뇌손상으로 인한 하악운동의 변화 (A Case Report on Abnormal Jaw Movements Associated with Brain Injury)

  • A 42-year old male patient was referred to the Department of Oral Medicine, Kyungpook National University Hospital due to the chief complaint of limite mouth opening. Three years ago, the patient was diagnosed as an infarction of both cerefellar hemispheres, acute obstructive hydrocephalus and acute epidural hematoma of frontal lobe at the department of neurosurgery.Both of the infarcted cerevellar hemispheres and the epidural hematoma of frontal lobe were removed with suboccipital and frontal craniectomu. After the brain surgery jaw opening range was decreased progressively and ultimately mouth opening became almost impossible. Spasmodic and rhythmic contractions of the masseter muscles occurred intermittently during daytime as well as sleeping. Food intake was available only through Levin -tube. Actibe jaw opening exercise was prescribed with the aids of tongue blades. A moist hot pack and indomethacin phonophesis were also applied 20 minutes three times a day to decrease discomfort muscle activities. After a month of treatments, the opening range was increased to 5mm at the premolar area and oral food intake was possibel. The L-tube was removed and the patient was discharged.

Status of Oral Care according to the Type of Surgery for Inpatients at Dental Hospitals

  • Background: The aim of this study was to understand the importance of professional oral care for inpatients by examining the type and frequency of surgery and hospital discharge period at dental hospitals, and identify the types of professional oral care actually in progress. Methods: In this study, the type of surgery and length of hospitalization were investigated among patients admitted to the dental hospital for oral and maxillofacial surgery, and the professional oral care status of inpatients who had difficulty self-managing their oral care was retrospectively identified by collecting data on oral care before and after surgery, including the type and frequency. Results: The majority of inpatients at dental hospitals were male (57.6%), elderly patients over 60 years accounted for 20% of patients, and the average length of hospitalization was 4 days. In the 20s (aged 20~29 y), the number of orthognathic surgery patients (73.1%) was high, and the incidence of cysts was high in middle-aged patients. Regarding the oral care of hospitalized patients, scaling was performed once by a dentist before surgery. After surgery, surgical dressings using H2O2 balls were applied and oral care education was introduced before discharge. Conclusion: Based on the results of this study, professional oral care is essential to prevent infection and complications caused by oral bacteria among inpatients at dental hospitals. It is necessary to use various oral hygiene aids for inpatients and to conduct effective oral care instruction according to each patient's situation. In addition, it is necessary to raise awareness and the role of dental hygienists in professional oral care.

  • 본 연구는 대학생의 구강검사 및 구강미생물검사, 설문조사를 통해 구강건강상태와의 관련성을 분석하여 구강건강증진을 위한 구강보건교육프로그램 개발에 필요한 기초자료를 제시하고자 한다. 대학생 95명을 대상으로 한 연구결과 칫솔질 교육을 받지 않은 학생이 높은 구강미생물의 활동성을, 구강미생물의 양이 많은 학생이 주관적으로 치아통증을 더 느꼈다. 우식치아지수는 구취 느끼는 증상이 더 컸으며, 상실치아지수는 구강미생물의 종류와 양에서 차이가 있었고, 우식경험영구치지수에서 구강보조용품 사용하지 않는 경우 더 좋지 않은 구강건강상태를 보였다. 본 연구결과를 바탕으로 구강보건행위와 주관적 구강건강인식, 구강미생물 정도는 객관적인 구강건강상태와의 유의한 관계가 있음을 확인하였다. 대학생의 구강건강 증진을 위해 생애주기별 맞춤형 구강보건교육 매체의 개발 및 공유 확산이 필요할 것이다.

Oral lesions associated with human immunodeficiency virus in 75 adult patients: a clinical study

  • Objectives: The objective of this study was to investigate the presence of oral lesions in human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS) patients in a descriptive cross-sectional study, and to establish their presence according to levels of CD4+ cells (including the CD4+/CD8+ cell ratio). Materials and Methods: A total of 75 patients infected with HIV were included. Oral lesions were observed and classified using World Health Organization classification guidelines. Potential correlations between the presence and severity of oral lesions and CD4+ cells, including the CD4+/CD8+ cell ratio, were studied. Results: The most frequent oral lesion detected was oral pseudomembranous candidiasis (80.0%), followed by periodontal disease (40.0%), herpetic lesions (16.0%), hairy leukoplakia (16.0%), gingivitis (20.0%), oral ulceration (12.0%), Kaposi's sarcoma (8.0%), and non-Hodgkin's lymphoma (4.0%). The CD4+ count was <$200cells/mm^3$ in 45 cases (60.0%), between $200-500cells/mm^3$ in 18 cases (24.0%), and >$500cells/mm^3$ in 12 cases (16.0%). The mean CD4+ count was $182.18cells/mm^3$. The mean ratio of CD4+/CD8+ cells was 0.26. All patients showed at least one oral manifestation. Conclusion: There was no correlation between the CD4+/CD8+ cell ratio and the presence of oral lesions. The severity of the lesions was more pronounced when the CD4+ cell count was less than $200cells/mm^3$.

Isolation and Identification of Candida dubliniensis and Distribution of Candida spp. from Oral Cavity of Healthy People

  • Candida spp. are yeast form fungi, which cause an opportunistic infections in a immune suppressed patients however it is a normal flora of the respiratory system, the gastrointestinal system, and the urogenital system of healthy person. It is investigated that the distribution of Candida spp. cause an oral disease from oral cavity of healthy people and also identified Candia dubliniensis. Distribution and identification of the yeast form fungi in oral cavities of healthy people was investigated by an automatic identifier, VITEK2 system. We found 21 strains of Candida albicans, 3 strains of Candida famata, one strain of Candida tropicalis, Candida haemulonii, Candida krusei, and Candida dubliniensis. In addition, one strain of Cryptococcus spp., Saccharomyces spp., and two unknown strains were isolated. Candida dubliniensis which forms a mass by more than 2 chlamydospores was isolated from a healthy person for the first time. Candida dubliniensis was not grown at $42^{\circ}C$ whereas Candida albicans was grown well. It is known that Candida dubliniencis was isolated in AIDS while it is found in healthy people from this study, which will be helpful to investigate the distribution of Candida spp.

  • Acquired Immune Deficiency Syndrome is the state which develops after complications with the infection from HIV. Irrespective of their state, all HIV infections have infectivity. According to a 2013 U.N. AIDS global report, the number of reported newly infected with HIV is constantly falling, while that of Korea has been increasing over recent years and it surpassed 10,000 in 2013. This phenomenon might be attributed to the unusualness of the blood test for early detection of HIV infection. From this fact, we can assume that we have strong possibilities for encounters with infections in the office. But many misconceptions about the disease makes patients try to hide their medical history, which can lead to a nationwide spread of the infection without proper management. Even though it may be difficult to take care of HIV patients in smaller dental offices, large scale hospitals have the means to arrange protocols to treat them. We present a case about dental treatment of a patient with AIDS that has a history of cerebral infarction and hepatitis type B under general anesthesia. The purpose of this case report was to discuss the special considerations of dental care for patients with HIV.

신속 구강 점막 도말 HIV 항체 검사에 대한 일반인/의료진의 사용 후 반응도 연구 (Response of ordinary person/medical team after using of rapid oral HIV screening test)

  • The aim of this study is to evaluate the patient's and clinician's response to rapid oral HIV screening test. Attitude assessment survey was carried out on patients test with rapid oral HIV screening test and clinicians who conducted the test. The patients and clinicians were present at the dental hospital and clinics which were participating in the research project, between March and May 2010. Rapid oral HIV screening test is safe and efficient to the patients. It also demonstrated reliability to accuracy and confidentiality, and high recommendation to rapid oral HIV screening test. The clinician survey revealed high test convenience and high appraisal to patient satisfaction. In conclusion, if rapid oral HIV screening test becomes more common, patients will receive safe and convenient and high reliable HIV infection test, while clinicians will be able to create safe dental care environment.

High-performance liquid chromatographic assay and oral pharmacokinetics of new anti-HIV uracil derivatives, KR-V analogues, in rats

  • A number of uracil derivatives have been developed as anti-AIDS drugs having a mechanism of inhibiting cellular reverse transcriptase. A simple and rapid assay technique for recently synthesized KR-V analogues was developed using a high-performance liquid chromatography, and oral pharmacokinetics was examined for assessing their oral bioavailabilites. Plasma samples were analyzed by reversed-phase HPLC using an ODS column with an ultraviolet detection system. All the analogues were eluted within 12 min and the LOQ was 15-30 ng/$m\ell$. The extraction recoveries were higher than 85%, except KR-V1039, 1068 and 1720 having ester group. This chromatic method was well applied to the kinetic studies for KR-V analogues. Among 16 analogues tested in the present work, the 6 compounds including KR-V1123, 1122, 1784, 1783, 1736 and 1700 were found to be bioavailable for oral administration to rats.

충치가 치주염으로 빠르게 진행하면 에이즈 의심해야

  • 에이즈 환자에게서 흔히 나타나는 구강 내 병변은 충치, 구강칸디다증, 구강 모상 백반증(oral hairy leukoplakia),재발성 아프타성 구내염(recurrent aphthous stomatitis),재발성 포진성 구내염(recurrent herpetic stomatitis),치주염, 카포시 육종, 비호치킨성 림프종 등이다. 이 중 에이즈 환자에게 비교적 특징적인 병변은 구강 칸디다증, 입안털백색판증, 카포시 육종 등이지만 일반인들에서 흔히 관찰되는 충치라고 하더라도 치주염이나 농양으로 빠르게 진행한다면 에이즈를 의심해 보아야 하며, 반복적인 아프타성 궤양이나 포진성 궤양이 유달리 크고 오래 지속된다면 역시 에이즈의 가능성을 생각해 봐야 한다. 구강 병소는 에이즈 환자에게서 흔히 발견되며 이 질환의 초기 상태를 제시할 수 있다는데 의의가 있다.

일부지역의 구강위생용품 사용실태에 관한 조사연구 (A study on usage status of oral hygiene devices in some area)

  • Objectivs : The purpose of this study is to provide basic data for the use of oral hygiene devices and to improve oral health care continuously by encouraging the use of oral hygiene devices. Methods : The survey was carried out by questionnaire research targeting 395 respondents in metropolitan area in June-August 2009. The collected data is analyzed by statistics program SPSS version 12.0. 1. General characteristics of subjects are analyzed by percentage and descriptive statistics. 2. Recognition about how to use oral hygiene devices, usage of oral hygiene devices, recognition of dental brush usage, Effect after using oral hygiene devices are analyzed by mean difference test(t, F verification). 3. Effect after using oral hygiene devices, relation of oral hygiene devices usage with subjective oral health status, oral hygiene devices usage and participation in education are analyzed by Correlation, Regression analysis. Results : 1. Investigation shows that most oral health devices are rarely used except toothbrush(4.57) in case of the use of oral hygiene devices. 2. In the case of awareness of oral hygiene devices usage, almost everyone knows how to use dental brushes(MEAN 4.16) the most. And the next ones are like these-tooth picks(MEAN 3.38), dental floss(MEAN 3.28), mouth rinse(MEAN 3.24). And they don't know how to use the other oral hygiene devices usage. 3. respondents who use dental floss(${\beta}$=.238, p<.001), interdental brush(${\beta}$=.242, p<.001) and water pik(${\beta}$ =.180, p<.05) recognize that the result and effect after using them are more positive than the others in different case. 4. Respondents who use toothbrush(${\beta}$=.119), dental floss(${\beta}$=.120) and interdental brush(${\beta}$=.136). frequently evaluate their subjective oral health status positively in case of frequency of using oral hygiene devices and subjective oral health status(p<.05). 5. Respondents who use toothbrush and interdental brush properly and frequently, who are older than the others show more concern about participating in education of "How to use oral hygiene devices" than the others(p<.05). Conclusions : In conclusion, to prevent oral disease, the opportunity of participating in education about correct knowledge and proper way should be expanded. And we should contribute to oral health improvement through developing, executing and informing systematic, popular oral health education with active one.
