Background: The purpose of this study is to investigate that effect of manual therapy using pelvic compression belt on hip abductor strength and balance ability in total knee replacement (TKR) patients. Methods: The subjects consisted of twenty two post-TKR patients. Participants were randomly assigned to a pelvic belt group (n=11) and a placebo group (n=11). All participants underwent manual therapy including range of motion exercise, soft tissue mobilization around knee joint, strengthening exercise (Quad set, SLR, sidelying hip abduction, standing hamstring curls, sitting knee extension, step-up, wall slide to $45^{\circ}$ knee flexion). Manual therapy was executed five times a week for 2 weeks. Outcome measures included hip abductor strength by using Biodex system 4 pro, anterior to posterior balance, medial to lateral balance, total balance by using Biodex balance system SD. Results: After the completion of the manual therapy, hip abductor strength was showed statistically significant improvements in pelvic belt group (p<.05). Anterior to posterior balance, medial to lateral balance, total balance were showed statistically significant improvements in pelvic belt group and placebo group (p<.05). There was a statistically significant difference between the two groups in hip abductor strength and there was no statistically significant difference in balance. Conclusions: This results suggest that manual therapy using pelvic compression belt has could be used for selective muscle activation of the hip abductor muscle and has useful in hip abductor strength and balance ability in TKR patients.
Purpose: The purpose of this study was to evaluate the effects of mobilization application for the lower extremity after fibula Pilon fracture operation patient. Methods: The subjects was 62 years old male who was injury of Lt. fibula shaft Fx., 3 cuneiform & cuboid Fx., 2.3.4 metatarsal bone Fx., We were compared to result of physical therapy between pre and post exercise for 2weeks. Results: The results of this study were summarized below; The mobilization application of the Lt. lower extremity was significantly differences of the ROM at pre and post therapy after 2 weeks, especially in knee flexion ($40^{\circ}$). The increased of accessary movement was evaluated to increased of the physiologic movement about the joints of the lower extremity. Conclusion: We consider that factors of therapy result were not only fracture types, operation and reduction methods for the fibula Pilon fracture but also the ability of physical therapist's manual techniques.
Background: The purpose of this study is investigate the effects of cervical range of motion (ROM) and forward head posture (FHP) on cervical manual traction in normal adults. Methods: The subjects were randomly assigned to either the experimental group(Female 9, Male 6) who conducted the manual cervical traction or the control group(Female 9, Male 6) who did not conduct the any intervention. The subject in the EG have conducted the 1 set of 10 minutes of manual cervical traction per day, 2 times a week for 6 weeks. The cervical ROM was measured by the digital inclinometer and The factors of FHP was measured by cranial vertebral angle (CVA) and cranial rotation angle (CRA). Results: Comparing the cervical ROM and FHP between the experimental and control groups before and after the experiment, it could be seen that flexion, extension, right lateral flexion, left lateral flexion, CVA and CRA of the experimental group has been increased. Thus, cervical manual traction was resulted in the increased cervical ROM and decrease FHP. Conclusion: In this study, it was confirmed that cervical manual traction affects increase cervical ROM and decrease FHP that play a important role in neck stability and mobility.
Background: The present study aimed to investigate the immediate effects of Soft Tissue Mobilization (STM) before Mobilization with Movement (MWM) on ankle ROM, pennation angle, balance in stroke patients. Methods: A total of 22 subjects were randomly assigned to one of two groups: the experimental group and the control group. The experimental group received intervention STM before MWM. STM was applied for one minute, MWM was applied one set of six times, in a total 3 sets. The passive ankle joint range of motion (ROM) was measured using a goniometer, the pennation angle was measured using RUSI, and the balance was measured using Timed Up & Go Test. Results: The ROM of the ankle dorsi-flexion, muscle tissue (pennation angle) and balance were significantly increased. Conclusion: In this study, it was confirmed that the ankle dorsi-flexion ROM, pennation angle of the medial gastrocnemius muscle, and balance were significantly improved in the group where STM was performed before the MWM intervention. Therefore, the physiotherapists should consider these results in their intervention. If MWM is applied to stroke patients, applying STM first is a better intervention.
Purpose: This study aimed to investigate the effects of myofascial release technique on pain threshold and hormonal changes in patients with adhesive capsulitis of the shoulder. Methods: Eight patients with adhesive capsulitis were treated with the myofascial release technique. Myofascial release is a form of manual therapy that involves the application of a low load, long duration stretch to the myofascial complex, intended to restore optimal length, decrease pain, and improve function. Blood tests and pressure pain threshold (PPT) examinations were performed on their first visit. On their second visit, the myofascial release technique was applied to the shoulder for 20 min. Then, blood tests and PPT were re-evaluated to determine the effects of the myofascial release technique on pain threshold and hormonal changes. Results: Pain threshold increased from 2.92 to 24.13 lb after treatment. Epinephrine decreased from .13 to .08 ng/mL whereas norepinephrine increased from .25 to .41ng/㎖ after treatment. Conclusion: Myofascial release technique in patients with adhesive capsulitis increased pain thresholds, norepinephrine and decreased epinephrine levels.
Background: The Purpose of this study was investigated the effects of the Self Sling Suspension on the numbness, radiated pain, visual analogue scale (VAS), Cobb's angle, Jochumsen's depth, neck disabliity index (NDI). Methods: This study was carried out with a 4 forward head posture (FHP) & 4 herniated nucleus pulposus (HNP) survivors. Outcomes such as the Numbness, Radiated Pain, VAS, Cobb's angle, Jochumsen's depth and NDI were measured pre- and post-intervention for both groups. Results: 2 HNP decrease was found numbness & radiated pain. After the intervention, was decreased VAS, NDI and was increased Cobb's angle, Jochumsen's depth. Conclusions: Applying Self Sling Suspension treatment to FHP & HNP patients proved to have a positive effect.
도수치료는 전 세계적으로 다수의 지역에서 각각 독립적으로 시행되고 발전되어왔던 학문으로 2,500년의 역사를 가진 아주 오래된 치료 방법이다. 최초의 역사적 기록은 기원전 4세기경 그리스에서 확인할 수 있는데 이후 오랜 세월 동안 도수치료는 의학계와 같이 연구하고 발전하기도 했지만 대립된 길을 걸어왔던 것도 사실이다. 우리가 도수치료를 제대로 이해하려면 의학적 치료의 한계를 넘어서기 위해 19세기 후반에 정립된 카이로프랙틱, 정골의학, 물리치료학의 관점에서 각각 연구되고 발전되어 온 도수치료의 세 분야에 대해 알고 있어야 한다. 아주 오랜 옛날부터 시작했고 전 세계적으로 사용된 치료법임에도 불구하고 도수치료에 대한 문헌들은 많지 않고 근거 수준이 낮은 논문들이 대부분이다. 그러나 현재 그 효과에 대한 논란에도 불구하고 도수치료는 점점 더 많은 환자들에게 시술되고 있는 실정이다. 저자들은 이 논문에서 고대부터 도수치료의 역사와 한국 도수치료의 역사, 그리고 현재 도수치료에 대한 최신 지견을 논하고 그것을 근골격계 통증의 치료를 담당하는 의사들에게 소개하고자 한다.
Background: Patellofemoral pain syndrome is very common knee problem and altered hip rotation may play a role in patellofemoral pain. The purpose of this case study is to describe the manual therapy of and the therapeutic exercise for a patient with asymmetrical hip rotation and patellofemoral pain. Method: The patient was a 29 years old woman with an 3 month history of anterior right knee pain, without known trauma or injury. Prior to intervention, her score on the VAS was Max 6 to Min 4. Left hip internal rotation was less than right hip internal rotation, and manual muscle testing showed weakness of the left hip internal rotator and abductor muscles. The intervention consisted of manual therapy and therapeutic exercise for three times a weeks, two weeks for increasing right hip medial rotation, improving left hip muscle strength, and eliminating anterior right knee pain. Result: After intervention for 2weeks, passive left and right hip medial rotations were symmetrical, and her right hip internal rotator and abductor muscle grades were Good plus. Her VAS score was Max 2 to Min 0. Conclusion: Manual therapy and therapeutic exercise is effective in improving for patient had patellofemoral pain with pattern of asymmetrical hip rotation.
Purpose : The purpose of this study was to understanding of a hemophiliac with hemarthrosis and provided information of rehabilitative therapy. Methods : The General understanding and rehabilitative therapy of a hemophiliac with hemarthrosis considered the matter carefully using a related publications and a internet searching investigation. Results : From the physical therapeutic method for pain alleviation, decreasing edema, ability of muscular strength and control, range of motion(ROM) in joint, prevention of damage on hemorrhaged joint in the hemophiliac with hemarthrosis, rehabilitative therapy for exercise of ROM, muscular strength, proprioceptive sense was most effective for patients. Conclusion : Physical therapists known the rehabilitative therapy should helped a hemophiliac with hemarthrosis to prevent from permanent change.
Objectives : The purpose of this study was to introduce the Chuna Manual Therapy (CMT) using Bong (a type of stick which is called 'bong') as a part of Oriental Medicine. Methods : We searched several traditional methods of CMT using Bong, either individual contact to specialist of CMT using Bong or referred to publications, and summarized briefly for introduction. Authors also made a comparative study between existing CMT and CMT using the bong. Results & Conclusions : The indications of Bong CMT are regarded as acute or chronic pain syndrome, whiplash associated disorders, facet syndrome, vertebral misalignment, chronic fatigue syndrome, obesity and also lower extremity length difference caused by malalignment of vertebrae and pelvic bone. The Meridian Muscle Therapy by pressing down using the Bong can be carried out on the imbalances of the muscle by shortening and lengthening contraction. CMT with Bong is considered more effective than other existing CMT in terms of effectiveness. In the case of pelvic correction which needs a tremendous amount of force, it can reduce the force required effectively. This fact can be inferred by the theory of composition and decomposition of force during the transmission of power. We can perform Bong CMT feeling less fatigued subsequently than general CMT. Pressing down with flexed fingers to grip bong acts on the contraction of flexor digiti and extensor digiti muscle, this protects the $doctor^{\circ}{\emptyset}s$ wrist joints from injury. The bong which acts as a tool between the doctor and the patient, while being given treatment, absorbs and spreads out the direct impact from the patient to the doctor. CMT with Bong is able to apply to both existing massage therapies with the hand.
