Purpose: This study was performed to evaluate relationship between the inferior alveolar nerve injury and the findings of panoramic and tomographic images for preventing inferior alveolar nerve injury after the 3rd molar extraction. Material and Method: From April, 2005 to June, 2005, The 190 patients who visited in the Department of Oral and Maxillofacia Surgery, Chonnam National University Hospital and the panoramic radiographies were taken for extraction of the mandibular third molar, was selected. Among 215 mandibular third molars, Scanora tomographic imagings were taken in the 90 teeth which were overlaped to the mandibular canal in the panoramic imagies. In panoramic radiographies, the angulation, the level, the root morphology, and the superimposition sign of the mandibular third molars with the mandibular canal were evaluated. In the tomographic radiographies, the location and distance of the mandibular third molar from the canal were also evaluated. The relationships between these findings and the inferior alveolar nerve injury were examined. Results: In the panoramic findings, the inferior alveolar nerve injuries were occurred in the darkened roots (5 molars, 7%), the uncontinuous radiopaque image (3 molars, 7%), and the depositioned mandibular canal (2 molars, 10%). In the tomographic findings of 90 molars, 20 molars also had the superimposition imagies. Five molars in those molars (25%) had the inferior alveolar nerve injury after extraction. There were 10 patients who had the inferior alveolar nerve injury. The sensory was began to be recovered in 9 patients, except 1 patient, within 2 weeks, then fully recovered within 3 months. Conclusion: These results indicate that the depth mandibular third molar and the superimposition sign may be related with the risk of the inferior alveolar nerve injury after extraction.

TMJ Osteoarthritis is a degenerative disease caused by overload on joint tissue, and often accompany with local tenderness on the joint, crepitus by mandibular movement, restriction of mandibular movement, and anterior openbite. In general, panoramic radiography, TMJ panoramic radiography, and transcranial radiography are conducted to diagnose osteoarthritis after clinical examination, however, these radiographic evaluations are limited in detecting minute bony changes of early pathologic lesion. The aim of this pilot study was to evaluate the limitation and usefulness of several TMJ imaging techniques, so we compare the findings from clinical examination, plain film radiographs, tomograph, and bone scan from 81 patients with temporomandibular joint disorder. The proportion of patients showing same findings in plain film radiographs, TMJ tomograph, and bone scan was high, however, it is desirable that clinician should conduct phased imaging examinations according to the clinical findings due to the possibilities of false negative findings in diagnosis of osteoarthritis.
Purpose: The aim of this study was to determine the prevalence of incidental findings(IFs) on digital dental panoramic radiographs(DPRs) of asymptomatic patients attending a general dental practice. Materials and Methods: This was a retrospective study of 6,252 consecutive digital (photostimulatable phosphor) DPRs of patients who visited a Canadian general dental practice for a complete new patient examination. The IFs were grouped into dental-related anomalies, radiopacities and radiopacities in the jaws, changes in the shape of the condyles, and other findings in the jaws, such as tonsilloliths and mucosal antral pseudocysts. Their prevalence was determined. Results: Thirty-two percent of the DPRs showed at least 1 IF. The highest prevalence was found for dental-related anomalies(29% of all DPRs), of which impacted teeth were the most prevalent finding (24% of all DPRs), followed by idiopathic osteosclerosis(6% of all DPRs). A lower prevalence was noted for tonsilloliths(3%), and the prevalence of root tips, mucosal antral pseudocysts, and anomalies in condylar shape was approximately 1% each. Conclusion: The observed prevalence of 32.1% for IFs of any type underscores the need for a dental practitioner to review the entire DPR when a patient presents for an initial dental examination (or check-up) or for dental hygiene. Only a single IF (a central giant cell granuloma) provoked alarm, as it was initially considered malignant. Similarly, impacted teeth and suspected cysts need careful evaluation upon discovery to determine how they may be optimally managed.
Bile acids and synthetic bile acid derivatives induce apoptosis in various kinds of cancer cells and thus have anticancer properties. Recently, it has been suggested that autophagy may play an important role in cancer therapy. However, few data are available regarding the role of autophagy in oral cancers and there have been no reports of autophagic cell death in OSCCs (oral squamous cell carcinoma cells) induced by HS-1200, a synthetic bile acid derivative. We thus examine whether HS-1200 modulates autophagy in OSCCs. Our findings indicate that HS-1200 has anticancer effects in OSCCs, and we observed in these cells that autophagic vacuoles were visible by monodansylcadaverine (MDC)and acridine orange staining. When we analyzed HS-1200-treated OSCC cells for the presence of biochemical markers, we observed that this treatment directly affects the conversion of LC-3II, degradation of p62/SQSTM1 and full-length beclin-1, cleavage of ATG5-12 and the activation of caspase. An autophagy inhibitor suppressed HS-1200-induced cell death in OSCCs, confirming that autophagy acts as a pro-death signal in these cells. Furthermore, HS-1200 shows anticancer activity against OSCCs via both autophagy and apoptosis. Our current findings suggest that HS-1200 may potentially contribute to oral cancer treatment and thus provide useful information for the future development of a new therapeutic agent.
The purpose of this study was to examine the oral health care aware-ness of pregnant women and their actual oral health care in an effort to provide information on how to assist pregnant women to have the right knowledge on oral health and improve their oral health care. The subjects in this study were pregnant women who used obstetrics and gynecology hospitals or participated in pregnancy/child-rearing programs in the region of P. After a self-administered survey was conducted in July and August 2008, the following findings were given: 1. As for the best case of oral health care, the largest number(77.2%) of the pregnant women investigated brushed all the teeth, gums and tongue when they did toothbrushing. The smallest number of the women(6.8%) spent three minutes or more brushing their teeth. 2. As to dental treatment experience during pregnancy by age, 27.0 percent of the age 26-30 group had ever received dental treatment during pregnancy, which was higher than the rates of the other age groups with the same experience. 3. Regarding the necessity of oral health education geared toward pregnant women, 94.1 percent of the age 26-30 group and 96.3 percent of the group of age 31 and up felt the need for that, which were significantly higher than 72.7 percent of the age 20-25 group who agreed to the necessity of that education. 4. As to connections between oral health status and oral health care, the women who were in good oral health got 6.60 on oral health care. They scored significantly higher than those who were in a moderate state of oral health and who were in bad oral health, as the latter two groups respectively got 5. There was a significant correlation between oral health state and oral health care and between oral health knowledge and oral health care. The better oral health status led to better oral health knowledge, and the better oral health knowledge was followed by better oral health care. 6. As for factors affecting oral health care, oral health knowledge had the largest impact on that, followed by age, oral health status, experience of receiving oral health education for pregnant woman, dental treatment experience during pregnancy, monthly income and stress caused by oral diseases. Given the findings of the study, oral health education should be provided in light of the special physical and mental state of pregnant women. They should be encouraged to receive possible dental treatment during pregnancy if necessary, and they should learn about how to cope with a dental disease in case of develop it.
Objectives : The purpose of this study was to examine whether the subjective oral health awareness and oral health behavior of Korean adults would affect their oral health indexes. It's meant to utilize existing data of epidemiological and alternative indexes in an effort to have a comprehensive and understanding of the relationship between the subjective oral health awareness and oral health behavior of Korean adults. And the following findings were obtained Methods : The subjects in this study were 7,285 adults who were selected from the third-year(2009) raw data of the fourth national health & nutrition survey. Results : As for the relationship between oral health awareness and oral health indexes, there were statistically significant differences in DMFT index, FS-T index, T-health index and CPI index according to self-rated health status, self-rated oral health state, necessity of dental treatment and oral health concern. Concerning the relationship between oral health behavior and the oral health indexes, whether they got a dental checkup over the past year, daily toothbrushing frequency, use or nonuse of oral health supplies and mastication problems made statistically significant differences to DMFT index, FS-T index, T-health index and CPI index. The variables that had a significant impact on oral health were selected from among the variables of oral health awareness and oral health behavior that affected oral health, and the variables were selected as independent variables. And then the oral health indexes were selected as dependent variables, and a multiple regression analysis was carried out by using the selected independent and dependent variables. As a results, it's found that the variables made a 22.4% prediction of DMFT index; 51.3% for FS-T index; 52.0% for T-health index; 47.4% for CPI index. Conclusions : The above-mentioned findings illustrated that the relationship between the subjective oral health awareness and oral health behavior of the Korean adults exercised an influence on their oral health indexes. Accurate and effective oral health plans should be mapped out by grasping the oral health status of adults from diverse angles to facilitate the maintenance and promotion of their oral health status.

Purpose This study is to report presurgical magnetic resonance imaging(MRI) findings of the temporomandibular joint which had perforation in the disc or its surrounding tissues and to improve its diagnostic rate using MRI. Patients and Methods The sample consisted of patients who visited the TMJ clinic at Yongdong Severance Hospital, Yonsei University, during the years, 1992 and 1997. They were diagnosed as TMJ internal derangement and received surgical treatment. We divided them into two groups. The first group comprised of 85 joints with perforated disc or its surrounding tissues and which were confirmed surgically. The second group of 62 joints which only had disc displacement without perforation, hyperemia or adhesion served as control. Results The preoperative diagnostic sensitivity of TMJ perforation using MRI was 74.1%. The MRI findings for diagnosis of the TMJ perforation were degenerative change of the condyle head or the articular fossa, bone to bone contact between the condyle head and the articular eminence or the articular fossa, bony spurring or osteophytosis of the condyle head, flattening of articular surface of the condyle head or the articular eminence, discontinuity of the disc and the arthrographic effect due to joint effusion. Conclusion The preoperative diagnostic sensitivity of TMJ perforation using MRI in this study was 74.1% which was lower than the diagnostic rate using the arthrogram. Further investigations are needed to improve the diagnostic accuracy of TMJ perforation using MRI.

Purpose: The aim of this study was to evaluate the computed tomography (CT) imaging findings and clinical symptoms of patients who complained of neurosensory disturbances after mandibular implant surgery, and to investigate the relationships of these parameters with the prognosis for recovery. Methods: CT scans were reviewed in 56 patients with nerve disturbance after mandibular implant surgery. Two oral radiologists classified the imaging findings into intrusion, contact, close, and separate groups according to the distance from the inferior border of the implant to the roof of the mandibular canal (MC). The symptoms of 56 patients were classified into 8 groups and the frequency of each group was investigated. Patients were categorized according to symptom improvement into no recovery and recovery groups, and the relationships of recovery with the CT classification and specific symptom groups were analyzed. Results: Thirty-eight of the 56 nerve disturbance cases showed improvement. The close and separate groups in the CT classification had a strong tendency for recovery (90.9% and 81.8%, respectively) (P<0.05). Although the lowest recovery rate was found in the intrusion group, it was non-negligible, at 50%. The 6 patients with a worm crawling feeling all improved, while the 8 cases with a tightening sensation showed the lowest recovery rate, at 12.5%, and the symptom of a tightening sensation occurred only in the intrusion and contact groups. Conclusions: The closer the implant fixture was to the MC on CT images, the less likely the patient was to recover. Regarding paresthesia symptoms, while a worm crawling feeling is thought to be a predictor of recovery, a tightening sensation appeared to be associated with a lower recovery rate.
Purpose: This study was performed to introduce an in vivo hybrid multimodality technique involving the coregistration of micro-computed tomography (micro-CT) and high-resolution magnetic resonance imaging (HR-MRI) to concomitantly visualize and quantify mineralization and vascularization at follow-up in a rat model. Materials and Methods: Three adult female rats were randomly assigned as test subjects, with 1 rat serving as a control subject. For 20 weeks, the test rats received a weekly intravenous injection of 30 ㎍/kg zoledronic acid, and the control rat was administered a similar dose of normal saline. Bilateral extraction of the lower first and second molars was performed after 10 weeks. All rats were scanned once every 4 weeks with both micro-CT and HR-MRI. Micro-CT and HR-MRI images were registered and fused in the same 3-dimensional region to quantify blood flow velocity and trabecular bone thickness at T0 (baseline), T4 (4 weeks), T8 (8 weeks), T12 (12 weeks), T16 (16 weeks), and T20 (20 weeks). Histological assessment was the gold standard with which the findings were compared. Results: The histomorphometric images at T20 aligned with the HR-MRI findings, with both test and control rats demonstrating reduced trabecular bone vasculature and blood vessel density. The micro-CT findings were also consistent with the histomorphometric changes, which revealed that the test rats had thicker trabecular bone and smaller marrow spaces than the control rat. Conclusion: The combination of micro-CT and HR-MRI may be considered a powerful non-invasive novel technique for the longitudinal quantification of localized mineralization and vascularization.
Objectives : This study was to evaluate the association between perceived oral health and perceived oral symptoms among adults in Daegu. Methods : All 437 subjects aged 18 or more selected convenience sampling were surveyed cross-sectionally via the self-administrated questionnaire. The questionnaire was measured perceived oral symptoms and perceived oral health, and also obtained socio-demographic characteristics, oral health behaviors. To assess the crude associations, bivariate analysis were applied. For the adjusted association between perceived oral health and perceived oral symptoms, multivariate linear regression multiple regression analysis was conducted. Results : 33.2% of the adults rated their perceived oral health was good, and 30.9% as poor. Older age, low education, had peridontal disease was negatively perceived their oral health(p<0.05). As oral symptoms were more frequently perceived, the perceived oral health were negative. Among the factors of perceived oral symptoms, trouble biting/chewing, poor periodontal status, trouble of appearance of teeth were positively associated with the perceived oral health after adjusting for socio-demographic characteristics, oral health behaviors in the regression model. Age, education, income, recent dental treatment, and all perceived oral symptoms showed the highest impact of association with perceived oral health in the baseline-category logit model. Conclusions : Perceived oral health are significantly associated with perceived oral symptoms among adults in Daegu. The findings of this study will be helpful to design plans of oral health promotion in welfare institutions to increase the oral health related quality of life among the adults.
