Relationship between the prevalence of temporomandibular joint disorders in some male high school students and computer utilization (일부 남자 고등학생의 측두하악장애 관련 증상의 유병률과 구강악습관 및 컴퓨터 이용시간과의 관계 연구)

  • Objectives : The purpose of the study is to verify the correlation between teenagers' use of internet and temporomandibular joint disorders(TMJD) because TMJD patients gradually increase in high school students. Methods : Subjects were 221 male high school students attending liberal arts and commercial schools from 9 to 21 of April, 2012. Students completed self-reported questionnaire. Results : TMJD symptoms included crackling joint (32.1%), headache (18.6%), malocclusion (8.1%), and jaw fatigue and muscular aching (7.2%). Students having at least one symptom accounted for 49.8%. Students having at least one poor habit accounted for 83.7% and 34.0% of the students used internet for 3 hours.. Score of students experiencing shoulders and chest pain was $5.13{\pm}1.10$ (p<0.05). Score of TMD afflicting area revealed $4.67{\pm}2.73$ (p<0.05) in mastication pain ($4.13{\pm}2.73$ (p<0.05)) due to long hours of internet use. Score of jaw fatigue related to muscular pain was $4.31{\pm}1.96$ (p<0.01). Duration of computer use was related to perceived TMJD symptoms (r=.153). Long duration of computer use was related to poor oral cavity habits (r=.157). Conclusions : Male high school students can perceive slight TMD which needs self management and regular check-ups and try to prevent TMD by utilizing measurement tools on TMD. Also, this study helps to prevent from worsening TMD by shortening hours of computer utilization.

Analysis of related factors for dental fear (치과치료 공포감 관련요인분석)

  • Objectives : The purpose of this study is to seek analysis of related factors for dental fear and to know the fear component affecting the treatment of the dental treatment. Methods : Total of 320 females and males aging from 10 to 40 years old with dental treatments done from May to July 2010 have participated with self-entry survey method utilizing the Dental Fear Survey questionnaire. The data received was analyzed using the descriptive statistic, t-test, ANOVA and multiple correlation analysis with level of significance as p<0.05. Results : The dental fear factor were significantly high in woman by $2.79{\pm}0.92$ and in 40's age group by $2.82{\pm}0.84$ (p<0.05). Participants with frequent tooth ache, experiencing pain while treating the tooth ache and breaking the dental treatment appointment showed higher fear factor. Thoughts of having poor dental health condition caused bad influence on fear factor. Overall, the factors affecting the fear of the dental treatments were gender, an ache while treating, number of missed appointment, and distrust of the dentist, stimulation factor were the factors affecting the treatment. Conclusions : The fear factors in the dental treatment were higher with gender, related characteristics of the oral health and distrust of the dentist. Therefore accurately recognizing the sensitive patient with dental treatment, proper management of the toothache, effort to increase the creditability of the doctors and separately treating patient customized for each individual patient will reduce the fear of the dental treatment.

Psychological analysis of dental orthodontic patients using SCL-90-R (간이정신진단검사를 이용한 치과교정환자의 심리분석)

  • Objectives : This research was investigated to evaluate the psychological characteristics and changes of psychological state among dental orthodontic patients. Methods : This cross-sectional questionnaire research was conducted with 230 dental orthodontic patients in Busan from December 23, 2009 to March 6, 2010. The psychological analysis of dental orthodontic treatment patients was performed with Symptom checklist-90-revision(SCL-90-R) considering 5 general characteristics and 2 treatment period related characteristics. Estimated psychological results were changed T-score. Data analysis was performed with descriptive analysis, t-test and ANOVA using SAS(ver 9.1) program. Results : The levels of T-score of SCL-90-R were $43.88{\pm}7.50$ in hostility(HOS), $43.38{\pm}4.64$ in phobic anxiety(PHOB), $43.20{\pm}6.24$ in somatization(SOM), $42.13{\pm}6.71$ in paranoid ideation(PAR), $41.39{\pm}8.16$ in interpersonal sensitive(I-S), $41.01{\pm}7.90$ in obsessive-compulsive(O-C), $40.96{\pm}5.37$ in psychoticism(PSY), $40.96{\pm}5.19$ in anxiety(ANX) and $39.81{\pm}6.80$ in depression(DEP), respectively. The T-score of phobic anxiety in before treatment group was higher than that of treatment groups. The T-score of interpersonal sensitive and paranoid ideation in over 36 months treatment period group were higher than that of other treatment period groups. Conclusions : Phobic anxiety(PHOB), interpersonal sensitive(I-S) and paranoid ideation(PAR) were affected in pre-treatment and long-term treatment dental orthodontic patients. Psychological management methods considering treatment period are needed to improve mental health of dental orthodontic patients.

Surface changes of denture base resin according to two toothpastes and a kitchen detergent (일부 세치제와 주방세제 사용에 의한 의치상 레진의 표면변화)

  • Objectives : This study evaluated the changes in surface roughness of denture base resin according to the types of denture cleansers. Methods : A denture base resin(Vertex RS, Dentimax, Netherland) was used. Two toothpaste(Antiplaque, Bukwang, Korea; 2080, Aekyung, Korea) and a kitchen detergent(Trio, Aekyung, Korea) were used as a denture cleanser. The specimens were put on the V8 crossbrushing machine(Sabri enterprises, Downers grove, IL, USA) to reproduce toothbrushing and the toothbrushes were flat, round end and soft type. The surfaces of denture base resin specimens were observed by profilometer(SJ-400, MITUTOYO, Japan) and SEM(S-3000N, Hitachi Co., Ibaraki, Japan). Results : 1. According to the result of measuring surface roughness, there was statistically significant difference in Ra, Rq, and Rz(p<0.05). 2. As for Ra, Rq and Rz, Antiplaque toothpaste showed the highest roughness, and there was significant difference from other groups(p<0.01). 2080 toothpaste, Trio, and distilled water were classified as the same group. 3. According to the result of observation with the SEM, the surfaces of the Antiplaque toothpaste group after toothbrushing showed the greatest roughness, and the surfaces of 2080 toothpaste, Trio, and distilled water groups were rough in order. Trio and distilled water had the surfaces similar to those before toothbrushing. Conclusions : Studies to compare the efficacy of denture management methods and examine the effects of denture cleansers on denture materials will be helpful for dental hygienists and dentists providing patients with proper information and education. And it will be also useful for denture users' oral health.

  • In the last decade, the replacement of old PFM restorations has been a common dental procedure, especially on the anterior regions. Gingiva conditions in most re-treatment cases are not healthy with the old restorations due to ill-fitting margin, improper tooth reduction and poor oral hygiene. Practitioners must carefully evaluate and manage periabutmental gingiva for final restorations. The role of provisional restoration became a major clinical concern at this point. Well-fabricated provisional restorations with precise-fit margin and proper anatomy are critical for achieving good gingival health. Thus, the key of success in replacing old PFM crown depends on the proper tissue management by precise provisional restorations.

  • Dental panoramic radiography is an imaging technique which shows the information of teeth, jaws and superficial structures on a single image. In this study, we propose the clinical dose reference for dental panoramic radiography. Dental panoramic radiography is an application which can increase the radiation does of oral cavity. It is very important to study the real condition of management for these panoramic X-ray equipments. Since there was no researches on dental panoramic equipments in domestic and foreign study groups, we measured and analyzed the dose such as effective dose, DAP and DWP of panoramic radiography.

  • Osteopetrosis is characterized by impaired osteoclast function and increased bone density. Infantile osteopetrosis is a severe form of the disease and has characteristics such as diffusely sclerotic skeleton, pancytopenia, cranial nerve entrapment, infection susceptibility, and abnormal craniofacial appearance. Patients with infantile osteopetrosis often experience developmental delay, and may have a short life span. A 14-month-old girl with osteopetrosis presented to the department of pediatric dentistry. Incipient caries on deciduous incisors were observed. The patient revisited 4 years of age. Besides medical problems, oral complications such as growth retardation, narrow upper arch, crowding, dental caries, and abnormal tooth development were observed. After consultation with her pediatrician, dental treatments were performed on the deciduous molars under sedation after a prophylactic antibiotic injection. At a periodic follow-up, multiple deciduous teeth were treated and extracted, and oralrehabilitation with a removable partial denture was initiated. Patient with osteopetrosis are highly susceptible to infection because of their compromised immune system and problems associated with wound healing that lead to osteomyelitis or sepsis development. Active participation in dental care for sugar intake management and proper oral hygiene are obligatory.

Caries Management of High-Risk Children by Caries Risk Assessment (우식위험평가에 의한 우식 고위험 유아의 치아우식 관리)

  • The purpose of this study was to classify risk groups according to Caries Risk Assessment (CRA) and to investigate the effect of caries prevention program after 1 year of caries prevention intervention program in 6-year-old infants with high caries risk. The subjects were selected based on responses to CRA questionnaires. At the first visit, oral examination, Cariview, bacteria and saliva flow test were performed. The caries risk group was classified accordingly. The subjects were given fluoride application and oral health education every four months and evaluated the same as the first visit after 1 year. As a result of classifying the risk level according to CRA, more than 80% of the subjects were in the high or extreme high risk. The dft index was increased in all risk groups after the intervention. There was a significant difference between the before and after intervention (p<0.05). The Cariview score showed a slight decrease after the intervention in the moderate and high risk groups. As a result of the evaluation of bacteria test, Streptococcus mutans were decreased to ${\geq}10^5CFU/ml$ saliva after intervention in all groups. Lactobacilli were decreased after intervention in high risk and extreme high risk groups. As a result of saliva flow, there was significant difference between caries risk groups before and after intervention (p<0.05). In conclusion, regular caries management has been shown to influence caries risk factors in high-caries risk children. Also, it is necessary to find out periodical dental risk management system which is suitable for domestic situation through the related studies.

Prosthetic rehabilitation by obturator considering the biomechanics in partially edentulous patient after maxillectomy (상악골 절제술을 받은 부분 무치악 환자에서 생역학을 고려해 제작한 구개 폐색장치를 이용한 보철 수복 증례)

  • Patients who went through maxillectomy can have severely impaired swallowing, mastication, and pronunciation functions because of palatal defects. Leakage occurs through the nasal cavity while eating, chewing becomes difficult due to the loss of teeth and alveolar ridges, and oral and nasal passages are not separated, leading to hyper-nasal sound, and significantly reducing the quality of life. To prosthetically reconstruct the defect, the weight of the obturator should be reduced as much as possible to minimize dropout because of gravity, and the bulb of the obturator should be properly extended into the defect to get additional retention and stability. In this case of a partially edentulous patient who underwent additional maxillary resection because of tumor recurrence, a metal framework was designed by applying the basic design principles of removable partial dentures. An obturator with improved retention, stability, and support was fabricated through functional impressions. The patient was satisfied with the improved facial expression, mastication, swallowing, and pronunciation, and showed stable occlusion and oral hygiene management during the follow-up period.

Oral Health Status and Dental Treatment Need of Liver Transplant Candidates (간 이식 예정 환자의 구강건강상태 및 치과치료 요구도에 관한 연구)

  • Liver transplantation is definitive treatment for the patients suffering from hepatitis, severe liver cirrhosis and liver cancer. In these patients, systemic infections under immunosuppression may occur easily. Therefore, primary object of dental treatments before liver transplantation is absolute removal of oral infection source. In addition, comprehensive dental management plan is essential for success of liver transplantation. The present study has been performed to investigate decayed, missing and filled permanent teeth index(DMFT index), degree of oral hygiene, past medical history, need of dental treatment, completion of dental treatment need and time interval between dental visit and operation date of liver transplantation in liver transplant candidates. Obtained results were as follows; 1. Decayed teeth of the patients were 2.68, missing teeth were 4.02 and filled teeth were 3.42. DMFT index was 10.12. 2. Twenty percents of patients showed moderate to severe food impactions, 42.2% of patients had moderate to heavy calculus and 37.8% of patients displayed gingival inflammation with swelling. 3. Patients needed periodontal treatments more than any other dental treatments. Periodontal treatments were needed for 88.9% of patients, operative & endodontic treatments were 46.7% of patients and 33.3% of patients needed for oral & maxillofacial surgical treatments. 4. Among 90 patients, time interval between scheduled operation date of liver transplantation and dental visit was within 2 weeks for 32.2% of patients, within 1 week for 20.0% of patients. In conclusion, most liver transplant candidates needed dental treatments for removal of potential infection sources. However because of insufficient interval between dental visit and operation date, they had taken liver transplantation procedures without comprehensive dental management. Development of preventive and comprehensive dental management program is mandatory for these patients. Cooperative interdisciplinary management will play a positive role for successful liver transplantation.
