중풍 한의변증 표준화를 위한 증상의 중요도 적용 연구 (Application Study of Symptoms Weight For Standard of Korean Medicine Pattern Identification In Stroke)

  • The diagnosis of pattern identification in Korean Medicine depend on Korean Medicine doctor's experience and information. So, Pattern identification in Korean Medicine raise a question of objectification. This study is to standard stroke pattern identification in Korean Medicine. Weight of symptoms was given important(1points), very important(2points) by stroke specialist. So weight of symptoms is used two method. One is total sum and the other is total man. One had been compared Pattern identification between diagnosed patients by medical specialists and by applicated weight in case report form. The other had been compared Pattern identification between intersubjectivity by medical specialists and by applicated weight in case report form. It is 38%(total sum) or 40%(total man) concordance rate between diagnosed patients by medical specialists and by applicated weight in case report form. It is 82.4% concordance rate between intersubjectivity by medical specialists and by applicated weight in case report form. To acquire more concrete data on this theme, we need further and large scale of prospective researches.

  • Puerperants need postpartum care for the recovery to the condition before pregnancy. The postpartum care used to be done at home however, nuclear families and the industrialization of society brought postpartum care home, postpartum care home has two major problems; first, there is no rules and regulations for postpartum care home. Second, no reasonable guidelines of postpartum care home exist. This study inquires into those problems and directions of legalization and reasonable guidelines of postpartum care home. In sum, the legal system for the postpartum care home has to be built and the supervision has to be done by the specialist, in particular, by the oriental medical doctor. The postpartum care home should be supervised for the healthy rehabilitation of puerperants mental, social health as well as their bodies. Also the neonate that has wick immune system requires special care for the prevention of infection and for the adaptation to the environment.

한방 공공의료의 활성화에 관한 연구 - 공중보건 한의사의 활동을 중심으로 - (A Study on Activation of Oriental Medicine in Public Health Sector : The role of Oriental Public Health Physicians)

  • From 1998, Oriental Medical Physicians(OMP) is distributed in Public Health Sector. but long term plan for Oriental Medicine in Public Health is not existed. So, this study is designed for the activation of OMP Subjects in this study were comprised 3 groups of oriental medicine related persons, the group of which are Students of 11 Oriental Medical Schools, Oriental Medical Physicians in Public Health Sector, Specialist Croup of Oriental medicine Policy(total 1,458 persons). Data were collected from July 1st to November 30, 1999. Direct interview with key persons, systematic interview by using of interview protocol, e-mail and facsimile have been conducted. The results of survey were coded by Excel 5.0, and analysed with SAS 6.12 statistical package. Inter-group difference determined by T-test, and descriptive statistics have been examined. Major findings can be epitomized as follows. 1) OMP disposition to multifarious organizations and institutes such as Public Health Centers, Public Health Sub-centers, Public Hospitals, Private Hospitals in Remote-Vulnerable Area, Community Social Welfare Centers, Institutes for Heath Policy Research, etc, will promise an effective use of Oriental Medical Physician. 2) Average number of patients treated by OMP was 22.8, average budget for oriental medical department, in which OMP were affiliated, was 39.6 million Won per year. Direct cost per every patient visit was 7,210.9 Won, which is considered expensive for public health service. Therefore, development and transformation for Oriental Medical Service in Public Health Sector is desirable in economic and political aspects. 3) It is recommended that ${\ulcorner}Advisory\;Committee{\lrcorner},{\ulcorner}Planning\;Commission\;for\; Public\;Health\;in\;Oriental\;Medicine{\lrcorner}$ should be established for the activation and for the support of Oriental Medical Physician in Public Health Sector, 4) Most effective programmes for oriental public health doctor are health service programmes for the elderly, Home health visit, chronic degenerative disease control services(p<0.001). 5) Standard guideline for facilities and equipments of Oriental Medicine Department in Public Health Center is needed for optimal supply of resources and activation of public health activity.

모유수유 중 인삼의 적응증과 용량에 관한 기존한약서 고찰 (Textual Research : Indication and Dosage of Panax Ginseng during Lactation)

  • Purpose: To provide basic informations for guidelines of Ginseng usage during lactation. Method: Based on a regulation of scope and preparation of herbal prescriptions by Ministry of health and welfare of Korea, we selected the formulae for postpartum care and disease in 7 Korean Medical Classics. And we searched the number of formulae including Ginseng, dosage and indications of Ginseng in formulae in those books. Results: The range of Ginseng dosage during lactation from medical classics is 1.5-18.75g/day for medical purposes. Indications of Ginseng are lethargy, excessive bleeding, asthma, fever, cold, pain, dizziness, mental disorder, spasm, digestive problem, constipation, diarrhea, urinary incontinence, edema, breast engorgement, lack of breastmilk, breast ulcer, etc. But Ginseng had not been used in the case of stroke, epistaxis, beginning of mastitis, tumor of lower abdomen. Conclusions: Ginseng should be recommended less than 1.5g/d as a nutritional supplement. If breastfeeding woman wants to take Ginseng as a medication or as a tonic, she should consult with a official specialist about proper prescription and dosage instead of risky random medication.

비만의 변증 진단을 위한 판별모형 (The Discrimination Model for the Pattern Identification Diagnosis of Overweight Patients)

  • The study was to investigate the agreement rate between the statistical diagnosis of pattern identification by discriminant analysis and the clinical diagnosis of pattern identification by medical specialist in obese patients with BMI$\geqq$23. The agreement rate of deficiency of the spleen, phlegm-retention, deficiency of Yang, retention of undigested food, stagnation of liver Gi, and blood stagnation are 0.40, 0.33, 0.52, 0.76, 0.71, and 0.66, respectively and accuracy rate and prediction rate using linear discriminant function are 0.59 and 0.61, respectively. Therefore, the complementary management in CRF questionnaires and/or consultation from experts will improve the accuracy and prediction rate, which will be helpful for pattern identification of obesity by clinical experts.

대야보건소에 내원한 정신분열병 회원 20례에 대한 체질진단소견 (Study of Sasang Constitutional Tendency on the 20 Cases of Schizophrenic Patients Visiting Daeya Public Health Center)

  • The causes of schizophrenia are multifactorial and stress-diathesis model is one of the supporitive causes. This study is aimed for Sasang Constitutional tendency of schizophrenic patients. 20 Subjects are recruited from Gunsan Mental Health Center. They agreed with this study in advance and visited Daeya Public Health Center to interview with Sasang Constitutional Specialist. Their Sasang Constitution types are determined by QSCC II and interview with specialist. Taeumin and Soyangin are prevelant among the 20 cases of schizophrenic subjects visiting Public Health Center; 10 subjects are Taeumin, 9 subjects are Soyangin and 1 subject is Soeumin.

  • Objectives: The objectives of this study are to determine both what information Korean Medicine(KM) doctors want from medical classics and how classic KM knowledge should be structured. Methods: KM doctor survey was conducted in October 2012 via E-mail to determine what information Korean Medicine doctors want to collect from a larger volume of existing classics. Specialist survey was made in October 2012 through questionnaire and 8 workshops were held between April 2012 ~ February 2013 to discuss how to reorganize classic KM knowledge. Results: With two surveys and 8 workshops, we built a conceptualization of the classic KM knowledge system for KM practice. KM doctors wanted to engage with the design of KM knowledge system informed by authentic medical classics, and hoped that more information on common disorders would be added to the table of contents of classic KM books than now. A comprehensive knowledge system was also required to maximize information sharing. Conclusions: Future KM knowledge systems need to be more comprehensive and include more information about disorders with which patients commonly consult KM doctors.

의사인력의 지역간 분포양상 및 공중보건의사의 영향 (Geographical Distribution of Physician Manpower under the Influence of Public Health Physician)

  • The purpose of this research is to assess the geographical distribution of physicians and dentists and the degree of maldistribution of the physician. Data were obtained form the Korean Medical Association's report on physicians registry and census for 1990. To assess the degree of disparity in the rural-urban distribution of physician manpower and to identify changes in the distribution pattern, the Gini index of concentration was used. Major findings are as follows; 1. Urban-rural disparity in the distribution of physician manpower exists in all categories of manpower, i.e. physician, dentist, oriental medical doctor, general practitioner, medical specialist, practitioner, public health physician and public health dentist. Urban area which had 74.4% of nation's population, accounted for over 90% of all physician manpower. 2. In terms of the ration of physician manpower per 10, 000 population, in urban area, they were 8.2 physicians, 2.7 general practitioners, 5.5 specialists, 3.0 practitioners, 1.8 dentists and 1.3 oriental medical doctors. In rural area, the ratios were 1.4 physicians, 0.6 general practitioners, 0.9 specialists, 1.0 practitioners, 0.4 dentists and 0.4 oriental medical doctors. 3. Gini indicies computed to measure inequality of physician manpower distribution were 0. 3675 for physicians, 0.3372 for general practitioners, 0.3338 for specialists, 0.2263 for practitioners, 0.3132 for dentists and 0.3293 for oriental medical doctors. 4. Inspite of increase in the number of physician manpower, urban concentration of physician manpower intensified from 1980 to 1990. However, the Gini index for all physician manpower fell by 18.3~36.7% from 1980 to 1990, indicating more even distribution. 5. In rural area, the public health physicians and dentists had increased the ratios of physicians, general practitioners, practitioners and dentists per 10, 000 population remarkebly, and had decreased the Gini indicies of physicians, general practitioners, practitioners and dentists. Thus, public health physicians and dentists contributed to improve the distribution of physician manpower in rural area. Based on the results of this study, long-term and rational manpower policies should be developed to solve the problem of geographical maldistribution of physician manpower as well as short-term policy for inducing physicians to the rural areas.

중풍환자의 음허변증 진단지표에 관한 연구 (Study of The Diagnostic Indicators of Deficiency of Eum Pattern Identification In Stroke Patients)

  • The purpose of this study was to investigate that which symptoms are adequate indicator of the deficiency of Eum pattern in the stroke patients. In the time period Dec. 2006 to Aug. 2007, 479 patients with a first-ever stroke admitted in the department of Internal Medicine of 12 Oriental Medical Hospitals were included. Patients were hospitalized within 1 months after the onset of stroke. Stroke patients had been interviewed by resident who studied standard operation procedures in Fundamental Study for Standardization and Objectification of Differentiation and Pattern Identification of Syndrome of Oriental Medicine for Stroke. Eum-deficiency patients was confirmed by medical specialist diagnosis, resident diagnosis, case report form analysis without a dissenting voice. Deficiency of Eum group included 65 case, Non Eum group 414 case out of 479 patients. dryness of mouth, short and rapid purse, white face and reddish zygoma, mirror-like tongue were higher among Eum group. Eum and Non Eum patients do not significantly differ in reddened tongue, dryness in tongue, night sweat, palpitation, afternoon tidal heat, palmar heat, sores of the mouth or tongue. This study was insufficiency because sample size is very small. More data from prospective cohort studies will help to Korean Standard Differentiation of the Symptoms and Signs for the Stroke.

중증 성인형 아토피 피부염을 사상처방으로 치료하여 호전된 환자 15례에 대한 1년간의 추적 관찰 (One Year Follow up for Severe Adult Atopic Dermatitis of 15 Patients After Sasang Constitutional Therapy)

  • Background : Atopic Dermatitis is a chronic and recurrent skin disease. it is characterized as itch-scratch cycle and topical or systemic use of corticosteroids is required in western medicine. Though its effects on major symptoms are impressive, these treatments are prone to several side effects and tend to recur after months or years. So potential treatments have been actively studied in Estern medicine, especially Sasang constitutional therapy, which has fewer side effects. Objectives : The purpose of this study was to examine the effect of Sasang constitutional therapy on treating severe adult Atopic Dermatitis and preventing its recurrent symptoms. Methods : fifteen patients who had severe adult Atopic Dermatitis attended treatment of Sasang constitutional therapy and were followed up for one year. An oriental internal specialist first isolated Sasang constitution and a distinctive body type. Grading of Atopic dermatitis was measured by the guidelines offered by Rajka G. and Langeland T. Results : After treatment with Sasang constitutional therapy, the patients symptoms were improved and the mean Rajka and Langeland score reduced significantly from $7.73{\pm}0.79\;to\;3.4{\pm}10.91$. During the follow-up period of one year, the mean Rajka and Langeland score was $2.72{\pm}2.06$ which was lower than last treatment. Conclusions : The results of this study support Sasang constitutional therapy as an effective therapy for severe adult Atopic Dermatitis. Follow up data suggest that this Eastern medical approach is of greater clinical value than the western medical approach in the long term.
