METHYL MERCAPTAN CONCENTRATION DURING EXPERIMENTAL GINGIVITIS IN MAN (실험적 치은염에서 구강내 휘발성 메틸머캅단 농도 변화에 관한 연구)

  • The purpose of present study was to evaluate the relationship between the early change of gingival condition and methyl mercaptan concentration during experimental gingivitis. Ten men(23-25 years old) whose gingiva were clinically healthy were selected. The participants have ceased to perform all forms of oral hygiene during 14 days and then did thorough plaque control for 7 days. For each subject, the methyl mercaptan concentration was measured by $B.B.Checker^{(R)}$ (Bad Breath Checker with printer, Tokuyama Soda Co.,LTD., Japan)before experiment and 1,4,7,14,21 days during experiment. Plaque index(Silness & $L\ddot{o}e$), gingival sulcus depth and sulcus bleeding index($M\ddot{u}hlemann$ & Son)score were recorded. The results were as follows. 1. Methyl mercaptan concentration increased continuously from the first day to the 14th day, decreased on the 21th day but it was still higher(P<0.001). 2. Plaque index score and sulcus bleeding index score tended to increase on the 4th day, markedly increased on the 14th day and returned to baseline level on the 21th day. 3. There was parallel relationhsip among methyl mercaptan concentration, plaque index score and sulcus bleeding index score. This result suggests that methyl mercaptan concentration increased with deterioration in gingival health, but decreased during recovery of normal health condition.

  • Freeman-Sheldon Syndrome (FSS, also known as "Whistling Face Syndrome") is a rare genetic condition which characteristically includes a small "whistling" mouth, a flat mask-like face, club feet, joint contractures usually involving the fingers and hands, and under-development of the cartilage of the nose. Intelligence is usually normal. Most of the features of this syndrome are due to muscle weakness. The patient, 11 years old boy was consulted from pediatrics to pediatric dentistry due to dental management. After clinical & radiographic examinations, severe multiple problems were found. Dental problems were microsomia(whistling mouth) & micrognathia, perioral muscle contracture, restricted mouth opening, poor oral hygiene & care, generalized dental caries, high palatal vault, severe malocclusion & crowding. And Orthopedic problems, ophthalmic & respiratory, anesthetic problems were found. Then He also had psychiatric problem, hospital(dental) phobia due to previous medical history(frequent hospitalization). And he had genital problem, cryptochidism, too. Due to these intricate problems, he suffered with feeding, swallowing difficulties and showed growth retardation. For enhancing patient's oral health, pediatric dentist, orthodontist, oral surgeon, pediatrician, psychiatrist, orthopedist, they all agree with early, cautious intervention and treatment. So, he has been treated by multidisciplinary care, now he is recovering general health maintenance.

Investigation of factors influenced on accuracy of polyvinylsiloxane (일부 시판 폴리비닐실록산 인상재의 정밀도에 영향을 미치는 요인분석)

  • In this study, polyvinylsiloxane impression materials were investigated to examine the characteristics of the impression materials that affect the accuracy of the final restoration. The flow property of impression materials which can reproduce the detail in the oral cavity and accurately duplicate it, and the tear strength and strain-in-compression which can cause problems when it is being removed from the oral cavity were studied. The results are as follows. 1. As for the flow properties of impression materials, Imprint II was 18.24${\pm}$0.30, which was the highest: and Twinz was 8.9${\pm}$0.62, which was the lowest. There was no significant difference among Imprint II, Examixfine, and Genie(p<0.05). 2. As for the tear strength of impression materials, Imprint II had the highest level, while Genie had the lowest value. There was no significant difference between Twinz and Genie(p<0.05). 3. As for the strain-in-compression, there were significant differences by impression material groups(p<0.05); the strain-in-compression of Examixfine was shown to be the highest at 5.56${\pm}$0.56, Twinz and Imprint II followed respectively, and Genie has the lowest at 3.19${\pm}$0.23. 4. Flow showed the correlation with tear strength but no significant connection to strain-in-compression. Also, there was no significant correlation between the tear strength and strain-in-compression (p<0.05). Making impression to reproduce oral tissue and tooth is an important part of making final restoration. The accuracy of impressions is influenced by the methods of taking impression or other condition. However the property itself of impression materials is the most essential and the materials with proper qualities should be selected.

  • A 26-year-old female patient with Down syndrome visited to recieve dental treatment under gnenral anesthesia 6 years ago. The patient had difficulties in oral examination, radiograph taking and laboratory test. The patient had congenital heart disease and medical consultation based on the echocardiography was provided by a cardiologist indicating that the patient could tolearte general anesthesia during dental treatment. And two times of general anesthesia were administered during a dental treament with the interval of 3 years and no postoperpative complicaton was reported. At the third dental operation, the patient had a relatively good condition and her prescreening test revealed no abnormalities. Without further consultation with a cardiologist, general anesthesia was administered to the patient. Anaesthesia was based on thiopental and ventilation of desflurane and $N_2O$ in oxygen via an endotracheal tube with an appropriate monitoring. During the maintenance of anesthesia, the blood pressure of the patient started to drop and the oxygen saturation also began to decrease. Consequently, the proceding operation was discontinued and also inhalation anesthesia was ceased. As the patient was recovered from anesthesia, her systemic conditions were alleviated. After the complete recovery of the patient, she visited the cardiologist, and the cardiologic test revealed her severe right ventricular dilatation. In the anesthesia of patients with congenital heart disease, information on their systemic conditions needs to be undated from the medical consultation, which assures the safety of treatment.

Prevalence of pineal gland calcification as an incidental finding in patients referred for implant dental therapy

  • Purpose: Pineal gland calcification has been proposed to play a role in the pathogenesis of Alzheimer disease. This study evaluated the prevalence and extent of pineal gland calcification in cone-beam computed tomography (CBCT) scans of patients referred for dental implant therapy who could possibly be a vulnerable group for this condition. Materials and Methods: A retrospective evaluation of 500 CBCT scans was conducted. Scans that showed the area where the pineal gland was located were included. The scans were initially screened by a single observer to record the prevalence and extent of calcification. Six weeks following the completion of the study, another investigator randomly reviewed and selected 50 scans to investigate inter-observer variation, which was evaluated using reliability analysis statistics. The prevalence and measurements of the calcifications were reported using descriptive statistics. The chi-square test was used to compare the prevalence between males and females. Results: The prevalence of pineal gland calcification was 58.8%. There was no statistically significant correlation between age and the extent of the calcification. The prevalence of calcification was 58.6% in females and 59.0% in males. The average anteroposterior measurement was $3.73{\pm}1.63mm$, while the average mediolateral measurement was $3.47{\pm}1.31mm$. The average total calcified area was $9.79{\pm}7.59mm^2$. Conclusion: The prevalence of pineal gland calcification was high in patients undergoing implant therapy. While not all pineal gland calcifications lead to neurodegenerative disorders, they should be strongly considered in the presence of any symptoms as a reason to initiate further investigations.

A study on the health science college students' dental health behaviors (일부 보건계열 대학생의 구강보건행태에 관한 조사연구)

  • I got the following conclusion that analyzes the dental health recognition, attitude and the action of the students who major in health section executing questionnaire about dental health behaviors from March 3 through 26 with health-related university students in some area in order to arrange the fundamental data that manage their mouth health rationally by using the investigation and comparison by sex and department in dental health teaching. The results obtained were as follows : 1. A large number of students recognized dental health as an important problem, but they believed that the level of their knowledges on dental health was not so high and female students were more negative than male students with regards to their own dental health condition. 2. 49.8% of the students know about tap water fluoridation program control business and 60.7% agreed and boy's approval rate was higher than girl's. But 42.2% of the not-related buccal department's students have a reservation about tap water fluoridation program control business. 3. Average brushing number of the boy is 51.0% 2 times a day according to sex, A girl appeared highest by 48.3% 3 times a day and there was statistically significantly different according to (pE0.01). there wasn't statistically significantly different according to department. Also, the period of the brushing was the most right after a breakfast, in the order of before sleep and after dinner. 4. Whether they go to the dentist's or not, 90.8% of them say 'yes', the result showed statistically significant difference according to sex (pE0.01), but It didn't show statistically significant difference according to department. Also, the purpose for visiting the dentist's office lately was the most caries treatment. and there was statistically significantly different according to sex (pE0.01), but there was not statistically significantly different according to department. 5. Oral health education-related subject approval's rate when the education course open is 65.0% there was statistically significantly different according to department (pE0.01). Also when oral health education-related subject as the liberal arts open' the rate of taking a course application showed 50.2% high appear. there was statistically significantly different according to department(pE0.01).

The management effect of TDC(Total Dental Care) internship program (일개 대학 치위생과 대학생들의 TDC(Total Dental Care) 인턴십 프로그램의 운영효과)

  • Objectives : The purpose of the study is to identify the efficiency of total dental care(TDC) internship program. Methods : Dental hygiene students participated in the total dental care(TDC) internship program for 4 weeks in dental clinics practice. A self-reported questionnaire was filled out from June to July in 2012. The questionnaire consisted of oral health condition and dental care services by the students, dentists, and dental hygienists after TDC internship program. Results : Satisfaction degree of the students was $3.92{\pm}0.68$ points. The patients satisfied with TDC internship program and it was helpful to their dental health care(4.0 points). There were significant changes in tooth brushing method, frequency and O'Leary plaque index. Satisfaction degree of the dentists was 4.09 points and that of the dental hygienists was 3.80 points. Conclusions : TDC internship program is very helpful for the students not only to play an outstanding role in their dental care activities after employment but also to establish the real identity of the dental hygienist.

  • The purpose of this study was to test the capacity of automatic tablet crusher. The Automatic tablet crusher not only saved crushing time, but also removed floating fine powders that was occurred during crushing and caused allergy. The characteristics of powder produced by this crusher were as follows: First, to meet the condition as powder after pulverizing 5∼10 g tablet it took 6 to 12 seconds crushing time. But six seconds would do to get powder f3r oral administration. Second, when the quantity of medicine was measured by HPLC, powder from automatic crusher showed higher than that from pestle. HPLC chromatogram indicated that there was no change of medicine in the process of pulverization.

Red fluorescence of oral bacteria interacting with Porphyromonas gingivalis (Porphyromonas gingivalis가 일부 구강미생물의 형광 발현에 미치는 영향)

  • Objectives: Dental plaque is composed of 700 bacterial species. It is known that some oral microorganisms produce porphyrin, and thus, they emit red fluorescence when illuminated with blue light at a specific wavelength of <410 nm. Porphyromonas gingivalis belongs to the genus Porphyromonas, which is characterized by the production of porphyrin. The aim of this study was to evaluate red fluorescence emission of some oral microorganisms interacting with P. gingivalis. Methods: Five bacterial strains (P. gingivalis, Streptococcus mutans, Lactobacillus casei, Actinomyces naeslundii, and Fusobacterium nucleatum) were used for this study. Tryptic soy agar medium supplemented with hemin, vitamin K3, and sheep blood was used as a growth medium. The fluorescence emission of bacterial colonies was evaluated under 405 nm-wavelength blue light using a Quantitative Light-induced Fluorescence Digital (QLF-D) camera system. Each bacterium was cultured alone and co-cultured in close proximity with P. gingivalis. The red/green (R/G) ratio of fluorescence image was calculated and the differences of R/G ratio according to each growth condition were compared using the Mann-Whitney test (P<0.05). Results: Single cultured S. mutans, L. casei and A. naeslundii colonies emitted red fluorescence (R/G ratio=$2.15{\pm}0.06$, $4.31{\pm}0.17$, $5.52{\pm}1.29$, respectively). Fusobacterium nucleatum colonies emitted green fluorescence (R/G ratio=$1.36{\pm}0.06$). The R/G ratios of A. naeslundii and F. nucleatum were increased when P. gingivalis was co-cultured with each bacterium (P<0.05). In contrast, the R/G ratios of S. mutans and L. casei were decreased when P. gingivalis was co-cultured with each bacterium (P=0.002, 0.003). Conclusions: This study confirmed that P. gingivalis could affect the red fluorescence of other oral bacteria under 405 nm-wavelength blue light. Our findings concluded that P. gingivalis has an important role for red fluorescence emission of dental biofilm.

  • We have investigated the genotoxicity of 3 chemicals, aluminum oxide, calcium oxide, sodium tetraborate using mammalian erythrocyte with micronucleus induction. It was performed using 9 week male ICR mice. At 24 hours after treatment with 3 chemicals with oral route, mice were sacrificed and bone marrow cells were prepared for smear slides. As a result of counting the micronucleated polychromatic erythrocyte (MNPCE) of 2,000 polychromatic erythrocytes (PCE), all treatment groups did not show statistically significant increase than negative control group. And there was no clinical sign related with injection of the 3 chemicals. It was concluded that the 3 chemicals did not induce micronucleus in the bone marrow cells of ICR mice, and these results indicate that the 3 chemicals have no mutagenic potential under the condition in each studies.
