A study on the correlation of the college life and oral prophylaxis in some students studying dental hygiene (일부 치위생과 학생들의 대학생활과 치면세마 교과목과의 상관관계에 대한 연구)

  • The purpose of this study is to examine the satisfaction for college life and oral prophylaxis subject and improve the satisfaction for this subject by satisfying students' desire through the results of correlation, and to inspire a professional sense by helping students enjoy healthy college life. In order to achieve these purposes, 127 senior students of the Dental Hygiene Department, Kwangju Health College took part in this study to examine the satisfaction for a study course, a lab, professors, a college, and subjects. The following results were obtained. 1. The overall satisfaction for the Dental Hygiene Department was 2.83 and the highest dissatisfaction factors included difficult subjects and non-aptitude. The overall satisfaction for professors was 3.33 and students wanted professors' human contact with students and a well-prepared lecture. The overall satisfaction for college life was 2.65, suggesting not very high satisfaction, and the highest dissatisfaction factors included similar life to high school and surrounding environment. 2. The overall satisfaction for theory was 3.83 and for practice was 3.91 in oral prophylaxis. While the highest satisfaction was found in 'the objective and direction of class' and 'class and a supplementary lesson in not giving a lecture', the lowest satisfaction was found in 'experience in reading textbook-related materials or books'. 3. The correlation of the satisfaction of theory and practice of oral prophylaxis was examined. The satisfaction of oral prophylaxis theory was positively correlated with a school course, professors, and college, while the satisfaction of oral prophylaxis practice was positively correlated with a school course, professors, college, satisfaction of oral prophylaxis theory, and present health condition(p<0.05). 4. Factors influencing the satisfaction for the theory of oral prophylaxis included professors, while factors influencing the satisfaction for the practice were statistically significant in professors and health condition(p<0.05). Based on these results, in order for professional dental hygienists to be developed as professionals serving society, it is recommended to enhance the satisfaction for an oral prophylaxis subject and that professors should provide warm-hearted support and become a role model as an educator.

An convergence study on the effect of plaque removal through the number of operations of the electric toothbrushes (전동칫솔의 작동횟수에 따른 치면세균막 제거율에 관한 융합연구)

  • The study was to expand the choice of electric toothbrush by number of operations of the plaque removal effect on the tooth surface. Three different toothbrushes were selected for oscillations and pulsations type of electric toothbrush. A is Oral-B D12 (7,600 pulsations), B is Oral-B D16 (20,000 oscillations and a 7,600 pulsations), C is Oral-B D20 (40,000 oscillations and an 8,800 pulsations). In each trial, each tooth was brushed for 2 seconds. For the 10 trials. And each tooth was coated with 1layer, 2layers and 3layers. After the experiment, the plaque removal effect was calculated and analzed using the Image J program(p<0.05). According to the study, C toothbrush had the highest rate of plaque removal effect. Based on this results of the study, dental worker will need to evaluate oral hygiene and recommend appropriate toothbrushes according to oral conditions.

Clinical outcome of conservative treatment of injured inferior alveolar nerve during dental implant placement

  • Objectives: Infererior alveolar nerve (IAN) damage may be one of the distressing complications occurring during implant placement. Because of nature of closed injury, a large proportion is approached non-invasively. The purpose of this study was to analyze the outcomes of conservative management of the injured nerve during dental implant procedure. Materials and Methods: Sixty-four patients of implant related IAN injury, who were managed by medication or observation from January 1997 to March 2007 at the Department of Oral and Maxillofacial Surgery, Seoul National University Dental Hospital, were retrospectively investigated. The objective tests and subjective evaluations were performed to evaluate the degree of damage and duration of sensory disturbance recovery. Tests were performed on the day of the first visit and every two months afterward. Patient's initial symptoms, proximity of the implant to the IAN, time interval between implant surgery and the first visit to our clinic, and treatment after implant surgery were analyzed to determine whether these factors affected the final outcomes. Results: Among the 64 patients, 23 had a chief complaint of sensory disturbance and others with dysesthesia. The mean time until first visit to our hospital after the injury was 10.9 months.One year after nerve injury, the sensation was improved in 9 patients, whereas not improved in 38 patients, even 4 patients experienced deterioration. Better prognosis was observed in the group of patients with early visits and with implants placed or managed not too close to the IAN. Conclusion: Nearly 70% of patients with IAN injury during implant placement showed no improvement in sensation or dysesthesia with the conservative management. Earlier decision for active treatment needs to be considered because of possibility of deterioration of symptoms and unsatisfactory recovery.

Research on the Smoking Conditions of College Students and the Behavior and Awareness of Oral Hygiene (일부 대학생의 흡연 실태와 구강건강에 관한 인식 및 행태 조사연구)

  • This study was conducted to investigate the differences between smokers and non-smokers' oral hygiene and college students' smoking awareness. The survey was carried out based on the 260 college students enrolled in four-year universities located in Kyungbuk, Daegu, Gangwon-do. Collected date used the SPSS 17.0 to perform the frequency and cross-analysis. 1. The daily smoking amounts of less than half pack and the smoking duration of more than 1 year and less than 3 years were to be the highest. The initiation of smoking was triggered by the curiosity and the proximal social environment. The reason of smoking was to relieve stress and habitual smoking respectively. 2. There was a significant difference between smokers and non-smokers' tooth brushing time, regular dentist visit, recent dental admission, the purpose of dental visits, scaling and oral health education. 3. There was a significant difference between smokers and non-smokers' awareness of oral hygiene in the subjective oral hygiene status. 4. There was a significant difference between smokers and non-smokers' awareness towards the impact of second-hand, after meal and one or two cigarette smoking. Based on the above results, smokers lack the awareness of smoking and oral hygiene compared to non-smokers. Therefore, the activation and the development of programs for the smoking and oral hygiene education targeting college students are considered to be necessary.

THE THREE DIMENSIONAL ANALYSIS OF VOLUMETRIC AIRWAY CHANGE IN ORTHOGNATHIC SURGERY OF MANDIBULAR PROGNATHISM (하악 전돌증 환자의 악교정 수술에서 기도 공간의 부피변화에 관한 3차원적 분석)

  • Orthognathic surgery changes patient's mandibular position and environment of related anatomic structures. Many clinicians were interested in these changes and studied about this problem. However, most of them were based on two dimensional cephalogram. According to the development of image and computer system, it would be possible that the airway change is analyzed with three dimensional CT. So we tried to measure the volumetric change of airway and analyzed the relationship between the airway structure and volumetric change. Nineteen patients who experienced orthognathic surgery due to mandibular prognathism were analyzed with 3D CT data (preoperative and postoperative 6 months) and 2D lateral cephalometry. Volumetric change was measured and 3 dimensional change of related structure was assessed with simulation program ($V-works^{(R)}$, 4.0 Cybermed, Korea). Ten patients showed the decrease of airway volume change and nine showed the increase of airway volume change. Volumetric change was determined by dimensional change of mandible and hyoid bone. The dimensional positions of mandible and hyoid bone were the key factor for determining the airway change after surgery. Airway change is also predictable with the dimensional change of mandible and hyoid bone.

Stemness and Proliferation of Murine Skin-Derived Precursor Cells under Hypoxic Environment

  • Skin-derived precursors (SKPs) have potential to differentiate to various cell types including osteoblasts, adipocytes and neurons. SKPs are a candidate for cell-based therapy since they are easily accessible and have multipotency. Most mammalian cells are exposed to a low oxygen environment with 1 to 5% $O_2$ concentration in vivo, while 21% $O_2$ concentration is common in in vitro culture. The difference between in vitro and in vivo $O_2$ concentration may affect to the behavior of cultured cells. In this report, we investigated the effect of hypoxic condition on stemness and proliferation of SKPs. The results indicated that SKPs exposed to hypoxic condition for 5 days showed no change in proliferation. In terms of mRNA expression, hypoxia maintained expression of stemness markers; whereas, oncogenes, such as Klf4 and c-Myc, were downregulated, and the expression of Nestin, related to cancer migration, was also downregulated. Thus, SKPs cultured in hypoxia may reduce the risk of cancer in SKP cell-based therapy.

Orofacial Soft Tissue Reconstruction with Locoregional Flaps in a Health Resource-Depleted Environment: Experiences from Nigeria

  • Background Reconstruction of orofacial soft tissue defects is often challenging due to functional and aesthetic demands. Despite advances in orofacial soft tissue defect reconstruction using free flaps, locoregional flaps still remain an important option, especially in health resource-depleted environments. This retrospective study highlights our experiences in oral and maxillofacial soft tissue reconstruction using locoregional flaps. Methods A twenty-three years retrospective analysis of all patients managed in our department was undertaken. Information was sourced from patients' case notes and operating theater records. Data was analyzed using SPSS ver. 16 (SPSS Inc.) and Microsoft Excel 2007 (Microsoft). Results A total of 77 patients underwent orofacial soft tissue defect reconstruction within the years reviewed. Males accounted for 55 (71.4%) cases and trauma was the main etiological factor in 45 (58.4%) of the patients treated. When sites of defect were considered, the lip, 27 (32.1%), was the most frequent site followed by the nose, 17 (20.2%). Forehead flap, 51 (59.3%), was the most commonly used flap. Complications noted were tumor recurrences at the recipient bed in 3 (3.9%) cases, tumor occurrence at the donor site in 1 (1.3%) case and postoperative infection in 11 (14.3%) cases. Conclusions Locoregional flaps still have an important role in the rehabilitation of patients with orofacial soft tissue defects. They remain a vital tool in the armamentarium of the reconstructive surgeon, especially in health resource-depleted environments where advanced reconstructive techniques may not be feasible.

A Study on the Occupational Diseases and Dental Hygiene of Laborers in Cement Industry (시멘트 취급 근로자들에 대한 구강위생실태 연구)

  • The purpose of this study was to examine the actual dental hygiene status of workers in cement industry in an effort to serve as a basis for enhancing their dental hygiene. The subjects in this study were 420 laborers from the cities of Curi, Donghae and Samcheok who handled cement. As a result of conducting a survey, the following findings were given : l. The workers investigated found their working environment satisfactory, getting a mean score of 323. They also expressed a high job satisfaction with a mean score of 333. 2. They didn't have a big interest in oral health. By age group, those who were aged between 36 and 40, or who cared less about health management, showed a higher interest in oral health. Besides, the laborers who kept smoking for a longer period, or who drunk more, expressed greater interest. 3. The toothbrushing method was considered most important for periodontal health, by 45.0% of the workers, and the next most crucial one was regular dental examination, followed by refraining from smoking and staying away from sweet food in the order named. 4. Concerning daily mean toothbrushing frequency, 455% brushed their teeth three times a day on the average. More than half them didn't pay enough attention to toothbrushing. 5. Regarding scaling, the large number of the workers, 42.4%, had no experience to get their teeth scaled, 37.6%, the greatest percentage, didn't have their teeth scaled because it seemed to make their teeth painful or cold. 6. As fororal health education experience, 67.6%, the great number of them, had no experience to receive dental health education. The above-mentioned findings suggest that the cement-related workers generally neglected dental health management. They should be encouraged to correct their wrong oral health knowledge or habit to make their oral cavity more healthy. To make it happen, it's required to provide oral health education and promote organized dental health projects.

Anti-inflammatory Effects of Gangyeong-Tang (康寧湯) on Mice injected LPS in Vagina (강녕탕(康寧湯)의 LPS 질내 주입 생쥐에 미치는 항염증(抗炎症) 효과)

  • Objective: We made the genital organs inflammatory mice model by vaginal injection of Lipopolysaccharide(LPS), and we intended to make study about anti-inflammatory effects of Gangyeong-Tang, among oral and rectal medication and Herbal-Acupuncture treatment. Method: The female ICR(20-30g) mice were used, the temperature was controlled within $22{\pm}0.5^{\circ}C$ and water and food was not limited. The environment was manipulated to simulate 12 hours of day and 12 hours of night. After LPS injection into vagina, we confirmed on-set of inflammation. 2 days before LPS injection, we started to medicate Gangyeong-Tang in Oral and Rectal and Aqua-acupucture treatment. After 3days from LPS injection, we mesured the White Blood Cell(WBC), Interrleukin-6(IL-6), Tumor Necrosis Factor-${\alpha}$(TNF-${\alpha}$) in blood which was collected from the Retro-orbital Plexus. Results: 1. We made the genital organs inflammatory mice model by vaginal injection of LPS successfully. 2. The number of WBC was decreased significantly as we medicated Gangyeong- Tang in Oral 1g/kg dose, 3g/kg dose and Rectal 1g/kg dose, rectal 3g/kg dose. 3. The concentration of IL-6 was decreased significantly as we medicated Gangyeong-Tang in all group of the Oral, Rectal, Herbal-Acupuncture treatment. 4. The concentration of TNF-${\alpha}$ was decreased significantly as we medicated Gangyeong-Tang in Oral 3g/kg dose and Rectal 1g/kg dose, Rectal 3g/kg dose group. Herbal-Acupuncture treatment group datas showed reductive tendency. Conclusion: As a result of this experiment, we made the genital organs inflammatory mice model by vaginal injection of LPS successfully and demonstrated anti-inflammatory effect of Gangyeong-Tang.

Quantitative analysis of periapical lesions on cone beam computed tomograph and periapical radiograph (Cone beam형 전산화단층영상과 치근단방사선영상의 치근단 병소에 대한 정량적인 분석)

  • Purpose: To detect the progression of experimentally induced periapical lesions on periapical radiograph and cone beam computed tomograph (CBCT) by quantitative analysis. Materials and Methods: After the removal of coronal pulps from premolars of two Beagle dogs, the root canals of premolars were exposed to oral environment during one week and then sealed for 70 days. Digital periapical radiographs and CBCTs were taken at baseline and every 7 days for 77 days after pulp exposure. We examined occurrence and areas of periapical bone resorption. Three comparative groups of CBCT radiographs were prepared by average projection of thin slabs with different bucco-lingual thicknesses (0.1, 3.0, and 8.0 mm) using a 3D visualization software. Radiographic densities were compensated by image normalization. Digital images were processed with mathematical morphology operations. The radiographic density and morphological features of periapical lesions were compared among three groups of CBCT in different time points. Results: In the CBCT group with 0.1 mm thickness, radiographic density (p<0.05) and trabecular bone area (p<0.01) were significantly decreased at the fifth week. However, in the CBCT groups with 3 mm and 8 mm thickness and periapical radiographs, none of densitometric and morphological features showed any significant differences in different time points. Radiographic density of periapical lesion showed increasing tendency at the eleventh week after pulp exposure. Conclusion: Radiographic detection of periapical lesions was possible at the fifth week after pulp contamination by quantitative method and was affected by buccolingual bone thickness.
