Increasing injection frequency enhances the survival of injected bone marrow derived mesenchymal stem cells in a critical limb ischemia animal model

  • Critical limb ischemia (CLI) is one of the most severe forms of peripheral artery diseases, but current treatment strategies do not guarantee complete recovery of vascular blood flow or reduce the risk of mortality. Recently, human bone marrow derived mesenchymal stem cells (MSCs) have been reported to have a paracrine influence on angiogenesis in several ischemic diseases. However, little evidence is available regarding optimal cell doses and injection frequencies. Thus, the authors undertook this study to investigate the effects of cell dose and injection frequency on cell survival and paracrine effects. MSCs were injected at $10^6$ or $10^5$ per injection (high and low doses) either once (single injection) or once in two consecutive weeks (double injection) into ischemic legs. Mice were sacrificed 4 weeks after first injection. Angiogenic effects were confirmed in vitro and in vivo, and M2 macrophage infiltration into ischemic tissues and rates of limb salvage were documented. MSCs were found to induce angiogenesis through a paracrine effect in vitro, and were found to survive in ischemic muscle for up to 4 weeks dependent on cell dose and injection frequency. In addition, double high dose and low dose of MSC injections increased vessel formation, and decreased fibrosis volumes and apoptotic cell numbers, whereas a single high dose did not. Our results showed MSCs protect against ischemic injury in a paracrine manner, and suggest that increasing injection frequency is more important than MSC dosage for the treatment CLI.

  • This experiment was carried out to investigate the optimal dose of intravitreal gentamicin that decreases intraocular pressure effectively and minimizes complications in dog. After inhalation anesthesia, gentamicin was injected intravitreally into the left eyes at doses of 10, 15 and 20 mg with 1 mg dexamethasone, respectively. Sterilized isotonic saline and dexamethasone mixture into the right eyes for control. Six dogs were used in each group. Intraocular pressures were measured using applanation tonometer(Mentore, Tono-Pen) until 5 months after injection of gentamicin. Ocular examinations were performed using direct ophthalmoscopy. The ocular volumes of both eyes were measured. Intraocular pressures of eyes injected with 10. 15 and 20 mg of gentamicin were decreased significantly compared with control eyes. Severe corneal opacity and neovascularization occurred in 20 mg treated group. Intraocular hemorrhage was observed in 3 dogs of 20 mg treated group. Ocular volume was significantly decreased(p <0.05) in 20 mg treated group, compared with 10 and 15 mg treated group. It is considered that intravitreal gentamicin injection at dose of 10 mg or 15 mg decrease intraocular pressure effectively and minimize complications such as corneal opacity, hyphema and phthisis bulbus.

  • A new method is proposed to fabricate a reusable qualtz master with order of 100 nm dot pattern on its surface. Some fabrication conditions such as dose are investigated to find optimal condition. This reusable qualtz master is used directly as a stamper to injection mold the dot patterns. Polycarbonate and Polyoxymethylene are used as molding materials and the effect of the mold temperature is also investigated to see the moldabilty of the injection molding for very fine dot features.

  • In vivo activity of recombinant human erythropoietin (rh-EPO) has been examined using polycythemic model in mice and acute hemorrhage model in rats. The number of reticulocytes in blood stream was increased after a single injection of rh-EPO depending on the dosage of rh-EPO in polycythemy model. It seemed that optimal dose of rh-EPO for polycythemic mice was around 1-10 U/kg. Rh-EPO also showed the effectiveness for increase of reticulocyte numbers both in male and female rats after bleeding. The number of reticulocytes and the change of hemoglobin concentration in the blood stream of normal rats has been examined after injection of rh-EPO. The maximum value of reticulocyte was observed on the 6th day of the injection in these normal rats. In addition, the increase of reticulocyte and the concentration of hemoglobin were dependent on the dosage of rh-EPO. The increase of hemoglobin concentration was continued to the 9th day after injection. In this study, the efficacy of rh-EPO was confirmed in both mice and rats.

소아 복부 CT 검사에서 체중에 기반한 조영제 주입 프로토콜 적용에 따른 조영증강의 최적화 (Contrast Optimization using of Weight-based Injection Protocol in Pediatric Abdomen CT Examination)

  • 본 논문의 목적은 복부 CT 검사를 시행하는 소아환자에게 체중 기반 조영제 프로토콜을 적용함으로써 고정 투여법보다 조영제를 감소시키면서 최적의 문맥기 화질을 달성하는 것이다. Discovery 750HD(General Electric medical systems, Milwaukee, USA)를 이용하였으며, 만 18세 미만의 소아 남자 85명과 여자 82명, 총 167명을 대상으로 연구하였다. 300 mgI/ml(Xenetix, Guerbet, France)의조영제를 몸무게 2배로 고정 주입한 그룹과 체중기반 프로토콜을 적용하면서 주입량을 10%씩 단계적으로 감소시키면서 생리식염수를 주입한 그룹을 구분하였으며, 스캔 지연 시간을 추가로 변화시키면서 복부 장기의 CT 감쇄계수와 SNR을 비교 평가하였다. 또한 조영 증강의 정도와 심장 주변의 빔 경화 인공물을 정성적으로 평가하였다. 체중기반 프로토콜을 적용하고 20%의 조영제를 감소한 그룹이 몸무게 2배로 고정 주입한 그룹과 조영 증강이 가장 유사하였으며, 그리고 20%의 지연시간을 가진 그룹이 가장 조영 증강 효과가 높았다. 조영제 주입 후 적절한 지연시간은 실질 장기의 조영 효과를 상승시켰으며, 생리식염수를 적용한 주입 프로토콜은 심장 주변의 인공물이 감소시켰다. 결론적으로 소아 복부 CT 검사 시, 체중 기반 프로토콜의 적용과 적절한 지연시간의 조절은 불필요한 조영제 사용을 억제하고 최적의 문맥기 영상의 특성화를 가능하게 한다.

PET-CT 검사에서 18F-FDG 투여량 감소에 대한 고찰 (Reduction of Injection Dose in 18F-FDG Fusion PET)

  • 최근 방사선 노출에 대한 사회적인 이슈와 함께 의료용 방사선의 이용에 대한 환자의 관심도 증가하고 있다. 현재 본원에서 시행하는 PET-CT 검사의 $^{18}F$-FDG는 EANM 권고기준대비 40% 초과하여 투여하고 있다. 따라서 최신 기술이 탑재된 장비의 성능을 파악하여, 진단적 가치를 보존하면서도 환자의 피폭선량을 최소화 시킬 수 있는 적정한 $^{18}F$-FDG의 투여량에 대해 알아보고자 한다. PET-CT 장비로는 2007년 설치된 Biograph Truepoint 40 (siemens, USA)스캐너와 2011년 설치된 동일회사의 Biograph mCT 64 (siemens, USA)를 사용하였고, 각 장비의 고유성능을 평가하기 위해 scatter phantom을 이용하여 NECR을 평가하였다. 또한 각 장비의 영상에 대한 평가를 위해 NEMA IEC Body Phantom에 $^{18}F$-FDG를 3.7, 4.44, 5.18 MBq/kg을 주입하고, 각각에 대해 bed 당 60, 70, 80, 90, 100, 110, 120초씩 받은 데이터로 장비간의 SNR을 평가하였으며, 임상적 평가를 위해 $^{18}F$-FDG 3.7, 4.44, 5.18 MBq/kg을 주입한 환자들의 데이터를 이용하여 SNR을 비교 평가하였다. 실험 결과 mCT 64의 peak NECR값은 1.65e+005 cps이고, 이것은 Turepoint 40보다 10 % 높은 수치였다. NEMA IEC body phantom을 이용한 SNR값은 $^{18}F$-FDG 3.7 MBq/kg 주입한 경우 mCT 64가 검사 시간에 따라 평균 17.9% 높았고, 4.44 MBq/kg 주입한 경우는 평균 17.4% 높았으며, 5.18 MBq/kg 주입한 경우는 평균 17.1% 높았다. 임상 환자 영상의 경우 mCT 64의 SNR값은 16.5이고, 이것은 Turepoint 40 장비보다 25% 높았다. 다시 말해, 최근 장비의 발전으로 인해 장비 성능의 평가 항목 중 하나인 NECR은 10% 증가하였고, 영상 질의 평가 항목 중 하나인 SNR은 평균 17.5% 증가하였다. 더불어 bed당 10초의 시간을 늘리면 주입량을 10% 감소를 하여도 SNR은 유지 할 수 있었다. 그러므로 신규 도입한 장비의 정확한 성능테스트를 통해 영상의 질 저하 없이 투여량을 저감하여 환자의 방사선 피폭선량을 줄일 수 있고, 또한 방사선 노출에 대한 환자의 불안감을 감소시킬 수 있을 것으로 사료된다.

효소-항체의 결합 및 효소면역측정 방법의 연구 (Study on Antibody-enzyme Coupling and Enzyme Immunoassay Methods)

  • Alakaline phosphatase (ALP)- or horseradish peroxidase (HRP)-antibody conjugate was used frequently on the immunological detection methods such as enzyme-linked immunosobent assay (ELISA), immunobolt, immunohistochemistry. The classical enzyme-antibody coupling method by one-step (direction) injection of glutaraldehyde bring into being disadvantage such as low sensitivity of antigen detection because of homopolymers. This study was modified with the dialysis glutaraldehyde method to provide simple coupling through E-amino residues present in most protein. The dialysis glutaraldehyde coupling effects were better than the classical one-step glutaraldehyde injection in antigen detection of ELISA and immunobolt. Optimal dose of the dialysis glutaraldehyde solution was 0.10-0.25 %. This results suggest that the dialysis glutaraldehyde coupling method can readily applied to antigen detection of in vitro and in vivo.

Epidural Steroid Injection in Korean Pain Physicians: A National Survey

  • Background: Epidural steroid injection (ESI) is one of the most common procedures for patients presenting low back pain and radiculopathy. However, there is no clear consensus on what constitutes appropriate steroid use for ESIs. To investigate optimal steroid injection methods for ESIs, surveys were sent to all academic pain centers and selected private practices in Korea via e-mail. Methods: Among 173 pain centers which requested the public health insurance reimbursements for their ESIs and were enrolled in the Korean Pain Society, 122 completed questionnaires were returned, for a rate of 70.5%; also returned were surveys from 39 academic programs and 85 private practices with response rates of 83.0% and 65.9%, respectively. Results: More than half (55%) of Korean pain physicians used dexamethasone for ESIs. The minimum interval of subsequent ESIs at the academic institutions (3.1 weeks) and the private practices (2.1 weeks) were statistically different (P = 0.01). Conclusions: Although there was a wide range of variation, there were no significant differences between the academic institutions and the private practices in terms of the types and single doses of steroids for ESIs, the annual dose of steroids, or the limitations of doses in the event of diabetes, with the exception of the minimum interval before the subsequent ESI.

정수장 유입조류 전처리를 위한 천연조류제거제(W.H.)의 최적주입농도 결정 (Decision Algorithm of Natural Algae Coagulant Dose to Control Algae from the Influent of Water Works)

  • 하천과 호수의 부영양화로 인하여 남조류가 대량으로 증식하게 되면 고유의 생물독소로 인한 위해뿐만 아니라 정수처리 과정에서 경제적 손실을 야기할 가능성이 있다. 현재 상용화되어있는 천연조류제거제인 M사의 W.H. 응집제(이하 W.H.)는 참나무 유래 성분의 살조 및 타감작용을 이용한 응집.부상공정을 통하여 조류를 사전에 제거함으로써 정수공정에 미치는 영향을 효과적으로 저감할 수 있다. 그러나, W.H.를 활용한 응집 부상공정은 정수처리의 전처리공정으로 적용된 사례가 없기 때문에 최적주입농도의 결정기법에 대한 보고 또한 전무한 실정이다. 본 연구에서는 (1) 한강에서 채취한 복합 조류와 (2) 남조류를 선택적으로 대량 배양하여 광조건 하에서 W.H. 투여량 및 조류농도 등의 여러 조건을 변화시키면서 Jar-test를 시행하여 응집 부상공정에서의 조류의 제거기작을 검토하였다. Jar-test 결과를 바탕으로 IBM-SPSS를 활용한 다중회귀분석을 실시하여 최적 W.H. 주입농도를 결정하기 위한 Chl-a 농도와 탁도를 변수로 하는 두 가지 선형식을 도출하였다. 또한 유입수질의 변동에 따라 W.H. 주입농도를 신속하게 결정하고 자동화할 수 있는 자동제어 로직의 프로토타입(Prototype)을 제시하였다.

Novel anatomical proposal for botulinum neurotoxin injection targeting depressor anguli oris for treating drooping mouth corner

  • The depressor anguli oris (DAO) muscle is a thin, superficial muscle located below the corner of the mouth. It is the target for botulinum neurotoxin (BoNT) injection therapy, aimed at treating drooping mouth corners. Hyperactivity of the DAO muscle can lead to a sad, tired, or angry appearance in some patients. However, it is difficult to inject BoNT into the DAO muscle because its medial border overlaps with the depressor labii inferioris and its lateral border is adjacent to the risorius, zygomaticus major, and platysma muscles. Moreover, a lack of knowledge of the anatomy of the DAO muscle and the properties of BoNT can lead to side effects, such as asymmetrical smiles. Anatomical-based injection sites were provided for the DAO muscle, and the proper injection technique was reviewed. We proposed optimal injection sites based on the external anatomical landmarks of the face. The aim of these guidelines is to standardize the procedure and maximize the effects of BoNT injections while minimizing adverse events, all by reducing the dose unit and injection points.
