임산(臨産) 의론(醫論)과 의방(醫方)에 대한 문헌고찰(文獻考察) - 불수산(佛手散)을 중심으로 - (A Literature Review about Labor theory and practice - Focused on Bulsusan(佛手散) -)

  • Object : From the ancient times, the importance of childbirth has been well recognized by Korean Traditional Medicine. Numeral methods have been developed to ease the labor process and keep the mother and child healthy from conception to labor. Bulsusan(佛手散) is one of the main remedies to healthy labor in KTM, both widely known and applied as well. Method : This paper examines the labor theory and practice of KTM focused on Bulsusan which is composed of Angelica gigas Nakai(當歸) and Cnidium officinale MAKINO(川芎). Result : 1. From the ancient times until the Q$\bar{i}$ng period, much attention was placed to the handling of the placenta, as it was conceived as bearing much relation to the health of the mother and her fate, and thus included in the labor process. 2. There was a recognition of the 'birth pulse[離經脈]', an intense change in the pulse that presents itself prior to somatic signs of labor. 3. There were numerous prescriptions that were administered beforehand to ease the process. They are mostly constituted with medicinals that nurture Gi(氣) and stimulate its flow, which in turn makes the fetus firm and reduces the volume, easing the labor process. 4. The medical practice of labor-induction was called 'Choesaeng(催生)'. The prescriptions which functioned as such were mostly constituted with blood medicinals such as Angelica gigas Nakai and Cnidium officinale MAKINO, those which nurture both Gi(氣) and blood, and medicinals that physically lubricate the labor pathway such as honey, oil and Talcum(滑石). Conclusion : Bulsusan can be used in most problems concerning pregnancy and labor, and cases of emergency blood loss due to injury. The term 'bulsu(佛手)' infers to the medical ability of the great doctor who takes care of major blood-loss situations resulting from discharge of dead fetus, cesarean delivery, etc. The prescription name takes after this meaning, as it deals with similar conditions in its effect.

Evaluation of Atopy Dermatitis Information on the Internet in Korea

  • The internet is closely related to the lives of modern people in the 21st Century. Exchanging information with others over the internet in any place through is possible nowadays and such exchanging information has been developed at rapid speed also in the areas of medical care. The internet is an important and rapidly-evolving source of health-related information, and many patients with atopy dermatitis attempt to find information from the internet. This study was performed to investigate the current information available in atopy dermatitis on the internet in Korea. We chose three portal sites and two network communities to analyze the information in which the topic of 'atopy' could be found. Among the 2,024 web sites and 5,613 network communities, 98.2% of the web sites and 71.7% of the communities were related products or service advertisements, and some of them provided incorrect of exaggerated information. There were only 42 web sites and 75 available communities with genuine information, and most of these also presented unattested treatment methods, including alternative therapies. It is quite understandable that patients with atopy dermatitis want to seek information related to their disease and the internet has the potential to be a useful source of information. However, there are no regulations to control the accuracy and quality of information on the internet. We should take an active role in identifying the accuracy and evaluating the quality.

지역 노인의 일반적 특성에 따른 낙상공포감과 낙상효능감 연구 (Study on Relationship of Fear of Falling and Falls Efficacy for the Elderly in Local Community)

  • The object of this study was to investigate the relation between variables that is related to the general characteristics, fear of falling and falls efficacy. Data were collected from a convenience sample of 105 elders. Fear of falling and falls efficacy are measured structured questionnaires each. This study analyzed by SAS program. There was a significant difference of fear of falling according to education level(p=.003), body mass index(p=.016). There was a significant difference of falls efficacy according to gender(p<.001), education level(p=.047), body mass index(p=.019), number of chronic disease(p=.021). Fear of falling and age(r=0.23, p=.018) had significant positive correlations in the elders. Falls efficacy and number of chronic disease(r=-0.26, p=.010) had significant negative correlations in the elders. Fear of falling and falls efficacy are mutually contradictious concept. Also, both are important for the health of elderly. This result can be used for the elderly care and development of the program for the improvement of the falling efficacy.

Aripiprazole-Related Oculogyric Dystonia

  • Objectives : Aripiprazole is unique drug among the SGA (Second generation antipsychotics) in its pharmacology and pharmacokinetics,but is similar in clinical efficacy. Aripiprazole acts as a partial agonist at dopamine D2 receptors, activating the receptor but eliciting a reduced response compared to the natural neurotransmitter. There are some side effects of aripiprazole, the most common side effects of aripiprazole are headache, nausea, vomiting, insomnia, tremor, constipation and EPS. Difficulty in opening eyes is not defined EPS yet, but it is a rare but important side-effect symptom of aripiprazole. Methods : This article is about a case of side-effect symptom of aripiprazole, 26-year-old single female suffering from schizophrenia had difficulty in opening eyes while she was taking antipsychotics. During the hospitalization, the relaxation therapy is helpful not only to reduce tension in the eyelids but also to headache. Results : It is important that early recognition of aripiprazole-induced oculogyric dystonia can prevent life-threatening complications. Education medical staff to this easily treatable reaction will improve overall quality of health care. Conclusions : This case notifies the need for awareness of the risk of acute oculogyric dystonia in adolescent female patients receiving aripiprazole.

  • Objectives : This research was investigated the effect of the Ginseng Radix plus Crataegi Fructus on the injury of brain tissue in the mice with Alzheimer's disease induced by ${\beta}A$. Methods : Observed a change of the injury of brain tissue and reduced the infarction area of hippocampus in the mice with Alzheimer's disease induced by ${\beta}A$. Results : 1. The Gin-CF extract reduced the infarction area of hippocampus, and controlled the injury of brain tissue in the mice with Alzheimer's disease induced by ${\beta}A$. 2. The Gin-CF extract reduced the Tau protein, GFAP protein, and presenilin1/presenilin2 protein (immunohistochemistry) of hippocampus in the mice with Alzheimer's disease induced by ${\beta}A$. Conclusion : These results suggest that the Ginseng Radix plus Crataegi Fructus extract may be effective for the prevention and treatment of Alzheimer's disease. Investigation into the clinical use of the Ginseng Radix plus Crataegi Fructus extract for Alzheimer's disease is suggested for future research.

  • Objectives : This research was investigated the effect of the Ginseng Radix plus Crataegi Fructus on the gene expression in relation to Alzheimer's disease. Methods : Observed gene expression of the Ginseng Radix plus Crataegi Fructus extract on $IL-1{\beta}$, IL-6, $TNF-{\alpha}$, COX-2, and NOS-II mRNA of BV2 microglia cell line treated with lipopolysacchride. Results : The Ginseng Radix plus Crataegi Fructus extract suppressed the gene expression of $IL-1{\beta}$, IL-6, $TNF-{\alpha}$, COX-2, NOS-II mRNA in BV2 microglia cell line treated with lipopolysacchride. Conclusion : These results suggest that the Ginseng Radix plus Crataegi Fructus extract may be effective for the prevention and treatment of Alzheimer's disease. Investigation into the clinical use of the Ginseng Radix plus Crataegi Fructus extract for Alzheimer's disease is suggested for future research.

판례분석을 통한 한방의료행위개념의 법적 근거 고찰 (A Study on the Legal Aspect of the Concept for Medical Practice in Korean Medicine through Cases Analysis)

  • Objectives : The lawsuits associated with medical practice in Korean medicine are increasing gradually. However, the clear definition for medical practice in Korean medicine has not been existed in Korean law. Only we may understand the concept regulated by judicial precedents of the court of justice or the authoritative interpretation by the government. Methods : For study, a database was established for medical lawsuits involving Korean medicine(1968~2009, n=130). Results : According to court rulings, the medical practice in Korean medicine is an act to diagnose a person's illness, prescribe and treat to cure based on traditional Korean medicine, to be understood as a medical care, to have some factor to create or increase danger for the preservation of health or hygiene, and to be practiced by medical specialists based on their professional knowledge. Conclusions : But, such definition is not proper and exceedingly vague. Besides medical circumstances Koreans Medicine are changing, and new precedent to the definition of the practice of medicine is establishing. Therefore the meaning and scope of the medical practice in Korean medicine should be modified and amended, reflecting these conditions.

견비통 완화 도인기공체조에 대한 기초적인 연구 (Basic Study on the Doin Gigong Exercise for the Reduction of Shoulder-arm Pain)

  • Gigong exercise is simple form's game for health care. Gigong exercise is the training method in mind and body, and it prevent or cure the disease by becoming larger the human's nature-healing power This syudy intends to identify the effective processing channel of the Doin Gigong Exercise for the reduction of shoulder-arm pain. The processing channel of the Doin Gigong Exercise for the reduction of shoulder-arm pain cinsist of warming, main, finishing exercise. The warming exercise start breathing and concentrate upon shoulder-arm exercise. The upper and lower sides join-hands exercise give help smoothly the Gi-circulation of human body in every direction. The finishing exercise consist of experience to Gi with the object of maintaining human comfortable condition. As mentioned above, the Doin Gigong exercise is effective intervention for the reduction of shoulder-arm pain.

뉴로피드백 훈련이 장기요양시설노인의 뇌기능지수에 미치는 효과 (The Effects of Neurofeedback Training on Brain Function Quotient of Elderly with Long-term Care Insurance Service)

  • Purpose: Recently, Neurofeedback training system that based on biofeedback of brain wave was introduced. This study was performed to identify the effects of the improvement of brain function by Neurofeedback training on elders(the 2nd or 3rd grade of long-term care insurance services). Methods: A quasi-experimental design using a nonequivalent control group, pre-post test was used. Total 11 elderly were enrolled in this study (experimental group 5, control group 6). The intervention was conducted 3 times a week for 30 minutes from January to June, 2012 (total 60 times). Chi-square test and Mann-Whitney U-test were used to analyze the data. Results: After the Neurofeedback intervention, attention quotient (AQ), anti-stress quotient (ASQ), emotion quotient (EQ) and brain quotient (BQ) of the experimental group were significantly better than those of the control group. Conclusion: The findings indicate that the Neurofeedback training program was effective in reducing fatigue by AQ, increasing the physical and mental stress resistance by ASQ, emotional balance by EQ and improving of total brain function by BQ. Therefore Neurofeedback training be used as an effective training intervention for the health of elderly in geriatric facility.

근거 중심 Kampo medicine 임상진료지침의 현황 (Evidence-based Clinical Practice Guidelines On Kampo (Japanese Herbal) Medicine : The Current State of Kampo Clinical Practice Guidelines)

  • Objectives : In 2007, a survey of how Kampo was regarded in Japanese clinical practice guidelines (CPGs) was first conducted by the Special Committee for Evidence Based Medicine (EBM), namely the Japan Society for Oriental Medicine (JSOM). A manual for CPG development was also published in 2007 by the Japan Council for Quality Health Care, and the revised edition came out in 2014. The purpose this study is to review the current state in CPGs applied to Kampo after 2007, while focusing on how Kampo products being regarded evidence based branch of medicine by CPG developers. Methods : Sources include the Kampo CPGs website of Japan Society for Oriental Medicine (JSOM) and MINDS (Medical Information Network Distribution Service) website of Japan Council for Quality Health Care. Results : Among the 784 CPGs existing by the end of 2015, 91 CPGs were considered containing descriptions of Kampo. Furthermore, 28 type A Kampo CPG (KCPG) which had quality of evidence and strength of recommendation with references were found. Also, most of type A KCPGs relied on the MINDS Handbook for Clinical Practice Guideline Development that was published in 2007. Conclusions : The number of KCPGs are increasing yearly. However, there is still not much Kampo evidence found in CPGs in Japan. Overall, it could be said that we need to not only make evidence vertically but preach it horizontally well.
