Osteopetrosis is an uncommon hereditary bone disorder whose prominent radiologic feature characterized by increased bone density. The authors reported a 7-year-old male patient who referred from local dental clinic for dental problems such as early exfoliation of deciduous teeth(#54,73,83) and delayed eruption of permanent teeth(#31.41.36.46). The patient appeared as a poorly developed. Dental X-ray films showed early exfoliation of deciduous teeth, delayed eruption of permanent teeth, and rampant caries. Lateral view of skull demonstrated increased opacity of calvarium, facial bones, and skull base. Generally the skeletal density is greatly increased throughout all bones. Facial CT showed poor development of paranasal sinuses and mastoid air cells. No hematopoietic and neurologic complications such as anemia, thrombocytopenia, blindness and deafness were found. Also mental retardation was not found. The final diagnosis of this case was a osteopetrosis tarda. Sometimes patient with osteopetrosis tarda may be developed dental problems prior to severe systemic symptoms. The dentist can be the first clinician to see the patient. It is very important for the dentist to have the knowledge of the osteopetrosis and to care the patient's dental problems to prevent complication such as osteomyelitis of jaws.

This is a report of adenoid cystic carcinoma occurred in the palate in 30-year-old patient with a complaint of exophytic mass. The authors diagnosed it as adenoid cystic carcinoma by the clinical examination, radiographic findings and histopathological findings. The obtained results are as follows: 1. In clinical examination, asymptomatic exophytic mass of palate was observed. 2. In radiographic findings, soft tissue mass infiltrated the left maxillary sinus, nasal cavity, infraorbital fossa, hard palate, pterygopalatine fossa and pterygoid plate, and enhanced soft tissue mass was also observed in CT. 3. In histopathological findings, tubular and solid patterns of glandular structures were observed and the infiltration of tumor cells into the nerve fibers was also observed. 4. Two years after radical surgery, radiation therapy and chemotherapy, the perineural spread to orbital area was observed. 5. Much longer follow-up than 5 years is needed for early diagnosis of recurrence and distant metastasis.

Early diagnosis and treatment of osteomyelitis provide good prognosis and prevent severe complications. Therefore, it is important to early diagnose and treat before the bony changes are observed in conventional radiograms. Authors experienced three cases of early osteomyelitis, and scintigrams were useful to differentiate them from other diseases. The purpose of this report was to aid in the early diagnosis and treatment of osteomyelitis occurred in the jaws. The characteristic features were as follows: 1. In clinical examination, the patients complained mild pain and localized swelling in the jaws. 2. In radiographic findings, the conventional radiograms showed relatively mild bony changes in the jaws. 3. The more severe periosteal reactions were observed in radiograms in children than in adult patient. 4. It showed marked increased uptake of radioisotopes in all scintigrams. 5. The three phase bone scannings were helpful to differentiate osteomyelitis from soft tissue diseases.

A 66-year-old man visited author's institute complaining of the swelling on the submandibular gland area. Clinically, the exophytic mass penetrated the skin of the submandibular area. On MRI, the lesion occupied the left submandibular space and extended downward, protruding exterior to the subcutaneous fat layer, but the center of the lesion was located on the side of the skin and the growth exterior to the skin was prominent. Demarcation of the lesion and the submandibular gland was unclear. Histopathologically the epithelial nests and keratin production were seen, then the biopsy result was squamous cell carcinoma. The stroma of lesion showed a myxoid characteristic and some ducts showed metaplasia of the ductal cells, which suggested the gland-origin carcinoma. However, lots of keratin production and carcinomatous change of cells continuous to the normal epithelium of the skin, the skin-origin carcinoma invading into the submandibular gland area could not be excluded.

Ameloblastic odontoma is a mixed odontogenic tumor, which is characterized as being extremely rare, displaying aggressive clinical behavior, resembling a mixed radiopaque and radiolucent radiographic appearance and containing both ameloblastoma and a composite odontoma. Seven-year-old boy complaining of delayed eruption of the left lower permanent 1st molar came to the department of Oral & Maxillofacial Radiology at Chonbuk National University Hospital. The list of our radiographic differential diagnosis included the dental follicle, compound odontoma, complex odontoma and cystic odontoma. The microscopic analysis showed the lesion with the characteristics of an ameloblastic odontoma. The obtained results were as follows; 1. The area was asymptomatic, but the delayed eruption of the left lower permanent 1st molar was observed. Radiographically, well-defined widened pericoronal radiolucency containing several tooth-like radiopacities involving the impacted left lower permanent 1st molar and well-defined round radiolucency containing irregular radiopaque mass were observed. 3. Histopathologically, several tooth-like structures, odontoma components with the area of typical enamel, dentin and pulp, and ameloblastic components with typical follicular ameloblastoma were observed.

Purpose: This study was performed to analyze human maxillary and mandibular trabecular bone using the data acquired from micro-computed tomography (micro-CT), and to characterize the site-specific microstructures of trabeculae. Materials and Methods: Sixty-nine cylindrical bone specimens were prepared from the mandible and maxilla. They were divided into 5 groups by region: the anterior maxilla, posterior maxilla, anterior mandible, posterior mandible, and mandibular condyle. After the specimens were scanned using a micro-CT system, three-dimensional microstructural parameters such as the percent bone volume, bone specific surface, trabecular thickness, trabecular separation, trabecular number, structure model index, and degrees of anisotropy were analyzed. Results: Among the regions other than the condylar area, the anterior mandibular region showed the highest trabecular thickness and the lowest value for the bone specific surface. On the other hand, the posterior maxilla region showed the lowest trabecular thickness and the highest value for the bone specific surface. The degree of anisotropy was lowest at the anterior mandible. The condyle showed thinner trabeculae with a more anisotropic arrangement than the other mandibular regions. Conclusion: There were microstructural differences between the regions of the maxilla and mandible. These results suggested that different mechanisms of external force might exist at each site.

The odontogenic myxoma is an infiltrative benign tumor of bone that occurs almost exclusively in the jaw bones and comprises 3% to 6% of odontogenic tumors. This neoplasm is thought to arise from the primitive mesenchymal structures of a developing tooth, including the dental follicle, dental papilla, or periodontal ligament. Radiographically the odontogenic myxoma may produce several patterns: unicystic, multilocular, pericoronal, and radiolucent-radiopaque, making the differential diagnosis difficult. In this report, two cases of the odontogenic myxoma in the jaw bones are presented. The first case involved only the mandible, while the second case involved the maxilla. Both cases presented extensive multilocular radiolucencies characteristic of odontogenic myxoma.
Purpose: The purpose of this study was to evaluate the morphologic features of posterior edentulous mandible for Korean patients using cross-sectional CT images. Materials and Methods: Computed tomographic cross-sectional views taken in 2004 and 2005 at Seoul National University Dental Hospital were analyzed by an oral and maxillofacial radiologist. Four indices were measured to meet the purpose of this study: 1) the horizontal distance between the alveolar crest and mandibular canal (Type), 2) the angle of the mandibular long axis (Angle), 3) the bucco-lingual location of mandibular canal, and 4) the depth of the submandibular fossa. The averages and standard deviations of the measurements were compared according to the location (the first and second molar area) and sex of the patients. Results: Statistically significant difference was found in Type, Angle, and submandibular fossa depth between the first and second molar area (p<0.05). However, there was no significant difference between men and women in any of the measured indices. Most of the mandibular canals were located in lingual side of posterior mandible. Conclusion: More care should be taken when an implant is installed on the mandibular second molar area.

Actinomycosis is defined as a chronic, specific, suppurative, granulomatous disease caused mainly by the anaerobic, gram positive organism, Actinomyces israelii. Actinomycosis in the salivary gland is a rare disease that is caused by an inhabitant of the normal flora. We report the case of the actinomycosis of submandibular gland. A 53-year old man presented with the swelling on left submandiblar area. The lesion was not painful but had been increasing for about 10 days. In the CT view, the internal portion of the mass showed homogeneous moderate signal. The mass had continuities with the inferior portion of the left enlarged submandibular gland. In the MRI, there was a mass that showed a buldging pattern inferiorly in the left submandibular gland without bony invasion sign. The biopsy shows the colony of special organism. Many filaments are discovered with clubbed ends diffused from center of colony. We diagnosed this disease as actinomycosis in the submandibular gland by the postoperational biopsy.
