A Survey on Parent's Recognition and Retrospective Study on Effect of Herbal Medication (한방 소아과에 내원한 환아 보호자 일반적인 인식도 조사와 한약치료후의 반응에 대한 후향적 연구)

  • Objective : In this study we aimed to· investigate the parental recognition on Herbal Medication. The result showed that parents concerned about the effect of Herbal Medication on liver function and gaining weight. Therefore, we had a retrospective study on the effect of Herbal Medicaion on liver function and gaining weight. Method : In parent's recognition study, we surveyed 354 parents of children who make a first visit to Department of Oriental Pediatrics in ${\bigcirc}{\bigcirc}$ Medical Center from 3 October to 2 November 2005. In retrospective study, we examined medical record of 22 children who visited to Department of Oriental Pediatrics in Kyunghee Medical Center from 30 September 2005 to 1 January 2005. Height, weight, BMI, body fat mass, body fat ratio, AST and ALT were estimated at two points; before and after Herbal Medicaion treatment. Result: The mean of expectation of parents on the efficacy of Herbal Medication was 75.14 on the VAS scale, which was interpreted as significantly high. The result showed the effect of Herbal Medicaion on liver function(48%) and gaining weight(53%) took the largest portion. When we observe the effect of Herbal Medication on Liver function, after Herbal Medication treatment we have slightly increased AST and slightly decreased ALT, but they were not statistically significant. Also we observe the effect of Herbal Medication on getting obesity after Herbal Medication treatment. The result showed that we have slightly decreased BMI, body fat mass, body fat ratio, but they were not statistical1y significant. Conclusion : Our report shows parents of children have high expectation and stand favorable on the efficacy of the Herbal Medication. We found that parents concerned the most about the effect of Herbal Medicaion on liver function and gaining weight. Our retrospective study showed that Herbal Medicaion treatment on children has relationship with neither liver function nor the degree of obesity.

Modulation of obesity by Gyeongshingangjeehwan18 in ob/ob mice (Ob/Ob 마우스에서 경신강지환(輕身降脂丸)18의 비만조절)

  • Objectives : This study was undertaken to verify the effects of Gyeongshingangjeehwan18 (GGEx18) on obesity using ob/ob male mice. Methods : Eight-week old mice (wild-type C57BL/6J and ob/ob) were used for all experiments. Wild-type C57BL/6J mice were used as lean control and obese ob/ob mice were randomly divided into 5 groups: obese control, GGEx15, GGEx16, GGEx17, and GGEx18. After mice were treated with several kinds of GGEx for 11 weeks, body weight gain, feeding efficiency ratio, plasma lipid and glucose metabolism. Results : 1. Compared with obese controls, GGEx-treated mice had lower body weight gain and feeding efficiency ratio, the magnitudes of which were prominent in GGEx16 and GGEx18. 2. Consistent with their effects on body weight gain, GGEx16 and GGEx18 not only decreased plasma triglycerides levels, but also increased HDL-cholesterol concentration. 3. CT analysis revealed that visceral fat areas were decreased in all treatment groups compared with obese control mice. The decrease in visceral fat area was prominent in GGEx16 and GGEx18, although they were not statistically significant. 4. The size of adipocytes were significantly decreased by GGEx18, whereas the adipocyte number per unit area was significantly increased, suggesting that GGEx18 decreased the number of large adipocytes. Hepatic lipid accumulation was decreased by GGEx16 and GGEx18, and the inhibitory effect was most effective in GGEx18. 5. Plasma GOT and GPT concentrations were significantly lower following GGEx16 and GGEx18 treatment compared with obese controls. Organ weights were not changed by GGEx treatment, indicating GGEx do not show any toxic effects. Conclusions : These results suggest that GGEx may regulate obesity. Of the 4 compositions, GGEx18 seems to be most effective in improving obesity and lipid disorders.

Effects of Low Calorie Diet and Platycodon Grandiflorum Extract on Fatty Acid Binding Protein Expression in Rats with Diet-induced Obesity

  • Obesity can be defined as a metabolic disease due to an increased fat accumulation in the body caused by an imbalance of calorie intake and output The prevalence of obesity has increased substantially over the past 2-3 decades in developed and developing countries. The health impact of weight gain is so marked that obesity has now been classified as a major global public health problem In order to investigate the effect of diet conversion and oral administration of Platycodon grandiflorum extracts on the treatment of obesity, male Spraque-Dawley rats were divided into four groups: a group converted to normal diet (Control group), a group maintained high fat (30%) diet (H), and two groups with Platycodon grandiflorum extract added to the previously mentioned two groups. All animals were fed high fat diet for 7 weeks to induce the obesity. Then they were divided as mentioned above. Animals were fed experimental diet and Platycodon grandiflorum extract (150 mg/ml/rat/day) for 7 weeks. Body weight, adipose tissue weight (subcutaneous, epididymal, peritoneal fat pads) and serum lipids (total cholesterol and triglyceride) showed some differences among groups. The Platycodon grandiflorum feeding markedly decreased both body weight and adipose tissue weight in control group compared to H, high fat diet maintaining, group. Platycodon grandiflorum extracts significantly decreased the concentrations of serum lipids compared to H group. Fat cell numbers and sizes were significantly reduced in the oriental medicinal herb extract administrated group. Increased fatty acid binding protein (FABP) expression in high fat diet group was decreased by the dietary conversion to normal diet and the oral administration of Platycodon glandiflorum extracts. In contrast, there was no significant effect on FABP expression in the high fat maintenance group. In this study, the conversion from high fat diet to low fat or normal diet had a beneficial effect on body weight loss and serum lipid profiles. Dietary Platycodon glandiflorum extracts had an additive beneficial effect on the prevention and treatment of obesity.

The Inhibitory Effects of Sansayukbokhap-bang(SSYBHB) on the Obese-Mouse Induced High Fat Diet (산사육복합방(山査肉複合方)이 생쥐의 비만억제에 미치는 영향)

  • Objectives In order to investigate the effects of Sansayukbokhap-bang (SSYBHB) on the hematological and histological changes. Methods C57BL/6 mice were fed with high fat diet. C57BL/6 mice were divided into four groups and fed for 15weeks. Results 1. The body weight of SSYBHB intake mice was significantly lower than high fat diet group. 2. The final increase of body weight was decreased significantly. 3. The levels of ALT, AST, total cholesterol, LDL-Cholesterol, triglyceride, Leptin were decreased significantly. 4. The levels of creatinine were decreased but did not show significance. 5. The level of HDL-cholesterol and the expression of ${\beta}$3AR mRNA gene in 3T3-L1 Adipocytes were increased significantly. 6. Adipocytes' size was decreased significantly. 7. The expression of ${\beta}$3AR mRNA gene, Leptin mRNA gene and serotinin mRNA gene in Adipocytes tissue was decreased significantly. Conclusion Based on these results, it is proved that SSYBHB is effective on the therapy of obesity by referring to obese-gene and obese inhibitory. So, it is espected that the clinical application of SSYBHB can help the treatment of obesity.

The comparative study on body shape perception, obesity and weight control between obese outpatients and general population in $20{\sim}30$ age bracket (2,30대 여성을 대상으로 일반인과 비만치료를 위해 내원한 환자간의 체형인식과 비만도 및 체중조절실태 비교 연구)

  • Purpose : This study was carried out to compare actual obesity rate, body shape perception and weight control between outpatients visiting obesity center and general population in the $20{\sim}30$ age bracket. Methods : The respondents were 72 females including 38 outpatients and 34 general population in the $20{\sim}30$ age bracket. We compared the results of the questionnaires about body shape, obesity and weight control between two groups. Data were analyzed by Student T-test and Chi-square test. Results : In questionnaire survey results, the average BMI, obesity rate, body shape distortion of the respondents who have normal weight, concern for weight control in outpatients group were significantly higher than general population group. Satisfaction on weight control in distorted group was lower than undistorted group. Conclusion : A distortion of the body shape caused unsatisfaction on weight control and it was significantly high in outpatients group, especially among the respondents who have normal weight. It is considered that the distortion of the body shape should be corrected by counselling for proper treatment.

Analysis of Safety of Mahuang in Studies for Treatment of Obesity (비만 치료 연구에서 마황의 안전성 분석)

  • Objectives: The purpose of this study is to analyze the adverse events and safety of Mahuang used as anti-obesity treatment in Korean medicine. Methods: PubMed, EMBASE, Cochrane, KoreaMed, Oriental Medicine Advanced Searching Integrated System (OASIS) and SCIENCE-ON were used as searching for randomized controlled trials investigating the safety of Mahuang for treating obesity from January, 2000 to July, 2021. Results: Total 6 studies were included for analyzing. In most studies, heart rate was higher in experimental group than in placebo group with statistically significant difference. Both systolic and diastolic blood pressure had no significant differences between both groups, but systolic blood pressure got lower in experimental group. Most adverse events reported in experimental group were dry mouth, constipation, diarrhea, palpitation and insomnia, but serious adverse events were not reported. Conclusions: This study provides the evidence about the safety of Mahuang for treating obesity. However, increase of heart rate and minor adverse events could be occurred. Further study is needed to confirm the safety of Mahuang.

The Effect of Education Program on Weight Control in Taeumin Childhood Obesity (태음인 비만아를 위한 체중조절 프로그램의 실시효과)

  • Objective; Childhood obesity has become one of the most common health problems facing children in the world. Although a number of regimens have been developed and distributed for the treatment of adult obesity, few studies have focused on therapeutic programs for obese children. The objective of this study was to develop and measure the effects of Taeumin childhood obesity by a sasang constitution­based education program. Methods ; A total of 14$(\%RBW\geq20)$ among 18 taeumin with childhood obesity from June to August 2003 participated in the constitution-based education program. It was designed to examine the effects of a constitution-based education program through anthropometric measurements, body composition indicates, biochemical indicates and serum level of taeumin childhood obesity. Results; The results of this study were as follows: 1. The anthropometric measurement of subscapular skinfold thickness was reduced after 4 weeks of the constitution-based education program. 2. The body composition indicates reduction in degree of obesity, percent body fat, and fat distribution. The other side increased in height, total body water, soft lean mass, and lean body mass. 3. For biochemical indicates, HDL-cholesterol was increased significantly after 4 weeks of the constitution-based education program. The other side decreased in AST and ALT. 4. There was a significant reduction in BMI and $\%RBW$ after the constitution-based education program. Conclusion : A constitution-based education program for weight control results in reduced degree of obesity among taeumin children.

The Effects of Zymolysis-Dietotherapy with Herbal medicine on Obesity in Sasang Constitution (발효식이요법과 한약의 병행치료가 사상체질별 비만치료에 미치는 영향)

  • Obhectives : The purposes of this study are to evaluate the effect of zymolysis-dietotherapy with herbal medicine on the obesity in Sasang constitution Methods : Seventy five patients were classified into four obesity groups by Body Mass Index, and four Sasang constitution groups by QSCC. Weight, body fat, body fat rate, BMI were compared in each group. Results : Weight, body fat, body fat rate, BMI were decreased significantly in all groups. There was statistically significant difference in the loss of weight, BMI between Taeumin and others. Decrement of body fat rate is not statistically significant between Taeumin and others. Conclusions : In conclusion, it was proved that zymolysis-dietotherapy with herbal medication is effective on the obesity treatment of all Sasang constitution groups. And the group of Taeumin lose more weight.

Effect of Ganjeonggyeok Acupuncture and Saengkankunbi-tang Treatment in the Changes of LFT and Lipid-obesity Index Values (간정격 시술 및 생간건비탕 투여가 간기능 및 비만도 변화에 미치는 영향)

  • Objectives : This study was aimed to figure out how effectively acupuncture treatment and herb-medicine work on patients with elevation of LFT value and lipid-obesity index. Methods : The subject were 23 patients visited to Dept. of Acupuncture and Moxibustion, Kwangdong Oriental Medicine Hospital, who complained chronic fatigue and wandering pain from Oct. 2007. to Oct. 2008. Before treatments, they were tested for LFT, total cholesterol, triglyceride, body fat ratio and obesity index. They were treated with Ganjeonggyeok acupuncture weekly for 1 month. During acupuncture treatments, we applied Sangkankunbi-tang to all patients twice a day. After 1 month, they were retested for LFT, total cholesterol, triglyceride, body fat ratio and obesity index. Results : 1. After treatments, we could get the result of dropping means of AST, ALT and $\gamma$-GTP value. Statistical significance of ALT and $\gamma$-GTP were recognized before and after treatments, but AST was not recognized. 2. After treatments, we also could get the result of dropping means of total cholesterol, triglyceride, body fat ratio and obesity index. Statistical significance of all values were recognized before and after treatments. 3. Among 23 patients, 10 cases were reported that they were not within normal limits at least one in AST, ALT and $\gamma$-GTP. After treatments of those patients, we could get the result of dropping means of AST, ALT and $\gamma$-GTP value. Statistical significance of all values were recognized before and after treatments. 4. Among 23 patients, 19 cases were reported that they were not within normal limits at least one in total cholesterol, triglyceride, body fat ratio and obesity index. After treatments of those patients, we could get the result of dropping means of total cholesterol, triglyceride, body fat ratio and obesity index. Statistical significance of all values were recognized before and after treatments. Conclusions : Though further study is necessary, our finding suggest that the conduction of Ganjeonggyeok acupuncture and Sangkankunbi-tang treatments have helped to decrease the figures of LFT and lipid-obesity index in patients.
