Medpor Craniotomy Gap Wedge Designed to Fill Small Bone Defects along Cranial Bone Flap

  • Objective : Medpor porous polyethylene was used to reconstruct small bone defects (gaps and burr holes) along a craniotomy bone flap. The feasibility and cosmetic results were evaluated. Methods : Medpor Craniotomy Gap Wedges, V and T, were designed. The V implant is a 10 cm-long wedge strip, the cross section of which is an isosceles triangle with a 4 mm-long base, making it suitable for gaps less than 4 mm after trimming. Meanwhile, the Medpor T wedge includes a 10 mm-wide thin plate on the top surface of the Medpor V Wedge, making it suitable for gaps wider than 4 mm and burr holes. Sixty-eight pterional craniotomies and 39 superciliary approaches were performed using the implants, and the operative results were evaluated with respect to the cosmetic results and pain or tenderness related to the cranial flap. Results : The small bone defects were eliminated with less than 10 minutes additional operative time. In a physical examination, there were no considerable cosmetic problems regarding to the cranial bone defects, such as a linear depression or dimple in the forehead, anterior temporal hollow, preauricular depression, and parietal burr hole defect. Plus, no patient suffered from any infectious complications. Conclusion : The Medpor Craniotomy Gap Wedge is technically easy to work with for reconstructing small bone defects, such as the bone gaps and burr holes created by a craniotomy, and produces excellent cosmetic results.

슬개골전 점액낭염의 관절경적 치료 - 경피적 연차봉합술 3례 보고 - (Arthroscopic Treatment of the Prepatellar Bursitis - A Report of Three Cases of Percutaneous Mattress Suture Technique -)

  • 급성 외상에 의해 발생한 슬개골전 점액낭염 중 그 크기가 거대하고 보존적 요법에 반응하지 않는 3례에 대하여 관절경을 통해 점액낭을 절제하고 경피적 다발성 연차봉합을 시행한 바, 그 술기를 소개하고 1년이상 추시상 모든예에서 합병증이나 재발없이 만족할 만한 결과를 얻었기에 그 증례를 보고하고자 한다. 본 술기는 통상적인 절개방법에서의 피부 반흔에 따른 문제와 감염 및 재발 등의 합병증을 최소화하기 위한 보완적 술기로 고안되었으며 얇고 손상받기 쉬운 피부를 보호할 수 있는 장점이 있고 점액낭이 제거된 공간에 부가적인 연차봉합을 통해 사강(dead space)을 배제하고 분리된 조직간의 견고한 접촉을 유지해 줌 으로써 술후 조기 운동을 가능케하고 재발의 가능성을 줄일 수 있어 선택적으로 적용할만한 유용한 술기라 사료된다.

A simple technique for impression taking of teeth and functionally generated paths

  • The objective of this case report is to introduce a simple technique for simultaneously taking a closed-mouth impression and functionally generated path (FGP) for a full coverage crown restoration. A monolithic zirconia crown was the restoration of choice. An alginate impression of the abutment tooth was taken to fabricate a custom-made closed-mouth impression tray covering the abutment tooth and the adjacent teeth. The tray had an FGP table and an abutment tray in cameo and intaglio surfaces, respectively. The impression was taken with silicone impression material after adjusting the abutment tray and inscribing the FGP using self-curing acrylic resins. Plaster casts were made from the impression, and a zirconia crown was fabricated. The crown was cemented to the abutment tooth with minimal adjustments. This simple technique resulted in a well-fitting crown that accounted for mandibular movements. Using the custom closed-mouth impression tray incorporating an FGP table simultaneously aids in fabricating an accurately fitting restoration that incorporates harmonious mandibular movements using a single impression capture.

하악 과두 골절의 관혈적 정복시 고정 방법에 따른 임상적 평가 (CLINICAL EVALUATION OF TREATMENT OUTCOME OF PLATING TECHNIQUE OF FIXATION FOR MANDIBULAR CONDYLAR FRACTURE)

  • The purpose of this study was to compare a sample of patients who had condylar fractures treated with open reduction using different plating techniques, to evaluate which plating technique is useful for stable fixation for fractures of the mandibular condyle and to evaluate effectiveness of resorbable miniplate. There were 60 patients (41 males, 19 females) whose condylar fractures were treated with open reduction. Rigid fixation was performed with a single miniplate, double miniplate ot one miniplate & one microplate and single resorbable plate. All patients remained intermaxillary fixation for 1 week postoperatively. Active physiotherapy was started after 2 weeks postoperatively. Radiographic evaluation (plate fracture, plate bending, screw loosening, displacement of condyle etc.) was performed at pre-operative, immediate, 2 weeks, 1 month, 3 months, 6 months after surgery. Clinical evaluation included degree of mouth opening, occlusion, mandibular lateral excursion, infection and facial nerve paralysis. In radiographic evaluation, displacement of fractured condylar segment associated with plate bending or screw loosening were showed 6 cases and 3 cases for single miniplate system and resorbable miniplate system. There was no patients who have this problem on double miniplate system. The results revealed that the application of two miniplates were more recommendable than single miniplates. When we select resorbable miniplate system, we should consider the type of fractures, post-operative treatment protocol and surgical technique.

Clinical Applications of the Tubular Retractor on Spinal Disorders

  • Tubular retractor system as a minimally invasive surgery (MIS) technique has many advantages over other conventional MIS techniques. It offers direct visualization of the operative field, anatomical familiarity to spine surgeons, and minimizing tissue trauma. With technical advancement, many spinal pathologies are being treated using this system. Namely, herniated discs, lumbar and cervical stenosis, synovial cysts, lumbar instability, trauma, and even some intraspinal tumors have all been treated through tubular retractor system. Flexible arm and easy change of the tube direction are particularly useful in contralateral spinal decompression from an ipsilateral approach. Careful attention to surgical technique through narrow space will ensure that complications are minimized and will provide improved outcomes. However, understanding detailed anatomies and keeping precise surgical orientation are essential for this technique. Authors present the technical feasibility and initial results of use a tubular retractor system as a minimally invasive technique for variaties of spinal disorders with a review of literature.

Vivadent의 변연누출에 관한 실험적 연구 (Marginal Leakage Test on 'Vivadent' Composite Resin)

  • The purpose of this study was to evaluate the marginal sealing ability of 'vivadent.' Using freshly extracted human teeth and 2% aqueous methylene blue, the marginal leakage of dye in restorative materials such as vivadent with acid etching technique, Durafill with acid etching technique, silar with acid etching technique, Adaptic, and Amalgam were investigated at 37℃ and under temperature cycling in range of 4℃-60℃. The results were as follows; 1. All filling materials showed some degree of marginal penetration by 2% methylene blue dye. 2. Vivadent with acid etching technique revealed effective marginal sealing ability, but under temperature cycling it showed increased marginal leakage. 3. All resins showed greater marginal leakage than amalgam restoration. 4. Vivadent had the most effective marginal sealing ability in experimented resins.

분리성 대동맥판막하 협착의 수술 (Surgery for Discrete Subaortic Stenosis)

  • Concerning the operative technique for the relief of discrete subvalvular aortic stenosis, emphasis by some authors has been placed on the superiority of blunt dissection in the enucleation of the entire, obstructing structure such as membranous or fibrous ring stenosis compared with the conventional resection by sharp dissection technique. Here report a case operated applying the technique of blunt dissection, analogous to endarterectomy, which resulted in good clinical improvement. The technique is such that the tissue causing obstruction is peeled off the ventricular septum along the plane of cleavage using a dissector, starting under the commissure between right and left coronary cusps. As for the origin of stenosis this case imply a possibility of different mechanism of etiology evidenced by that the tissue removed consisted of elastic fibers only.

  • Between January 1980 and July 1984, 321 cases of open heart surgery for infants or small children under 10Kg of body weight were performed, which occupied the great part of total open heart surgery done in the same period. The mean age was 16.58.7 months [ranging from 2 days to 51 months], and the mean body weight was 7.8a1.8Kg [from 2.8 to 10Kg] which was below the third percentile compared with the mean age. The technique of deep hypothermia with total circulatory arrest, which contributed to great improvement in operative condition, was used increasingly and widespreadly in this period. For each anomaly, the number of patients and operative deaths were as follows: VSD, 11 of 184 [6.0%]; TOF, 8 of 47 [17.8%]; TGA, 13 of 30 [43.3%]; ASD, none of 9; TAPVR, 1 of 8 [12.7%]; C-ECD, 3 of 6 [50%]; Tricuspid Atresia, 4 of 5 [80%]; Pulmonary Atresia, 2 of 4 [50%]; Congenital Mitral Anomaly, 1 of 3 [33.3%]; P-ECD, none of 3

Orbital Floor Fracture

  • The medial wall and floor of the bony orbit are frequently fractured because of the delicate anatomy. To optimize functional and aesthetic results, reconstructive surgeons should understand the anatomy and pathophysiology of orbital fractures. Appropriate treatment involves optimal timing of intervention, proper indications for operative repair, incision and dissection, release of herniated tissue, implant material and placement, and wound closure. The following review will discuss the management of orbital floor fractures, with the operative method preferred by the author. Special considerations in operation technique and the complication are also present in this article.

  • This paper presents a model and analysis techniques for a unilateral external fixator to achieve fracture reduction and deformity correction in long bones precisely. through fixator joint adjustment. Combining the kinematic analysis with a graphic model of the tibia and the fixator allowed 3D simulation and visualization of the adjustments required to reduce fracture or correct bone deformity after osteotomy. The model and analysis technique can be used for fixator evaluation and clinical application planning.
