The purpose of this study was to determine whether extension exercise is effective in reducing low back pain. Nine patients with low back pain were randomly assigened to one of two groups-one was an experimental group in which the patient were treated with extension exercise(McKenzie method) and conservative therapy. The other was a control group in which the patients were treated with conservative therapy only. Treatment was performed for a period of ten days during Which we examed each patient three times: the first exam was done at the begining of treatment, the second after fives days, and the third after ten days. We used 'The Oswestry low back pain disability questionnaire'(r=.99) as the examination tool. The results were as follows: 1. Both experimental and control groups showed nonsignificant differences before and after treatment, and the between group difference was also non-significant(P>.05). 2. There was no effective reduction of low back pain by MecKenzie's extension exercise(p>.05). We admit that our study protocol had several shortcomings. If they had been accounted for, the result might have been different. Further study should be done with a better experimental design, a larger sample, and a longer experimental duration.
This study was designed to determine the efficacy of ultrasound and laser therapy for sub-acute lower back pain. Twenty-seven patients with sub-acute low back pain were recruited, who were randomly assigned to three groups: Ultrasound group (actual ultrasound, 1.1 MHz, $1.0W/cm^2$, duty cycle 100%, 10 min/session, n=9), laser group (actual laser, 904 nm, 155 ns, 13.5 W, 12 mW, 90 sec/point, n=9), and control group (placebo ultrasound or placebo laser, n=9). All of treatments including placebo procedures were applied to patients over a period of 2 weeks, five times a week. Visual Analogue Scale (VAS), Modified Schober's Test (MST), and Modified Oswestry Disability Questionnaire (MODQ) were used by the clinical and functional evaluations before and after intervention. At post-hoc, significant differences were observed in all groups with respect to VAS, MST (p<.05), except MODQ. VAS and MST score were more significantly improved in the ultrasound group than the laser and control group (p<.0167). However, no significant difference was present between the laser group and the control group. Therefore, this study revealed that ultrasound therapy was effective in pain relief and improvement of lumbar mobility in patients with sub-acute lower back pain. However, laser therapy did not show the effects for sub-acute lower back pain.

Purpose: The aim of this study was to determine the effectiveness of breathing exercises and lumbar segmental exercises on the segmental stability of patients with chronic back pain. Methods: Fifty-nine patients, who suffered from chronic low back pain, were enrolled in this examination. They were divided randomly into three groups: experiment group 1 underwent breathing and segmental stabilization exercises (n=20), experiment group 2 experienced segmental stabilization exercises (n=20), and the control group was given the modality treatment (n=19). The measurements were assessed through an Oswestry disability questionnaire (ODQ), as well as a lumbar segmental instability test (LSIT). Results: The ODQ results for experimental groups 1 and 2 were similar (p<0.05), both before and after six weeks of exercise, but different among the three groups (p>0.05). The differences in segmental instability of each of the three groups were similar (p<0.05), and also similar among the three groups (p<0.05). Conclusion: These findings suggest that lumbar segmental stabilization exercises are efficient in increasing the segmental stability and alleviating pain in patients with chronic back pain. Additional studies on this subject will be needed to improve the clinical applications in the future.
Objective: The purpose of this study is to investigate the effect of stabilization exercise on whole-body vibration on pain, dysfunction, psychosocial factors, balance ability, and abdominal contraction with patients with low back pain. Design: A randomized controlled trial Methods: A total of 34 patients with low back pain were assigned randomly to experimental group (n=17) and control group (n=17). Both groups underwent a neuromuscular stabilization exercise program. In addition, the experimental group implemented the neuromuscular stabilization exercise program using whole-body vibration. All interventions were applied 60 min per session, 3 times per week for total 4 weeks. Numeric Rating Scale (NRS), Korean version of Oswestry Disability Index (K-ODI), Fear-Avoidance Beliefs Questionnaire (FABQ), balance ability, muscle thickness and contraction ratio were compared to evaluate the effect on intervention. Results: Both groups showed significant differences in NRS, balance ability, and muscle thickness in contraction, contraction ratio before and after intervention (p<0.05). In addition, the experimental group showed significant difference in the amount of change in NRS, balance ability and muscle thickness in contraction, contraction ratio values than the control group (p<0.05). Conclusions: Neuromuscular stabilization exercise program combined with whole-body vibration stimulation has been proven to be an effective and clinically useful method to decrease pain, dysfunction, increase balance ablilty, and transverse abdominis muscle thickness in contraction and contraction ratio for patients with low back pain.

Background: This study investigated the effects of pelvic floor stabilization exercise of pain, disfunction, psychosocial, electromyography (EMG) activity on the lower back pain with postpartum. Methods: The study included 20 postpartum women who were randomly assigned to a sling exercise group (SEG, n=10) or a general physical therapy group (GPTG, n=10). Outcomes were assessed using to lower back pain with postpartum the quadruple visual analogue scale (QVAS), the Korean version of the Oswestry disability index (KODI), the fear avoidance belief questionnaire (FABQ), the inventory of functional status after childbirth (IFSAC), the edinburgh postnatal depression scale-Korean (EPDS-K), and trunk muscle activity before and after a 4-week exercise intervention. Statistical analysis were performed using a mean, standard deviation, crosstab test, paired t-test, independent t-test. Kolmogorove-Smirnov test was used for test of normality. Results: Compared to the GPTG, the SEG showed significant improvement in the QVAS, KODI, FABQ, IFSAC, and EPDK-K scores (p<.05) after 4 weeks. Conclusion: Postpartum pelvic strengthening exercise proved to have a positive effect.
This study aimed to investigate the effect of the abdominal drawing-in maneuver (ADIM) and abdominal expansion maneuver (AEM) on trunk stabilization, as well as trunk muscle activities and differences in quadruple visual analogue scale, Korean Oswestry Disability Index, and Fear Avoidance Beliefs Questionnaire scores, in patients with chronic low back pain and lumbar spine instability. To increase intra-abdominal pressure during the trunk stabilization exercise, the technique of pushing the abdomen out using diaphragmatic abdominal breathing suggested by Pavel Koral was used, which we termed the AEM. Fifty patients who tested positive on more than three of the five lumbar spine instability tests were separated from 138 patients with chronic low back pain of these patients, 16 were placed in the control group (trunk stabilization exercise), 17 were placed in the ADIM group (trunk stabilization exercise with ADIM), and 17 were placed in the AEM group (trunk stabilization exercise with AEM). Each group participated in the study for 30 minutes three times weekly for 4 weeks. Surface electromyography was used to measure the trunk muscle activities during the kneeling forward and supine bridging positions, and one-way repeated analysis of variance was used to determine the statistical significance of the trunk muscle activities in the rectus abdominis, internal oblique (IO), erector spinae, and multifidus (MF) muscles. The ADIM and AEM groups showed relatively larger improvements in psychosocial and functional disability level than control group. There were significant changes among the three groups, those from the measured values of the AEM group was significantly higher than the other two groups in changes in IO and MF trunk muscle activities (p<.05). This finding demonstrates that trunk stabilization exercises with AEM is more effective than ADIM for increasing trunk deep muscle activity of chronic low back pain patients with lumbar spine instability.

Objectives : The main purpose of this study is to investigate the efficacy of Kyejibokryong-whan(Guizhifuling-wan) and examine the blood stasis for patients with motor vehicle accident. Methods : Of the patients who have visited Dong-Shin university oriental hospital for general pan due to motor vehicle accident from June 2010 to November 2010, we enrolled 29 patients and performed blood stasis diagnosis. Patients were divided into groups according to blood stasis findings. Patients who had more than twenty blood stasis diagnosis point were classified as blood stasis group. Patients who had under twenty blood stasis diagnosis point were classified as non blood stasis group. We administered Kyejibokryong-whan(Guizhifuling-wan) to all group. The effect was assessed by VAS(visual analogue scale), SF-MPQ(short form-McGill pain questionnaire) ODI(Oswestry disability index) & NDI(neck disability index). Results : The pain & function index decreased in both groups after treatment. Conclusions : The difference between blood stasis group and non blood stasis group were not significant at after treatment stage. This data suggested that the method of classifying blood stasis is useful and the Kyejibokryong-whan(Guizhifuling-wan) can be effective means that decrease pain caused by motor vehicle accident.
Objectives: This study was performed to report the case of oriental medical treatment for the left lower limb monoplegia after herniated intervertebral disc(HIVD) operation at lumbar(L)-spine. Methods: A 38-year-old man who underwent lumbar HIVD operation at a hospital admitted with motor weakness of left lower limb, a little decreased left leg radiation pain and low back pain. We treated him by acupuncture, herbal medicine, bee venom injection moxibustion, cupping treatment physical therapy and measured with visual analogue scale(VAS), Oswestry disability index(ODI), Roland-Morris disability questionnaire(RMDO) and manual muscle test(MMT) from 8th February to 4th May 2011. Results: After treatment most symptoms decreased, VAS score changed from 10 to 3, ODI changed 71% to 37% RMDQ changed 13 to 8 and MMT changed from 3 to 5. Conclusions: Our study suggested that oriental medical treatments are significantly applicable to the monoplegia and pain after lumbar HIVD operation. And further studies ire required to identify underlying mechanism of the treatment.

Objective : The purpose of this study was to investigate the prevalence of and the relevant risk factors for lumbar spondylosis (LS) among middle-aged and elderly rural Korean residents and to explore the association between radiographic LS and lower back pain (LBP) in relation to age and gender. Methods : This community-based, cross-sectional study evaluated 1512 subjects with available radiograph. The prevalence of LBP was obtained using a questionnaire and disability resulting from LBP was measured using a validated Korean version of the Oswestry disability index (ODI). In lumbar spine radiographs, vertebral levels from L1/2 to L4/5 were evaluated for the presence of osteophytes and joint-space narrowing (JSN), and Kellgren-Lawrence (KL) grading was applied. Results : Of 4261 subjects aged 40-79 years, data from 1512 subjects were included. The prevalence of radiographic LS indicated by grade ${\geq}2$ osteophytes and JSN were 53.9 and 15.8%, respectively. Seventy-three percent of subjects had KL grade ${\geq}2$ spondylosis and LBP was present in 36.5% of subjects. Although LS was more common among males, the prevalence of LBP was higher among females. Age, male gender and history of hand or knee arthritis were risk factors for LS. LS was significantly associated with LBP mostly among females over 60 years old and correlated with the ODI after adjusting for age and gender. Conclusion : Our study among rural Korean residents revealed a high prevalence of LS and LBP. The association between LS and LBP was observed mostly among females and LS was significantly correlated with the severity of back pain.
Objectives: This study is designed to compare clinical symptoms between Symptom-giving pelvic girdle relaxation and Separation of the pubic symphysis. And report the clinical effectiveness of Traditional Korean Treatment on two case. Methods: Two patients who delivery in one week ago had pain that located symphysis and around. Treatment was acupuncture, Placenta Herbal Acupuncture and herbal medicine. The progress of symptom and aggressiveness of pain and disability were measured by Visual Analogue Scale, McGill pain Questionnaire-short form, Oswestry Disability Index and Active Straight Leg Raise test. Results: After treatment most two patients' pain and disabilty were improved. At first case, width of symphysis pubis were increased in normal range. Conclusions: These cases shows that Traditional Korean therapy might be effective in decreasing symptoms on separation of symphysis pubis during delivery and Symptom-giving pelvic girdle relaxation.
