Background: In the past decade, the number of anatomic total shoulder arthroplasty (aTSA) procedures has steadily increased. Patients over 65 years of age comprise the vast majority of recipients, and outcomes have been well documented; however, patients are opting for definitive surgical treatment at younger ages.We aim to report on the effects of age on the long-term clinical outcomes following aTSA. Methods: Among the patients who underwent TSA, 119 shoulders were retrospectively analyzed. Preoperative and postoperative clinical outcome data were collected. Linear regression analysis (univariate and multivariate) was conducted to evaluate the associations of clinical outcomes with age. Kaplan-Meier curves and Cox regression analyses were performed to evaluate implant survival. Results: At final follow-up, patients of all ages undergoing aTSA experienced significant and sustained improvements in all primary outcome measures compared with preoperative values. Based on multivariate analysis, age at the time of surgery was a significant predictor of postoperative outcomes. Excellent implant survival was observed over the course of this study, and Cox regression survival analysis indicated age and sex to not be associated with an increased risk of implant failure. Conclusions: When controlling for sex and follow-up duration, older age was associated with significantly better patient-reported outcome measures. Despite this difference, we noted no significant effects on range of motion or implant survival. Level of evidence: IV.

골연골종은 골종양중 가장 흔한 양성 골졸양으로, 양성 골종양의 40%를 차지하고 있으나, 척추에서의 병변은 적어 2%에 지나지 않아, 경추 제5, 6번간 극돌기에 발생한 골연골종 1예를 en bloc을 통한 절제 생검을 시행하고, 그 발생 부위의 희귀성에 비추어 보고하는 바이다.

Suture anchors are commonly used in shoulder surgeries, especially for rotator cuff tears. Peri-anchor cyst formation, however, is sometimes detected on follow-up radiologic image after surgery. The purpose of this report is to discuss the case of a patient who presented with regression of extensive peri-anchor cyst on postoperative 4-year follow-up magnetic resonance imaging and had good clinical outcome despite peri-anchor cyst formation after arthroscopic rotator cuff repair.

Although Achilles tendinitis is a relatively common disease, tuberculous involvement of Achilles tendon is rare. We report a case of tuberculous Achilles tendinitis, which was successfully treated with chemotherapy and a combined surgical procedure (Achilles tendon parital excision and FHL tendon transfer).

Pigmented villonodular synovitis (PVNS) in ankle is relatively uncommon. This disorder results in increased proliferation of synovium causing villous or nodular changes containing histiocytes, fibroblasts, multinucleated giant cell, and hemosiderin. PVNS is classified into two different type : localized and diffuse. Diffuse type of PVNS in ankle is more common than localized type. Also, recurrence of diffuse type is more frequent. We report a case of diffuse type of PVNS which was recurred soon after the excision.

Arthroscopy of the elbow is a very precise and demanding procedure due to the closeness of the recommended portals to neurovascular structures and complexity of articular geometry. So to establish safe portal is not always reproducible especially in case of stiff elbow, even for the experienced arthroscopist. We described new tip of elbow arthroscopy using a new starting portal. This procedure is almost always reproducible even in stiff elbow, and minimizes risk of damage to neurovascular structures.

Tarsal coalition is a congenital failure of segmentation resulting in fibrous, cartilaginous, or bony union between tarsal bones. Although single tarsal coalitions are common, dual tarsal coalitions are a rare occurrence. We repport of unilateral and bilateral dual calcaneonavicular and talocalcaneal coalitions.

Accessory navicular is a congenital anomaly appearing in the secondary ossification center on the tuberosity of the navicular that may cause flatfoot. Bipartite medial cuneiform is another rare congenital anomaly occurring as two primary ossification centers in the medial cuneiform. The authors report a rare case of symptomatic bilateral accessory navicular with bipartite medial cuneiform and flatfoot deformity in a 19-year-old man with a review of the literature.

Intra-osseous ganglion is an uncommon benign bone tumor caused by an intramedullary metaplastic event. Two cases of the intraosseous ganglion of the foot are presented; one in the metatarsus and the other in the talus. Follow-up averaged 14 months. Satisfactory results were obtained with curettage, cancellous bone graft from the ipsilateral calcaneus. No recurrence and complications occurred during the period of follow-up.

Extracellular matrix(ECM) is composed of the ground materials(proteoglycan) and nano size diameter fibrous proteins(ex. collagens) that together form a composite-like structure. In this study, fibrous scaffold with biomimetic architecture based on collagen nanofibers interpenetrated in PLGA/chitosan microfibrous matrix. Chitosan was selected for its structure similarity to glycosaminoglycan and neutralizing capacity for PLGA acidic metabolite. Collagen nanofiber were prepared by electrospinning. (omitted)
