Phenytoin Induced Erythema Multiforme after Cranial Radiation Therapy

  • The prophylactic use of phenytoin during and after brain surgery and cranial irradiation is a common measure in brain tumor therapy. Phenytoin has been associated with variety of adverse skin reactions including urticaria, erythroderma, erythema multiforme (EM), Stevens-Johnson syndrome, and toxic epidermal necrolysis. EM associated with phenytoin and cranial radiation therapy (EMPACT) is a rare specific entity among patients with brain tumors receiving radiation therapy while on prophylactic anti-convulsive therapy. Herein we report a 41-year-old female patient with left temporal glial tumor who underwent surgery and then received whole brain radiation therapy and chemotherapy. After 24 days of continous prophylactic phenytoin therapy the patient developed minor skin reactions and 2 days later the patient returned with generalized erythamatous and itchy maculopapuler rash involving neck, chest, face, trunk, extremities. There was significant periorbital and perioral edema. Painful mucosal lesions consisting of oral and platal erosions also occurred and prevented oral intake significantly. Phenytoin was discontinued gradually. Systemic admistration of corticosteroids combined with topical usage of steroids for oral lesions resulted in complete resolution of eruptions in 3 weeks. All cutaneous lesions in patients with phenytoin usage with the radiotherapy must be evoluated with suspicion for EM.

성인에서 편도적출술후 정맥내 동통자가조절법에 의한 동통조절 효과 (Effect of Intravenous Patient Controlled Analgesia for Postoperative Pain in Adult Tonsillectomy)

  • Postoperative pain following tonsillectomy remains a significant obstacle to speedy recovery and smooth convalescence. Inadequate analgesia causes poor oral intake and influences the length of hospital stay and ability to return to normal activity. Patient Controlled Analgesia (PCA) is a method of analgesia adminstration that consists of a computer driven pump with a button that the patient may press to adminster a small dose of analgesic drug. The aim of this study was to examine whether Intravenous Patient Controlled Analgesia (IV-PCA) can reduce postoperative pain after tonsillectomy. The 100 patients undergoing tonsillectomy with general anesthesia were divided into two groups. The PCA group patients (n=80) received a mixture of nalbuphine and ketorolac by Walkmed PCA infusor during first 48 postoperative hours. In control group (n=20), the patients received oral acetoaminophen (Tyrenol) regularly and tiaprofenic acid (Surgam) intramuscularly on a p.r.n basis. Analgesic efficacy was evaluated with visual linear analogue scale (VAS) and the adverse effects were evaluated with 4 point scale. The patients of PCA group had less pain than those of control group. The adverse effects in the PCA group were nausea and vomiting. This study suggests that IV-PCA may be safe and effective method of pain control after adult tonsillectomy and is better accepted than oral or intramuscular pain medications.

Effects of Alanine and Glutamine Supplementation on Alcohol Metabolism in ICR Mice

  • This study was conducted to investigate the effects of oral supplementation of alanine and glutamine on alcohol metabolism. The subjects were 70 male ICR mice weighing 25-30 g. The animals were raised on standard rations artier weaning. After 24 hours of fasting, all the animals were given a peritoneal injection of 20% alcohol. Then, they were randomly divided into two groups: control and experimental. Fifteen minutes after the injection of alcohol, the mice in the experimental group wer given an oral solution of alanine(5 mM, 2 g/kg B. W) and glutamine (5 mM, 2g/kg B.W). The concentration of alcohol in the blood was measured in all the mice 20 minutes after they received the alochol, and the measurements continued every 20 minutes up to 140 minutes. The experimental group sustained lower blood alcohol levels at every 20 minute time interval compared to the control group, showing that oral supplementation of alanine and glutamine increases the rate of alcohol metabolism. Furthermore, the total amount of alcohol remaining in the blood, determined by using the Area Under the Curve (AUG) method, was lower in the group supplemented with alanine and glutamine, However, the effectiveness of alanine and glutamine in increasing the rate of alcohol metabolism, compared to the control group, diminished with time throughout the experiment. In conclusion, alanine and glutamine supplementation appears to promote alcohol metabolism shorthy after alcohol intake.

Oral administration of ginseng berry concentrate improves lactate metabolism and increases endurance performance in mice

  • In the present study, to determine the efficacy of oral supplementation of ginseng berry extracts in augmenting exercise performance and exercise-associated metabolism, male mice were given orally 200 and 400 mg/kg of body weight (BW) of GBC for nine weeks. Although there are no differences in pre-exercise blood lactate levels among (1) the control group that received neither exercise nor GBC, (2) the group that performed only twice-weekly endurance exercise, and (3) and (4) the groups that combined twice-weekly endurance exercise with either 200 or 400 mg/kg GBC, statistically significant reductions in post-exercise blood lactate levels were observed in the groups that combined twice-weekly endurance exercise with oral administration of either 200 or 400 mg/kg GBC. Histological analysis showed no muscle hypertrophy, but transcriptome analysis revealed changes in gene sets related to lactate metabolism and mitochondrial function. GBC intake increased nicotinamide adenine dinucleotide levels in the gastrocnemius, possibly enhancing the mitochondrial electron transport system and lactate metabolism. Further molecular mechanisms are needed to confirm this hypothesis.

보조적 프로바이오틱스 복용을 통한 치주 병원성 세균 및 전신질환 지표 변화: 증례보고 (Changes in periodontal pathogens and chronic disease indicators through adjunctive probiotic supplementation : a case report)

  • Objectives: This case study aimed to evaluate changes in periodontal pathogens and systemic disease indicators following the adjunctive use of probiotics for periodontal treatment. Methods: Two adults, a 64-year-old male and 71-year-old female, were selected with ethical approval and underwent comprehensive oral and systemic health assessments before and after probiotic intake with periodontal debridement. Results: There was a significant reduction in the periodontal pathogens, particularly Porphyromonas gingivalis and Treponema forsythia, and no adverse systemic indicators were observed. Moreover, a trend toward improved lipid profiles was noted, suggesting a potential positive impact on systemic health. Conclusions: This study shows the potential role of probiotics in enhancing oral health and preventing systemic diseases, thus highlighting the need for further research and clinical trials.

콜라겐 펩타이드 섭취 무모생쥐에서 UVB 조사에 의한 피부 손상에 미치는 효과 규명 (The Effect of Collagen Peptide Intake on UVB-induced Skin Damage in Hairless mice)

  • 본 연구에서는 Tilapia fish에서 얻은 콜라겐 펩타이드 (TFCP)의 섭취가 UVB를 조사한 무모생쥐에서 어떠한 효과를 나타내는지 확인하고자 하였다. 무모생쥐에 1주일에 3번 UVB를 조사하여 광노화를 유도하였고, TFCP 545, 1090 mg/kg을 매일 총 12주간 경구투여 후, 주름생성, 피부 두께, 홍반, 피부 수분량, 하이드록시프롤린, MMPs 및 filaggrin의 발현량을 측정하였다. UVB만 조사 한 군과 비교하여 UVB를 조사하고 콜라겐 펩타이드를 섭취시킨 군에서 주름생성, 피부 두께 및 홍반이 감소하였고 피부 수분량은 증가하였다. 또한, 콜라젠 양과 하이드록시프롤린의 양도 TFCP를 섭취한 군에서 UVB 조사군에 비해 유의하게 회복되는 것을 확인하였다. TFCP를 섭취한 군에서 자외선에 의해 활성화되는 MMP-2, MMP-9 뿐만 아니라 MMP-3, MMP-13의 mRNA 발현량이 억제됨을 확인하였고, filaggrin의 단백질 발현량도 증가하였다. 이러한 결과들을 통하여 TFCP의 섭취는 자외선에 의한 주름 생성을 억제하고 피부 손상을 회복시킨다는 것을 확인하였다. 따라서, TFCP는 피부미용식품 소재로서 활용 가능성이 있음을 알 수 있다.

예탁선양치(預託線量値)에서 본 성인(成人)과 유아(幼兒)와의 비교연구(比較硏究) (Comparative Study on the Committed Dose Equivalent for Adults and Infants)

  • 국제 방사선 망어위원회 (ICRP)에서 새로이 권고한 체내(體內) 피폭선량(被曝線量) 산정법(算定法)(ICRP Pub. 30)을 이용하여 I-131, I-133, Cs-134 및 Cs-137 네가지 핵종의 단위 방사능섭취당 하중(荷重) 예탁선량(預託線量)($W_T\;H_{50}$)을 몸무게 70 kg, 나이 25세의 성인(成人)과 10 kg, 1세의 유아(幼兒)에 대하여 계산하였다. 경구(經口) 또는 흡입(吸入) 경로를 통해 섭취된 요오드의 신진대사(新陳代謝)를 무기질(無機質), 갑상선(甲狀腺) 유기질(有機質)구역으로 구성된 삼단계(三段階) 구획형(區劃型) 모델을 이용하여 묘사하였다. 이 구역을 사이의 전달방정식을 풀어 섭취후 각 구역들에 있어서 요오드의 양을 계산하였다. Caesium은 섭취후 곧, 전달구역을 동해 전신에 퍼진다. 이 경우 전신에 있는 caesium의 생리학(生理學) 적(的) 반감기(半減期)에 의하여 전신을 두 조직(組織)구역으후 나눈 전신(全身) 구획형(區劃刑)모델을 이용하여 전신에서의 caesium의 양을 계산하였다. 계산결과, 경구(經口) 또는 흡입(吸入), 어떤 경로이든지 단위 방사능성취로 인한 I-131 I-133의 하중(荷重) 예탁(預託) 선량(線量)($W_T\;H_{50}$)은 유아(幼兒)의 경우가 성인(成人)보다 10배정도 많은 값을 나타냈다. 한편, Cs-134과 Cs-137에 대해서는 단위 방사능 섭취당 $W_T\;H_{50}$ 값은 성인(成人)과 유아(幼兒)가 서로 비슷한 값을 나타내었다.

흡연 여부에 따른 한국 성인의 음료섭취와 치주질환의 연관성 (Association between Beverage Consumption and Periodontal Disease by Smoking Status among Korean Adults)

  • 한국 성인들의 흡연여부에 따른 음료섭취량과 치주질환의 연관성을 확인하기 위하여 제6기 국민건강영양조사에 참여한 만 19-64세 중 음료섭취조사와 건강설문조사 및 구강검진을 완료한 9,042명을 연구대상으로 하였다. 치주질환의 주요 위험요인인 흡연여부를 층화변수로 사용하여 복합표본 통계분석을 하여 다음과 같은 결론을 얻었다. 비흡연군에서 치주질환자는 비치주질환자에 비해 커피의 섭취가 많았고, 우유, 탄산음료, 과일주스의 섭취는 적었다. 흡연군에서 치주질환자는 비치주질환자에 비해 커피의 섭취가 많았고, 우유, 액상요거트, 탄산음료, 과일주스의 섭취는 적었다. 외래 변수 보정 후 비흡연군에서는 커피의 섭취가 많은 사람이, 흡연군에서는 우유의 섭취가 적은 사람이 치주질환과 연관성이 있었다. 결론적으로 일부 음료의 섭취빈도는 치주질환과 연관성이 확인되어 치주질환 예방 교육 시 음료섭취의 중요성을 강조하고 식이지침에도 음료섭취에 관한 내용을 추가할 것을 제언한다.

국내 시판 일부 불소함유치약 사용 시 연령별 어린이 불소섭취량 (Relationship of the use of some fluoride containing dentifrice on the korean market to children's fluoride intake in different age groups)

  • Objectives : The purpose of this study was to examine the amount of daily fluoride intake among children using fluoride-containing dentifrice in an effort to pave the way for the selection of criteria for the development of safe dentifrice for different age groups to make a contribution to children's oral health. Methods : This study was implemented over approximately six months from May to October 2008 by recruiting subjects, asking their consent, conducting a survey, collecting samples and analyzing the collected data. The subjects in this study were preschool residents in Seoul and Gyeonggi province, who were at the western age of 2 to 5. The amount of fluoride withdrawn from their one-time toothbrushing was measured in two different ways. One was by using HMDS-facilitated modified diffusion method and fluoride ion electrode, and the other was by applying ion chromatography without the diffusion procedure. Results : The fluoride intake accounted for $46.5{\pm}19.1$ percent of the amount of fluoride used, and that percentage was statistically significantly different according to age(p<0.01). The one-time fluoride intake from dentifrice per weight(kg) was a mean of $0.009{\pm}0.006mg$. As a result of multiplying this amount by daily toothbrushing frequency, the daily fluoride intake from dentifrice per weight(kg) appeared to be $0.023{\pm}0.016mg$ on average. There was a large difference among the children in that regard, since that ranged from a low of 0.003mg to a high of 0.070mg. And age made a statistically significant difference to that(p<0.01). Conclusions : The above-mentioned findings of the study showed that the children's daily fluoride intake from dentifrice per weight was lower than the recommended daily fluoride intake from diets per weight for young children aged 1 to 12(0.05-0.07mg). However, there was a great disparity among the children in that aspect, and they are likely to take more fluoride from other things as well. Therefore it is required to prepare separate criteria for preschool and school-aged children.

여대생의 단맛 인지도에 따른 식행동 및 간식류를 통한 당류 섭취량 평가 (Dietary Behaviors and Total Sugar Intake from Snacks of Female College Students according to Sweet Taste Perception)

  • Increasing sugar intake of population has become a nutritional issue in Korea. Sweet taste perception may be related to behaviors such as eating sweet food including high sugars and total sugar intake. This study aimed to evaluate objective and subjective sweet taste perception and the association among objective sweet taste perception, dietary behaviors related to eating sweet snack food including high sugar, and total sugar intake from the snacks. Participants were 261 healthy female college students (mean age: $21.0{\pm}1.6years$), who were divided into three subgroups based on oral sweet taste evaluation using a sweet taste assessment tool provided by Ministry of Food and Drug Safety: sweet-seeker group (n=139), medium sweet-seeker group (n=54), and unsweet-seeker group (n=68). There was no significant difference in weight and body mass index (BMI) among the three groups; however, the sweet-seeker group had significantly higher sweet taste preference than that of the other groups. Though more people in the sweet-seeker group thought they tended to eat sweet foods than the medium sweet-seeker and unsweet-seeker groups, over half of the sweet-seekers did not think they tended to eat sweet foods. The sweet-seeker group was more likely to eat sweet snacks such as breads, chocolate products, sugar-sweetened milk, and so on than the unsweet-seeker group. Total sugar intake from the selected sweet snacks was 44.4 g for the sweet-seeker group, 34.4 g for the medium sweet-seeker group, and 28.0 g for the unsweet-seeker group with a significant difference. These results indicated the absence of relationship between objective sweet taste perception and the obesity index; however, significant associations were detected among objective sweet taste perception, eating sweet snacks and total sugar intake from the snacks. We also found high disagreement between objective and subjective sweet taste perception of the subjects. The present study provided the novel insight that measuring objective sweet taste perception may be useful for assessing the risk of high sugar consumption and undesirable dietary behaviors.
