아로마 마사지요법이 퇴행성 슬관절염 노인의 통증, 수면 및 활보장에 미치는 영향 (The Effects of Aromatherapy Massage on Pain, Sleep, and Stride Length in the Elderly with Knee Osteoarthritis)

  • Purpose: The purpose of this study was to investigate the effects of aromatherapy massage in the elderly with knee osteoarthritis. Methods: Participants were assigned randomly to an intervention (n=21) group and a control (n=21) group. In order to increase a lasting effect of aromatherapy massage, an essential oil used in the form of cream. Each participant had aromatherapy massage on lower legs for 20 minutes each time twice a week for four weeks. Numerical rating scale was used for self-report of pain intensity. Sleep disturbance was measured by Korean Sleep Scale consisting of 15 items. Stride length had been obtained by measuring the distance from the heel of one foot to the heel of the other foot. Results: There was a significant difference between the pain scores of the experimental group and that of control group after the aromatherapy massage (p=.001). Whereas, no significant differences between sleep scores of the two groups (p=.592) was found. The experimental group's stride length significantly increased than those of the control group (p=.009). Conclusion: Aromatherapy massage could be recommended as an effective intervention to decease pain and to increase stride length in the elderly with knee osteoarthritis.

운동프로그램이 퇴행셩 슬관절염 노인의 체력에 미치는 영향 (The Effects of Exercise Program on Physical Fitness in Elderly with the Knee Osteoarthritis)

  • Purpose: This study was to determine the effects of exercise program for knee osteoarthritis on the physical fitness in elderly. The progressive exercise program included flexibility, strength, endurance, active range of motion and function activities. Methods: Nineteen subjects with osteoarthritis of the knees randomized to exercise group(n=10) or control group(n=9), but fourteen subjects completed the trial with an adherence of 70% to the intervention and 77% to the control. The physical fitness(back muscle strength, grip strength, flexibility, muscle power, agility, equilibrium) were evaluated with Helmas(Health Management System) made by Sewoo system at pre- and post- intervention. Results: The data were analysed with the two-way repeated ANOVA and paired t-test. The physical fitness had no difference among the groups and times except muscle power. Within experimental group, muscle power and agility were significantly increased between pre and post intervention, while the control group showed no change. Conclusion: Exercise program for knee osteoarthirits appear to be effective in muscle power and agility.

Effects of Electromagnetic Acupuncture Combined with Integrative Korean Medicine Treatment on Pain and Dysfunction in a Patient with Knee Osteoarthritis: A Case Report

  • Knee osteoarthritis (KOA) is a prevalent degenerative joint disease causing significant pain and dysfunction. This case report presents the use of electromagnetic acupuncture utilizing a Whata 153 device generating a magnetic field to enhance acupuncture stimulation for the treatment of KOA. A 69-year-old female diagnosed with KOA experienced a reduction in pain (numerical rating scale score from 7 to 4), improved gait, and decreased stiffness and swelling after daily electromagnetic acupuncture treatments during hospitalization. In addition, the Korean Western Ontario and McMaster Universities Osteoarthritis Index scoreimproved from 20 to 14, and the patient rated her overall improvement as "significantly improved" on the patient's global impression of change scale. Although these findings suggest potential benefits of electromagnetic acupuncture for KOA, the case report design limits its generalizability. More controlled trials are warranted to confirm the efficacy and safety of electromagnetic acupuncture as a treatment of KOA.

골수 흡인-혈소판 풍부 혈장과 히알루론산의 관절강내 주사의 효과 비교 (Effects of Bone Marrow Aspirate Concentrate-Platelet-rich Plasma Versus Hyaluronic Acid on Patients with Knee Osteoarthritis: A Randomized Controlled Trial)

  • Objective: To compare the therapeutic efficacy of the bone marrow aspirate concentrate (BMAC)- platelet-rich plasma (PRP) complex with hyaluronic acid in patients with knee osteoarthritis. Method: Thirty-four patients with knee osteoarthritis participated in this study. Seventeen patients in the study group underwent BMAC and PRP extraction followed by intra-articular injection of BMAC-PRP complex within affected knee. Seventeen patients in the control group underwent intra-articular injection of hyaluronic acid. Knee injury, osteoarthritic outcome score (KOOS), and EuroQol-5D (EQ-5D) questionnaire were evaluated before, one month, three months, and six months after the injection. Results: There were statistically significant temporal differences in total KOOS scores in both BMAC-PRP and HA groups. However, there were no significant group difference in the study period. In the Sports and Recreational Function Scale, there was statistically significant improvement in the BMAC-PRP group compared to the HA group at three months (p=0.041). There were no side effects or complications in both groups. Conclusion: Intra-articular injection of BMAC-PRP showed better functional recovery in the OA at three months and this can be an alternative treatment in terms of functional recovery in the OA in addition to the decrease of pain.

계지가부자탕(桂枝加附子湯)이 MIA로 유도된 골관절염 Rat 모델에 미치는 영향 (Effects of Gyejigabuja-tang on MIA-induced Osteoarthritis in Rats)

  • Objectives This study was carried out to investigate the effects of Gyejigabuja-tang extracts on the Monosodium iodoacetate (MIA) induced osteoarthritis in rats. Methods Osteoarthritis was induced by injection of MIA into knee joint cavity of rats. Rats are divided into 4 groups (normal, control, positive comparison group, GBT group, each n=5). Normal group was injected by normal saline into knee joint cavity only. Control group was induced for osteoarthritis by MIA and oral medicated with distilled water. Positive comparison group was injected with MIA and taken Joins tablet 25 mg/kg. GBT group was injected with MIA and taken Gyejigabuja-tang extracts 300 mg/kg. Positive comparison group and GBT group were oral medicated for each substance once a day for 4 weeks. ALT, AST and creatinine were evaluated for hepatotoxicity and nephrotoxicity. Hind paw weight bearing ability was examined and inflammatory cytokines (IL-$1{\beta}$, IL-6, TNF-${\alpha}$), $PGE_2$, $LTB_4$, osteocalcin and deoxypyridinoline (DPD) within serum were analysed. Knee joint structures were observed by Hematoxylin & Eosin (H&E), Safranin-O staining method. Results 1. Function of liver and kidney was not affected. 2. Hind paw weight bearing ability was significantly improved. 3. IL-$1{\beta}$, IL-6 and TNF-${\alpha}$ in experimental group were significantly decreased compared with control group. 4. $PGE_2$, $LTB_4$, Osteocalcin and DPD in experimental group were decreased compared with control group. 5. In histopathologic observation, injury on synovial membrane and cartilage of experimental group was lesser than control group (H&E, Safranin-O staining). Conclusions Based on these results, it can be suggested that Gyejigabuja-tang has anti-inflammation effects on the MIA-induced osteoarthritis in rats.

  • Objectives : In cases of osteoarthritis, the hypofunction of the cartilage and joint leads to a limited range of joint motion, swelling, and pain, which is generally treated using pharmaceutical drugs (e.g., anti-inflammatory agents, cartilage protectants, and nonsteroidal anti-inflammatory drugs) or replacement arthroplasty. However, long-term drug treatment is associated with adverse effects on the gastrointestinal systems. The present study aimed to evaluate the ability of Giheolsotong-hwan to treat of osteoarthritis symptoms in the MIA-induced rat model based on histological analysis, and factors that are associated with inflammation and bone mineral metabolism. Methods : Giheolsotong-hwan was administered orally at doses of 200 mg/kg/day or 400 mg/kg/day for 2 weeks before direct injection of monosodium iodoacetate ($3mg/50{\mu}{\ell}$ of 0.9% saline) into the intra-articular space of the rats' right knee. The rats subsequently received the same doses of oral Giheolsotong-hwan for another 4 weeks. We evaluated the treatment effects based on serum biomarkers and histopathological analysis of the knee joints. Results : Compared to those in control rats, the Giheolsotong-hwan treatments significantly decreased the serum concentration of inflammation factors (i.e., $IL-1{\beta}$, IL-6, $TNF-{\alpha}$, $PGE_2$, and $LTB_4$), and bone degrade factors (i.e., MMP-9, CTX-II, and COMP). In addition, the Giheolsotong-hwan treatments significantly increased the concentration of glycosaminoglycans of bone defence factors, but no chage the TIMP-1. Furthermore, the Giheolsotong-hwan treatments effectively preserved the knee cartilage and proteoglycan. Conclusion : The results indicate that Giheolsotong-hwan treated osteoarthritis symptoms. Thus, Giheolsotong-hwan may be a novel oriental therapeutic option for the management of osteoarthritis.

무릎밴드 가압과 가온요법이 퇴행성 무릎관절염 노인의 혈류속도에 미치는 효과 (The Effect of Compression Knee Band and Heat Treatment on Blood Velocity of the Elderly with Osteoarthritis)

  • The purpose of this study was to investigate the change in blood velocity(mm/sec) when compression and/or heat were applied to the knee joint for the elderly who has both normal and painful legs with osteoarthritis(OA). Experimental compression knee band was prepared from the 3D knee data of the average women in 60's. 3D replica of knee was reduced by 7, 10, and 13% from the nude pattern in course direction. Clothing pressure was measured at the front and back of each healthy and painful knee of elderly women for one minute while standing and sitting on the chair. Blood velocity was measured at 13 cm upper from the mid-patella for 15 minutes. Results are as follows: first, compression or heating treatment itself did not change blood velocity of both legs; second, combination treatment with heating and compression was effective to increase blood velocity. In details, for healthy legs, combination treatments with compression by 10% reduced pattern(about 1.3kPa) and heating($43^{\circ}C$) induced the maximal blood velocity, however, for knees with OA, 7% reduced pattern(about 1.0kPa) with simultaneous heating($43^{\circ}C$) was more effective than other cases. These results indicated that pain and spasticity of knee joint with OA could be reduced by applying heat and compression therapy, where the compression level of painful knee should be slightly lower than of healthy leg.

Potential Benefits of a Selective Region High-frequency Diathermy with Therapeutic Exercises on Older Persons with Degenerative Knee Osteoarthritis: A Case Report

  • Objective: The purpose of this case study was to investigate selective region high-frequency diathermy at trigger points with therapeutic exercises on pain, function, balance and gait in older patients with degenerative knee osteoarthritis (DKO). Design: A case report. Methods: The patient who participated in this study was a 71-year-old woman, who had been diagnosed with moderate osteoarthritis with grade II Kellgren & Lawrence grading scale. The intervention consisted of selective region high-frequency diathermy at trigger points, with hip and knee stretching and strengthening exercises. The participant was given assessments before and after every intervention session using the Visual Analogue Scale (VAS), Western Ontario and McMaster Universities Arthritis Index (WOMAC), the Timed Up and Go test (TUG) and the 10 Meter Walk Test (10MWT). The participant performed the intervention 18 times for a total of 30 minutes each. Results: As a result of this study, the patient VAS decreased to 3 points, and the WOMAC decreased to 53 points. In addition, the TUG decreased to 3.25 s and the 10MWT decreased to 1.14 s. Conclusions: The results of this study suggest that selective region high-frequency diathermy at trigger points with therapeutic exercises may be an effective intervention to decrease pain, improve knee function, balance and gait in patients with DKO. The selective region high-frequency diathermy with therapeutic exercises may be feasible and provide potential benefits for rehabilitation of DKO.

슬관절 전치환술 후 한의 핵심 결과 지표를 개발하기 위한 임상 평가지표에 대한 문헌 연구 (A Literature Study about Clinical Outcome Parameters for Total Knee Replacement to Develop Core Outcome Set for Osteoarthritis by Korean Medicine Treatment)

  • Objectives Osteoarthritis is hard to manage with both conventional and Korean medicine treatment. The core outcome set (COS) to demonstrate the effectiveness of Korean medicine treatment has not been established yet. We aimed to present preliminary data of COS by performing a literature review on the evaluation indices used in existing clinical research. Methods We examined the literature from 2000 to 2017 in two Korean electronic databases (Korea citation index and oriental medicine advanced searching integrated system) by searching for the following 3 terms 'total knee replacement (Korean)', 'total knee replacement,' and 'knee surgery.' We found 333 articles; among them, 50 duplicates were removed. Finally, we selected 160 articles after complete screening. We then extracted measured indices and clinical outcomes from the selected articles and categorized the relevant criteria. Results According to this study, the hospital for special surgery and knee society, range of movement angle, cross leg, Berg balance scale and balance ability, muscle strength, 6 minutes walking test, visual analogue scale, self-efficacy, the 12-item and 36-item short form survey and self-rated health status are the most commonly used outcomes of knee. Conclusions This study found that the several categories after total knee replacement (TKR) are being evaluated in the literature, and we were able to verify the most frequently used evaluation indices in these categories. The results of this study will be used to establish evaluation indices for the treatment of TKR in the future using Korean medicine.

관절 초음파를 이용한 퇴행성 슬관절염 평가에 관한 연구 (Study of the Evaluation of the Knee Osteoarthritis Using Ultrasonography)

  • Objectives : The aim of this study was to evaluate the usefulness of ultrasonography of knee osteoarthritis. Methods : 124 osteoarthritis patients classified by the American College of Rheumatology (ACR) clinical criteria were included in the study. Simple X-ray and ultrasonography examination (effusion, osteophyte, articular cartilage, Backer's cyst) of knee in pain were performed. Also VAS, KWOMAC score were checked. Results : X-ray showed abnormal findings in 112 patients, but ultrasonography showed abnormal findings in all 124 patients. 12 Patients with unremarkable knee X-ray showed effusion and osteophyte in the ultrasonography. Effusion was discovered only in 18 cases of X-ray and 62 cases of ultrasonography. Group with effusion showed higher VAS mean figure but couldn't retrieve significant difference. KWOMAC showed no significant difference. Conclusions : Osteoarthritis patients with no abnormality in X-ray diagnosed abnormal findings through sonograph, which manifest that sonograph can be utilized for differential diagnosis especially sensitive on the problem of effusion.
