A Musculoskeletal Model for Biomechanical Analysis of Transfemoral Amputees Climbing Stairs

  • Understanding the characteristics of amputee gait is key in developing more advanced prostheses. The aim of this study was to quantitatively analyze a stair-climbing task for transfemoral amputees with a prosthesis and to predict the muscle forces and joint moments at musculoskeletal joints using a dynamic analysis. A three-dimensional musculoskeletal model of the lower extremities was constructed from a gait analysis using transformation software for two transfemoral amputees and ten healthy people. The measured ground reaction forces and kinematical data of each joint from the gait analysis were used as input data for an inverse dynamic analysis. Dynamic analyses of an transfemoral amputee climbing stairs were performed using musculoskeletal models. The results showed that the summed muscle forces of the hip extensor of an amputated leg were greater than those of a sound leg. The opposite was true at the hip abductor and knee flexor of an amputated leg. We also found that higher moments at the hip and knee joints of the sound leg were required to overcome the flexion moment caused by the body weight and amputated leg. Dynamic analyses using musculoskeletal models may be a useful means to predict muscle forces and joint moments for specific motion tasks related to rehabilitation therapy.

  • This is a report on a aseptic synovitis diagnosed and treated arthroscopically following the meniscal repair using biodegradable Meniscus Arrow$^{(R)}$(Bionix Inc, Malvern, USA). Histological examination revealed chronic nonspecific synovitis and birefringent material. Immunohistochemical tests were positive in lysozyme, ${\alpha}$-1-antitrypsin and ${\alpha}$-1-antichymotrypsin. The patient was received arthroscopic synovectomy, and then pain and swelling of the knee joint was relieved. Range of motion was improved to full range. As far as we know, this is the first case of aseptic synovitis after meniscal repair using biodegradable Meniscus Arrow$^{(R)}$.

  • Considerable attention has been focused on digital X-ray systems with transmission. However, only a few attempts have been made using X-ray backscatter systems. This study developed a Data Acquisition System (DAS) for backscattered X-rays. The imaging system used X-ray backscatter at low energy levels, which has unique advantages in terms of acquiring information that cannot be obtained using the transmission system. Such systems are of potential benefit in the medical, industrial, security and military fields because of its practical benefits and the low doses used. The developed system is a similar body scan system. Experiments were carried out using a several metal materials and a fish, which produces results similar to those obtained using the human body during X-ray testing.

  • Kinematic and kinetic studies were performed to investigate the walking characteristics on a treadmill with various slopes at the same speed of 1.25m/sec. Six different slopes of the treadmill were selected . -4%(-$2.3^{\circ}$), 0%($0^{\circ}$), 5%($2.9^{\circ}$), 10%($5.7^{\circ}$), 15%($8.6^{\circ}$), and 20%($11.3^{\circ}$). With increased slopes of the treadmill, both hip and knee flexion angles significantly increased at initial contact, and the maximum hip flexion during swing phase and the maximum knee flexion during stance phase also significantly increased Ankle dorsiflexion angle at initial contact and the maximum dorsiflexion increased with increased slopes. However, the maximum plantarflexion in early swing was slightly reduced with increased slopes. Hip extension in late stance and the maximum knee flexion in early swing was not changed sigilificantly with increased slopes. As for the vertical ground reaction force, compared to the yond level walking, both the first and the second peak forces increased, but the mid-support force decreased.

  • Arteriovenous malformation (A-V malformation) is defined as an abnormal connection between arteries and veins that lead to A-V shunting with an intervening network of vessels. A-V malformation is a rare condition, and spontaneous regression is also rare. A-V malformation becomes symptomatic when the surrounding tissue and osseous structures are negatively affected. A-V malformation has a high recurrence rate and is relatively hard to treat. In this case, a huge mass with pulsatile and bruit on the medial plantar area were observed. With the diagnosis of A-V malformation in accordance with the results from ultrasonography, magnetic resonance imaging and computed tomography angiography, and mass excision with feeding vessel ligation through plantar midfoot approach was completed successfully.

Morphometric Analysis of the Ureter with Respect to Lateral Lumbar Interbody Fusion Using Contrast-Enhanced Computed Tomography

  • Objective : To analyze the anatomical location of the ureter in relation to lateral lumbar interbody fusion and evaluate the potential risk of ureteral injury. Methods : One hundred eight patients who performed contrast-enhanced computed tomographic scans were enrolled in this study. The location of the ureter from L2-L3 to L4-L5 was evaluated. The distances between the ureter and psoas muscle, intervertebral disc, and retroperitoneal vessels were also recorded bilaterally. Results : Over 30% of the ureters were close to the working corridor of extreme lumbar interbody fusion at L2-L3. Most of the ureters were close to working corridor of oblique lumbar interbody fusion, especially at L4-L5. The distance from the ureter to the great vessels on the left side was significantly narrowing from L2-L3 to L4-L5 (28.8±9.5 mm, 22.0±8.0 mm, 15.5±8.4 mm), and it was significantly larger than that on the right side (12.3±6.1 mm, 7.4±5.7 mm, 5.4±4.4 mm). Conclusion : Our findings indicate that the location of the ureter varies widely among individuals. To avoid unexpected damage to the ureter, it is imperative to directly visualize it and verify the ureter is not in the surgical pathway during lateral lumbar interbody fusion.

DESIGN OF AIR SEAT CUSHION ORTHOSIS FOR PLEGIA

  • The design of an air seat cushion for preventing decubitus ulcer includes many design factors such as the even distribution of interface pressure, the minimization of mean and peak interface pressure values, and the reduction of interface shear force and pressure gradient. It involves the anatomic condition of plegia's buttock as well as air pressure in air cells of cushion. As a result, a suitable design of the cushion satisfying the all requirements is a difficult problem. Therefore, an appropriate and effective numerical tool to develop an air cushion orthosis is required. The purpose of the present study was to develop an air seat cushion orthosis having optimized air cells for evenly distributed interface pressure between the buttock and cushion surface. For the purpose, an advanced finite element (FE) model for the design of air cushion was developed. Since the interface pressure and shear force behavior, as well as stress analyses were primary concern, a FE air cell model was developed and verified by the experiments. Then, the interactions of two cells were checked. Also, the human part of the developed numerical model includes every material property and geometry related to buttock and femoral parts. For construction of dimension data of buttock and femoral parts, CT scans were performed. A commercial FE program was employed for the simulation representing the seating process on the orthosis. Then, sensitive analyses were performed with varying design parameters. A set of optimal design parameters was found satisfying the design criteria of the orthosis. The results were utilized to produce a prototype of the orthosis. Experimentally, the buttock interface pressure distributions from the optimized and previous ones were compared. The new seat orthosis showed a significantly improved interface pressure characteristics compared to the most popular one in the market. The new orthosis will be used for the development of the AI(artificial intelligent) controlled seat orthosis fur prevention of decubitus ulcer fur various plegic patients and the elderly.

The Analysis and Treatment of Benign Bone Tumor by Curettage and Debridement with Bone Graft Substitutes (양성 골종양의 절제 및 소파술 후 사용한 골이식 대체물의 결과 및 분석)

  • Purpose: The purpose of this study was to determine if bony union can be obtained when a bone graft substitute is transplanted in order to treat a benign bone tumor and if there is a difference between calcium sulfate and hydroxyapatite in the healing procedure when the degree of the bony union after the bone graft are compared. Materials and Methods: This study selected 20 cases, in which a curettage on the benign bone tumors was conducted and a bone graft substitute was transplanted. The area of the focus, the new bone formation, the recurrence of the focus in the plain radiographs and histological findings were observed. Results: Twenty cases (13 males, 7 females) were evaluated. Their mean age at surgery was 15.8 years (2~45), and the mean follow-up period was 3 years. The mean area of focus was 30.7 $cm^3$ in the radiographs, and 19 cases showed successful results in the ultimate visit, while 1 case has a recurrence of the focus. Conclusion: Calcium sulfate has osteoconduction and superior bioavailability, and is absorbed in vivo in proportion to the new bone formation. On the other hand, hydroxyapatite has good osteoconduction. It can result in better bone formation when it is combined with an autologous bone graft, autologous bone marrow, and an allogenic bone graft, but is absorbed in vivo more slowly than the former.

Adipose-Derived Stem Cells Suppress Inflammation Induced by IL-1β through Down-Regulation of P2X7R Mediated by miR-373 in Chondrocytes of Osteoarthritis

  • Adipose-derived stem cells (ADSCs) were previously considered to have an anti-inflammatory effect, and Interleukin-$1{\beta}$ ($IL-1{\beta}$) was found to be a pro-inflammatory factor in chondrocytes, but the mechanism underlying ADSCs and $IL-1{\beta}$ is unclear. In this study, we investigate whether P2X7 receptor (P2X7R) signalling, regulated by microRNA 373 (miR-373), was involved in the ADSCs and $IL-1{\beta}$ mediated inflammation in osteoarthritis (OA). Chondrocytes were collected from 20 OA patients and 20 control participants, and ADSCs were collected from patients who had undergone abdominal surgery. The typical surface molecules of ASDCs were detected by flow cytometry. The level of nitric oxide (NO) was determined by Griess reagent. Concentrations of prostaglandin E2 (PGE2), interleukin 6 (IL-6), matrix metallopeptidase 3 (MMP-3) were detected by enzyme-linked immunosorbent assay (ELISA). The expressions of IL-6, MMP-3, miR-373 and P2X7R were determined by real-time polymerase chain reaction (PCR), and Western blot was used to detect the protein expression of P2X7R. The typical potential characters of ADSCs were verified. In chondrocytes or OA tissues, the miR-373 expression level was decreased, but the P2X7R expression was increased. $IL-1{\beta}$ stimulation increased the level of inflammatory factors in OA chondrocytes, and ADSCs co-cultured with $IL-1{\beta}$-stimulated chondrocytes decreased the inflammation. OA chondrocytes transfected with the miR-373 inhibitor increased the inflammation level. The miR-373 mimic suppressed the inflammation by targeting P2X7R and regulated its expression, while its effect was reversed by overexpression of P2X7R. $IL-1{\beta}$ induced inflammation in OA chondrocytes, while ADSCs seemed to inhibit the expression of P2X7R that was regulated by miR-373 and involved in the anti-inflammatory process in OA.
