A Case Report on the improvement of Range of Motion and Pain Relief for Patients Diagnosed with Supraspinatus Tendinosis, Subacromial Bursitis and Subdeltoid Bursitis treated with Megadose Shinbaro Pharmacopuncture (극상근건염과 견봉하점액낭염 및 심각근하점액낭염으로 진단받은 견비통 환자를 대상으로 대용량 신바로 약침치료를 통한 통증경감과 가동범위 호전에 대한 증례보고)

  • Objectives : The purpose of this research is to show the effectiveness of the Korean medical treatment on patients diagnosed with supraspinatus tendinosis, subacromial bursitis and subdeltoid bursitis treated by Korean medical treatment Including megadose shinbaro pharmacopuncture. Methods : We used megadose shinbaro pharmacopuncture on patients who received treatment at Bucheon Jaseng Korean medicine hospital from December, 2016 to January, 2017. Also we checked NRS, SPADI, range of motion(ROM) and shoulder physical examination to follow the scale showing the improvement of the symptoms of the patients. Results : All 4 patientes showed the improvement of NRS, SPADI range of motion(ROM) and shoulder physical examination. Conclusion : This research showed that megadose shinbaro pharmacopuncture was effective on the treatment of shoulder pain patients diagnosed with supraspinatus tendinosis, subacromial bursitis and subdeltoid bursitis.

Effect of Simple Acupuncture and Electroacupuncture at Bai Hui($GV_{20}$) on Heart Rate Variability in Healthy Adults with Stress Task (백회(百會)($GV_{20}$) 단순 자침과 전침 자극이 정신적 스트레스를 가한 정상 성인의 심박변이도에 미치는 영향 비교)

  • Objectives : This study was performed to investigate the effect of simple acupuncture and electroacupuncture at Bai Hui($GV_{20}$) on heart rate variability in healthy adults with stress task. Methods : 40 healthy volunteers participated in this study. HRV was recorded before and after stress. And then simple acupuncture(SA group) was applied at $GV_{20}$ or electroacupuncture(EA group) was applied at the same spot in each treatment group for 20 minutes. No treatment was performed in control group for 20 minutes. Then HRV was recorded. Results : 1. In all groups, LF norm, LF/HF, HF norm showed significant changes after mental stress. 2. Control group showed no significant change. In SA group, HF norm LF, LF norm and LF/HF showed a significant change after treatment. In EA group, Mean HR, HF, LF norm, HF norm and LF/HF showed a significant change after treatment. In both treatment groups, there were significant differences after treatment compared to control group. 3. In EA group, HF and HF norm decreased significantly after treatment compared to SA group. Conclusions : These results suggest that simple acupuncture and electroacupuncture at $GV_{20}$ affect the balance of the autonomic nervous system and that electroacupuncture at $GV_{20}$ enhances parasympathetic activation more than simple acupuncture.

Preliminary Study to Develop the Instrument on Pattern Identifications Knee Osteoarthritis (퇴행성무릎관절염 변증도구 개발에 관한 기초연구)

  • Objectives The aim of this study was to develop a standard tool of pattern identification for Knee Osteoarthritis, which will be applied to clinical research. Methods The advisor committee for this study was organized by 11 panel of experts (Korean Rehabilitation Medicine professors, Acupuncture and Moxibustion professors belonging to Korean Medicine colleges, Principal Researcher of Korea Institute of Oriental Medicine, Doctor of Korean medicine). The pattern identifications and symptoms for this tool were extracted from published Korean and Chinese literature. Through the discussion among internal experts and consultation from advisors, the Instrument on Pattern Identifications for Knee Osteoarthritis was developed. Results 1) Five pattern identifications (The Wind, Chill, and Moisture, The Moist-Heat, Blood Stasis, Yang Deficiency of Spleen and Kidney, Yin Deficiency of Liver and Kidney) were set for the tool. 2) The mean weights which represent the importance of each symptom and scored on a hundred-point scale was obtained. 3) The Instrument on Pattern Identifications for Knee Osteoarthritis was designed in the self-reporting format composed of 46 questions. Conclusions The Instrument on Pattern Identifications for Knee Osteoarthritis was created through this study. Though this study is not proved about validity, reliability, the instrument of pattern identification for Knee Osteoarthritis is meaningful and expected to be applied to the subsequent.

  • Objective The purpose of this study was to investigate clinical studies between 2015 and 2020 on the traditional Chinese medicine treatment for carpal tunnel syndrome (CTS) using the China National Knowledge Infrastructure (CNKI) database. Methods We searched clinical studies on the Chinese medicine treatment published in traditional Chinese medical journals on CTS through the CNKI search. Fifteen published studies between 2015 and 2020 were chosen and examined for the year of publication, sample size, study design type, diagnosis and evaluation of diverse criteria, treatment period, purpose of the study, and classification based on the treatment technique. Results Out of 73 studies, 15 had practiced various Chinese medical treatments, such as acupuncture, moxibustion, manipulation, herbal medicine application therapy, and external herbal treatment. Conclusions Various studies have investigated the Chinese medicine treatment for CTS in China. To envelope the treatment of CTS in Korean medicine, steady and valuable therapies for high-level clinical research are required. In the future, this study could be helpful in the development of the treatment of CTS with Korean medicine.

  • Objectives : To report cases of HIVD of L-Spine successfully treated with Korean medical treatment including Samul-tang(Siwu-tang) Gagam. and Chuna manual treatment. Methods : Three patients diagnosed with HIVD of L-Spine were treated with Korean medical treatment including Samul-tang(Siwu-tang) Gagam and Chuna manual treatment. Numeric Rating Scale(NRS), Oswestry low back pain Disablility Index(ODI) scores and EQ-5D were collected before and after treatment for comparison. Results : There was a significant decrease in NRS, ODI scores.And, there was a significant increase EQ-5D. The mean NRS score decresed from 10 to 1.33, ODI from 66.66 to 8.88. Tne mean EQ-5D score incresed from 0.225 to 0.856. Conclusion : The result suggests Korean medical treatment including Samul-tang(Siwu-tang) Gagam and Chuna manual treatment can be an effective treatment on patients with HIVD of L-Spine.

Reliability and Validity Analysis of an Instrument for Pattern Identification and Evaluation in Chemotherapy-Induced Peripheral Neuropathy (항암제 유발 말초신경병증 변증평가도구의 신뢰도 및 타당도 평가)

  • Objectives: The purpose of this study was to evaluate the reliability and validity of an instrument for pattern identification and evaluation in chemotherapy-induced peripheral neuropathy (CIPN).Methods: The study consisted of 66 patients with CIPN (visual analog scale ≥30 mm, Eastern Cooperative Oncology Group ≤2). The test-retest reliability of the instrument for pattern identification and evaluation in CIPN was tested twice with the same patients in one week.Results: Pearson’s coefficient test-retest reliability of the instrument was 0.601-0.777. The internal reliability of each part was 0.619-0.811. To confirm the validity of the instrument for pattern identification and evaluation for CIPN, the correlation between the score for each identification pattern and that of other CIPN instruments was analyzed. The results revealed a positive correlation between the Functional Assessment of Cancer Therapy-General and CIPN-20.Conclusion: The reliability of the instrument for pattern identification and evaluation in CIPN was moderate, and the validity analysis revealed a positive correlation.

Efficacy and Safety of the Antidiabetic effect of Hwangryunhaedok-tang for Type 2 Diabetes Mellitus Patients without Complications: A Systematic Review and Meta-Analysis (합병증을 동반하지 않은 2형 당뇨병 환자에 대한 황련해독탕의 혈당 강하 효과에 대한 유효성과 안전성 : 체계적 문헌 고찰과 메타분석)

  • Objectives: The purpose of this review is to assess the efficacy and safety of Hwangryunhaedok-tang for type 2 diabetes without complications by systemic review and meta-analysis of randomized controlled trials (RCTs). Methods: We searched the following up to March 31. 2021: PubMed, EMBASE, The Cochrane Library, CNKI, CiNii, KISS, KMBASE, OASIS, ScienceON. Data extraction and assessment of Cochrane's risk of bias (RoB) tool were performed by two independent authors, and if there was disagreement between two researchers, it was resolved through the intervention of a third researcher. Results: A total of seven trials are included in this systemic review. The treatment group (Hwangryunhaedok-tang plus conventional treatment) showed more statistically significant effects than did the control group (conventional treatment only) in fasting plasma glucose level, 2-hour postprandial plasma glucose level, glycated hemoglobin A1c, fasting insulin level, and HOMA-IR. There was no significant difference between the treatment group (Hwangryunhaedok-tang alone) and the control group (metformin alone). Conclusion: Hwangryunhaedok-tang might have efficacy and safety in controlling blood sugar level and improving insulin-resistance. However, the number of studies included in the meta-analysis was insufficient, and the Hwangryunhaedok-tang used in the included studies was not standardized. Also, the quality of the involved studies was generally low. Therefore, further studies are needed to determine the efficacy and safety of Hwangryunhaedok-tang treatment for type 2 diabetes mellitus.

A Retrospective Chart Review of 122 Inpatients with Knee Osteoarthritis Treated with Korean Medicine: An Analysis of the Effects of Treatment

  • Background: Korean medicine treatment was assessed in patients with knee osteoarthritis (OA) according to subgroups of: sex, age, cause of knee OA, body mass index, hospitalization period, history, OA compartment, phenotype, and comorbidity. Methods: A retrospective review was performed of 122 inpatients who were admitted to the Hospital of Korean Medicine for Korean medicine treatment of knee pain, and were diagnosed with knee OA based on magnetic resonance imaging findings. Analysis of patient subgroups (sex, age, cause of knee OA, body mass index, hospitalization period, history, OA compartment, phenotype, and comorbidity) was carried out and treatments including acupuncture, cupping, pharmacopuncture, herbal medicine, chuna therapy, medicinal steaming therapy, manual therapy, and extracorporeal shock wave therapy were listed. The numeric rating scale (NRS), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), and 5-level EuroQol- 5 Dimension (EQ-5D-5L) scores were measured before and after treatment to assess the effects of treatment on pain and quality of life. Results: Seventeen males and 105 females were included in this study. Most patients were in their 60s. In the total study population, NRS, WOMAC, and EQ-5D-5L scores were improved statistically significant when comparing before and after treatment. The NRS and WOMAC scores improved statistically significant in the medial, patellofemoral, medial + patellofemoral, medial + lateral + patellofemoral compartment. Conclusion: Korean medicine treatment significantly reduced pain, stiffness, and physical dysfunction, and improved the quality of life of patients with knee OA, suggesting that it may be an effective alternative to the current conservative treatments.

  • Last year, the elderly population aged 65 years and over exceeded 14.2 percent of the total population, and Korea has entered the ageing society. At the same time, social interest in improving the health level and quality of life in the elderly has also increased. In particular, pain is one of the most common diseases experienced by middle-aged and elderly people and is one of the factors that closely affect the quality of life. The purpose of this study was to analyze the factors affecting the pain level and pain region of Korean medicine users and the use of Korean medicine in the Korean middle aged and elderly using the 6th Korean Longitudinal Study of Ageing (KLoSA) panel. As a result, the main influencing factors of Korean medical care were subjective factors such as gender, subjective health status, and education level. In particular, half of the middle-aged and elderly who used Korean medicine experienced back and knee pain, and generally complained of low level of pain. This study suggests the need for research and development of Korean medicine that can control and relieve the pain of the middle-aged and elderly.

  • Cervical spondylotic myelopathy (CSM) is a severe disease that usually needs surgical treatment. We have treated two CSM patients with Korean traditonal medicine, which includes acupuncture, pharmacopuncture, herbal medicine and manual treatment (Chuna). For objective evaluation, we used Nurick's classification, JOA (Japanses Orthopedic Association) score, grip and release test and hand grasping power measurement. Post-treatment analysis has shown that grip and release test score and grasping power of two patients were both remarkably improved, along with the improvements on both Nurick's classification and JOA score. This study showed that Korean traditional medicine could be effective conservative treatment for CSM.
