.

Role of Micronutrients in Skin Health and Function

  • Skin is the first line of defense for protecting our bodies against external perturbations, including ultraviolet (UV) irradiation, mechanical/chemical stress, and bacterial infection. Nutrition is one of many factors required for the maintenance of overall skin health. An impaired nutritional status alters the structural integrity and biological function of skin, resulting in an abnormal skin barrier. In particular, the importance of micronutrients (such as certain vitamins and minerals) for skin health has been highlighted in cell culture, animal, and clinical studies. These micronutrients are employed not only as active compounds in therapeutic agents for treating certain skin diseases, but also as ingredients in cosmetic products. Here, the author describes the barrier function of the skin and the general nutritional requirements for skin health. The goal of this review is to discuss the potential roles and current knowledge of selected micronutrients in skin health and function.

고령사회 대응 연구개발 지원방안 연구 (A Study on R&D program for the aging society)

  • The purpose of this study is to explore the research and development(R&D) programs for effectively response of population aging. We analyzed of aging-related R&D support status by National Science and Technology Information Services(NTIS) and reviewed of foreign aging-related R&D programs. It was found that the overall lack of support systems, the R&D support status for Korea's aging society is very low performance and the difference appears in the annual support status, etc. It is very important to enhance the effectiveness of R&D programs in order to effectively prepare for aging society. I suggested support measures of the R&D for aging society by survey research. The result of this study will help in planning of the R&D for aging society and contribute to increasing of welfare for the aged.

체중조절 운동을 하고 있는 비만 성인의 식사행동과 영양 섭취 상태 및 건강상태에 관한 연구 (A Study on the Eating Behavior, Nutritional Status and Health Condition of Obese Adult Attending a Weight Control Exercise)

  • The purpose of this study was to investigate eating behavior, nutritional status and health condition of obese adult attending a weight control exercise. The subjects of this study consisted of 81 adult in some areas of Daegu. Their weight, height, triceps, midarm circumference were measured and their dietary intake and eating behavior were obtained by using questionnaires. Anthropometry was conducted to determine obesity. Average age of men and women were 38.1 and 35.2 years old respectively. Their intake of energy and protein were 69.5% and 97.4% of RDA respectively. Energy intake of normal and obese group was 74.4% and 64.9% of RDA respectively. Protein intake of normal and obese group was 99.7% and 95.3% of RDA respectively There was a negative correlation between PIBW and energy intake, PIBW and carbohydrate intake, BMI and energy intake, and BMI and carbohydrate intake. A relative magnitude of factors affecting weight decrease rate was analyzed by Stepwise multiple regression analysis. Overall results about relative influence of independent variables to dependent variable(weight decrease rate) indicated that the BMI(p<0.01) was the most significantly correlated with weight decrease rate in all subjects. The results of this study suggest that the extensive nutrition education in weight control program should be emphasized to prevent obesity early.

Women's Employment in Industries and Risk of Preeclampsia and Gestational Diabetes: A National Population Study of Republic of Korea

  • Background: Some working conditions may pose a higher physical or psychological demand to pregnant women leading to increased risks of pregnancy complications. Objectives: We assessed the association of woman's employment status and the industrial classification with obstetric complications. Methods: We conducted a national population study using the National Health Information Service database of Republic of Korea. Our analysis encompassed 1,316,310 women who experienced first-order live births in 2010-2019. We collected data on the employment status and the industrial classification of women, as well as their diagnoses of preeclampsia (PE) and gestational diabetes mellitus (GDM) classified as A1 (well controlled by diet) or A2 (requiring medication). We calculated odds ratios (aORs) of complications per employment, and each industrial classification was adjusted for individual risk factors. Results: Most (64.7%) were in employment during pregnancy. Manufacturing (16.4%) and the health and social (16.2%) work represented the most prevalent industries. The health and social work exhibited a higher risk of PE (aOR = 1.11, 95% confidence interval [CI]: 1.03-1.21), while the manufacturing industry demonstrated a higher risk of class A2 GDM (1.20, 95% CI: 1.03-1.41) than financial intermediation. When analyzing both classes of GDM, women who worked in public administration and defense/social security showed higher risk of class A1 GDM (1.04, 95% CI: 1.01, 1.07). When comparing high-risk industries with nonemployment, the health and social work showed a comparable risk of PE (1.02, 95% CI: 0.97, 1.07). Conclusion: Employment was associated with overall lower risks of obstetric complications. Health and social service work can counteract the healthy worker effect in relation to PE. This highlights the importance of further elucidating specific occupational risk factors within the high-risk industries.

Association between periodontal flap surgery for periodontitis and vasculogenic erectile dysfunction in Koreans

  • Purpose: The National Health Insurance Service-National Sample Cohort and medical checkup data from 2002 to 2013 were used to evaluate the association between periodontal surgery for the treatment of periodontitis (PSTP) and vasculogenic erectile dysfunction (VED). Methods: Bivariate and multivariate logistic regression analyses were applied to a longitudinal retrospective database to assess the association between PSTP and VED while adjusting for the potential confounding effects of sociodemographic factors (age, household income, insurance status, health status, residence area, and smoking status) and comorbidities (diabetes mellitus, angina pectoris, cerebral infarction, and myocardial infarction). Results: Among the 7,148 PSTP within the 268,296 recruited subjects, the overall prevalence of VED in PSTP was 1.43% (n=102). The bivariate analysis showed that VED was significantly related to PSTP (odds ratio [OR], 1.99; 95% confidence interval [CI], 1.38-2.06; P<0.001), and this was confirmed in the multivariate analysis after adjusting for sociodemographic factors and comorbidities (OR, 1.29; 95% CI, 1.06-1.58; P=0.002). Conclusions: Subjects with a history of periodontal flap surgery had a significantly higher risk of VED, after adjusting for potential confounding factors. Further studies are required to identify the key mechanisms underlying the association between severe periodontal disease and VED.

Variation in Meal-skipping Rates of Korean Adolescents According to Socio-economic Status: Results of the Korea Youth Risk Behavior Web-based Survey

  • Objectives: To identify and evaluate the trend of meal-skipping rates among Korean adolescents with their contributing causes and the influence of household income level on meal skipping. Methods: Using 2008, 2010, and 2012 data from the Korea Youth Risk Behavior Web-based Survey of 222 662 students, a cross-sectional study with subgroup analysis was performed. We calculated odds ratios for skipping each meal 5 or more times in a week by household socio-economic status using a multiple logistic regression model. The secular change in the meal-skipping rates by the students' family affluence scale was analyzed by comparing the meal-skipping students within each subgroup and odds ratios for the same event over time. Results: Through 2008 to 2012, most of the meal-skipping rates generally showed a continuous increase or were almost unchanged in both sexes, except for breakfast skipping in several subgroups. Students in low-income households not living with both parents had the highest meal-skipping rates and odds ratios for frequent meal skipping. In a time-series subgroup analysis, the overall odds ratios for the same event increased during 2008 to 2012, with a slight reduction in the gap between low and higher income levels with regard to meal skipping during 2010 to 2012. Conclusions: Household socio-economic status and several other factors had a significant influence on Korean adolescent meal-skipping rates. Although the gap in eating behavior associated with household socio-economic differences is currently decreasing, further study and appropriate interventions are needed.

What Explains Socioeconomic Inequality in Health-related Quality of Life in Iran? A Blinder-Oaxaca Decomposition

  • Objectives: This study aimed to explain the health-related quality of life (HRQoL) gap between the poorest and the wealthiest quintiles in the capitals of Kermanshah and Kurdistan Provinces (Kermanshah and Sanandaj), in western Iran. Methods: This was a cross-sectional study conducted among 1772 adults. Data on socio-demographic characteristics, socioeconomic status (SES), lifestyle factors, body mass index, and HRQoL of participants were collected using a self-administered questionnaire. The slope and relative indices of inequality (SII and RII, respectively) were employed to examine socioeconomic inequality in poor HRQoL. Blinder-Oaxaca (BO) decomposition was used to quantify the contribution of explanatory variables to the gap in the prevalence of poor HRQoL between the wealthiest and the poorest groups. Results: The overall crude and age-adjusted prevalence of poor HRQoL among adults was 32.0 and 41.8%, respectively. The SII and RII indicated that poor HRQoL was mainly concentrated among individuals with lower SES. The absolute difference (%) in the prevalence of poor HRQoL between the highest and lowest SES groups was 28.4. The BO results indicated that 49.9% of the difference was explained by different distributions of age, smoking behavior, physical inactivity, chronic health conditions, and obesity between the highest and lowest SES groups, while the remaining half of the gap was explained by the response effect. Conclusions: We observed a pro-rich distribution of poor HRQoL among adults in the capitals of Kermanshah and Kurdistan Provinces. Policies and strategies aimed at preventing and reducing smoking, physical inactivity, chronic health conditions, and obesity among the poor may reduce the gap in poor HRQoL between the highest and lowest SES groups in Iran.
