뇌졸중 환자의 양${\cdot}$한방 협진에 대한 연구 (A Study on Cooperative Treatment with Both the Western and Oriental Medical Department in C.V.A patients)

  • The primary purpose of this study was to provide the basic materials for C.V.A inpatients' actual use of medical service at the hospital with both the western and oriental medical department and the attitude on cooperative treatment. The results of this study were as follow: First, the subjects' general characteristics including job, monthly mean income and age made statistical differences to their pathological characteristics such as part of primary paralysis, detailed name of disease and cause of elicitation. Second, their general characteristics including religion and job produce statistical difference to their actual use of medical service, like medical institution form, term of treatment and type of medical institution at first-aid. Third, through the awareness of cooperative treatment system, the effect of C.V.A treatment and the shorten of the C.V.A treatment term were higher at oriental medical department inpatients and cooperative treatment serviced inpatients than western medical department inpatients and cooperative treatment non-serviced inpatients. Fourth, the biggest problem on current dual medical system is increase medical expenses and the biggest reason on not vitalized cooperative service is prejudice of both parts.

침 금속재료에 대한 생체적합성 연구 -생물학적 특성을 중심으로- (A Study for Biocompatibility of Acupuncture′s Metal Materials, - Focused on Biological Character -)

  • Objectives : To present criteria for acupuncture standardization, which will improve quality of acupuncture and secure safety, through studies of the biocompatibility of acupuncture materials. Methods : Acupuncture needles distributed in Korea were studied. Intracutaneous reaction tests and sterility tests were executed to study the biocompatibility of acupuncture. Results & Conclusions : All 4 kinds of acupuncture needles distributed in Korea were found suitable for medical use, through intracutaneous reaction tests and sterility tests. However, the medical instrument standards of the Korea Food and Drug Administration lack criteria concerning intracutaneous reaction tests and sterility tests. The criteria for these tests should be standardized in order to secure the biocompatibility of acupuncture needles.

"급유방(及幼方)" 에 기재된 의안(醫案)에 대한 연구(硏究) (A study on the Medical records in the Gupyubang)

  • Objectives : The Medical Records(醫案) are important materials in studying the developmental process of Korean Traditional Medicine. The purpose of this study was to investigate the medical records which were described in Gupyubang(及幼方), the first Korean book that specialized in pediatrics. Methods : 85 Medical records about medical traits of diagnosis and treatments in Gupyubang were analyzed and those were translated in Korean. Results : Medical records were analyzed as follows; The number of Male was 76 and the number of Female was 9. Prepubertal period was 16 and preschool period 14. Acute febrile convulsion was 6. Measles and blood symptom was 5. Ring worm pain, vomiting & diarrhea, colic pain was 4. Most of them were self treatment except one. 77 cases were improved and 6 cases were dead or worse. The medical records used oral medication, external medication, acupuncture, moxibustion and surgical manners. Conclusion : This study showed that Gupyubang is a experiential book and alse the medical records in Gupyubang was usefulness and had practical value.

3T3-L1 세포에서 소맥엽 에탄올추출물의 지질생성 억제효과 (Inhibitory Effect of Triticum aestivum Ethanol Extract on Lipid Accumulation in 3T3-L1 Preadipocytes)

  • Non-alcoholic fatty liver disease is known to be frequently associated with obesity and type 2 diabetes. We examined the effects of EtOH extracts from Triticum aestivum on lipid accumulation during the differentiation of 3T3-L1 preadipocytes to screening the candidate materials in preventing non-alcoholic fatty liver disease. The lipid level in adipocytes was determined by Oil Red O staining. The treatment of 50% ethanol, but not water and 100% ethanol extracts, from Triticum aestivum at concentration of 0.5 $mg/ml$ inhibited lipid accumulation in 3T3-L1 cells, revealing no cell toxicity. Thus, the fractions of $CH_2Cl_2$, EtOAc and BuOH were separated from 50% EtOH extract to characterize anti-adipogenic effect. The $CH_2Cl_2$ fraction at concentration of $50{\mu}g/ml$ effectively inhibited the lipid accumulation in the adipocytes compared to those of EtOAc and BuOH at concentration of $50{\mu}g/ml$. The intracellular triglyceride accumulation also was significantly reduced by treatment of $CH_2Cl_2$ fraction in concentration-dependent manner. Western blot analysis showed that the $CH_2Cl_2$ fraction attenuated the intracelluar level of fatty acid synthase(FAS) accompanied by attenuated expression of Peroxidase proliferator-activated receptor ${\gamma}$ ($PPAR{\gamma}$) adipogenic transcription factor. These results suggest that $CH_2Cl_2$ fraction from 50% EtOH extract of Triticum aestivum may has the potent anti-adipogenic effects by inhibiting the transactivation of $PPAR{\gamma}$.

조선시대(朝鮮時代) 소아경풍(小兒驚風)에 사용된 약재의 변화 (Evolution of "Medicinal Material Usage Targeted at Infantile Convulsion" of the Chosun Dynasty)

  • Infantile convulsion has always been the subject of concern as it is a common yet fatal disease among infants. During the Koryo dynasty and the early years of Chosun dynasty animal and mineral medicinal materials were used to relieve heat. These are replaced with more subtle medicinal materials later on in the Chosun dynasty when differentiation of chronic and acute infantile convulsion first emerged. As such, perception on appropriate medicinal materials is constantly renovated in the course of time.

"동의보감"(東醫寶鑑) 내경편(內景篇)에 수록된 의안에 대한 연구 (Study on the Medical Records in Naekyung-pyun of Dongeuibogam)

  • The Medical records (醫案) are very important materials in studying the developmental process of traditional Korean medicine. In this study, we investigated the medical records which are described in Naekyung-pyun(內景篇) of Dongeuibogam(東醫寶鑑), the most famous medical literature in Korean. There are 50 medical records in Naekyung-pyun and all of these records are cited from 12 Chinese medical literatures. The medical records were mainly cited from medical literatures written in Jin-Yuan(金元) and Ming(明) dynasty such as Yixuegangmu(醫學綱目) and Danxizanyao(丹溪纂要), although most records were written by medical scholars in Jin-Yuan(金元) dynasty, including Zhudanxi(朱丹溪) and Zhangzihe(張子和). Most of records were omitted and/or modified reflecting the Purpose of editors of Dongeuibogam. In addition, medical essay(醫論) was changed into medical record in some cases. The authors of the records used oral medication, acupuncture-moxa, psychological treatment and surgical manners. And in some cases of only having principal of treatment, the editors newly added the herbal prescription. The further study on medical records in Korean traditional medicinal literatures, would reveal the developmental progress of Korean medicine and inform more actual proof on medical condition.

Identification of Angelica Species by Pyrosequencing

  • Objective : Angelica species are some of the most medicinally important materials in Oriental medicine. This study was performed to determine if Angelica species could be identified by genetic analysis and to verify Pyrosequencing analyses, which were used to assess genetic variation. Methods : The DNAs of Angelica acutiloba, Angelica gigas and Angelica sinensis were extracted. We have investigated the typing of single-base variations of Angelica species in DNA by using Pyrosequencing. Results : Angelica gigas showed a different pattern compared with Angelica acutiloba and Angelica sinensis. The peak of Angelica gigas was very weak in the second C nucleotide base compared with that of the others. The peak of Angelica acutiloba was present in the fourth C nucleotide base compared with that of the others. From these results we verified that our Angelica species-specific sequencing primer was well designed. Conclusion : Pyrosequencing analysis might be able to provide the identification of the Angelica species.

소아용 입욕제품의 분석 및 고찰 (Analysis of Baby Bath Preparation)

  • Objectives: The purpose of this study is to analyze the baby bath preparation and provide necessary information on the upcoming herbal bath preparation for atopic dermatitis. Methods: We selected 113 baby bath preparation by searching typing in "baby bath preparation" in 6 major web-search-engines, and 17 web shopping malls in Korea. 11 items were evaluated under three criteria : type of product, function and ingredient of goods. Results: Result showed that the most common type of bath preparation were liquid type. 96% of the products contained medical agents. Ingredients of the medical agents were herbal medicine, aroma oil, spring and sea ingredients, vitamin and extract. 33% of the products were bath preparation for the atopic dermatitis and 74% of the products were only for the baby. Conclusions: It is necessary to make a government level guideline for natural materials used in bath preparation, and to develop new products contained herbal medicine abide by oriental medical theory.

백혈병의 임상관찰 및 변증에 대한 연구 (Clinical Observation and Diagnosis in Leukemia)

  • Leukemia is a blood disease that occurs in the abnormal process of leukocyte maturation. Its main medical treatment is chemical therapy and bone medical transplant. But its treatments give rise to ill effects and sequela. At present, the cause of leukemia is not fully understood. But oriental concept of curing leukemia is the holistic thinking that emphasizes the unity, wholeness, and the relationship between the human organs, including western medical concept of blood cause. So it is estimated that holistic medical treatment in oriental medicine could present new medical way of curing leukemia. I read over the diary on struggles against leukemia, survey on the leukemia patients, and, medical charts in the oriental clinics that treated in the holistic way, and I interviewed the leukemia patients. With these materials, I classified the early main attacking symptoms according to the chief complaints of one hundred leukemia patients. With these results collected, I present new alternative treatment by oriental medical diagnosis. The chief early complaints that leukemia patients give are fatigue, enervation, cold, contusion, fever, high fever, sweating at sleep, myalgia, arthralgia, and dizziness, in order of main symptoms. Fatigue occurs mainly by spleen and stomach weakness and marrow shortage. So it is estimated that It is important to treat the spleen that is responsible for supplementing the marrow. Because the leukemia patients have anemia and hemorrhage, to treat the spleen is important for hematopoiesis and controlling blood. In case of cold, it penetrates into the body when the body is weak. So its treatment is to increase body's health. But the cause of fever is difficult to classify into outer cause and inner cause. But in case of children under 14 years old, fever is the main sypmptom. I think this is because children have the body with pure vital energy. Hemorrhage is thought to be the result of yin-lack and heat-miasma of spleen and stomach channels. Contusion occurs from the qi-weakness and the not-controlling-blood. Sweating at sleep is from the yin-weakness. It is found with all weak symptoms. Dizziness is from the yin-blood impairment. Weight-loss is from the marrow shortage. Myalgia and arthralgia is mainly from inner weakness, not outer maisma. Most leukemia patients have the idea that holistic treatment of leukemia could be of assistance and give help to the low immunity. So it is expected that holistic medical treatment could contribute to knowing the cause and treatment of leukemia, and give people reliability on oriental medical treatment, through the profound diagnosis of leukemia.

아토피 피부염 증례를 이용한 표준화환자 프로그램 연구 (Study of Standardized Patient Program Using Case Report of Atopic Dermatitis)

  • Objectives: The purpose of this study was to design a standardized questionnaire for patients with atopic dermatitis in oriental dermatology. Methods: We selected atopic dermatitis from the evaluation of students' satisfaction conducted by the Department of Oriental Dermatology, Dong-guk University College of Oriental Medicine in 2011. To design the questionnaire for atopic dermatitis, we made up descriptive materials consisting of history of the present illness, patient risk factors, past medical history, personal history, and physical examination. Results: Standardized patients (SP) were used in teaching and practicing interviewing and physical examination skills for students. The SP offers a readily available tool for teaching and evaluating the clinical performance and interpersonal skills of medical students. However, the SP program needs to be more thoroughly researched and developed. Research of pattern identification in atopic dermatitis is needed to design the questionnaire. Conclusions: Using an SP program is strongly recommended to teach and evaluate in oriental medicine clinical clerkships.
