Selective factor of dental institution according to health-promotion lifestyle in the elderly (노인의 건강증진생활양식에 따른 치과의료기관 선택 요인)

  • Objectives : The aim is to examine what a selective factor of dental institution in the elderly is, which can be used as a strategy for vitalizing hospital management, and which difference there is in selection of dental institution according to health-promotion lifestyle. Methods : Questionnaire survey of individual interview was carried out targeting 177 old people in over 65 years old of Jeonju, Iksan, and Gunsan in Jeollabuk-do Province by convenience sampling from August 16, 2010 to August 27. The analysis was performed by using SPSS WIN 12.0 program. Results : 1. The average mark of health-promotion lifestyle in the elderly was 2.69 points. The health responsibility was the highest with 2.91 points. 2. The mark of health-promotion lifestyle was higher in the higher educational level of the elderly(p<0.01), when monthly income is more than 2 million won(p<0.01), and in the better subjective awareness of oral condition(p<0.01). 3. The group with high mark of health-promotion lifestyle regarded reliability(p<0.001), economic efficiency(p<0.01), and facilities & cleanliness(p<0.01) as important factors when selecting dental institution. Conclusions : The development and expansion in health-promotion program targeting the elderly are seen to be necessary that reflect difference by sphere in health-promotion lifestyle. There will be necessity for pursuing even a strategy for the elderly in the group who have relatively low characteristic in the practice level of health-promotion lifestyle. Also, given understanding characteristics on the elderly and focusing on management proper for the needs of medical consumers, the successful medical management is thought to be likely possible.

The Realities of Smoking among Some College Students and Their Relevant Knowledge (일부 대학생들의 흡연실태 및 지식도)

  • The purpose of this study was to examine the state of smoking among some college students, the smoking motivation of smoker students, what made them quit smoking and how they got ready for giving up smoking, as smoking had a huge impact on national health and there was a desperate necessity for preventive antismoking education programs to let students keep away from smoking. After a survey was conducted with self-administered questionnaires from October 1 through November 1, 2006, the responses from 400 students were gathered, and 384 answer sheets were analyzed except 16 incomplete ones. The findings of the study were as follows: 1. By gender, 62.4% of the male students and 28.6% of the female students were smokers, and the state of smoking was statistically significantly different according to their gender(p<0.05). 2. The largest number of the male students, which accounted 44.1%, started smoking in their high school days, and 39.5% of the females did that in middle school. As for a daily mean amount of smoking, 41.8% of the male students, the greatest percentage, smoked 10 to 19 cigarettes a day, and one to nine cigarettes were most common among the females, which represented 57.9 %. But the gap between the two was insignificant(p>0.05). As many as 83.6% of the males and 86.8% of the females had ever attempted to quit it. 3. Concerning awareness of antismoking policies, they felt that a raise in cigarette price would lead to less smoking, and that antismoking advertisement or posters through television or newspaper would deliver the same results as well. There was a statistically significant difference among their awareness(p<0.05). 4. Regarding preparation stage for quitting smoking, 31.3% of the male smokers had no intention to give it up, and 23.2% considered it. 36.7%, the largest percentage, got ready to refrain from it. Among the female smokers, 34.2% had no plans to abstain from smoking, and 36.8% took it into consideration. 23.7% got ready to do that. The females who got ready for that were outnumbered the males who did. 5. As to connections between the state of smoking and relevant knowledge, the students didn't have a good knowledge on that, and a statistically significant difference existed between the smokers and nonsmokers in smoking knowledge. 6. As for knowledge about oral diseases, they were highly cognizant of the relationship of smoking to tooth discoloration, nicotine stomatitis and bad breath, but they didn't know well about its relationship to delayed recovery from dental treatment, implant failure and ozena. Their smoking condition made a statistically significant difference to their knowledge(p<0.05). In the future, sustained research efforts should be channeled into determining how much smoking affects health and concerns oral diseases, and antismoking counseling programs should be prepared to bolster people's awareness of oral health.

  • Gingivitis is the most prevalent type of periodontal disease and the dental plaque is considered as a major contributory factor. As the poor oral hygiene is firmly related to the occurrence of periodontal disease, pediatric dentist should make every effort to promote the oral health and control the plaque effectively for the high risk patients, especially for those who are under orthodontic treatment. P.M.T.C.(Professional Mechanical Tooth Cleaning), introduced by Dr. P. Axelsson in 1969, is a very effective method of plaque removal and can be performed by specially trained personnel. Two pediatric orthodontic patients were treated by P.M.T.C. for the elimination of gingivitis and gingival swelling. Signi ficant improvements of gingival condition were achieved in both cases but additional preventive programs and home care along with professional office care seem to be necessary for the best result.

S대학 신입생의 구강위생상태 및 치주조직질환의 이환율에 관한 연구

  • In order to appreciate the school dental services of the primary, middle, and high school and to get the basic data for the university dental service programs in Korea, the author had examined the oral health status of 1,300 freshmen in S University. Then, the followed results were obtained from the calculation and evaluation of DEMT index, DT index, MT index, FT index, DT rate, MT rate, FT rate, debris deposit rate, calculus deposit rate, periodontal disease rate, and abnormal jaw relation rate. 1. DMFT index in the freshmen was higher than that of the Korean youth in the same age. 2. It seems the above result to be appeared from the influence of the intake of the more carbohydrate foods in the freshmen. 3. In the freshmen, MT rate was lower, and FT rate higher than in the Korean youth in the same age. 4. The oral hygiene condition was good in 3.15% of the freshmen. 5. Calculus was not deposited in 8.69% of the freshmen. 6. In 9.69% of the freshmen, the periodontium was healthy. 7. In Korea, the school dental services in the primary, middle, and high schools must be improved. 8. There was the abnormal jaw relation in 12.92% of the freashmen.

Avian Pox Infection in a Eurasian Jay (Garrulus glandarius) in Korea (어치에서 계두의 자연발생 증례보고)

  • A Eurasian jay (Garrulus glandarius) with lethargy, dyspnea, and poor physical condition was rescued and referred to the Animal Health Center, Seoul Zoo, Korea. The jay's eyelids were sealed with dark-colored hard scabs and it died the day after arrival. At necropsy, diphtheritic membranes on the tongue and oral mucosa were apparent, suggesting avian poxvirus infection based on the appearance of the oral diphtheritic lesions. PCR was conducted using a tissue sample to confirm the causative agent. The jay was diagnosed with an avian pox viral infection. This report describes the first case of a natural avian pox infection in a Eurasian jay in Korea.

Influence of metabolic on periodontal disease in Korean adults (한국 성인의 대사 증후군이 치주질환에 미치는 영향)

  • Objectives: The purpose of the study is to investigate the influencing factors of metabolic syndrome on the periodontal disease in the Korean adults over 40 years old. Methods: The data were extracted from the Fifth National Health Nutrition Survey 2010. The subjects were adults over 40 years old. The periodontal diseases were examined by direct survey. The questionnaire consisted of the general characteristics of the subjects, oral health behavior, metabolic syndrome, and relation to periodontal diseases. CPI(community periodontal index) was measured. CPI indicates periodontal condition and classified into alveolar arch of the upper jaw and the lower jaw. A periodontal disease is defined as $CPI{\geq}3$(over 4 mm). The variables for metabolic diseases included hypertension, diabetes mellitus, dyslipidemia, and obesity measured as BMI. Results: Diabetes mellitus is closely related to periodontal diseases prevalence. Low level of HDL increase the periodontal diseases. Number of toothbrushung, use of dental floss and interdental toothbrush as for the number of toothbrushing, more than twice toothbrushing a day, and use of dental floss and interdental toothbrush reduced periodontal diseases. Conclusions: The risk factors of periodontal disease in Korean adults over 40 years old were sex, age, residence, monthly income, education level, smoking, diabetes and low HDL cholesterol.

Paralytic Ileus Secondary to Electrolyte Imbalance: A Case Study in a 16 Year Old Female

  • Paralytic ileus is a metabolic state in which the intestines fail to transmit peristalsis due to failure of the neuromuscular mechanism in the small intestines and colon. It is a major cause of morbidity in hospitalized patients especially during late presentations and points of mismanagement. The causes include infections, electrolyte imbalance (hypokalemia, hyponatremia), surgeries and medications. When the exact cause of the disease condition is identified and corrected, paralytic ileus is usually resolved. This case report is that of a 16 year old female who was admitted and managed as a case of paralytic ileus. The patient presented with symptoms of fever, abdominal pain, abdominal distension, vomiting and inability to pass stool or flatus. There was associated body weakness, reduced urine output and weight loss. She was properly examined clinically and sent for various investigations. Investigations such plain abdominal X-Ray, serum electrolyte estimation, chest X-Ray and full blood count were carried out. The results of the investigations done were in keeping with the diagnosis of paralytic ileus, electrolyte imbalance and ongoing sepsis. She was subsequently managed through nil per oral, adequate fluid rehydration, antibiotics and correction of electrolyte imbalance. Following stable clinical state and investigation results, she was discharged and advised on follow-up.

Breast and Cervical Cancer Related Practices of Female Doctors and Nurses Working at a University Hospital in Turkey

  • Background: Breast and cervical cancers are among the most frequent and most fatal cancers in women. Life span of patients may be increased and quality of life improved through early diagnosis and treatment. This investigation was performed in order to determine knowledge and practices of female health personnel working at a university hospital regarding breast and cervical cancers. Materials and Methods: This descriptive investigation was performed in Erciyes University Hospitals in 2014. A total of 524 female health personnel were included in the study. Data were collected through a questionnaire of 36 questions prepared by the researchers. The Chi square test and logistic regression were used for statistical analyses. Results: The mean age of the study group was $32.8{\pm}6.9$ years, 18.3% being doctors and 81.7% nurses. Of the study group, 60.5% stated that they performed self breast-examination, 4.4% underwent HPV testing, 26.3% thought about taking an HPV test, 34.7% of those who are 40 years and over had mammography regularly and 19.5% of those who were married had a Pap smear conducted regularly. Most important causes of not performing the methods for early diagnosis of breast and cervical cancers are "forget and neglect". Conclusions: It was concluded that female doctors and nurses do not pay sufficient attention to screening programs for breast and cervical cancers. The importance of early diagnosis and treatment should be emphasized during the undergraduate education and in-service training programs. Health condition of personnel and their utilization of preventive health care should be followed by occupational physicians.

The Study on Burden of Mothers of Children with Nephrotic Syndrome (신증후군 환아 어머니의 부담감에 대한 연구)

  • This study was designed to identity the degree of burden felt by mothers of children with nephrotic syndrome. Also, relations between the subject characteristics and burden were investigated to provide basis data for their family health and nursing intervention. The study subjects were composed of 70 mothers of nephrotic syndrome patients whose children were hospitalized in 2 Pediatric wards of University Hospital in Seoul and 1 in Pusan from Mar. 2nd, 1998 to May. 30th in the same year The questionnaires were used which dealt with burden of mothers. the questionnaires for this study designed and used by researcher placed their basis in Burden Measurement Instrument developed by Montgomery et. al (1985) and the reliability of the used instrument was .78. The data analysis was done by SPSS, t-test, ANOVA and Stepwise Multiple Regression. The results of were as follows. 1. Mean score of burden of subjects was 60.82(Maximum 86, standard deviation 1.244). 2. Of the mothers characteristics, the score of burden was high in case of no religion and low income. 3. Of the patients characteristics, the score of burden ranked as high in MCNS, doing oral therapy and injection therapy at the same time, and negative perceived patients' condition. 4. The degree of burden felt by mothers of children with nephrotic syndrome was significantly predicted by the level of pt's diagnosis(11%), pt's condition(8%), economic state (6%) and pt's sex (5%), respectively. In conclusion to above study, the researcher suggests that the development of instrument for measurement of burden is in much need. Relations between burden and social support should be studied to lessen burden of mothers of children with nephrotic syndrome.

The necessity of ban on opening and operating the multiple medical institutions in medical law in Dental case (의료법에서의 의료기관 이중개설 금지조항의 필요성에 대한 치과 사례연구)

  • In accordance with Article 33(8) of the Korean Medical Law, it is stated that a medical person cannot open or operate a medical institution by borrowing the name of another medical person. However, the publicity of medical care is threatened by the recent illegal network dental clinics. The purpose of this study is to investigate the actual condition of illegal network dentistry and to analyze the cases and to find out the reason why the prohibition of double opening & operating of medical institution. As a result, the illegal network dental clinics treated less health care insurance treatment such as dental caries and periodontal treatment than general dental hospitals. In contrast, the rate of implementation of illegal network dentistry was high in endodontics treatment and extraction, which could lead to uninsured treatments such as crowns and implants. As a result of Supreme Court precedent analysis, it is concluded that illegal act is not only the opening of a medical institution by borrowing the name of other medical personnel, but also the duplicated operation which has the authority to make decision about management matters of medical institutions. The results of the patient's case survey also showed that excessive dental treatment due to such as dental staff incentive system. In conclusion, the illegal network dental clinics not only threatens the oral health of the public, but also causes leakage of health insurance premiums. In other words, the ban on opening and operating the multiple medical institution should be strictly applied as a strong protection device for protecting the patient in dental case.
