Analysis of Joint Moment in the Intact Limb With Uni-Transfemoral Amputee During Level Walking (편측 대퇴절단자의 보행 시 건측 하지 관절 모멘트 분석)

  • The purpose of this study was to determine the differences in joint moment in the intact limb of uni-transfemoral amputees and to identify the implications of knee osteoarthritis. As an experimental method, three-dimensional gait analysis was performed on 10 uni-transfemoral amputees and 10 healthy males. Kinematics and kinetics at the hip, knee, and ankle joint were calculated. As a statistical method, independent t-tests were conducted to perform a comparison between the transfemoral amputee group and the control group. The results showed that the external knee adduction moment increased in the transfemoral amputee group (.22 Nm/kg) compared with that of the control group (.13 Nm/kg) at terminal stance (p=.008). External knee flexion moment also increased in the transfemoral amputee group (.24 Nm/kg) but this difference was not statistically significant. External hip flexion moment increased in the transfemoral amputee group (1.35 Nm/kg) compared with that of the control group (.45 Nm/kg) at initial stance, and external hip extension moment decreased in the transfemoral amputee group (-.26 Nm/kg) compared with that of the control group (-.76 Nm/kg) at terminal stance. Although external ankle plantarflexion moment of the transfemoral amputee group increased, it was not found to be statistically significant. The results suggest that the intact limb joint moment of the uni-transfemoral amputees during walking can be different from that of healthy subjects. In conclusion, it was found that there is a link between the increase of external knee adduction moment and the prevalence of knee osteoarthritis in uni-transfemoral amputees. This result is expected to provide some objective data for rehabilitation programs related to knee osteoarthritis in transfemoral amputees.

Joint Pain in Patients with Osteoarthritis: Based on the 5th KNHNES (우리나라 골관절염 환자의 관절통증: 2012년 국민건강영양조사 자료를 이용하여)

  • Purpose:The purpose of this study was to examine the degree of joint pain, limitation of daily activities, and frequency of outpatient visits. Methods: This secondary analysis study used the data from the fifth Korean National Health and Nutrition Examination Survey (KNHNES). The data about general characteristics, prevalence and treatment of osteoarthritis, limitation of activities of daily living (ADL) and frequency of outpatient visits and hospitalization in 424 osteoarthritis patients over 50 years old were derived from the database. Data were analyzed with complex samples in SPSS ver. 20.0. Results: Among people with knee joint pain, 79.3% reported they had more than 4 out of 10 points of the degree of pain. Meanwhile, 97.3% of people with hip joint pain reported that they had more than 4 out of 10 points of the degree of pain. People reporting 10 out of 10 point of pain were 21.4% of those with knee joint pain and 25% of those with hip joint pain. The mean of the degree of knee joint pain was 6.35, and the mean of the degree of hip joint pain was 6.89 out of 10 points. About 50% of people with osteoarthritis visited an outpatient clinic within the last 2 weeks, and 27% had limitation of ADL. Conclusion: It is necessary to develop an integrated intervention program to improve quality of life in patients with osteoarthritis.

The Effects of Femorotibial Angle of a Contact Lateral-Wedged Insole with Strapping in Patients with Varus Deformity Osteoarthritis of the Knee (밀착성 외측 쐐기 스트랩 깔창이 내반슬 골관절염 환자의 대퇴경골각에 미치는 영향)

  • The purpose are to assess the efficacy of a contact lateral-wedged insole with strapping on the femorotibial angle in patients with varus deformity osteoarthritis of the knee after treatment period. 25 outpatients with knee osteoarthritis (OA) were randomized to be treated with a contact lateral-wedge insole with strapping. Standing radiographs were used to analyze the femorotibial angle for each subject. The result of repeated two-way ANOVA's reveled that a contact lateral-wedged insole with strapping produced significantly differences in the femorotibial angle between groups after treatment period (P<0.05). and repeated one-way ANOVA's reveled that it produced significantly differences in the femorotibial angle between experimental groups after treatment period (P<0.05). We suggest that these results may be beneficial for manufacturing foot orthotic devices, such as wedged insoles, to control medial and later compartment forces in the knee varus-valgus deformity.

  • Objectives : This study is performed for proposing reasonable clinical study protocol at International standard clinical study of osteoarthritis of Knee Joint. Methods : We gave the eighteen oriental medicine doctors the questionnaire and the video about female patient who diagnosed Knee joint of osteoarthritis above 60 years old and they tested same patients using differentiation of symptoms and signs. Then they retested it after We educated them with diagnosis education text. Conclusion : The improvement of diagnosis agreement can be possible by developing standardized the same patients using differentiation of symptoms and by reeducation about it.

Efficacy and Safety of Pharmacopuncture and Bee venom Acupuncture for Knee Osteoarthritis: A systematic review and meta-analysis (퇴행성 슬관절염에 대한 약침 및 봉독 요법의 효용성 및 안전성: 체계적 문헌 고찰 및 메타 분석)

  • Objectives: The purpose of this study was to evaluate the efficacy and safety of pharmacopuncture and bee venom acupuncture for knee osteoarthritis. Methods: We searched for randomized controlled trials that investigated the effects of pharmacopuncture and bee venom acupuncture on knee osteoarthritis through the electronic databases including Pubmed, EMBASE, Cochrane, CiNii, CNKI, KMBASE, KISS, NDSL, and OASIS. Meta-analysis was performed by Review Manager software and the quality of included studies were assessed by the Cochrane risk of bias tool. Results: A total of 20 articles with 1536 participants were identified. 12 trials about phamacopuncture and 8 trials about bee venom acupuncture showed significant improvement than sham treatment, western medicine treatment, and other Korean medicine treatment such as acupuncture in diverse scales. Six trials reported adverse events. Conclusions: These findings showed certain efficacy and safety of pharmacopuncture and bee venom acupuncture. It would be helpful for patients and Korean medicine doctors in the choice of the treatment for knee osteoarthritis. Well-designed studies with long term follow up and more number of participants should be conducted to strengthen the evidence of the use of pharmacopuncture and bee venom acupuncture.

Effects of Jetongdan on the Quality of Life in Patients with Osteoarthritis of Knee (제통단이 퇴행성 슬관절염 환자의 삶의 질에 미치는 영향)

  • Objectives : to evaluate the effects of Jetongdan, the newly developed herbal medicine, on the quality of life in patients with osteoarthritis of knee Methods : Placebo-controlled, randomized clinical trial on the 80 patients with osteoarthritis of the knee was fulfilled. After enrollment, they took medication as they enrolled. And they were asked to answer the questionnaires (Korean Health Assessment Questionnaire (KHAQ), Lequesne's functional index (LFI), visual analogue scale (VAS)) and analysed with the erythrocyte sedimentation rate (ESR) at baseline, after 4 weeks and 8 weeks of medication. Results : Total KHAQ score, some categories of KHAQ like hygiene, activities were significantly improved after 8 weeks of medication. And the change of the KHAQ score was significantly correlated with the change of VAS. But LFI, VAS, ESR level was not significantly improved, and it was considered because of the short medication period and small size of study population. Conclusion : In conclusion, Jetongdan could improve the health-related quality of life in patients with osteoarthritis of knee. Further study in the large population, in long period were recommended.

The impact of joint mobilization with an elastic taping on immediate standing balance in patients with knee osteoarthritis. (무릎 관절염 환자에 대한 관절가동술과 탄력 테이핑 융복합 적용이 즉각적인 기립 균형에 미치는 영향)

  • The purpose of this study was to identify the immediate effect of the joint mobilization with an elastic taping on standing balance in patients with knee osteoarthritis. Thirty patients with knee osteoarthritis were randomly divided into three groups: a taping group, a joint mobilization group, and a joint mobilization with taping group. A foot pressure platform (Zebris) was used to evaluate standing balance ability, and the sway area, path length and average velocity were measured during eyes open condition and eyes closed condition. All the groups showed a significant improvement in the sway area during eyes closed condition after intervention, and the joint mobilization with taping group revealed significant improvements in the path length and average velocity. There was no significant difference in the standing balance ability among all the groups. Both the joint mobilization and taping method were effective methods for standing balance during eyes closed condition, and it has been found that the convergence of the two interventions had an effect on diverse balance variables. Thus, it is recommended to apply the convergence of the two interventions for patients with knee osteoarthritis.

  • Purpose: The purpose of this study was to investigate the effects of peer mentoring program on physical activity, knee joint function, self-care agency and social support, which are health conservation elements in elderly women with osteoarthritis. Methods: This study used a quasi-experimental research design. It is a pretest-and-post 1, post 2 test design of a non-equivalent control group. The subjects were elderly women aged over 65 who were diagnosed with osteoarthritis. A total of 60 patients (experimental group 30, control group 30) who registered with the Senior Welfare Center in City G and in Region D participated in this study. The data were collected from June 29th to September 4th, 2015. The collected data were analyzed with $x^2$ test, Fisher's exact test, independent t-test and repeated measurement ANOVA. Results: The experimental group showed a greater increase in physical activity, knee joint function, self-care agency and social support than the control group. Conclusion: The results indicated that the peer mentoring program is effective in increasing physical activity, knee joint function, self-care agency and social support of elderly women with osteoarthritis.

Effects of Foot Reflexo-massage on Physical, Psychological, Physiological Parameters in Patients with Osteoarthritis (발반사마사지가 골관절염 환자의 신체적, 심리적, 생리적 지수에 미치는 영향)

  • Purpose: This study was undertaken to investigate the effects of foot reflexo-massage on physical, psychological, and physiological parameters in patients with osteoarthritis of the knee. Method: This study was employed non-equivalent control group pretest-posttest time series experimental design. The subjects of this study were 47 elderly patients with osteoarthritis of the knee from two different nursing homes in Deajeon. The patients were divided into two groups; 26 patients in experimental group, the rest in control group. Each patient in experimental group received the 30-minute foot reflexo-massage, 3 times per week for 4 weeks. The dependent variables of each patient were measured before treatment, after 2 weeks and after 4 weeks during the treatment session. The collected data was analyzed using the Social Package Social Science(version 10.0) software program. Result: There were significant differences in the flexion angle of both knees; the flexion and extension muscle strength of both knees; systolic blood pressure; POMS; plantaris skin temperature and blood velocity of dorsalis pedis artery between the two groups over different the three measurement times. Conclusion: The results suggest that foot reflexo-massage could be an effective intervention to improve physical, psychological and physiological parameters for patients with knee osteoarthritis.
