구강병소에 대한 펄스형 Nd: YAG 레이저의 치료효과 (The Therapeutic Effects of a Pulsed Nd:YAG Laser on Oral Lesions)

  • In order to set the lasing variables and evaluate, clinically, the therapeutic effects of a pulsed Nd:YAG laser on oral lesion, the author applied the laser energy from a fiberoptic delivered, free running, pulsed Nd:YAG laser (wavelength 1064nm, Pulse duration 120$\mu$sec, fiber diameter 200$\mu$m/320$\mu$m) to 22 cases of oral soft tissue lesions and 6 cases of oral hard tissue lesions. The obtained results were as follows : 1. The effective excision with contact mode and the effective hemostasis of accompanied bleeding with noncontact mode were occurred by lasing on oral soft tissue lesions with fiber diameter of 320$\mu$m under the variables of 2.0~4.0W and 20~50Hz which were controlled into high power/low pulses for excision, low power/high pulses for hemostasis, low power in granulation tissue and high power in fibrous tissue according to therapeutic goals and tissue conditions. 2. About 50% of decreasing effect on hypersensitivity was occurred by lasing with non-contact and contact mode on cervical abrasion which caused dentinal hypersensitivity with fiber diameter of 320$\mu$m under the variables of 0.7 - 1.0W and 10Hz which were applied 2~3 times with 1 week interval. 3. The effective sterilization of infected root canal and lesion of periapical abscess was occurred by lasing with contact and spiral modes on wall of root canal and periapical abscess with fiber diameter of 200$\mu$m of which the tip was placed about 1mm shorter than root canal length under the variables of 1.OW and 10Hz.

  • Oral cancer ablation surgery results in tissue defects with functional loss. Accompanying neck dissection results in facial nerve weakness and dysmorphic changes. To minimize the complications after oral cancer surgery, accurate dissection without damaging facial nerve and vital structures are mandatory. Marginal mandibular branch of facial nerve should be dissected or contained in the superficial layer of deep cervical fascia to minimized facial palsy after operation. Reconstruction after cancer ablations is routine procedures and free flap reconstruction is the most commonly used. Radial forearm free flap is the most versatile flap to reconstruct soft tissue defects and it is easy to design according to the defect size and shape. However, donor site scar and secondary skin graft from thigh result in unesthetic and cumbersome wounds. Double layered collagen graft in the donor site could reduce secondary donor site for skin graft. In conclusion, oral and maxillofacial surgeon should know the exact anatomy of the face and neck during neck dissection. Radial forearm free flap is most versatile flap for soft tissue reconstruction and double collagen graft can reduce postoperative scar and there is no need for secondary skin graft.

Quantitative cone-beam computed tomography evaluation of hard and soft tissue thicknesses in the midpalatal suture region to facilitate orthodontic mini-implant placement

  • Objective: To identify the most favorable sites that optimize the initial stability and survival rate of orthodontic mini-implants, this study measured hard and soft tissue thicknesses in the median and paramedian regions of the palate using cone-beam computed tomography (CBCT) and determined possible sex- and age-related differences in these thicknesses. Methods: The study sample comprised CBCT images of 189 healthy subjects. The sample was divided into four groups according to age. A grid area was set for the measurement of hard and soft tissue thicknesses in the palate. Vertical lines were marked at intervals of 0, 1.5, and 3.0 mm lateral to the midpalatal suture, while horizontal lines were marked at 2-mm intervals up to 24 mm from the posterior margin of the incisive foramen. Measurements were made at 65 points of intersection between the horizontal and vertical lines. Results: The palatal hard tissue thickness decreased from the anterior to the posterior region, with a decrease in the medial-to-lateral direction in the middle and posterior regions. While the soft tissue was rather thick around the lateral aspects of the palatal arch, it formed a constant layer that was only 1-2-mm thick throughout the palate. Statistically significant differences were observed according to sex and age. Conclusions: The anterolateral palate as well as the midpalatal suture seem to be the most favorable sites for insertion of orthodontic mini-implants. The thickness of the palate differed by age and sex; these differences should be considered while planning the placement of orthodontic mini-implants.

치주질환 원인의 심한 골소실을 동반한 발치와에 대한 치조제 증강술과 연조직 대체제를 이용한 조직 재생 효과: 증례보고 (Regeneration of total tissue using alveolar ridge augmentation with soft tissue substitute on periodontally compromised extraction sites: case report)

  • 발치 후 시간이 지남에 따라 치조골이 흡수되는 것은 자연스러운 현상이다. 치조골 소실과 상부 연조직의 감소는 추후 임플란트 식립 및 임플란트의 장기 생존 가능성에 어려움을 준다. 이번 증례보고는 치주질환에 이환된 발치와에 치조제 증강술과 연조직 대체제 중 이종 콜라겐 기질을 함께 이용하여 임플란트 식립 부위의 연조직과 경조직을 증가시키는 데 중점을 둔다. 각각의 증례에서 치조골의 너비가 6 mm, 8 mm, 4 mm로 증가하였고, 임플란트 주변으로 치간 유두의 재생과 각각 4 mm, 6 mm, 4 mm의 협측 각화치은을 보여준다. 증대된 치조골은 임플란트 수술을 용이하게 하며 치간 유두와 각화치은은 심미적인 보철이 가능하게 한다. 이 연구는 치주질환이 이환된 발치와를 가진 환자에게 치조제 증강술을 시행하고 연조직 대체제를 추가로 사용함으로써 임플란트 식립을 위한 보다 나은 환경을 제공하고 심미적이고 예지성 있는 임플란트 수술을 위한 긍정적인 효과를 보여준다.

  • Purpose : The purpose of this study was to analyze the lower third facial changes in frontal view after mandibular setback surgery. Materials and Methods : In this study, fifteen subjects(6 males and 9 females) with class III dental and skeletal malocclusions who were treated with BSSRO(Bilateral Sagittal Split Ramus Ostetomy) were used. Frontal cephalometric radiographs were taken preoperatively and more than 6 months postoperatively, and hard tissue(H2-Hl) and soft tissue changes (S2-S1) were measured on vertical and horizontal reference lines. In 15 cases, changes which developed more than 6 months after surgery were studied. Results : The results were as follows. 1. In the facial height, hard tissue $decreased(2.46{\pm}2.76mm)$ with statistical significance(P<0.01), and soft tissue also $decreased(1.64{\pm}3.66mm)$. As a result, the facial height generally becomes shorter after sagittal split ramus osteotomy. 2. In the mandibular width, hard tissue $decreased(2.08{\pm}3.59mm)$ with statistical sgnificance(P<0.05), but soft tissue $increased (2.14{\pm}5.73mm)$ without statistically significant difference(P>0.05) postoperatively. 3. In the facial index, hard tissue $decreased(0.23{\pm}2.21%)$, but soft tissue $increased(2.41{\pm}3.46%)$ with statistical significance. Conclusion : One of the main purpose of orthognathic surgery is to achieve facial esthetics and harmony. In order to fullfill this purpose, it is important to carry out a precise presurgical treatment planning by estimating the changes of frontal profile after surgery.

상악 전치부 임플란트지지 수복금관의 주변 연조직 제원 변화에 관한 누년적 연구 (Alterations of the soft tissue dimensions around implant-supported singletooth replacements in the maxillary anterior region - A retrospective longitudinal study -)

  • This study was performed to investigate the soft tissue changes around single implant-supported crowns during followup periods. Twenty patients(31 implants) whose single missing tooth in the maxillary anterior region had been replaced with an single implant-supported crown were recruited for the study. Crown length, soft tissue level and papilla height at the single implant-supported crowns were measured at follow-up examination and calculated from the slides taken at time of crown placement. as well Papilla index was scored from the slides taken at the time of crown placement and follow-up examination. A very little amount of recession occurred and the soft tissue level moved more apically and the papilla height increased significantly (p<0.01). Especially, both mesial and distal papilla index at single implant-supported crowns increased significantly during follow-up periods (p<0.001). When the two slides taken at the time of crown placement and follow-up were compared simultaneously, except one site, papillae size increased at all sites. From the results of the study, the interdental papillae at the single implant-supported crowns seemed to regenerate significantly and their crown margins were stable during follow-up periods. Hence it is indicated that various surgical interventions at on early stage to enhance soft tissue esthetics arourd single implants may be unnecessary.

하악에 발생한 횡문근육종 (Alveolar rhabdomyosarcoma involving the mandibular ramus and its surrounding tissues)

  • Rhabdomyosarcoma, when it occurs in the head and neck, is primarily found in children. Alveolar rhabdomyosarcoma is rarely seen in the oral lesion, comparing to the embryonal and the pleomorphic variants. This is a report of a case of alveolar rhabdomyosarcoma in the mandible in a ten-year old girl who complained of a non-painful swelling on the right cheek. The right lower 1st molar was mobile. Her radiographs revealed an extensive radiolucency with somewhat irregular border on the right mandibular ramus. The right mandibular 1st and 2nd molars lost their lamina dura and were floating. CT images revealed smooth-outlined soft tissue mass occupying the pterygomandibular space, the infratemporal space, and the masseteric muscle with thinning and perforation of the right mandibular angle and ramus. Histopathological and immunohistochemical findings established the final diagnosis of alveolar rhabdomyosarcoma.

Evaluation of the postoperative maxillary sinus with computed tomography

  • Purpose: To evaluate the computed tomographic appearances of post-operative maxillary sinuses. Materials and Methods: 33 asymptomatic cases of post-operative maxillary sinus without evidence of any pathologic changes and clinical symptoms were selected. CT images were classified as opacification, soft tissue shadow, anterior wall depression, naso-antral communication, and compartmentalization. The relationships between the CT image and the age of patients at the time of operatation, and between the CT image and the duration of time elapsed since the surgical procedure were evaluated. Results: The most commonly presented radiological characteristics that occurred after the Caldwell-Luc procedure were opacification and soft tissue shadow. Anterior wall depression and naso-antral communication were radiographic indications that a Caldwell-Luc operation had been carried out. The age of patients when they had been first operated on, and the duration between the surgical procedure and the time of evaluation had no effect on the CT appearances of normal changes. In cases involving a longer time interval between the antral surgery and evaluation, the anterior wall depression with bony healing was more commonly observed than soft tissue healing. Conclusion: The radiographic information regarding the normal healing state using computed tomography can distinguish post-operative changes from inflammatory and cystic disease in patients who have undergone a Caldwell-Luc type of radical maxillary antrostomy.

Effect of bFGF and fibroblasts combined with hyaluronic acid-based hydrogels on soft tissue augmentation: an experimental study in rats

  • Background: Hyaluronic acid (HA) has been applied as a primary biomaterial for temporary soft tissue augmentation and as a carrier for cells and the delivery of growth factors to promote tissue regeneration. Although HA derivatives are the most versatile soft tissue fillers on the market, they are resorbed early, within 3 to 12 months. To overcome their short duration, they can be combined with cells or growth factors. The purpose of this study was to investigate the stimulating effects of human fibroblasts and basic fibroblast growth factors (bFGF) on collagen synthesis during soft tissue augmentation by HA hydrogels and to compare these with the effects of a commercial HA derivative (Restylane®). Methods: The hydrogel group included four conditions. The first condition consisted of hydrogel (H) alone as a negative control, and the other three conditions were bFGF-containing hydrogel (HB), human fibroblast-containing hydrogel (HF), and human fibroblast/bFGF-containing hydrogel (HBF). In the Restylane® group (HGF), the hydrogel was replaced with Restylane® (R, RB, RF, RBF). The gels were implanted subdermally into the back of each nude mouse at four separate sites. Twelve nude mice were used for the hydrogel (n = 6) and Restylane® groups (n = 6). The specimens were harvested 8 weeks after implantation and assessed histomorphometrically, and collagen synthesis was evaluated by RT-PCR. Results: The hydrogel group showed good biocompatibility with the surrounding tissues and stimulated the formation of a fibrous matrix. HBF and HF showed significantly higher soft tissue synthesis compared to H (p < 0.05), and human collagen type I was well expressed in HB, HF, and HBF; HBF showed the strongest expression. The Restylane® filler was surrounded by a fibrous capsule without any soft tissue infiltration from the neighboring tissue, and collagen synthesis within the Restylane® filler could not be observed, even though no inflammatory reactions were observed. Conclusion: This study revealed that HA-based hydrogel alone or hydrogel combined with fibroblasts and/or bFGF can be effectively used for soft tissue augmentation.

연조직 및 골 두께가 임플란트 주위 연조직 형태에 끼치는 영향에 관한 임상추적연구 (Influence of soft tissue and bone thickness on the dimensional change of peri-implant soft tissues;A clinical follow-up study)

  • The aim of this study was to investigate the influence of peri-implant soft tissue and bone thickness on the early dimensional change of peri-implant soft tissue. Seventy-seven non-submerged implants of 39 patients which had been loaded more than 6 months were selected for the study. Following clinical parameters were measured; bucco-lingual bone width of the alveolar bone for implant placement before implant surgery; distance between implant shoulder and the first bone/implant contact at the surgery; presence of plaque, probing depth, bleeding on probing, width of keratinized mucosa, mucosa thickness, distance between implant shoulder and peri-implant mucosa, crown margin location at follow-up examination. The results showed that distance between implant shoulder and peri-implant mucosa (DIM) was correlated with probing depth and width of keratinized mucosa (p < 0.05). In addition, mucosa thickness was also correlated with probing depth (p<0.05). However, the bone width of alveolar bone and soft tissue thickness were not found to be correlated with DIM. It is important to understand the meaning of peri-implant tissue dimension in relation to dimensional changes of peri-implant soft tissue which designates appearance of implant-supported restorations. Future study is needed to elucidate the significance of the buccal bone thickness and soft tissue thickness with respect to the change of peri-implant soft tissue margin with the use of an instrument capable of measuring buccal bone thickness directly.
