This study was the study verifying dental technicians' motivation and job satisfaction in Seoul through the positive method as well as its purposes were to investigated the effect of dental technicians manpower supply/demand prospect on motivation, job satisfaction and the effect of other job satisfaction factors on dental technicians motivation and job satisfaction. For then, general characteristics of study objects were classified by sex, married/unmarried, age, career, salary, highest level of education, working place, position and job as well as job satisfaction factors were classified with future supply/demand prospect, future prospect, working hours, working environment and salary. We conducted a frequency analysis, crossing analysis and multi-linear regression analysis about the effect of the classified articles on motivation and job satisfaction. Also, for verifying the relationship of motivation and job satisfaction, we conducted correlation analysis and its result is as followed. First, it showed that the job related with making orthodontic appliance influenced on motivation, among the age, career and job of the general characteristics, and other jobs between working place and job influenced on job satisfaction. Second, the dark future prospect was showed to influence on motivation and job satisfaction highly so we could grasp that negative opinions are included in that. And low-paying was showed to highly influence on job satisfaction s fall so we could grasp the subjective low-salary standard at the present. But we expected that the manpower supply/demand prospect would influence on motivation and job satisfaction but there was little influence. Third, the correlation between motivation and job satisfaction was showed to be somewhat high, motivation had the standard which is above average and job satisfaction had the standard which is below the average. So with a little improving of job satisfaction, it s possible that motivation can be very high. Therefore, in rapidly changing generation, we think that the dental technicians are actively coping with the reality of low-paying and high-working. But it's implying that anxiety for uncertain future was reflected. Furthermore, because institutional strategies for dental technicians' efficient management are insufficient in relation with policies so it's necessary the policy consideration for solve the job-unsatisfying factors actively.
Statement of problem: Injuries along with discomfort may result on the oral mucosa when non-rigid material is used as the major connector in construction of RPD, since nonrigid major connectors transmit unstable forces throughout the appliance. Titanium which recently draws attention as a substitute of Co-Cr had a difficulty in fabricating due to high melting temperature but the development of casting technique makes it possible to apply to the clinical case. Purpose: The purpose of this study was to investigate the rigidity and the castability of titanium upper major connector by design and make a comparison with Co-Cr major connectors which are widely used in clinical cases now. Material and methods: Casting was done using CP-Ti(Grage 2) (Kobe still Co., Japan) for the experimental groups, and 4 various designs namely palatal strap, U-shaped bar, A-P strap, and complete palatal plate were casted and 5 of each designs were included in each group. For the experimental group, Universal testing machine (Model 4502; Instron, Canton, Mass) was used to apply vertical torsional force vertically to the horizontal plane of major connector. In the second experiment, Vertical compressive force was applied to the horizontal plane of major connector. As a comparative group, Co-Cr major connector was equally manufactured and underwent the same experimental procedures Strain rate was measured after constant loading for one minute duration, and statistical analysis was done with SPSS ver.10.0 for WIN(SPSS. Inc. USA). From the one-way ANOVA and variance analysis (P=0.05), Scheffe's multiple comparison test implemented. Results: 1. Least amount of strain was observed with complete palatal plate followed by A-P bar, palatal bar, and the U-shaped bar having most amount of strain. 2. In all designs of titanium major connector, less strain rate was observed under compressive loading than under torsional loading showing more resistance to lateral force. 3. For titanium major connector, less strain rate was observed when the force is applied to the first premolar area rather than to the second molar area indicating more strength with shorter length of lever. 4. In Comparison of Co-Cr major connector with titanium major connector, palatal strap and U-shaped bar designs showed higher strength under torsional force that is statically significant, and under compressive force, no significant difference was observed expert for U-shaped bar. 5. In titanium major connector, complete palatal plate showed lowest success rate in casting when compared with the Co-Cr major connector. Conclusion: Above results prove that when using titanium for major connector, only with designs capable of generating rigidity can the major connector have almost equal amount of rigidity as Co-Cr major connector and show lower success rate in casting when compared with the Co-Cr major connector.
Purpose: This study examined the clinical success rate of Mg titanate implants (M Implant system, Shinhung, Korea), which employ a Mg coating method, by evaluating the marginal bone loss and implant stability using radiographs and Osstell$^{(R)}$, over a 1 year. Materials and methods: The locations of the implants placement were divided into 4 areas; the maxillary and mandibular premolars and molars. In the maxilla, 8 and 9 implants were inserted in the premolar and molar areas, respectively. In the mandible, 11 and 51 implants were inserted in the premolar and molar areas. Marginal bone loss and ISQ of all implants (79) were measured after insertion, mounting the prosthetic appliance, and 1, 3, 6, and 12 months after loading. The marginal bone loss was measured from the radiograph using XCP bite, which was customized, and the implant stability measured using Osstell$^{(R)}$. Fisher's exact test (${\alpha}$=.05) was used to compare the success rates of each region. Results: The mean marginal bone loss for the upper and lower jaws were 1.537 mm and 1.172 mm. The mobility showed a non-significant reduction or increase according with time. The success rates were accounted for 94.12% and 98.39% in the upper and lower jaws; the premolars and molars were accounted for 100% and 96.67%. The two cases of early failure resulted from failure of primary stability during implant insertion. The late failures were not observed for 1 year after adding a loading to the implants. Conclusion: The Mg titanate implant showed good primary stability and good clinical results in both healing and function.

To distalize the maxillary molars, the traditional techniques such as extra-oral traction, Wilson distalizing arches, removable spring appliances and Schwarz plate-type appliances have been used. But, these need considerable patient cooperation. For minimal patient compliance, many practitioners use the pendulum appliances. Several clinical studies demonstrated pendulum is effective molar distalization appliance in the growing patient(using the premolars and the palate as anchorage). But unfortunately, maxillary anterior teeth also shift mesially as the molar moves distally. As a result anchorage loss is occurred. To overcome these disadvantages, we used bone-supported pendulum, combined the conventional pendulum with Skeletal Anchorage System(SAS). The miniscrew was implanted in the anterior paramedian region of the median palatal suture, which has comparatively sufficient bone thickness and is low risk to damage on the dental follicles. We report three cases, using bone-supported pendulum for the maxillary molar distalization in children. After treatment, we find out anchorage stability, minimal unfavorable anterior tooth movement and sufficient molar distalization.

Occlusal disease is comparable to periodontitis in that it is generally not reversible. Occlusal disease, however, like periodontitis, often maintainable. It does itself to treatment and when restorative dentistry is utilized it becomes, in that sense, reversible. Moreover, a systematized and integrated approach will lead to a prognosis that is favorable and predictable. This approach facilitates development of optimum oral function, comfort, and esthetics, resulting in a satisfied patient. Such a systematized approach consists of four logical phase : (1) patient evaluation, (2) comprehensive analysis and treatment planning, (3) integrated and systematic reconstruction, and (4) postoperative maintenance. An integrated treatment plan is first developed on one set of diagnostic casts, properly mounted on a semiadjustable articulator using jaw relationship records. This is accomplished by using wax to make reconstructive modifications to the casts. These modified casts become the blueprint for planned occlusal changes and the fabrication of provisional restorations. The treatment goals are : (1) comfortably functioning temporomandibular joints and stomatognathic musculature, (2) adherence to the basic principle of occlusion advocated by Schuyler, (3) anterior guidance that is in harmony with the envelope of function, (4) restorations that will not violate the patient's neutral zone. This report shows the treatment procedures for a patient whose mandibular position has been altered due to posterior bite collapse. Migration of the maxillary anterior teeth had occurred, and the posterior occlusal contacts showed pathologic interference. Precise diagnosis using mounted casts was executed and prosthodontic reconstruction by the aid of an unconventional orthodontic correction on maxillary flaring was planned. An unconventional orthodontic correction can be accomplished by using preexisting natural teeth, which can be modified for use in active tooth movement or splinted together for orthodontic anchorage. This technique has an advantage over conventional fixed appliance orthodontic therapy because it can accomplish tooth movement concurrently with restorative and periodontal therapy. On occasion, minor tooth movement can be necessary to achieve the optimum occlusal scheme, crown form, and tooth position for the forces of occlusion to be displaced down the long axis of the periodontally compromised teeth. Once the occlusion, periodontal health, and crown contours for the provisional splinted restoration are acceptable, the final splinted restoration can be similarly fabricated, and it becomes an excellent orthodontic retainer.

As a common side effect of fixed orthodontic treatment, demineralization of the enamel adjacent to the bracket and band occurs in patients with poor oral hygiene. The purpose of this study was to investigate what is the most effective method to prevent demineralization around the fixed orthodontic appliance among various methods using fluoride. 80 extracted bovine incisors with a healthy surface were classified into four groups as experimental materials: (Group I) Control group, (Group II) V $varnish^{TM}$, (Group III) Tooth Mousse $Plus^{(R)}$, (Group IV) $Vanish^{TM}$ XT. After treatment for each group, mineral loss and Vickers surface microhardness were measured at 0, 30, 60 and 90 days after demineralization in artificial carious solution. Mineral loss was the lowest in group IV, followed by group II and group III, which showed a significant difference. The surface microhardness was the lowest in group IV, followed by group II and group III, which showed a significant difference. Through this study, group IV showed the best effect to prevent enamel demineralization around the bracket. Group III showed significant prevention of enamel demineralization compared with the control group, but the effect was less than that of the other groups.
Objective: The purpose of this study was to compare the cephalometric measurements of obese and non-obese Korean male patients with obstructive sleep apnea syndrome (OSA). Methods: Eighty-seven adults who had visited the Sleep Disorder Clinic Center in Keimyung University, Daegu, Korea were examined and evaluated with polysomnography (PSG) and lateral cephalogram. They were divided into 4 groups (non-obese simple snorers, obese simple snorers, non-obese OSA patients, obese OSA patients) according to AHI (Apnea-Hypopnea Index) and BMI (Body Mass Index). Results: The obese OSA group had the highest AHI among the 4 groups. The non-obese OSA group had a significantly steeper mandibular angle and shorter tongue length than the obese OSA group. The hyoid bone of the obese OSA group was positioned anterior and inferior as compared with the non-obese OSA group. Multiple regression analysis showed that tongue length in the obese OSA group and retroposition of hyoid bone in the non-obese OSA group were significant determinants for the severity of AHI. Conclusions: From a cephalometric point of view, the obese and non-obese pateints with OSA may be characterized by different pathogeneses. Therefore, they have to be managed by individualized treatment. For the obese OSA patients, weight control must be advised as a first choice and for the non-obese OSA patients, oral appliance, nasal CPAP, UPPP and others could be chosen according to the obstructive sites.
This study was conducted to investigate the changes in the structural parts of the craniofacial skeleton subsequent to chincap therapy in the juvenile skeletal Class III patients. The subject consisted of 29 Korean children(14 males, 15 females) who had skeletal Class III malocclusion and were undergone chincap therapy from the beginning of the treatment (and an auxilliary upper removable appliance, if necessary). The control group was composed of 21 children(10 males, 11 females) with skeletal Class III malocclusion who had no orthodontic treatment. Cephalometric data at the mean age of 7 and 2 years later were analyized by finite element method, and compared between groups by independent group t-test(p<0.05). The results of the present study were as follows; 1. There were no significant changes in the cranial base, posterior face, upper anterior face, ramus, chin and soft tissues by the chincap therapy. 2. The mandibular body showed significant differences in the minimum extention ratio and the overall shape ratio. This means that the vertical direction of growth was retarded by the chincap therapy. 3. The major direction of the growth in the maxillary basal bone was significantly more horizontal in the experimental group, which suggests that the vertical growth of maxilla was inhibited. 4. There was statistical difference in the major direction of the growth of the anterior face between groups. This may be due to the significant difference in the major direction of growth of the lower anterior face, supposed to be resulted from the mandibular rotation and/or displacement by the chincap therapy. The change in the oral functional space seemed to be caused by the same reason. 5. From the standpoint of these results, the retardation of growth, the changes of the growth direction and the morphological changes could be accepted partly, but the major effect of the chincap seems to be the rotation and the displacement of the mandible.
Objectives: Obstructive sleep apnea syndrome (OSAS) has drawn increasing attention as medical community has become to be aware of its co-morbidities and complications, especially cardiovascular complications and excessive daytime sleepiness with accident proneness. As of now, polysomnography is the standard tool to diagnose sleep apnea and estimate the treatment validity. However, its being rather expensive and inconvenient, alternate diagnostic tools have been proposed including wrist actigraphy. So far, actigraphies have been adopted usefully to field-survey sleep apnea prevalence. In this study, we attempted in a sleep laboratory setting to assess the supplemental value of actigraphy in diagnosing OSAS. Methods: This study was done at the Division of Sleep Studies, the Seoul National University Hospital. Thirty-seven clinically suspected cases of OSAS underwent the one-night polysomnography, simultaneously wearing an actigraphy on non-dominant wrist. We analyzed the data of 27 polysomnographically-proven OSAS patients (male:female 20: 7;age $47.6{\pm}12.9$ years old;age range 23 to 72 years) with no other sleep disorders. We calculated RDI (respiratory disturbance index) from the polysomnography data and FI (fragmentation index) from the actigraphy data. Pearson correlation was calculated in order to compare FI with RDI and to evaluate the supplemental diagnostic value of the actigraphy. Results: Mean total sleep time on polysomnography was $401.4{\pm}57.8\;min$ (range of 274.0 to 514.1 min). Mean RDI was $21.7{\pm}20.4/hour$. Mean FI was $21.9{\pm}13.0/hour$. RDI and FI showed significant correlation (r=0.55, p<0.01). Conclusions: Wrist actigraphy in OSAS patients generates a comparable outcome to polysomnography, in measuring the nocturnal sleep fragmentation. The actigraphy could be used supplementally in inpatients, outpatients, and field survey subjects, if polysomnography is unavailable or impossible. In follow-ups related with nasal CPAP (continuous positive airway pressure), upper airway surgery, and oral appliance in OSAS patients, the actigraphy might play a more dominant role in the future.
Background and Objectives: Obstructive sleep apnea (OSA) is often undiagnosed but is an important risk factor affecting the health of an individual. The level of awareness of the illness among patients with OSA is low and is not correlated with severity of the illness. This study was conducted to compare awareness of OSA symptoms and illness between patients with OSA and simple snorers. Materials and Methods: Two hundred eighty-two patients who were suspected of having OSA participated in this study. All subjects underwent overnight polysomnography. Those with an apnea-hypopnea index (AHI) ${\geq}5$ were classified as the OSA group, while those with an AHI < 5 were classified as the simple snoring group. A sleep questionnaire, which included items on awareness of the illness, OSA, and sleep symptoms, was administered to all subjects and their bed-partners. Results: Simple snorers were much more aware of their symptoms such as snoring, irregular breathing, and apnea than were patients with OSA. Bed-partners of simple snorers were also more aware of the participants' sleep symptoms than were partners of patients with OSA. However, the duration of OSA symptoms was longer in the OSA group. In the correlation analysis, the level of awareness of OSA symptoms was negatively correlated with AHI, age, body mass index, and Epworth Sleepiness Scale score. Among the sleep questionnaire and polysomnography results, only Pittsburgh Sleep Quality Index was positively correlated with level of awareness of OSA symptoms. The minority of the respondents had heard about the treatment methods of continuous positive airway pressure and oral appliance and preferred them as treatment options. Conclusion: This study suggests that simple snorers are more aware of their symptoms than are patients with OSA. A higher severity of OSA, represented by a higher AHI, is correlated with lower awareness of one's OSA symptoms.
