The General Characteristics and Risk Factors Study According to Onset Time in Acute Stroke Patients (급성기 중풍환자의 발병시간에 따른 특성 및 위험인자 연구)

  • Objectives : The aim of this study was to assess the distribution of general characteristics and risk factors according to onset time in acute stroke patients. Methods : We studied hospitalized patients within 4 wks after the onset of stroke who were admitted to the Department of Internal Medicine at Kyunghee University Oriental Medical Hospital, Kyunghee University East-West Neo Medical Hospital, Kyungwon University Songpa Oriental Medical Hospital, Kyungwon University Incheon Oriental Medical Hospital, and Dongguk University Ilsan Oriental Medical Hospital from April 2007 to August 2008. The 24-hour day was divided into quartiles (6:00 to 12:00, 12:00 to 18:00, 18:00 to 24:00 and 24:00 to 6:00). We analyzed the general characteristics and risk factors according to onset time in acute stroke patients. Results : 468 subjects were included into the final analysis. 1. The most common time period for ischemic stroke onset was between 6:00 and 12:00. It demonstrated a 50% excess risk between 6:00 and 12:00. 2. On the demographic variables of the patients, the rate of hypertension was significantly higher from 18: 00 to 24: 00 than 24:00 to 6:00. 3. There was no significant difference of stroke type, Sasang constitution, or lifestyle between times. 4. According to the blood test, high density lipoprotein cholesterol was significantly higher at 24:00 to 6:00 and lower at 12:00 to 18:00 than other time-groups. 5. The patients diagnosed with metabolic syndrome were significantly more at 12:00 to 18:00 and fewer at 24:00 to 6:00. 6. Hypertension and coffee drinking showed close relationship with patients whose stroke occurred while awake in multivariate analysis. Conclusions : The above results showed that the overall tendency of acute stage cerebral infarction patients varied according to their onset time. These results can be utilized in the future as a basis material.

  • Objectives : The aim of this study was to examine the Differences of symptom improvement depending on correspondance of Herb medicine with Oriental Medical Diagnosis in Acute Stroke Patient. Methods : We studied hospitalized patients within 10days after their ictus who were admitted at Kyunghee University Oriental Medical Center from May 2011 to October 2011. We compared the improvement of MI and SSS score between Correspondance group(n=9) and Non-correspondance group(n=13). Results : The patients who had corresponding herb medicine with Oriental Medical Diagnosis show much improvement of MI score, but there is no significant difference. There is no significant difference on improvement of SSS score, between Correspondance and Non-correspondance. Conclusion : This study provides evidence that correspondance of herb medicine with Oriental Medical Diagnosis is important to improve the symptoms of acute stroke.

  • Few studies have reported the effects of traditional Korean medical (TKM) therapy in patients with traumatic brain hemorrhage. The patient was a 93-year-old female who was unconscious after a traumatic intracerebral hemorrhage (ICH) and was not a candidate for any surgical treatment because of her age. We treated her with acupuncture and herbal medicine. After 28 days of treatment, we observed an improvement in consciousness on the Glasgow Coma Scale (GCS) and in other symptoms (aphasia, weakness in both legs, and headache).

  • Objectives: To report a case of a patient with a mild depressive episode accompanied by physical symptoms such as hyposmia visiting a Korean medicine hospital who was treated with oriental psychotherapy in parallel with existentialism-based psychotherapy showing improvement. Methods: For this patient, Korean medicine Psychotherapy was mainly used for treatment. In parallel, existential Psychotherapy, acupuncture treatment, and self-relaxation training were implemented. For estimating this treatment, changes in results of Beck Depression Index (BDI), Beck Anxiety Index (BAI), Numeric Rating Scale (NRS), Korean Version of Sniffin' Sticks Test (KVSS), Butanol Threshold Test (BTT), Cross-Cultural Smell Identification Test (CC-SIT), and somatic symptoms that the patient subjectively expressed were determined. Results: After the comprehensive treatment, it was confirmed that changes in BDI, BAI, KVSS, BTT, CC-SIT, NRS, and subjective somatic symptoms expressed by the patient all showed improvements. Conclusions: We propose that a comprehensive treatment using Korean medicines Psychotherapy in parallel with existential psychotherapy can be used for patients who complain of physical symptoms accompanied by depression and hyposmia.

Clinical Analysis of Patients Who Visited the Emergency Room of an Oriental Medical Center (한의과대학 부속 한방병원 한방응급실 내원환자에 대한 임상적 분석)

  • Objectives : By recording basic data of patients visiting the emergency room of the oriental medicine center, we can understand their characteristics and gain better insight about them. Methods : We performed a retrospective study of 2,674 patients who visited the emergency room of Kyunghee Oriental Medical Center, from June 1, 2006 to May 31, 2007. Results : 1. Of 2,674 total patients, the male to female ratio was 0.94:1 and the 6th decade of life was the peak age group. 2. The majority of patients visited our emergency room between 06:00 and 24:00(midnight). 3. Most patients visited on a Saturday or Sunday. 4. By monthly distribution, the patients increased slightly during June, July, August and October. 5. The majority of patients visited the emergency room within 24 hours of onset. 6. The patients were categorized as follows: neurological diseases 71.15%, musculoskeletal diseases 16.02%, internal medical disease 9.04%, psychiatric and related disease 3.79%. 7. The average transit time was about 1 hour 30 minutes. 8. Admission rate was 48% and the rate of admission to the intensive care unit (ICU) was 5% of total visiting patients. Conclusion : Assuming that this research will continue adding patient's data continuously, this work will help us to understand the features of patients who visiting the emergency room of the oriental medical center.

A Comparative Study between the Korean-medical and the Western-medical Psychotherapy on PTSD (외상 후 스트레스 장애(PTSD)의 정신치료에 대한 양·한방 비교 고찰)

  • Objectives: The purpose of this study is to compare the Korean-medical and the Western-medical psychotherapy on PTSD for improving the Korean-medical psychotherapy on this issue. Methods: We surveyed recent articles relating to psychotherapy on the incidence of PTSD. Results: Under the circumstances of this study, we note that applying the qigong therapeutics would be helpful when conducting relaxation training on the identified PTSD patients. We understand that taking a systematic desensitization style in the use of the kyungja-pyungji treatment is recommended. It would also be more effective to combine other treatments such as the use of the I-Jeong-Byeon-Gi Therapy and the five minds mutual restriction therapy. We have shown that by using the five minds mutual restriction therapy for the imagery rescripting has a better a synergy effect for optimum patient outcomes for patients who have been diagnosed with PTSD. Conclusions: Our study has shown that by improving the shortcomings of the Korean psychotherapy and utilizing Western psychotherapy's merits, we expect to increase the efficacy of the Korean psychotherapy with these recommendations for changes in this case.

  • Background : To understand Korean medicine it is crucial to first understand medical aspect of 'DongUiBoGam'. It is also meaningful that it became one of the influential book even to posterity. Also based on the apprehension, we can take a view of development of Korean medicine led by 'DongUiBoGam'. Objectives : This study aims to review the status, study field, specialist of DongUiBoGam. In the process, this review will grasp trends in this field of studies and will direct further researches into the right direction. Method : The computerized Korean databases were searched from their respective inceptions up to December 2008. The search terms used were 'DongUiBoGam' and random or Korean language terms related to DongUiBoGam. Several specialized journals were also manually searched for relevant articles. Result : Since the 2000s, DongUiBoGam papers in the Korean Literature is increased. Published 58 papers on The Korean Journal of Oriental Medical Prescription were more than the other journals. 58 papers published in The Korean Journal of Oriental Medical Prescription are the best in many areas related to Korean medicine. Most people have submitted papers related to DongUiBoGam was Woo-yeal Jeong. Conclusions : 'DongUiBoGam' research is conducted and current tendency and outlook for 'DongUiBoGam' is carried out focusing on several associations.

Cytoprotective Effects of Artemisia princeps Extract through Inhibition of Mitochondrial Dysfunction (애엽(艾葉)의 미토콘드리아 보호 효과)

  • Objectives : Artemisia princeps is used as moxa in moxibustion and traditional herbal medicine. And its extracts or compounds is known to have an efficacy of antioxidant, anti-diabete, anti-cancer, anti-inflammation and neuroprotection. This study was performed to investigate the cytoprotective effect of Artemisia princeps extract (APE) against arachidonic acid (AA)+iron-induced oxidative stress on HepG2 cell. Methods : The effects of APE on cell viability has been assessed using MTT assay. And flow cytometric analysis was performed to estimate APE's effects on mitochondrial function. To investigate its underlying mechanism, related protein was analysed by using immunoblot analysis. Results : Treatment of APE increased relative cell viability, prevented a decline of B-cell lymphoma-extra large (Bcl-xL) and cleavage of poly(ADP-ribose) polymerase (PARP) and procaspase-3, and also protected mitochondrial membrane permeability (MMP) against oxidative stress induced by AA+iron. In addition, APE treatment increased phosphorylation of AMP-activated protein kinase (AMPK) exerts a cytoprotective effect. Conclusions : This results demonstrate that APE has an ability to activation of AMPK which protects cells from AA+iron-induced oxidative stress and restores MMP.

  • 1. Objective : This study aims to report symptomatic improvement of a Soyangin patient reporting general weakness, dizziness, gait disturbance, cold sensitivity, nocturnal sweating, and anorexia after treatment with Hyeongbangjihwang-tang. 2. Method : The patient's subjective and objective symptoms were observed daily, and the VAS scores for the main symptoms were recorded daily throughout the hospitalization period. 3. Result : The general condition of the patient improved by the end of the hospitalization period, where the VAS score dropped from VAS 10 to VAS 5 and the symptoms of dizziness, general weakness, cold sensitivity, and nocturnal sweating disappeared by day 4 of admission without recurrence. 4. Conclusion : Although the patient did not report diarrhea, one of the main Mangeum symptoms, the general symptoms improved with Hyeongbangjihwang-tang medication. This indicates that Hyeongbangjihwang-tang can be used to treat chief and subsidiary symptoms related to Mangeum symptomatic pattern when applied to the appropriate base and manifesting symptomology.

Clinical Study of Oriental Medical Treatments for Neonates in Oriental Medicine Hospital Postpartum Care Center (한방병원 산후조리센터 내 신생아의 한방적 처치에 대한 임상적 고찰)

  • Objective : The aim of this study is to investigate the effects of oriental medical treatments for neonates in the Postpartum care center. Methods : This clinical study was carried out with the 186 neonates who admitted to the Postpartum Care Center of Conmaul Oriental Medicine Hospital, from November, 2003 to May, 2004. The data of sex, delivery method, gestation age, birthweight, symptoms, herbal prescription, improved condition and other treatments was collected via the medical charts. Results : Among 186 newborn babies, 48 cases had diarrhea. We used Gwakhyangjeonggi-san for 31 cases, Bulwhangumjeongnggi-san for 5 cases, compound prescription of the two for 8 cases. The effect of treatment was excellent for 25 cases, good for 11 cases, not good for 8 cases. In fever, there were 39 cases. We used Gwakhyangjeonggi-san and venesection of blood for 17 cases, only venesection of blood for 20 cases, and only Gwakhyangjeongg-isan for 3 cases. The effect of treatment was excellent for 37 cases, good for 3 cases. In cold, there were 4 cases. We used Samsoum for 1 case, Socheongryong-tang for 3 cases. The effect of treatment was excellent for 2 cases, good for 1 case, not good for 1 case. Conclusions : This study shows oriental medical treatments like acupucture, herbal medicine, and venesection of blood for neonates have significant effects. we will need further prospective studies about effects of these oriental medical treatments.
