Reviews of Literature on Dental Plaque Control and Oral Hygiene Education in Korea (치면세균막관리와 구강보건교육에 관한 국내문헌고찰)

  • The purpose of this study was to understand the effectiveness of oral health education (OHE) or oral hygiene instruction (OHI) involving professional plaque control/removal, as compared to conventional plaque control/removal. By means of a systematic review of the literature, in the review of literature by using systematic method, Korean articles of plaque control including OHE or OHI, were studied in order to analyze and conclude the literature to enhance oral health. We found that self and professional plaque control/removal, in addition to tooth brushing instructions, decreased incidence and prevalence of loss of teeth. Taken together, professional mechanical plaque removals were most effective 4~5 visits every 1~2 weeks. Use of disclosing agent was more effective than oral education or model education. In line with oral hygiene education, professional brushing, oral prophylaxis, scaling and root planing, it was advisable to repeat the training according to the characteristics of the patient. Routine OHE or OHI would be help to increase to oral health. For a quality of life related oral health, reliability and validity of data are needed to develop and its data should be applied to dental health insurance policy.

The 4th Year Appraisal for Effectiveness of School-based Oral Health Program in Daegu, Korea (대구광역시 초등학교 구강보건실 시범운영의 효과평가 - 사업 4차년도 평가 분석 -)

  • Objectives: The purpose of this study was to evaluate the long term effects of school-based oral health program on the decrease of dental caries among elementary school children. Methods: The subjects of this study were total 283 students of one elementary school in Deagu, Korea. They had received school-based oral health programs more than one year since 2004. This school-based oral health program included regular oral examination, fluoride mouthrinsing, pit-and-fissure sealing, APF gel application, tooth brushing instruction and chewing the xylitol tablets. The subjects' oral health status were examined and calibrated by a dentist every year from 2004 to 2007. Results: The change of DMFT index were as follows: (1)The subjects who entered the school at 2004 - 0.32(2004), 0.25(2005), 0.25(2006) and 0.38(2007), (2)the subjects who entered the school at 2005 - 0.18(2005), 0.31(2006) and 0.32(2007), and (3)the subjects who entered the school at 2006 - 0.19(2006) and 0.27(2007). Conclusions: This study partially showed the effects of school-based oral health programs on the increments of dental caries. This programs should be expanded widely among elementary schools in Korea.

THE STUDY OF CHANGES IN ORAL HEALTH CARE OF PRESCHOOLERS IN TAEBAEK CITY THROUGH ORAL HYGIENE EDUCATION (구강보건교육을 통한 태백시 미취학 아동들의 구강건강관리 변화에 대한 연구)

  • The objective of this study is to find the improvement in the dental health care of the children through oral hygiene education and TBI. In Taebaek public health care center, an oral hygiene education, TBI and fluoride gel application was conducted 2 times for about 631 preschool children living in Taebaek City. The ages of preschool children were from 4 to 7. To examine the changes, a questionnaire was written and distributed for the collecting the results after the education of the second half for children, parents and teachers. As a result, the children showed better understanding of the caries through oral hygiene education and reduction of fright and fears for dental treatment. Many children learned how to brush and there was a significant increase in the number of brushing. Children learned to be able to classify good food and bad food for teeth and showed the change in eating habits although in small part through the oral hygiene education. Through this study, it is judged that the oral hygiene education of the preschool children has effectiveness and it is in need for the expansion of oral hygiene education in homes, kindergartens and dental offices.

The Effect of Continuous Nutritional Education and Oral Mucositis Management on Nutritional Status of Patients Undergoing Hematopoietic Stem Cell Transplantation (지속적인 식이교육과 구내점막염 관리가 조혈모세포이식(HSCT) 환자의 영양상태에 미치는 효과)

  • Purpose: This study aimed to evaluate the effectiveness of continuous nutritional education and oral mucositis management on the nutritive status of patients who received hematopoietic stem cell transplantation (HSCT). Methods: After randomly allotting 72 patients who received HSCT to either an experimental group or a control group, intensive and continuous care for preventing malnutrition was conducted in the experimental group while usual routine care was conducted in the control group. The changes of the body scale, blood chemistry profile, oral intake calories, nausea and vomitus, and oral stomatitis scores were measured at three points during their hospitalization using a oral assessment guide and nutrition analysis program: admission, HSCT, and discharge day. The differences between the scores of two groups were analyzed by repeated measures analysis of covariance. Results: The number of total lymphocytes was significantly improved in the experimental group after transplantation (p<.001). Nausea and vomiting score was significantly decreased in the experimental group during the conditioning regimen (p<.001). Conclusion: It was found that continuous nutritional education and oral mucositis control is an effective intervention by improving immune condition. Further investigations concerning direct examination of oral intake with controlling the effect of the chemotherapy are needed to ultimately discern the impact of varying oral nutrition patterns during HSCT.

Updates of Nursing Practice Guideline for Oral Care (근거기반 구강간호 실무지침 개정)

  • Purpose: This study aimed to update the previously published nursing practice guideline for oral care. Methods: The guideline were updated according to the manuals developed by National Institute for Health and Care Excellence (NICE) and Scottish Intercollegiate Guidelines Network (SIGN), and a Handbook for Clinical Practice Guideline Developer Version 1.0. Results: Updated nursing practice guideline for oral care was consisted of 10 domains and 79 recommendations. The number of recommendations in each domain were: 5 general issues, 2 oral care indications, 9 oral assessment, 16 general oral care, 12 oral care for critically ill patients, 16 oral care for cancer patients, 12 oral care for cancer patients with oral complications, 5 oral care education, 1 oral care referral, and 1 documentation and report. In terms of grades for recommendations, 11.4% was grade A, 17.0% was grade B, and 68.2% was grade C. Twelve new recommendations were developed and 7 previous recommendations were deleted. Conclusion: Updated nursing practice guideline for oral care is expected to serve as an evidence-based practice guideline for oral care in South Korea. It is recommended that this guideline be spread to clinical nursing settings nationwide to improve the effectiveness of oral care practice.

Oral Health and Quality of Life of the Orphans in Dong-gu, Daejeon (대전 동구 보육원생의 구강건강 및 구강건강관련 삶의 질)

  • The aim of this study was to examine the effectiveness of oral health promotion program in a group of 9~18-year-old children and adolescents living in four orphanages in Dong-gu, Daejeon. The program was based on oral disease prevention program including oral health education, fluoride application and scaling every six months. Oral health status of total 109 orphans was examined by one dentists who were trained in 2010 Korean National Oral Health Survey. Dental caries index, community periodontal index and modified patient hygiene performance index (M-PHP) were checked using dental unit chair. Child oral health impact profile (COHIP) and subjective oral health recognition survey were carried out. Compared with data of 2010 national sample, the mean of decayed, missing and filled teeth showed no difference between the subjects and test values, but the means of decayed teeth, decayed surface, toothbrushing frequency of the subjects showed to become worse with advancing years in spite of oral health promotion program. COHIP, subjective oral health status showed lower than test values, too. In M-PHP and Calculus index, the subjects showed better by periodic oral health education and scaling. We suggest that oral health promotion program for orphans include oral disease treatment program as well as preventive program to improve oral health of orphans efficiently. And, oral health promotion program has to be connected with psychological support for improving quality of life of orphans.

Oral health education on recognition and their prevalence of dental caries comparative analysis of some primary school pupils' in Buckjeju-gun (북제주군 일부 초등학교 학동들의 구강보건교육에 관련된 인식도 및 영구치 우식경험도 비교 평가)

  • This study has been conducted with continuous dental sanitary education for primary school pupils for five years from 1997 through 2001, based on data obtained from a 97' survey on primary school pupils' recognition on dental hygiene education and their permanent dental health capacity. Following results were drawn through comparative analysis of data obtained during the survey period. Approx 70.77% of the examinees have experienced decay missing feeling (DMF) in the year 2001, suggesting a good effectiveness of dental hygiene education compared with 92.1 % of DMF rate in 1997. It has been found that pupils' knowledge and recognition on dental hygiene and management, etc were improved, as well as their eating habits and consciousness were changed. Comparative analysis of annual DMF showed that DMF rate, DMFT index, and DT rate were found to decrease every year, suggesting a improved dental health capacity. Grade level analysis revealed that DMFT index and DT rate were found to decrease every year during the survey period, suggesting pupils' dental management and consciousness were improved and changed. It has been found that DMF rate more significantly increased in a higher grade in 2001 than 1997. There was no difference in DMF rate between grades of primary school in 1997. However, in the year 2001 increment of approx 10% of DMF rate were observed in a higher grade.

Effects of Health Promotion Program on Physical Fitness and Quality of Life of Elderly Women Receiving Home Visiting Health Care Services (건강증진 프로그램에 참여한 방문건강관리 여성노인의 체력과 삶의 질)

  • Purpose: The study evaluated the effectiveness of health promotion program on the physical fitness and quality of life of elderly women receiving home visiting health care services. Methods: There were 122 elderly women participants. The data were collected between March and December 2019. The participants were provided with the 36-weeks health promotion program consisting of health education, such as nutrition, depression, urinary incontinence, fall, oral care, and exercises, such as stretching, weight-bearing exercise, and elastic resistance training. The balance, muscle strength, flexibility, and quality of life were measured before and after the program. The results were analyzed with paired t-test using the SPSS/WIN 26.0 program. Results: The dynamic balance, muscle strength, flexibility significantly increased. Conclusion: The health promotion program positively affected elderly women in terms of physical fitness, but there are limitations to increasing the quality of life of elderly women. Through this study, it is necessary to be supplemented in improving quality of life of elderly women.

Comparison of the Practical and Theoretical Knowledge of the Hepatitis B Virus among Dental Hygiene Students

  • Background: Hepatitis B is an important public health problem as one of the leading causes of morbidity and mortality. The aim of this study was to compare the theoretical and practical knowledge levels of dental assistant students about Hepatitis B Virus. Methods: The 1st and 2nd grade students of the Oral and Dental Health program were invited to participate in the survey. Out of the 68 invited students, 61 completed the questionnaire. The average ages of the male and female students surveyed were 20.27±1.45 and 19.56±1.16, respectively. A total of 34 questions were asked, of which 15 measured basic theoretical knowledge and 19 assessed basic practical knowledge. Results: There was no statistically significant difference between the students' knowledge for each question according to their sex and grade. When the questionnaire was grouped into basic theoretical and basic practical knowledge levels, both were observed to be high. While the lowest correct answer rate was 35.00% for the questions about practical applications, it was 31.14% for the questions measuring the level of theoretical knowledge. There was no statistically significant difference when the levels of knowledge of 1st and 2nd grade students were compared. Students answered the majority of the questions correctly, and ranged between 71% and 100%. Conclusion: Students' high level of basic theoretical knowledge can be a result of their in-class education on the fundamentals. However, their knowledge about the correct approaches in practical applications indicates the beneficial role of having well-defined criteria and prevention protocols that are required in hospitals and the effectiveness of their environmental orientations.

Evaluation of Dental Hygienist Job Validity according to Judgment Standard of Medical Practice in Medical Law (의료행위 기준에 따른 치과위생사 직무 타당도 평가)

  • The purpose of this study was to evaluate the validity of dental hygienist job according to judgment standard of medical practice in medical law. In this study, we conducted a self-administered questionnaire survey to evaluate the validity of dental hygienist job for 12 professors of dental college in Gangneung-Wonju National University from November 10 to 20, 2017. We investigated whether the dental hygienist job conforms to the three criteria of medical practice such as disease prevention and treatment, patient care, and health hazard. The response rates were scored and classified into four categories according to the final score. As a result of this study, dental hygienist job are classified into four categories according to judgment standard of medical practice. The higher the level of the category, the higher the degree of difficulty, and the higher the level of expertise and skills required. More than 50% of respondents answered that measuring the gingival pocket, bleeding on probing, professional tooth cleaning, oral health education, counseling after dental treatment are all three criteria for medical treatment. And these were classified into Level 4 group which requires the difficulty and expertise in the final score 4.3. It is necessary to develop and utilize standardized guidelines on the level of knowledge, education, and qualification standards required for dental practice in order to effectively allocate work among the dental personnel while ensuring the health rights of patients in the dental clinic field in Korea. In addition, there is a need to evaluate the various aspects of cost effectiveness, dental health service productivity, and health promotion contribution to dental hygienist jobs, And based on this evidence, it is necessary to continue to expand and adjust the dental hygienist job and to reorganize the dental workforce system.
