재발성 아프타성 궤양 환자의 타액에서 Herpes Simplex Virus, Varicella Zoster Virus, Helicobacter pylori 그리고 Candida 검출 (Detection of Herpes Simplex Virus, Varicella Zoster Virus, Helicobacter Pylori and Candida in Saliva of Patients with Recurrent Aphthous Ulceration)

  • 재발성 아프타성 궤양 환자의 타액에서 구강내 궤양성 병소를 유발할 수 있고 감염성이 비교적 높은 미생물로 알려진 Herpes Simplex virus, Varicella Zoster virus, Helicobacter pylori 그리고 Candida가 발현되는지 여부와 병소의 발생과 상관관계가 있는지를 알아보고자 조선대학교 치과병원 구강내과에 내원한 재발성 아프타성 궤양을 가진 환자 29명과 대조군 29명의 타액을 이용하여 PCR과 배양을 통해 발현율을 비교한바 다음과 같은 결과를 얻었다. 1. HSV DNA는 재발성 아프타성 궤양 환자군에서 41.4%, 대조군에서 55.2%가 발현되었으나 두 군간에 유의한 차이는 없었고(P>0.05), VZV DNA는 두 군에서 모두 나타나지 않았다. 2. H. pylori DNA는 재발성 아프타성 궤양 환자군에서 27.6%, 대조군에서 48.3%가 발현되었으나 두 군간에 유의한 차이는 없었다(P>0.05). 3. Candida는 재발성 아프타성 궤양 환자군에서 13.8%, 대조군에서 6.9%가 배양되었으나 두 군간에 유의한 차이는 없었다(P>0.05). 이상의 연구를 종합하여 보았을 때, HSV, VZV, H. pylori 그리고 Candida는 재발성 아프타성 궤양의 발생에 직접적인 역할을 한다고는 볼 수 없으므로 향후 더 많은 표본을 대상으로 다른 미생물이 병소 발생의 유발요인으로 작용하는지 연구하여야 할 것으로 사료된다.

주요 출혈성 질환자에서 치성감염 관리에 관한 임상적 연구 (A CLINICAL STUDY ON THE CARE OF ODONTOGENIC INFECTIONS IN THE PATIENTS WITH MAJOR BLEEDING DISORDERS)

  • This is a retrospective study on the care of odontogenic infections in admission patients with major bleeding disorders. The study was based on a series of 514 patients treated at Dong San Medical Center, Wonju Christian Hospital and Il San Health Insurance Hospital, from Jan. 1, 2000, to Dec. 31, 2002. The obtained results were as follows : 1. The cardiovascular disease was the most frequent cause of the systemic diseases with major bleeding disorders, and liver disease, cerebrovascular disease and renal failure were next in order of frequency. But, there was the most frequent dental consultation in the liver disease, owing to the many odontogenic infectious diseases. 2. Male prediction (66.3%) was almost existed in the odontogenic infectious patients with major bleeding disorders. But, there was slight female prediction (53.4%) in the cardiovascular disease. 3. The most common age group of the odontogenic infectious patients with major bleeding disorders was the fifty decade(27.2%), followed by the forty, sixty & thirty decade in order. 4. In the contents of chief complaints on the odontogenic infectious patients with major bleeding disorder, peak incidence was occurred as toothache (42.2%), followed by intraoral bleeding, ulcer pain, dental extraction in order. 5. In the diagnosis group of odontogenic infectious diseases, periodontitis, pulpitis and periapical abscess were more common. 6. In the treatment group of odontogenic infectious diseases, the most frequent incidence(44.2%) was showed in primary endodontic drainage(pulp extirpation, occlusal reduction & canal opening drainage) and followed by the incision & drainage, the medications & oral hygiene instruction, scaling, indirect pulp capping in order.

실제 임상현장에서의 간이식 환자 대상 Everolimus와 저용량 Tacrolimus 병용요법의 유효성 및 안전성 평가 (Real-World Efficacy and Safety of Everolimus with Low Dose Tacrolimus in Liver Transplantation Recipients)

  • Background: Post-transplant immunosuppression with calcineurin inhibitors (CNIs) is associated with kidney function impairment while mammalian target of rapamycin (mTOR) inhibitors, such as everolimus, can be used for its renal-sparing effects. In this study, we compared the efficacy and safety of everolimus with low dose tacrolimus (EVR+Low TAC) and conventional dose tacrolimus (TAC) in liver transplantation recipients. Methods: Medical records of recipients who received liver transplantation at Seoul National University Bundang Hospital from January 1st 2009 to December 31st 2018 were retrospectively reviewed. Cohort entry date was defined as the day everolimus was initiated and tacrolimus dosage was reduced. All patients were followed up for 1 year. Indicator of efficacy was the incidence of rejection and safety was evaluated by incidence of drug adverse events including renal function. Results: Among 118 patients, there were 40 patients (33.9%) in EVR+Low TAC group. Incidence of rejection, including both biopsy proven acute rejection and clinical rejection, was similar in two groups [7.5% (n=3) vs. 6.4% (n=5), p=1.000]. Renal dysfunction was less frequent in EVR+Low TAC [17.5% (n=7) vs. 35.9% (n=28), p=0.038]. However, incidence rates of dyslipidemia, oral ulcer were more frequent in EVR+Low TAC [45.0% (n=18) vs. 21.8% (n=17), p=0.009; 15.0% (n=6) vs. 1.3% (n=1), p=0.006]. Conclusions: In terms of prevention of rejection, EVR+Low TAC was as effective as TAC and had renal-sparing effect but was associated with increased risk of dyslipidemia and oral ulcer. This study demonstrates that EVR+Low TAC could be an alternative to liver transplant recipients with nephrotoxicity after administration of conventional dose tacrolimus.

지유가 흰쥐의 급성위염 및 소화성 궤양에 미치는 영향 (The Effects of Ethylacetate Fraction of Sanguisorba officinalis L. on Experimentally-induced Acute Gastritis and Peptic Ulcers in Rats)

  • 본 실험에서 흰쥐에 실험적으로 급성위염, 위궤양 및 십이지장궤양을 유발하고 지유 에틸아세테이트분획을 투여하여 항위염 및 항궤양에 미치는 영향을 검토하였던 바 다음과 같은 결과를 얻었다. 지유 에탄올 추출물을 n-헥산, 클로르포룸, 에틸아세테이트 및 n-부타놀과 물로 계통분획하여 농축하고 in vitro에서 항산화력을 측정한 결과 에틸아세테이트분획이 가장 우수하였다. 지유 에틸아세테이트분획 (SOL & SOH)의 HCI-EtOH급성위염에 대한 억제율은 각각 $27.7\%$, $31.7\%$로 대조약물로 사용된 cimetidine(CMT, $47.2\%$)의 억제효과에는 미치지 못했으나 CON에 비해 우수하였다. Indomethacin유발 위궤양에서, SOL & SOH의 위궤양억제율은 각각 $48.28\%$, $51.08\%$로 대조약물로 사용된 CMT($58.26\%$)의 억제효과에는 미치지 못했으나 CON에 비하며 유의 성있는 억제효과를 나타냈다. Shay 위궤양에서, 지유 에틸아세테이트분획을 십이지장에서 위내로 투여하였을 때 궤양 억제율은 SOL이 $42.86\%$로 CON에 비하여 억제되었으나 유의성 있는 효과는 아니었으며, 용량을 증량하여 투여한 SOH는 $47.71\%$로 대조약물인 omeprazol(OMP)의 저해효과($62.00\%$)에 미치지는 못했으나 CON에 비해 유의적민 효과를 나타내었다. 위액량 및 총위산분비량에서, 지유 에틸아세데이트분획 투여로 CON에 비하여 $32.11\%$$47.69\%$, $24.28\%$$34.69\%$의 감소로 유의성 있는 효과를 나타냈으며 OMP($55.69\%$, $41.62\%$)에 근접하게 감소되었다. pepsin 활성은 지유 에틸아세테이트분획 투여로 CON에 비해 각각 $19.55\%$, $\%21.45$가 저하되었으나 유의성있는 변화는 아니었다. 수침 $\cdot$ 구속 스트레스에 의해 유발된 궤양의 경우, SOL는 궤양억제율이 $9.23\%$ 로 CON와 유사하게 출혈을 동반한 깊은 궤양이 관찰되었으나 용량을 증량하여 투여한 SOH는 $48.45\%$로 CON에 비해 궤양의 깊이도 얕아졌고 출혈을 동반한 궤양도 감소되어 유의적인 억제율을 나타내었다. Cysteamine 유발 십이지장궤양에서, CMT는 유발된 십이지장 궤양을 CON에 비해 $54\%$ 정도 억제하여 우수한 항궤양효과를 나타냈고 시료투여군인 SOL 및 SOH는 CMT보다 우수하지는 않았으나 약 $35\∼42\%$ 정도의 궤양억제율을 나타냈다. 이상의 실험결과에서 지유의 항위염 및 항궤양효과는 항산화물질에 의한 결과로 판단되어지며, 앞으로 위염 및 위궤양 개선을 위한 건강 기능성 식품으로 개발이 기대된다

  • 자폐증 및 정신 지체가 있는 환자의 입술 깨물기 증상을 mouthguard를 사용하여 성공적으로 조절하였다. 치과적으로 접근 가능한 구강 내 자해 증상의 치료는 가철성 혹은 고정성 장치를 이용하거나 관련 치아의 발치, 악교정 수술 등을 시도할 수 있으며 각 환자에 맞는 적절한 치료법을 선택하는 것이 중요하다. 자해 행동이 일시적으로 개선되었다 해도 재발의 가능성이 높으므로 장기적인 관리와 소아정신과와의 협진 하에 정신심리학적 환경 개선 및 약물 치료가 병행되어야 할 것이다.

코카 콜라 경구 투여와 내시경적 주입법을 이용한 위석의 치료 1 예 (Gastric Phytobezoar Treated by Oral Intake and Endoscopic Injection of Coca-Cola)

  • Bezoars are collections or concretions of indigestible foreign material that accumulate and coalesce in the gastrointestinal tract; they usually occur in patients who have undergone gastric surgery and have delayed gastric emptying. Treatment options include dissolution with enzymes, endoscopic fragmentation with removal or aspiration, and surgery. Recently, the efficacy of nasogastric lavage or endoscopic infusion of Coca-Cola for the dissolution of phytobezoar have been reported. We report a case of phytobezoar successfully treated by oral administration and endoscopic injection of Coca-Cola. A 62-year-old woman was referred to Yeungnam University Hospital for epigastric pain. Upper gastrointestinal endoscopy revealed one very large, dark-greenish, solid bezoar in the stomach with gastric ulcer and duodenal bulb deformity. We performed endoscopic injection of Coca-Cola into the bezoar. The patient was instructed to drink four liters of Coca-Cola per day. At endoscopy two days later, the phytobezoar was easily broken into pieces. At endoscopy on the $11^{th}$ day of admission, the phytobezoar was decreased in size and removed by endoscopic fragmentation with a polypectomy snare. At follow up endoscopy after 13 days, the bezoar was completely dissolved.

정전가미이진탕(正傳加味二陳湯)의 랫드에서의 단회투여독성시험(單回投與毒性試驗) (Single dose toxicity study of Jengjengamiyjintang in rats)

  • The single dose toxicity of Jengjengamiyjintang, a herbal drug for duodenal ulcer was evaluated in male and female Sprague-Dawley (SD) rats. Jengjengamiyjintang was once administered to both sexes of rats at the dose levels of 2000, 1000, 500, 250 and 125mg/kg for oral route according to KFDA guidelines for single dose toxicity test (1999-61). In addition, vehicle control group was added in order to compare clinical signs, body weight changes and abnormal necropsy findings. After single administration, clinical signs were observed every twice a day for 14 days and body weights were measured 5 times including initial measurement on day 0. When observation period was over, the animals were sacrificed and macroscopic examination of major organs was conducted. Neither significant clinical signs nor death after administration was observed during the observation periods except for soft feces or diarrhea that were restricted to Day 1 of 2000 and 1000mg/kg-dosing groups. Although some accidental findings such as gross and histopathological changes of lung that were demonstrated in some animals of all experimental groups including vehicle control group, no abnormal necropsy findings and changes of body weight were observed at terminal necropsy in both sexes. From these results, it is considered that $LD_{50}$ of Jengjengamiyjintang is over 2000mg/kg in oral administration in both sexes of rats and approximated lethal dose was considered over 2000mg/kg.

Curcumin Blocks Naproxen-Induced Gastric Antral Ulcerations through Inhibition of Lipid Peroxidation and Activation of Enzymatic Scavengers in Rats

  • Curcumin is a polyphenol derived from the plant Curcuma longa, which is used for the treatment of diseases associated with oxidative stress and inflammation. The present study was undertaken to determine the protective effect of curcumin against naproxen-induced gastric antral ulcerations in rats. Different doses (10, 50, and 100 mg/kg) of curcumin or vehicle (curcumin, 0 mg/kg) were pretreated for 3 days by oral gavage, and then gastric mucosal lesions were caused by 80 mg/kg naproxen applied for 3 days. Curcumin significantly inhibited the naproxen-induced gastric antral ulcer area and lipid peroxidation in a dose-dependent manner. In addition, curcumin markedly increased activities of radical scavenging enzymes, such as superoxide dismutase (SOD), catalase, and glutathione peroxidase in a dose-dependent manner. Specifically, 100 mg/kg curcumin completely protected the gastric mucosa against the loss in the enzyme, resulting in a drastic increase of activities of radical scavenging enzymes up to more than the level of untreated normal rats. Histological examination obviously showed that curcumin prevents naproxen-induced gastric antral ulceration as a result of direct protection of the gastric mucosa. These results suggest that curcumin blocks naproxen-induced gastric antral ulcerations through prevention of lipid peroxidation and activation of radical scavenging enzymes, and it may offer a potential remedy of gastric antral ulcerations.

도시에 거주하는 중년 여성들의 골밀도와 이에 영향을 미치는 인자들에 관한 연구 2. 골밀도에 영향을 미치는 요인에 관한 연구 (Bone Densities of the Middle Aged Women Residing in the City and Related Factors)

  • This study was conducted to investigate the effect of physiological and behavioral factors on the bone density of 125 middle aged women who visited Saint Bundo Hospital. 16.1% of subjects had osteoporosis and 40.3% had osteopenia according to the measurement of the bone density of lumbar spine. Mean age was 56.9 in osteoporosis group and 53.7 in osteopenia group. It was significantly different from the mean age of control group, 50.7. The mean bone density of the women who had menarche after 15 years old was significantly lower than that of the women who had menarche before 15. But the age of menopause, the total year of menstruation, irregularity of the menstrual cycle and percentage of subjects who had ovariectomy were not significantly different among osteoporosis, osteopenia and control group. The use of medication such as oral contraceptive, steroid, depressant, diuretic, and Ca supplement and the preference of salty food were not significantly different among three groups. The percentage of subjects who had rheumatism, gastric ulcer, and pain in neck or shoulder was higher in osteoporosis and osteopenia group than in control group. This study shows that the age and the age of menarche affect the bone density, and that behavioral factors were not sig nificantly different in osteoporosis and osteopenia group compared to the control group. Further researches are needed to find out the effective way to minimize the effect of age and other physiological conditions on the decrease of bone density.

The Inhibitory Effect of Quercetin-3-O-${\beta}$-D-Glucuronopyranoside on Gastritis and Reflux Esophagitis in Rats

  • It was evaluated the inhibitory action of quercetin-3-O-${\beta}$-D-glucuronopyranoside (QGC) on reflux esophagitis and gastritis in rats. QGC was isolated from the herba of Rumex Aquaticus. Reflux esophagitis or gastritis was induced surgically or by administering indomethacin, respectively. Oral QGC decreased ulcer index, injury area, gastric volume, and acid output and increased gastric pH as compared with quercetin. Furthermore, QGC significantly decreased gastric lesion sizes induced by exposing the gastric mucosa to indomethacin. Malondialdehyde levels were found to increase significantly after inducing reflux esophagitis, and were reduced by QGC, but not by quercetin or omeprazole. These results show that QGC can inhibit reflux esophagitis and gastritis in rats.
