Review on Korean Herbal Medicine Complex Prescriptions and Single Herbal Medicines for Osteoporosis - Focused on Domestic Research Published Since 2013 (골다공증의 한방 복합 처방 및 단일 본초에 대한 국내 연구 고찰 - 2013년 이후 발표된 국내 연구를 중심으로)

  • The purpose of this study was to investigate the effect of single herbal medicines and Korean herbal medicine complex prescriptions for osteoporosis in Korea. We targeted studies published in domestic journals from January 1, 2013 to April 1, 2021. For domestic databases, RISS, NDSL, OASIS, DBpia, KISS, KISTI, and Korean Traditional Knowledge Portal were used. Words such as "osteoporosis", "herbal medicine", "herbal combination prescription" and "herbal" were combined and searched. A total of 32 studies were selected. Among them, 8 studies were conducted exclusively for animal studies, 18 studies were conducted on cell experiments, and 6 studies were conducted on both animal and cell experiments. There were 18 studies using a single herb and 14 studies using a combination of Herbal medicine. As a result of the investigation of a single herb, it was found that 補陽藥 was most widely used, and it was found that it was mainly used in the herbal combination prescription, mainly for 補陽藥, 補氣藥, and 補陰藥. Studies in animal experiments revealed significant results mainly in bone mineral density, serological indicators, and morphological indicators of bone tissue. In the study of cell experiments, the effects were found in factors related to the differentiation of osteoclasts and inhibitors of inflammation. Through the results of this study, we are able to confirm the effective single herbal medicines and herbal medicine complex prescriptions for the treatment of osteoporosis, and it is expected that they will contribute to clinical treatment.

Factors related to Osteopenia in Community People (일 지역주민의 골감소증 영향요인)

  • Purpose: The purpose of this study was to investigate bone mineral density (BMD) and risk factors related to osteopenia in community people. Methods: The subjects consisted of 145 community people. The variables of osteoporosis knowledge and preventive health promoting behavior were measured using questionnaires. BMD and BMI (body mass index) were measured. Dietary nutrients intake related to osteoporosis was measured using 24hours recall. Data analysis was performed using the SPSS 18.0 program, using mean, standard deviation, ${\chi}^2-test$, t-test, and logistic regression. Results: Eighty six people (59.3%) had an osteopenia. According to normal group and osteopenia group, there was significant difference in gender (${\chi}^2=6.72$, p=.010) and smoking (${\chi}^2=5.18$, p=.023). Predictors of osteopenia group were male (OR=2.65, CI=1.03-8.98), BMI(OR=0.86, CI=0.72-0.96). Conclusion: Health care providers must consider factors that affect osteopenia such as gender, BMI. These results suggest the need to instruct nursing activities and interventions which are beneficial for prevention of osteoporosis.

  • Osteoporosis, a disease characterized by low bone mass and microarchitectural deterioration of bone tissue leading to enhanced bone fragility and fracture risk, is a major public health problem. The diagnostic methods for osteoporosis include simple radiography, bone scan, DXA (Dual energy X-ray Absortiometry) and biochemical markers of bone turnover. Optimal treatment and prevention of osteoporosis require modification of risk factors, particularly smoking cessation, adequate physical activity, and attention to diet, in addition to pharmacologic intervention. The estrogens and raloxifene both prevent bone loss in postmenopausal women, and the estrogens probably also decrease the risk of first fracture. There is good evidence that raloxifene prevents further fractures in postmenopausal women who already have had fractures and some evidence that estrogen does as well. Bisphosphonate prevents bone loss and reduces fractures in healthy and osteoporotic postmenopausal women and in osteoporotic men as well. Risedronate is more potent and has fewer side effects than alendronate and reduces the incidence of fractures in osteoporotic women. Calcitonin increases bone mineral density in early postmenopausal women and men with idiopathic osteoporosis, and also reduces the risk of new fractures in osteoporotic women. All of the agents discussed above prevent bone resorption, whereas teriparatide and strontium increase bone formation and are effective in the treatment of osteoporotic women and men. New avenues for targeting osteoporosis will emerge as our knowledge of the regulatory mechanisms of bone remodeling increases, although issues of tissue specificity may remain to be addressed.

  • Objective : The purpose of this study was to inspect the effect of Oriental medicacl treatment with following up NTx and absorbing rate of bone in degenerative osteoarthritis. Methods : To obtain this result, we examinated 52 case of parents who visit Dept. of Acupuncture and Moxibustion Oriental Medical Hospital. Taejon University. We observe their age, sex, symptoms and change of NTx and ALP. Results : As a result of this examination, we detected the fact that it's difficult to decending NTx, but we are convinced that Oriental medicine has a good effect clinically. Conclusion : We take very useful result from this study, and want to be put this knowledge to practical use on treating Osteoporosis think the study like this must be go on continuously.

Development of Health Information Program for the Middle Aged Women (중년여성을 위한 건강정보 프로그램 개발과 건강교육 활용에 관한 연구)

  • The purpose of the study was to develop a Web site for the middle-aged women and test the usability of the site. Users' need for information were identified via online survey of 189 middle-aged women. It was found that they had various health problems but very few did any health prevention or health promotion activities. A web site for the middle-aged women was developed based on the result of needs assessment. The site includes content on exercise, diet including weight control, osteoporosis, climacteric, aging, breast and cervical cancer. Besides this health information, FAQ on women's health, my health knowledge test, statistics, link to related institutes and web sites, newspaper articles/mass media clips, journal articles, conferences, papers and other educational information were added. The site was developed as a subsystem of the Healthguide(http://healthguide.kihasa.re.kr). As a way of utilizing the homepage, twenty two women aged 40-50 were recruited at a community-based gathering and health promotion education program was conducted. To test the effectiveness of the education, knowledge, attitude and behavior changes of before and after education were compared. There were significant differences of knowledge and behavior in breast cancer, cervical cancer, osteoporosis, menopause and sexual life. It is expected that Web-based women's health information will contribute to women's health promotion and provide an effective learning media for health education.

A Study on the Degree of Knowledge of Menopause and Management of Menopausal Women (폐경기 여성의 폐경 지식 정도와 폐경 관리 실태 조사)

  • The purpose of study was to identify the degree of menopausal of age knowledge and management of menopausal women. The data were collected from April 1st to May 31th in 1998 and 368 total. Data were analyzed by SPSS/PC program. The results were as follows. 1. The average age was 45-50 years. General health state was good 71% mosty they were house keeper. 87.5% had religion. The married woman was 86.1%. Menopause treatment experience was 88.1%. The educational state was below middle school, 72.3%. 2. The high score of knowledge of menopause was the item of whether they conceive or not after menopause, irregularity of menopause, menopausal symptoms, of the possibility osteoporosis. 3. The score of knowledge was the highest at the group 45-50, the high education, and the high number of family members. 4. The score of management was the highest in self control category. No drinking & no smoking items were the highest. Sexual management and management by professional person were low score. 5. The higher score of the knowledge of menopause, the higher score of the management.

A Study of Educational Need, Attitude and Knowledge toward Menopause in Middle-aged Women (중년기 여성의 폐경에 대한 교육요구도, 지식과 태도에 대한 연구)

  • Purpose: The purpose of this study was to investigate educational needs, knowledge and attitude towards menopause in middle-aged women Method: The research design was a descriptive study. The subjects of this study were 231 middle-aged women. Data was collected from a structured questionnaire from October 2 to November 30, 2007. Result: The score of educational needs towards menopause was relatively high at 3.83/5. The score of educational needs related to 'Osteoporosis'(3.99 score), 'Cardiovascular disease'(3.91 score), and 'Hormone replacement therapy'(3.87 score) was relatively high. The score of know ledge towards menopause was relatively low at 17.28/30. The score of attitude towards menopause was neutral at 2.3/4. The relationship between knowledge and attitude towards menopause revealed a negative significant correlation(r=-.194, p=.003). The relationship between knowledge and educational needs towards menopause revealed a positive significant correlation(r=0.275, p<.0001). The degree of educational needs of women before menopause is higher than in menopausal women. Conclusion: There is a need to develop a continuing educational program according to age and menopause condition. Specialists(Nurses) must give information about menopause for a positive attitude toward menopause.

A critic review on the 'medical knowledge of menopausal syndrome' ('폐경기 증후군'에 관한 의학지식의 비판적 고찰)

  • The purpose of this study is to examine the medicalizing process of menopanse with literature review, and then is to explore the knowledge of 'menopausal syndrome' critically, focused on four critics on the biomedical model which were suggested by Mishler. Although menopause is a natural biological phenomenon, the view of many medical researchers and practitioners is that menopause is a disease. After synthetic estrogen was developel in 1938, physicians did agree on two basic assumptions : menopausal women should be managed by physicians, and medical intervention should be given. Menopouse was defined as a deficiency disease (estrogen difficiency) by Wilson in 60's and is redefined as a cause of disease(for example, osteoporosis, heart disease) at the present. But the other view of non-medical researcher is that 'menopausal syndrome' as a disease is constructed medically. It was reported that Only hot flush and sweating of physical symptoms experienced by menopausal women, were associated with menopause. Symptoms of menopausal syndrome are also related with symptons of aging. So, it cann't conclude that menopausal syndrome is resulted from menopause, and it cann't be only explained that menopausal syndrome is related causally to estrogen deficiency, and only treatment by ERT or HRT is best relevant. It cann't assume that menopausal syndrome is a common phenomenon to all menopause women, because culture affected on women's experience of menopause.

Diagnosis and Treatment of Menopause-Related Depression (폐경기 관련 우울증의 평가와 치료)

  • Recent Korean statistics show that a Korean woman can now expect to live until her mid-80s, which implies living at least one-third of her life after menopause. The menopausal transition is typically marked by intense hormonal fluctuations, accompanied by sleep disturbance, vasomotor symptoms(e.g., hot flashes, night sweats), increased risk for osteoporosis, cardiovascular disease, and developing depression as well as mood disturbances. These symptoms can affect a woman's quality of life negatively. Therefore, a comprehensive understanding of the accurate detection and appropriate treatment of various menopause-related symptoms including depression in the menopausal transitions and postmenopause is mandatory. This review primarily focused on the current knowledge about the treatment of menopause-related depression.

The Role of Jak/STAT Pathways in Osteoclast Differentiation

  • Osteoclasts are bone-resorbing cells of monocyte/macrophage origin and are culprits of bone destruction associated with osteoporosis, rheumatoid arthritis, and cancer bone metastasis. Recent advances in osteoclast biology revealed central roles of various cytokines in regulating osteoclastogenesis both in vitro and in vivo. However, exact underlying mechanisms including signaling pathways downstream of receptor ligation are still under pursuit. In the present review, the role of Jak/STAT proteins and their regulators will be discussed in connection with osteoclastogenesis, since growing evidence indicates that a number of cytokines and growth factors utilizing Jak/STAT signaling pathways affect osteoclastogenesis. A better understanding on the role of Jak/STAT pathways in osteoclast differentiation will not only strengthen our knowledge on osteoclast biology but also provide invaluable insights into the development of anti-resorptive strategies for treating bone-lytic diseases.
