The Septic Knee Arthritis Caused by Klebsiella pneumoniae in a Patient with Type 2 Diabetes Mellitus and Osteoarthritis of the Knee (제2형 당뇨 및 골관절염을 동반한 환자에서 발생한 Klebsiella pneumoniae균에 의한 화농성 관절염)

  • Klebsiella pneumoniae causes that liver abscess mostly, also spread to pneumonia, meningitis, urinary tract infections. Septic arthritis caused by K. pneumoniae is a quite rare and has not been reported in Korea. Therefore, the authors report a case of the septic arthritis in the knee joint caused by K. pneumoniae in a patient with type 2 diabetes mellitus and osteoarthritis of the knee that successfully treated by early detection and arthroscopic synovectomy.

Surgical Treatments and Clinical Outcomes for Idiopathic Osteoarthritis of the Tarsometatarsal Joints (족근 중족 관절의 특발성 골관절염에 대한 수술적 치료 및 임상적 결과에 대한 분석)

  • Purpose: The purpose of the study was to identify the subtypes of idiopathic osteoarthritis of the tarsometatarsal joints based on accompanying hindfoot, midfoot, or foot deformities and their corresponding surgical options and also to evaluate the overall clinical results. Materials and Methods: The study included 59 patients (67 feet) with idiopathic tarsometatarsal joint osteoarthritis. Tarsometatarsal fusion was performed for tarsometatarsal joint and accompanied secondary change was divided into subtypes and various bony reconstruction was carried out. The patients were evaluated with the AOFAS midfoot score and FFI. The average patient age was 60.2 years with 40.6 months follow-up. Fifty-four feet (80.6%) had been treated with realignment fusion. Twenty-six feet had first and second tarsometatarsal joint fusion, and 20 feet had first tarsometatarsal fusion only. Six subtypes were identified based on associated foot deformities: 1) in-situ without deformities (18%), 2) pes planovalgus (45%), 3) rockerbottom (15%), 4) cavus foot (1%), 5) hallux valgus (12%), and 6) hallux valgus with pes planovalgus or rockerbottom (9%). Plantar-medial closing-wedge resection was used in 10 feet to correct rockerbottom. For pes planovalgus, a medial sliding calcaneal osteotomy was done. Lateral column lengthening with medial sliding calcaneal osteotomy was done for severe pes planovalgus, and triple arthrodesis was done for rigid pes planovalgus. Hallux valgus was corrected with the Lapidus procedure (85.7%). Results: AOFAS midfoot scores improved from preoperative 34.1 points to postoperative 83.9 points (p<0.05). The Foot Function Index postoperatively also showed significant improvement (p<0.05), with a high satisfaction rate (86.6%). There were 29 complications, most commonly sesamoid pain. Conclusion: Idiopathic tarsometatarsal OA feet can be classified into six categories. Pes planovalgus feet should be treated with medial sliding calcaneal osteotomy, lateral column lengthening, or triple arthrodesis in addition to tarsometatarsal joint realignment fusion. Rockerbottom and hallux valgus deformities should also be addressed.

Comparative Study on the Effects of Bee Venom Pharmacopuncture According to the Treatment Method for Knee Osteoarthritis

  • Objectives: The purpose of this study is to compare the effects of bee venom pharmacopuncture (BVP) therapy according to the methods used to treat knee osteoarthritis (OA): intra-acupoint combined with intra-articular injection, intra-acupoint injection, and intra-articular injection. Methods: A total of 69 patients were recruited by the Department of Acupuncture & Moxibustion at Dong-Eui Oriental University Hospital from February 1 to July 23, 2012. The patients were assigned to 3 groups: the first group with intra-acupoint combined with intra-articular BVP Injection (the experimental group), the 2nd group with intra-acupoint BVP injection (control group II), and the 3rd group with intra-articular BVP injection (control group II). The participants were assigned in the order in which they were recruited. Treatments were done twice a week, for a total of 9 times. The effectiveness was assessed by using the visual analouge scale (VAS) and the Korea Western Ontario and McMaster Universities Osteoarthritis Index (KWOMAC). Results: All three groups exhibited significant VAS and KWOMAC effects. Moreover, the 4 week follow-up after the final treatment showed a persistence of BVP effects. However, when the groups were compared, no statistically significant differences in VAS and KWOMAC were noted, but when improvement was considered, the results showed that intra-articular injection was more effective than intra-acupoint injection. Especially, intra-acupoint combined with intra-articular injection was the most effective among the three treatments. Conclusions: Combining intra-acupoint with intra-articular injection, depending on the patient's symptoms, may produce better results when conservatively treating knee OA.

A Study on Effects of Tai-Chi Exercise Program on Joint Flexibility for Osteoarthritis Patients (태극권이 퇴행성관절염 환자의 관절 유연성에 미치는 효과)

  • Purpose: The purpose of this study was to examine effect of Tai-Chi exercise program on joint flexibility for osteoarthritis patients. To evaluate the effects, quasi-experimental study was used for pre and post test with a nonequivalent control group. Method: Fifty-tow subjects participated in the experiment(twenty-seven for experimental group and twenty-five for control group) and they were from two different senior citizen centers in J city. The experiment period was from April 2002 through August 2002. The experiment group took part in Tai-Chi exercise program for eighteen weeks(three times a week for four weeks and five times a week for fourteen week). The exercise was performed for 50 minutes at every time (15 minutes for warm-up, 30 minutes for Tai-Chi exercise, and 5 minutes for ending). In order to pursue study purpose, the SPSS/WIN 8.0 program was chosen for the statistical analysis; ANCOVA was employed in examining test hypothesis; t-test and $x^2$-test were used to examine homogeneity between experimental and control group. Result: The study results were as follows :Score of vertebral joint flexibility increased significantly after of Tai-Chi exercise program. Score of wrist joint extension increased significantly after of Tai-Chi exercise program. Score of wrist joint flexion increased significantly after of Tai-Chi exercise program. Score of elbow joint extension increased significantly after of Tai-Chi exercise program. Score of elbow joint flexion increased significantly after of Tai-Chi exercise program. There was a slight increase of knee joint extension in the Tai-Chi exercise group compared to control group, but without statistical significance. Score of knee joint flexion increased significantly after of Tai-Chi exercise program. Score of ankle plantar flexion increased significantly after of Tai-Chi exercise program. Score of ankle dorsiflexion increased significantly after of Tai-Chi exercise program. Conclusion: As shown in the results, Tai-Chi exercise program can be effective nursing intervention for osteoarthritis patient to improve joint flexibility.

Clinical Study on Effect of Rainbow Power Therapy on Osteoarthritis in Knee Joint (Rainbow Power 요법 시술이 퇴행성 슬관절염에 미치는 임상 효능연구)

  • Objective : This study is designed to find out the effects of Rainbow Power therapy on knee joint with osteoarthritis. Methods : Patient with the osteoarthritis of knee joints are divided into Rainbow Power therapy group(20 people) and none-Rainbow Power therapy group(20 people). After treatment, we evaluated the effect per each group. Results : 1. Fifty and Sixty aged patients were the most in the Rainbow Power group(30% each) and sixty aged patients were the most in the None-Rainbow Power group(45%). 2. In the both groups, 1-3 years duration of disease was the most(40%, 45% each). 3. In the none-Rainbow Power group, patients improved (Lysholm score : $49.20{\pm}11.54{\rightarrow}63.90{\pm}9.54$, p<0.05), but, did not improved in the part of squatting and dystrophy. In the Rainbow Power group, patients improved(Lysholm score : $46.55{\pm}9.89{\rightarrow}80.90{\pm}13.78$, p<0.05), but, did not improved only in the part of dystrophy. 4. The effects of treatment by VNRS were follow : After treatment, $4.10{\pm}1.22$ in Rainbow Power Group and $4.55{\pm}1.15$ in none-Rainbow Power group. 5. The effects of treatment by nine points scale were follow : In the Rainbow Power group, the Excellent was 7 cases(35%), the Good was 9 cases(45%), the Fair was 4 cases(20%) and there was no Poor. The Excellent was 6 cases(30%), the Good was 8 cases(40%), the Fair was 6 cases(30%) and there was no Poor.

A Retrospective Chart Review of 122 Inpatients with Knee Osteoarthritis Treated with Korean Medicine: An Analysis of the Effects of Treatment

  • Background: Korean medicine treatment was assessed in patients with knee osteoarthritis (OA) according to subgroups of: sex, age, cause of knee OA, body mass index, hospitalization period, history, OA compartment, phenotype, and comorbidity. Methods: A retrospective review was performed of 122 inpatients who were admitted to the Hospital of Korean Medicine for Korean medicine treatment of knee pain, and were diagnosed with knee OA based on magnetic resonance imaging findings. Analysis of patient subgroups (sex, age, cause of knee OA, body mass index, hospitalization period, history, OA compartment, phenotype, and comorbidity) was carried out and treatments including acupuncture, cupping, pharmacopuncture, herbal medicine, chuna therapy, medicinal steaming therapy, manual therapy, and extracorporeal shock wave therapy were listed. The numeric rating scale (NRS), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), and 5-level EuroQol- 5 Dimension (EQ-5D-5L) scores were measured before and after treatment to assess the effects of treatment on pain and quality of life. Results: Seventeen males and 105 females were included in this study. Most patients were in their 60s. In the total study population, NRS, WOMAC, and EQ-5D-5L scores were improved statistically significant when comparing before and after treatment. The NRS and WOMAC scores improved statistically significant in the medial, patellofemoral, medial + patellofemoral, medial + lateral + patellofemoral compartment. Conclusion: Korean medicine treatment significantly reduced pain, stiffness, and physical dysfunction, and improved the quality of life of patients with knee OA, suggesting that it may be an effective alternative to the current conservative treatments.

A Study on Usefulness of Planar Bone Scan and Bone SPECT in Diagnosis of Temporomandibular Joint Arthritides (측두하악관절의 관절염 진단에 있어서 골스캔과 단광자방출 전산화 단층촬영의 유용성에 관한 연구)

  • Temporomandibular joint arthritides is divided into osteoarthritis, osteoarthrosis and polyarthritis. Because the signs, symptoms and radiographical features of osteoarthritis and osteoarthrosis are similar without arthralgia, diffenential diagnosis is difficult. Also non-radiographically change in early Osteoarthritis leads to misdiagnose. Planar bone scan and SPECT are useful to detect bone change early. This study was carried out in order to make diagnostic criteria of planar bone scan and SPECT. Three hundred and four temporomandibular joints were examined with clinical examination, computerized tomograph, planar bone scan, and SPECT. The obtained results were as follows. 1. If temporomandibular joint simple uptake ratio of patient in twenties is over 1.397%, it's condition may be osteoarthritis. And simple uptake ratio over in thirties-fourties may mean osteoarthritis. 2. It may mean osteoarthritis of temporomandibular joint that the number of coronal and transverse SPECT frame with hot spot is over four. 3. Destructive stage may goes on, if simple uptake ratio is over 1.370% in tweenties and over 1.104% in thirties-fourties. 4. If the number of coronal SPECT frame with hot spot is over four, temporomandibular joint may be on destructive stage in tweenties, thirties-fourties. And if the number of transverse SPECT frame with hot spot is over three, it may be on destructive stage in all ages. 5. When patient complains subjective arthralgia and palpation arthralgia, bone change may be more active than each arthralgia. 6. Osteoarthritis may progress gradually worse in 4.5 anamnesis. And then it may be stable gradually and turn to osteoarthrosis.

Clinical Study on Effect of Carthmi-Flos Herbal acupuncture therapy on Osteoarthritis in Knee Joint (홍화약침(紅花藥鍼)의 퇴행성(退行性) 슬관절염(膝關節炎)에 대한 임상적 연구(硏究))

  • Objective : This study is designed to find out the effects of Carthmi-Flos herbal acupuncture theraphy on knee joint with osteoarthritis. Methods : 32 cases of diagnostic patient with the osteoarthritis of knee joints by biochemical method, X-ray analysis and hematologic & serologic tests are divided Carthmi-Flos herbal acupuncture theraphy Group(HAG, 16 peples) and non-Carthmi-Flos herbal acupuncture theraphy Group(NHAG, 16 peoples) at Ik-San Oriental medical hospital in wonkwang university from 1th June 2000 to 31th January 2002. After treatment, we evaluated the effect per each group . Results : 1. In the both HAG and NHAG females were much more than males. Sixty aged patients were the most in the HAG(56.3%) and Seventy aged patients were the most in the NHAG(68.8%). 2. Over 5 years duration of disease in HAG was the most(37.5%). 6 months and over 5 years duration of disease in NHAG was the most(each 31.3%). 3. Two groups were not improved chronic stage(over 5 years). 4. The effect of treatment by condition of patients was follows: After treatment, Grade IV & III were decreased 75.0% in HAG and 56.2% in NHAG 5. In the distribution of treatment progression : 15 cases(93.7%) were recoverd in HAG but 14 cases(87.5%) in NHAG. 6. After comparing the results before and after treatment, we found 93.3% of patients showed recovery at anterior area and 88.9% showed at posterior. Conclusion : These results shows that Carthmi-Flos(CF) herbal acupuncture treatment has good effect on osteoarthritis in knee joint. Futhermore it is possible that CF acupuncture is one of the promising candidates for long-term therapy of osteoarthritis.

  • This paper describes the possibility of evaluating and classifying arthritic pathology using the acoustical analysis of knee joint sound. Six normal subjects and 11 patients with knee problems were enrolled. Patients were divided into the 1st patient group which required an orthopeadic surgery and the 2nd patient group of osteoarthritis. During sitting and standing periods, subjects' active knee flexion and extension were monitored. Fundamental frequency, mean amplitude of pitch, jitter and shimmer were analyzed according to the position. The results demonstrate that the values of fundamental frequency, jitter and shimmer of the 2nd patient group were larger than others and changed unstably. The values of the standing position were larger than the sitting position.

  • In this study, we have evaluated and classified arthritic pathology using the acoustical analysis of knee joint sounds. Six normal subjects and 11 patients with knee problems were enrolled. Patients were divided into the 1st patient group which required an orthopeadic surgery and the 2nd patient group of osteoarthritis. During sitting and standing periods, subjects' active knee flexion and extension were monitored. Fundamental frequency, mean amplitude of pitch, jitter and shimmer were analyzed according to the position and the joint angle. The result showed that the fundamental frequency of the 2nd patient group and standing position was higher than the others, and that the pitch of sounds changed unstably.
