A Performance Enhancement of Osteoporosis Classification in CT images (CT 영상에서 골다공증 판별 방법의 성능 향상)

  • Classification methods based on dual energy X-ray absorptiometry, ultrasonic waves, and quantitative computed tomography have been proposed. Also, a classification method based on machine learning with bone mineral density and structural indicators extracted from the CT images has been proposed. We propose a method which enhances the performance of existing classification method based on bone mineral density and structural indicators by extending structural indicators and using principal component analysis. Experimental result shows that the proposed method in this paper improves the correctness of osteoporosis classification 2.8% with extended structural indicators only and 4.8% with both extended structural indicators and principal component analysis. In addition, this paper proposes a method of automatic phantom analysis needed to convert the CT values to BMD values. While existing method requires manual operation to mark the bone region within the phantom, the proposed method detects the bone region automatically by detecting circles in the CT image. The proposed method and the existing method gave the same conversion formula for converting CT value to bone mineral density.

An Implementation of Classification Method of Osteoporosis using CT images (CT 영상을 이용한 골다공증 분류 방법의 구현)

  • In this paper, we propose a method of measuring bone mineral density in a peripheral-type clinical X-ray CT using a phantom, and we propose a method of classifying osteoporosis using bone mineral density and bone structure parameters together. It segments the trabecular bone region and cortical bone region for the six sections of the phantom and calculates the average HU value of the segmented regions. By using these values, it derives an expression converting HU value to bone mineral density. It segments trabecular bone of 1 cm region in the end part of distal radius and extracts the bone mineral density and structural parameters for the trabecular bone region. We extracted bone mineral density and structural parameters for the 18 subjects each of normal and osteoporotic group. We carried out classification experiments using three classification methods; SAD, SVM, ANN. The sensitivity, specificity, accuracy, positive predictive value, negative predictive value, likelihood ratio of the classification was improved in the order of ANN, SVM and SAD. Also, The sensitivity, specificity, accuracy, positive predictive value, negative predictive value, likelihood ratio of the classification was improved when we use the bone mineral density and structural parameters together.

Relationship of nutrient intakes and bone mineral density of elderly women in Daegu, Korea

  • The purpose of this study was to examine the relationship between nutrient intake and bone mineral density (BMD) of elderly women in Daegu, Korea. In this study, the bone mineral densities of 101 elderly women in Daegu were measured, and their nutrient intake, dietary habits, and maternal factors were determined through a survey. The subjects were divided into the normal group, the osteopenia group, and osteoporosis group to find out if there is a correlation between bone mineral density and maternal factors, dietary habits, and nutrient intake through their T-scores, analyzed according to the standards of WHO. Classification by T-scores of the participants were the normal group 25.7%, the osteopenia group 39.6%, the osteoporosis group 34.7%. Menopause age of the osteoporosis group was lower, postmenopausal period was longer, and last delivery age was significantly higher than the normal group (p<0.05). Osteoporosis group had a lower percentage of 'everyday' fruit and vegetable intakes and higher percentage of 'never' dairy intake than of the normal group. Vitamin A, vitamin $B_1$, and vitamin $B_2$ intakes of participants in the osteoporosis group were lower than those in the normal group (p<0.05). In conclusion, nutritional education is necessary to encourage high intake of milk and vegetables and fruits along with calcium, vitamin A, $B_1$, and $B_1$ intakes and low intake of sodium for the prevention of elderly women's osteoporosis.

Usefulness of panoramic radiography in the diagnosis of osteoporosis (골다공증의 진단시 파노라마 방사선사진 유용성에 대한 연구)

  • Purpose: To assess the possibility of using panoramic indices as an indicator of jaw osteoporosis. Methods: Mandibular cortical width (MCW), degree of mandibular alveolar bone resorption (ABR) and morphology of mandibular inferior cortex (MIC) on panoramic radiograph were used as panoramic indices. These panoramic indices were compared with bone mineral density (BMD) of lumbar (L1-L4) and femoral neck measured by dual energy X-ray absorptiometry. We also compared MCW and ABR of young men with those of postmenopausal women. Results : There was a significant correlation between ABR and BMD of lumbar and femoral neck. And also significant correlation between MIC and BMD of lumbar and femoral neck. ANOVA test of BMD of lumbar and femoral neck showed significant differences according to morphologic classification of inferior cortex. There was significant difference in MCW and ABR between young men and postmenopausal women. Conclusion: Our results suggested that ABR and MIC on panoramic radiograph could be reliable in screening of osteoporosis.

Treatment Patterns of Osteoporosis and Factors Affecting the Prescribing of Bone-forming Agents: From a National Health Insurance Claims Database (건강보험 청구자료를 이용한 골다공증 치료제의 처방 양상과 골형성촉진제 처방에 미치는 영향요인)

  • Objective: To analyze osteoporosis treatment patterns and teriparatide prescription-associated factors in Korea by using a national health insurance claims database. Methods: We utilized the Health Insurance Review & Assessment Service National Patients Sample claims database to identify patients (aged ≥50 years) with at least one osteoporosis claim (International Classification of Disease 10th revision code: M80, M81, M82) and at least one prescription for osteoporosis medication (antiresorptive agents: bisphosphonates, selective estrogen receptor modulators, denosumab, and calcitonin; bone-forming agent: teriparatide) in 2018. Demographic characteristics and healthcare utilization patterns were analyzed. Factors associated with teriparatide prescriptions were assessed using a multivariate logistic regression model. Results: Records showed that 44,815 patients were prescribed osteoporosis medications in 2018; the percentage of patients prescribed each treatment was as follows: 86.6% bisphosphonates, 13.9% selective estrogen receptor modulators, 3.1% calcitonin, 2.1% denosumab, and 0.7% teriparatide. A greater proportion of patients prescribed teriparatide were ≥75 years (53.4% vs. 33.8%) and had fractures (63.9% vs. 12.8%) compared to the same for antiresorptives (p<0.001). Patients prescribed teriparatide had higher Charlson comorbidity index values (1.2±1.3 vs. 0.9±1.2) and were more frequently hospitalized (0.8±1.3 vs. 0.1±0.5) than those prescribed antiresorptives (p<0.001). Elderly patients (≥75 years old; adjusted OR=1.66; 95% CI 1.16-2.38) and those with fractures (adjusted OR=6.23; 95% CI 4.76-8.14) were more likely to be prescribed teriparatide than antiresorptives. Conclusion: Patients prescribed teriparatide were older and more likely to have severe osteoporosis than those prescribed antiresorptives.

The Relationship between Bone Mineral Density and Physical, Obstetric Characteristics in Middle-aged Women (중년 여성의 골밀도와 신체적, 산과적 특성과의 관계 연구)

  • Purpose: This study were to investigate BMD of middle-aged women and to examine the relationships between BMD and Physical, Obstetric characteristics Method: The data was collected from 119 healthy women who were 40-60 years old. they were examined for BMD at 4 regions(forearm, lumbar, femur, whole body), %fat by DEXA and investigated physical, obstetric characteristics using scale, questionnaire from January to March, 2001. Result: 1) According to bone diagnostic results by WHO classification, 95.8% of forearm and whole body BMD were normal but 21.8-48.7% of lumbar and femur BMD(neck, trochanter, ward's triangle) were diagnosed osteoporosis or osteopnea. 2) The bones were significantly positive correlations of each other (r=.19-.69, p=.04-.00) and there were significant correlations between BMD and physical, obstetric characteristics such as age (r=-.22, p=.02), weight(r=.36~.48, p=.00), height(r=.22, p=.02), %fat(r=.19, p=.04) and age of first delivery(r=-.28, p=.00). Conclusion: Based on this study, healthy middle-aged women were also exposed to risk of osteoporosis related to aging, change of physical conditions or hormonal release. Further research to develop nursing interventions for the purpose of preventing osteoporosis by modifying risk factors is suggested.

The effect of low-dose intravenous bisphosphonate treatment on osteoporosis in children with quadriplegic cerebral palsy

  • Purpose: Quadriplegic children with cerebral palsy are more susceptible to osteoporosis because of various risk factors that interfere with bone metabolism. Pamidronate is effective for pediatric osteoporosis, but there are no guidelines for optimal dosage or duration of treatment in quadriplegic children with osteoporosis. We aimed to evaluate the efficacy of low-dose pamidronate treatment in these patients. Methods: Ten quadriplegic patients on antiepileptic drugs (6 male, 4 female patients; mean age, $10.9{\pm}5.76years$), with osteoporosis and gross motor function classification system level V, were treated with pamidronate (0.5-1.0 mg/kg/day, 2 consecutive days) every 3-4 months in a single institution. The patients received oral supplements of calcium and vitamin D before and during treatment. The lumbar spine bone mineral density (BMD) z score and biochemical markers of bone metabolism were measured regularly during treatment. Results: The main underlying disorder was perinatal hypoxic brain damage (40%, 4 of 10). The mean cumulative dose of pamidronate was $4.49{\pm}2.22mg/kg/yr$, and the mean treatment period was $10.8{\pm}3.32months$. The BMD z score of the lumbar spine showed a significant increase from $-4.22{\pm}1.24$ before treatment to $-2.61{\pm}1.69$ during treatment (P=0.008). Alkaline phosphatase decreased during treatment (P=0.037). Significant adverse drug reactions and new fractures were not reported. Conclusion: Low-dose pamidronate treatment for quadriplegic children with cerebral palsy increased lumbar BMD and reduced the incidence of fracture.

Osteoporosis Measured by Quantitative Ultrasound and Its Risk factors in Middle-aged Women, Gyeonggi-do, Korea (중년여성의 정량적 초음파(Quantitative Ultrasound)로 측정한 골다공증 정도와 위험인자)

  • Purpose: To identify Osteoporosis and the related risk factors in middle-aged women, the descriptive survey was done. Method: The subjects were measured in 465 residents who were 40-60 aged healthy women. They underwent ultrasound measurement and health examination in G city's Health Center in Gyeonggi-do, Korea from July 2000 to March 2001. The self-reported questionnaire consisted of total 17 items about risk factors related to osteoporosis. The evaluation of bone density was based on ultrasound measurements of right heel. All data were analyzed by SAS-PC Program. Result: Mean age of the subjects was 46.7. Mean T score related to bone density was -1.30, ranged from -3.52 to 3.06. In diagnosis classification according to T score, 74.8% of subjects was normal, 12.9% was osteopenia, 12.3% was osteoporosis. Among risk factors, there were significant differences by age (t=15.35. p=0.000), parity (F=12.81, p=0.000), menopause status (t=22.05, p=0.000), period after menopause (F=5.20, p=0.006). The higher frequency of delivery, postmenopausal and longer period after postmenopause of subjects had the lower the bone density. Conclusion: It would be necessary to develop and apply the community-based health promotion program for middle-aged women to prevent osteoporosis.

Assessment of the Correlation for Geometry Transition using Bone Mineral Density in Proximal Femur (골밀도를 이용한 대퇴근위부 형태 변화의 상관관계 평가)

  • Osteoporosis is increasing in Korea as it becomes an aging society with the rapid economic growth and the development of medical technology. Osteoporosis also develops due to chemo and radiation therapy of cancer which also increases owing to Westernized diet. Osteoporosis is caused by reduced bone density, has close relationship with the change of geometry of proximal femur, which is a factor of hip fracture risk. The purpose of this study was the analysis of the correlations of osteoporosis and the change of geometry of proximal femur, which was observed according to T-score variance. The 350 male and female patients are chosen from D hospital in Busan, who were classified by age, sex and T-score values (normal, osteopenia, and osteo porosis). The results show that the age and gender have significant difference in the incidence of osteoporosis; the disease classification according to T-score value has significant difference in the geometry of the proximal femur such as Cortical ratio calcar, Cortical ratio shaft, Hip/shaft Angle, Strength index, Section modulus, CSMI, and CSA, and is highly correlated with the incidence of osteoporosis. Therefore, the findings of this research is that the change of the geometry of the proximal femur could be used as an indicator in the diagnosis of osteoporosis, could enhance the accuracy of the diagnosis in the future, and could be used as a clinical predictive factors through the analysis of the correlations of T-score variance and the geometry changes of the proximal femur.

  • Sarcopenia is the degenerative loss of muscle mass and function with aging. Recently sarcopenia was recognized as a clinical disease by the International Classification of Disease, 10th revision, Clinical Modification. An imbalance between protein synthesis and degradation causes a gradual loss of muscle mass, resulting in a decline of muscle function as a progress of sarcopenia. Many mechanisms involved in the onset of sarcopenia include age-related factors as well as activity-, disease-, and nutrition-related factors. The stage of sarcopenia reflecting the severity of conditions assists clinical management of sarcopenia. It is important that systemic descriptions of the disease conditions include age, sex, and other environmental risk factors as well as levels of physical function. To develop a new therapeutic intervention needed is the detailed understanding of molecular and cellular mechanisms by which apoptosis, autophagy, atrophy, and hypertrophy occur in the muscle stem cells, myotubes, and/or neuromuscular junction. The new strategy to managing sarcopenia will be signal-modulating small molecules, natural compounds, repurposing of old drugs, and muscle-specific microRNAs.
