단근 소구치의 근관계 형태에 따른 치근단 부위의 근관 형태 (The apical root canal shape according to the root canal system of premolars with single root)

  • Materials and methods: Sixty extracted premolars were assigned to three groups according to the root canal system (Weine's classification; type I, II and III) of 20 teeth each using radiographic examination. The root tip was cut horizontally 1 mm from the anatomical apex and the apical cross-section was visualized using microscope at x50 magnification and photographed. Minimum and maximum apical root canal diameter of each tooth was measured and classified into three types by canal morphology (round, oval and flattened shape). Statistical analysis was performed to compare the apical root canal diameter and morphology according to the root canal system. Results: In apical root canal morphology at cross-sectional view, the most common shape was round in type I, flat in type II, and oval in type III. In apical root canal diameters at cross-sectional view, there was a significant difference between the minimum and maximum diameter in all types (p<0.05). The maximum diameter was 0.331 mm in type I, 0.519 mm in type II, and 0.310 mm in type III. There was a significant difference among type I, III and type II (p<0.05). Conclusion: The morphology and diameter of apical root canal was different according to the root canal system. Therefore, clinicians should consider the apical file size in view of the apical root canal shape according to the root canal system.

Dentinal defects induced by 6 different endodontic files when used for oval root canals: an in vitro comparative study

  • Objectives: To compare the formation of dentinal defects using stainless-steel hand K-files (HFs), rotary files, reciprocating files, and Self-Adjusting File (SAF), when used for oval root canals. Materials and Methods: One hundred and forty extracted human mandibular premolar with single root and oval canal were selected for this study. Oval canals were confirmed by exposing to mesio-distal and bucco-lingual radiographs. Teeth with open apices or anatomic irregularities were excluded. All selected teeth were de-coronated perpendicular to the long axis of the tooth, leaving roots segments approximately of 16 mm in length. Twenty teeth were left unprepared (control), and the remaining 120 teeth were divided into 6 groups (n = 20) and instrumented using HF (size 40/0.02), Revo-S (RS; size 40/0.06), ProTaper NEXT (PTN; size 40/0.06), WaveOne (WO; size 40/0.09), RECIPROC (RC; size 40/0.06), and the SAF (2 mm). Roots were then sectioned 3, 6, and 9 mm from the apex, and observed under stereomicroscope, for presence of dentinal defects. "No defect" was defined as root dentin that presented with no visible microcracks or fractures. "Defect" was defined by microcracks or fractures in the root dentin. Results: The control, HF, and SAF did not exhibit any dentinal defects. In roots instrumented by RS, PTN, WO, and RC files exhibited microcracks (incomplete or complete) in 40%, 30%, 55%, and 50%, respectively. Conclusions: The motor-driven root canal instrumentation with rotary and reciprocating files may create microcracks in radicular dentine, whereas the stainless-steel hand file instrumentation, and the SAF produce minimal or less cracks.

A Case of Engorged Female Hard Tick in the External Auditory Canal of an Infant

  • An oval-shaped mass with a smooth surface was found in the ear canal of a 22-month-old male infant. Although the mass appeared to be almost entirely blocking the ear canal, it was successfully removed under a surgical microscope without general anesthesia at the outpatient department. Under an optical microscope with hematoxylin and eosin staining, the specimen was observed to have a cuticle with a serrated surface and a pore canal, as well as parts of the capitulum, salivary glands, muscles, midgut, and the legs. The specimen was identified as a hard tick of the family Ixodidae, based on gross and histological findings. This paper is the first report in Korea on the diagnosis and treatment of a tick bite in the ear canal.

Effect of ultrasonic cleaning on the bond strength of fiber posts in oval canals filled with a premixed bioceramic root canal sealer

  • Objective: This study aimed to evaluate the effect of ultrasonic cleaning of the intracanal post space on the bond strength of fiber posts in oval canals filled with a premixed bioceramic (Bio-C Sealer [BIOC]) root canal sealer. Materials and Methods: Fifty premolars were endodontically prepared and divided into 5 groups (n = 10), based on the type of root canal filling material used and the post space cleaning protocol. A1: gutta-percha + AH Plus (AHP) and post space preparation with ultrasonic cleaning, A2: gutta-percha + BIOC and post space preparation with ultrasonic cleaning, B1: gutta-percha + AHP and post space preparation, B2: gutta-percha + BIOC and post space preparation, C: control group. Fiber posts were cemented with a self-adhesive luting material, and 1 mm thick slices were sectioned from the middle and cervical third to evaluate the remaining filling material microscopically. The samples were subjected to a push-out test to analyze the bond strength of the fiber post, and the results were analyzed with the Shapiro-Wilk, Bonferroni, Kruskal-Wallis, and Mann-Whitney tests (p < 0.05). Failure modes were evaluated using optical microscopy. Results: The results showed that the fiber posts cemented in canals sealed with BIOC had lower bond strength than those sealed with AHP. The ultrasonic cleaning of the post space improved the bond strength of fiber posts in canals sealed with AHP, but not with BIOC. Conclusions: BIOC decreased the bond strength of fiber posts in oval canals, regardless of ultrasonic cleaning.

하악 제1 대구치 근단부의 형태학적 연구 (MORPHOLOGY OF THE APICAL ROOT CANAL SYSTEM IN KOREAN MANDIBULAR FIRST MOLAR)

  • 본 연구는 한국인의 하악 제1 대구치 60개를 대상으로 생리적 근단공의 크기와 형태, 근심 치근의 치근단 3mm에서 isthmus의 발생 빈도와 형태, 그리고 원심 치근의 치근단 3mm의 근관 형태를 입체현미경하에서 60배로 조사한 결과 다음과 같은 결론을 얻었다. 1.근심 치근은 두 개의 근단공을 가지는 비율이 높았으며(51.67%), 원심 치근은 하나의 근단공을 가지는 비율이 높았다(71.66%). 2. 부근단공의 빈도는 근심 치근의 1개의 근단공을 가지는 경우가 가장 높았다(26.07%) 3. 생리적 근단공의 크기는 근심 치근에서 하나의 근단공을 가지는 경우 0.329mm, 근심 협측 근단공은 0.266mm, 근심 설측 근단공은 0.246mm였으며, 원심 치근에서 하나의 근단공을 가지는 경우 0.375mm, 원심 협측 근단공은 0.291mm, 원심 설측 근단공은 0.237mm로 나타났다. 4. 근단공의 형태는 oval 형태 (69.93%)가 가장 많았다. 5.근심 치근의 치근단 3mm에서 isthmus의 발생빈도는 55%였으며, partial isthmus는 31.66%, complete isth-mus는 23.34%로 나타났다. 6.원심 치근의 치근단 3mm의 근관 형태는 oval형태(50.64%)가 가장 많았다. 이상의 연구 결과 하악 제1대구치의 성공적인 근관 치료를 위해서는 생리적 근단공의 실제 크기와 형태 및 isthmus의 존재에 대한사전 지식이 필요할 것이다.

수동과 엔진구동형 기구를 이용한 난원형 근관의 형성에 관한 주사전자현미경적 관찰 (A SEM OBSERVATION ON THE EFFICIENCY PREPARATION OF OVAL CANALS USING HAND AND ENGINE-DRIVEN INSTRUMENTS)

  • The purpose of this study was to evaluate the efficiency of the preparation of oval canals using hand and engine-driven instruments with SEM observation. Thirty single-rooted teeth with oval canal were used in this study. The teeth were divided into 3 groups. In group A the teeth were instrumented up to a size 35 K-file using RC-prep and irrigated with 5% NaOCl between each file size. In group B. the teeth were instrumented with Profile according to the manufacture's instructions using RC-Prep and irrigated with 5% NaOCl between each file size. In group C. the teeth were instrumented with GT file according to the manufacture's instructions using RC-prep and irrigated with 5% NaOCl between each file size. Then. in all teeth. a final flush of 5ml of distilled water delivered for 30s. Canals were dried with sterile standardized paper points. After preparing the canals, the teeth were sectioned along their mesial and diatal surfaces by using low-speed diamond disc. chisel and mallet. Each root section was then dehydrated in graded concentration of alcohol (70, 80, 90, 100%). mounted on an aluminum stub. sputter-coated with gold-palladium and observed with scanning electron microscope (HITACHI S-4200) in middle and apical area. The results of this study were as follows: 1. In the middle area. group B and group C showed less smear layer than group A and it was statistically significant (p < 0.05). 2. In the middle area. group B showed greater smear layer than group C. but it was not statistically significant (p > 0.05). 3. In the apical area, group C showed less smear layer than group A. and it was statistically significant (p < 0.05). 4. In the apical area. group A showed greater smear layer than group B. but it was not statistically significant (p > 0.05). 5. In the apical area. group B showed greater smear layer than group C. but it was not statistically significant (p > 0.05). 6. In all groups. the middle area was less smear layer than the apical area. and it was statistically significant (p < 0.05).

개에서 고막과 고실의 초음파학적 평가 (Ultrasonographic Assessment of the Tympanic Membrane and Tympanic Bulla in Normal Dogs: A Preliminary Study)

  • This study was aimed for ultrasonographic assessment of the tympanic membrane and the tympanic bulla in five healthy Beagle dogs. To improve an ultrasonographic image, the ear canal was filled with warm saline, and an 11 MHz linear probe and a 6.5 MHz convex probe were used. The structures of ear component such as ear canal, ear cartilage and tympanic membrane were easily identified. Especially, tympanic membrane was presented as a reflaction surface which was resulted from the different acoustic impedence between the fluid-filled anechoic ear canal and the gas-filled hyperechoic tympanic cavity in normal dogs. In five left-side ears, the saline was infused into the external ear canal after the tympanic membrane had been ruptured experimentally. Both anechoic fluid-filled ear canal and tympanic cavity were clearly identified. In five right-side ears, the surgically fluid-filled tympanic cavity was imaged as a hypoechoic oval shaped structure. When tympanic cavity and ear canal have been contained with fluid, it was difficult to identify whether the tympanic membrane was ruptured or not. For assessment of the ear structure with ultrasonography, the 11 MHz linear probe was considered as an optimal equipment for a serial assessment of ear canal, tympanic membrane and tympanic bulla whereas the 6.5 MHz convex probe was suitable to assess the tympanic cavity. The results suggest that ultrasonography with saline infusion into the ear canal can be used to find the intactness of the tympanic membrane and to assess the fluid- filld tympanic bulla.

Surgical importance of the tympanic bone: multidetector computed tomography findings

  • Objectives: To measure tympanic bone thickness (anterior-superior, anterior-inferior, and inferior wall), external ear canal length, and tympanomandibular distance that can be useful in cases that undergo tympanic bone resection. Materials and Methods: The temporal computed tomography (CT) images of 349 patients were retrospectively evaluated. The anterior-inferior, anterior-superior, and inferior wall thicknesses; tympanomandibular distance; and external auditory canal (EAC) bone canal length were measured from the narrowest part of the canal. The shapes of the EAC in the coronal and sagittal planes were also examined. Results: The numbers of female and male patients were similar, and the mean age was 49.45±13.95 years. The anterior-superior, anterior-inferior, and inferior wall thicknesses were 1.92±0.60, 2.54±0.74, and 9.16±2.20 mm, respectively. The anterior-superior and anterior-inferior wall thicknesses and canal lengths were greater on the right side (P<0.001). All measurement values were higher in males, except right tympanomandibular distance (P<0.05). A non-significant negative correlation was found between the age of the participants and the left anterior-inferior wall and tympanomandibular distance on both sides. Intra-observer agreement was high for all measurements. We observed four main shapes in the external ear canal in the coronal plane: Type 3, Type 2, Type 1, and Type 4 in order of frequency on the right, and Type 2, Type 3, Type 1, and Type 4 on the left. In the sagittal plane, we detected three shapes: oval (74.4%), triangular (16.3%), and round (9.4%). Conclusion: The anterior wall thicknesses and tympanomandibular distance should be measured on preoperative temporal bone CT to safely perform tympanic bone anterior resection, which is required in some otological procedures, and also to prevent temporomandibular joint damage.

The Clinical Application of Sound-Protection

  • Bekesy measured the sound transformation system of the middle ear 49 years ago. According to his reports, a ratio between the size of ear drum and the size of oval window is 17 : 1, and the lever function of the ossicles is physiologically 1.3 : 1. Therefore, the hearing might be aggravated to 27.5㏈ in the case of the vanishment of 3 ossicles. In 1952, Wullstein reported 5 types of tympanop-lasty and the fourth type among them was especially named for the sound-protection. The oval window is only exposed by the sound pressure and the round window is not exposed. According to the application by this idea, the post-operative hearing might be improved until 27.5㏈. Mean while, in 1942, Onchi verified through his experiment that the results of Bekesy's measurement was not completely conformed to Onchi result. Bekesy measured the sound pressure on the stapes plate of the oval window, on the other hand, Onchi measured the sound pressure on the surface of the perilymph of the oval window after removing the stapes plate(Fig. 1).(Figure omitted) The difference of their experiment is recognized that the impedance of the stapes plate exists or not (Fig. 1). Both Audiogrums are compared as Fig. 2. The result of IV type of tympanoplasty is success ful in 54% as the Table 1. (Table Omitted) The reason of unsatisfactory is caused by the thick and unmovable window-membrane and by the closing of air passage to the round window. The closing of the air passage to the round window is occurred by the adhesion between the grafting membrane and the surface of promontorium. In order to preserve this adhesion, I produce to transplantate the mucous membrane of the lip to the bone surface of tympanic cavity after removing the granulation tissue of the tympanic cavity and to form a membranous canal for the sake of air passage (Fig. 3). (Figure Omitted) The post-operative hearing by this method is shown as Fig. 4, 5. In other words, the post-operative sound pressure entered into the cochlea directly, by way of the oval window only, not by way of the round window, as a theorie of the sound protection.
