Surgical Treatment of Metastatic Lung Cancer (전이성 폐암의 외과적 치료)

  • There are follow-up data according to thirteen patients recieved the surgical resection for metastatic lung cancer arising from different primary tumor. The patients were received the surgical resection at Korean Cancer Center Hospital from July 1987 to Setember 1991 and followed-up to August 1992. There were 9 men and 4 women, ranging in age from 16 to 70 years[mean age, 42.8 years]. The primary tumors were 2 synovial sarcoma, 2 sarcoma, 2 osteosarcoma, 3 laryngeal ca, 1 melanoma, 1 ovarian ca and 1 bladder ca. The operative procedures were 5 wedge resections, 1 segmental resection, 5 lobectomies, 1 bil-obectomy and 1 pneumonectomy. There was no operative and hospital death. There were 3 deaths[each survival period: 2, 9 and 20 months, average 10.3 months]and 5 tumor recurrence during follow-up. At now, the average survival period of aliving patients is 29.1 months.

  • Thoracic sympathetic nerve block has a wide range of therapeutic applications which clinicians utilize neurolytics or perform operative sympathectomy. All methods have advantages and disadvantages. We performed "thoracic sympathetic ganglion cauterization" using resectoscope as it is less invasive and more effective than traditional operative methods. Successful procedures were performed involving 2 cases of idiopathic hyperhidrosis and 1 case of sympathetically maintained pain on chest and upper extremity. We experienced failure with one case of idiopathic hyperhidrosis due to severe pleural adhesion. There was also a case of complication of periganglional hemorrhage and parenchymal lung perforation which we successfully treated.

  • From Jan. 1981 to Dec. 1993, 24 cases of pulmonary hamartoma were experienced sugically at the Department of Thoracic Surgery, College of Medicine, Seoul National University. They consisted of 11 females and 13 males. The age distribution was 24 years to 71 years with a mean age of 49 years. They included 3 cases of endobronchial hamartoma, and 21 cases of pulmonary parenchymal hamartoma. Fifty eight percent of patients [14/24] were asymptomatic. One patient had a multiple pulmonary parenchymal hamartoma, and 1 patient also had combined lung cancer. The operative procedures were 9[37.5%] wedge resections, 5[20.8%] lobectomies, 3[12.5%] enucleations, 3[12.5%] segmental resections, 2[8.3%] bilobectomies, 1[4.2%] lobectomy and segmentectomy, and 1[4.2%] lobectomy, wedge resection, and enucleation. All of the operative results were excellent and without complication.

Traumatic Aortic Rupture - Report of 4 Case - (외상성 대동맥 파열: 4례 보고)

  • Four patients with traumatic rupture of aorta underwent operative repair at Seoul national university hospital. All cases were confirmed by preoperative aortography. Rupture site was aortic isthmus or just distal to it. Operations were somewhat delayed due to the low degree of suspicion and difficulties in diagnosis: ranging from 5 hours to 8 days. Operation was performed as same manners in all cases: resection of the ruptured portion and tubular woven dacron graft interposition in conjunction with shunt or bypass procedures, TDMAC-Heparin shunt between ascending and descending aorta was used in 3 cases, and LA-femoral centrifugal pump was used in one case. There were no intraoperative or postoperative mortality. Hoarseness was developed in all patients but paraplegia or other significant complications were not found in any of patients. We concluded that 1] high degree of suspicion is essential in the early diagnosis and treatment of traumatic aortic rupture and 2] any kind of shunt or bypass procedure is necessary in operative repair of traumatic aortic rupture and use of centrifugal pump without systemic heparinization is easier and safer procedure than others for the maintenance of adequate distal flow.

Surgical treatement of complex cardiac malformations (복잡심기형의 외과적 치료)

  • Thirteen patients with cyanotic cardiac malformations having more complex intracardiac defects, hemodynamics and operative procedures than ones in Tetralogy of Fallot undertaken total surgical corrections from July 1981 to August 1985. The cases of corrective surgery for complex cardiac malformations were 3.9% of all congenital cardiac malformations and 12.6% of cyanotic cardiac malformations. Six patients died within 30 days after surgery. So operative mortality was 46%; Transposition of the great arteries, two of 4 patients, due to low cardiac output syndrome and tracheal bleeding ; Univentricular heart, one of 3 patients, due to bleeding; Corrected transposition of the great arteries, one of 2 patients, due to acute heart failure; Tricuspid atresia, one of 2 patients, due to low cardiac output syndrome; Double outlet right ventricle, one of single patient, due to respiratory failure. The cases of surgical correction for complex cardiac malformations are progressively increasing in numbers. The more accurate evaluation of anatomical condition and hemodynamics in preoperative diagnosis, studies on applicable surgical procedure and perioperative care of patients are necessary in the improvement of clinical and surgical results.

  • The purpose of this prospective study was to assess the incidence of flare-ups among patients who received endodontic treatment for the last 11 months and to examine the correlation with pre-operative and operative variables. Data were gathered on 840 teeth of 755 patients over 11 months. At first visit, information was obtained on each patient as to their demographics, presenting signs, symptoms and diagnosis, and treatment performed. Treatment procedures were routine. If additional appointments were necessary, the access cavity was closed with a dry cotton pellet and ZOE.(omitted)

  • The recent development of new surgical techniques using lasers has increased the opportunities for open surgery involving minimal manipulation and faster and more accurate removal of lesions. The increasing use of laser technology requires nurses to play an extensive role. As assistants, nurses play an important role in maintaining the efficacy and safety of the laser device. In addition, they are also responsible for providing pre-and post-operative care to patients. Therefore, nurses should be aware of how to proceed with operative laser treatment for all surgical procedures and the steps for maintaining safety prior to, during, and after laser treatment. This review provides in-depth knowledge for nurses undertaking continuing education on lasers and patient care in the field of gynecology.

Early Results of Aortic Valve-sparing Procedures in Patients with Annuloaortic Ectasia (대동맥륜대동맥확장(Annuloaortic Ectasia) 환자에서 대동맥판막을 보존하면서 시행된 대동맥근부 및 상행대동맥 치환술의 단기 성적)

  • Aortic valve-sparing procedures could reduce valve-related morbidity, but may increase operative risks; therefore, these procedures could not be performed routinely. We attempted to find out the early results while focusing on the operative risks associated with these procedures in our hospital. Material and Method: From May 1996 to July 2003, 26 patients underwent these procedures including 15 patients with Marfan syndrome and 1 patient with Behcet disease. There were 17 men and 9 women with mean age of $37.9\pm19.2$ years (range: 6 months-74 years). Ten patients had ascending aortic dissection, 18 patients had more than moderate degree of aortic valve insufficiency (AI). Two types of valve-sparing procedures were performed: valve reimplantation in 14 and root remodeling in 12 patients. Associated procedures were performed as follows: aortic valve plasty in 6, mitral valve plasty in 5, hemi-arch replacement in 4, total arch replacement in 2, coronary artery bypass surgery in 1 and Maze procedure in 1 patient(s). Result: In four patients, valve-sparing procedures were converted to Bentall procedures during operation. Including these patients, there was no operative deaths, 3 patients underwent re-operation due to bleeding, 1 patient had permanent pacemaker. The median duration of ICU stay was 45.5 hours, the median duration of hospital stay was 10.5 days. In 22 patients excluding 4 converted patients, intraoperative transesophageal echocardiogram (TEE) showed less than mild degree of AI in all except one who had not received intra-operative TEE in the beginning and showed moderate degree of AI at discharge. The mean duration of follow-up was $21.2\pm27.4$ months. All patients were alive except one who died during other departmental surgery. In 3 patients, more than moderate degree of AI was recurred, but there were no reoperation. Conclusion: Aortic valve-sparing procedures could be performed relatively safely in selected patients who had annuloaortic ectasia.

Bronchoplastic Procedures for Bronchogenic Carcinoma (폐암 환자에서 기관지성형술)

  • Bronchoplastlc procedures including sleeve lobectomy were initially introduced for patients whose pulmonary function was insufficient to tolerate pneumonectomy In more recent years, sleeve lobectomy has evolved as an alternative to pneumonectomy in carefully selected cases of bronchogenic carcinoma, especially for centrally located lesions. Between 1992 and 1995, bronchoplastic procedures for bronchogenic carcinoma were performed in 15 patients and the majority of operative procedures were sleeve lobectomy (W: 12). All procedures were considered as complete and potentially curative. Mean age was 62.3 years (range 46 to 70 years) and there were 12 males and 3 females. Of 15 patiients, 7 underwent right upper sleeve, 2 underwent right lower sleeve, 5 underwent left upper sleeve, and 1 underwent right sleeve pneumonectomy. Postoperative staging was , stage I in 3, stage ll in 8, stage llla in 3 and stage lII in 1. The postoperative complications included anastomosis site obstruction due to granulation tissue in 1, local recurrence in 3, and wound infection in 1 There were 1 operative death due to sepsis and 2 late deaths. The three-year survival rate was 80%. The significant correlation was observed (r=0.11) between the predicted FEVI (1.851 L) and measured FEVI (1 762L). In conclusion, bronchoplastic procedure will have better prognosis than pneumonectomy in selected lung cancer patients because of preserving good function in remnant lung.

Open Mitral Commissurotomy (개방성 승모판교련절개술)

  • From July 1983 to June 1988, twenty six patients underwent mitral valve repair for pure mitral stenosis[21 patient] or for mitral stenosis with a mild degree of regurgitation[5 patient] at Chonbuk National University Hospital. All patients underwent open mitral commissurotomy and 17 patients required to additional procedures for relief of obstructive subvalvular lesion. There was no operative death. The patients were followed from one to sixty months[mean 24. 1Mo.] One late death occurred due to cardiac and renal failure 10 month postoperatively. Of the 25 surviving patients at the time of follow-up, 17 patients[68%] were in NYHA functional class I and 8 patients[32%] were in class II.
