A Clinical Aspect Analysis of the Cervical Necrotizing Fasciitis (경부에 발생한 괴사성 근막염에 대한 임상적 양상 분석)

  • Background and Objectives : Necortizing fasciitis in the cervical region is a very rare disease with high mortality and morbidity rates. The purpose of this study was to analyze the clinical characteristics, treatment results, complication and prognosis of necrotizing fasciitis patients. Materials and Methods : We retrospectively reviewed the inpatient charts treated for cervical necrotizing fasciitis at our Otorhinolaryngology clinic. We diagnosed necrotizing fasciitis by the clinical findings such as swelling, redness and pain of infected area and necrosis of subcutaneous fat layer and fascia observed during surgery. Twenty such patients were identified and treated from January 2011 to December 2016. Results : 20 adults consisting of 14 male and 6 females with cervical necrotizing fasciitis were diagnosed and treated. The most commonly known associated preceding illness were dental abscess and tonsillitis. The most pathogen was Streptococcus species (12/20), followed by Klebsiella pneumonia (6/20), Staphylococcus species (2/20). The mean duration of hospitalization was 32.2 days (range,14-86). The mean Modified Laboroatory Risk Indicator for Necrotizing Fasciitis (M-LRINEC) score is 7.4 and more than 4 points was eighteen. All patients received parenteral antibiotics and surgical drainage after admission. Conclusions : The reason for the high survival rate in the study was the early diagnosis, as well as the early surgical procedure and antibiotics. After the operation, frequent betadine irrigation could improve the patient's condition and recover without severe complication.

Performance Comparison of Machine Learning Algorithms for TAB Digit Recognition (타브 숫자 인식을 위한 기계 학습 알고리즘의 성능 비교)

  • In this paper, the classification performance of learning algorithms is compared for TAB digit recognition. The TAB digits that are segmented from TAB musical notes contain TAB lines and musical symbols. The labeling method and non-linear filter are designed and applied to extract fret digits only. The shift operation of the 4 directions is applied to generate more data. The selected models are Bayesian classifier, support vector machine, prototype based learning, multi-layer perceptron, and convolutional neural network. The result shows that the mean accuracy of the Bayesian classifier is about 85.0% while that of the others reaches more than 99.0%. In addition, the convolutional neural network outperforms the others in terms of generalization and the step of the data preprocessing.

Comparison of particle collection characteristics in a wire-cylindrical wet electrostatic precipitator with and without a water film (와이어-실린더형 습식 전기집진기의 수막 유무에 따른 집진 특성 비교)

  • People's environmental concerns for fine particles in Korea lead to the strong necessity of improving the performance of environmental control systems. Wet electrostatic precipitators (ESPs) are considered as one of the alternatives to overcome the limit of previous dry ESPs, the re-entrainment of collected particles during rapping and back corona problem for high electrical resistivity dusts etc. In this study, a wire-cylindrical ESP with a thin water film has been developed. Particle collection characteristics were compared in the ESP with operations of water film on and off. Particle collection efficiencies at various applied voltages as well as voltage-current curves were almost the same in the ESP with and without a water film. Particle collection performance for PM1.0, PM2.5 and PM10 in the wet ESP with a water film was constantly maintained with operation time even in the high dust loading environment. This results indicate that a uniform water film in our wet ESP was successfully formed with a very thin layer without any dry spot and therefore could continuously clean the collected particles on the inner wall of the ESP without any performance degradation.

Nail bed defect reconstruction using a thenar fascial flap and subsequent nail bed grafting

  • Background Full-thickness nail bed defects with significant exposure of the distal phalanx are typically challenging to reconstruct. We describe a novel method of nail bed defect reconstruction using a thenar fascial flap combined with nail bed grafting. Methods Full-thickness nail bed defects were reconstructed in a 2-stage operation involving the placement of a thenar fascial flap and subsequent nail bed grafting. A proximally-based skin flap was designed on the thenar eminence. The flap was elevated distally to proximally, and the fascial layer covering the thenar muscle was dissected proximally to distally. The skin flap was then closed and the dissected fascial flap was turned over (proximal to distal) and inset onto the defect. The finger was immobilized for 2 weeks, and the flap was dressed with wet and ointment dressings. After 2 weeks, the flap was divided and covered with a split-thickness nail bed graft from the great toe. Subsequent nail growth was evaluated on follow-up. Results Nine patients (9 fingers) treated with the novel procedure were evaluated at follow-up examinations. Complete flap survival was noted in all cases, and all nail bed grafts took successfully. Five outcomes (55.6%) were graded as excellent, three (33.3%) as very good, and one (11.1%) as fair. No donor site morbidities of the thenar area or great toe were observed. Conclusions When used in combination with a nail bed graft, the thenar fascial flap provides an excellent means of nail bed reconstruction.

Evaluation of Heat Waves Predictability of Korean Integrated Model (한국형수치예보모델 KIM의 폭염 예측 성능 검증)

  • The global weather prediction model, Korean Integrated Model (KIM), has been in operation since April 2020 by the Korea Meteorological Administration. This study assessed the performance of heat waves (HWs) in Korea in 2020. Case experiments during 2018-2020 were conducted to support the reliability of assessment, and the factors which affect predictability of the HWs were analyzed. Simulated expansion and retreat of the Tibetan High and North Pacific High during the 2020 HW had a good agreement with the analysis. However, the model showed significant cold biases in the maximum surface temperature. It was found that the temperature bias was highly related to underestimation of downward shortwave radiation at surface, which was linked to cloudiness. KIM tended to overestimate nighttime clouds that delayed the dissipation of cloud in the morning, which affected the shortage of downward solar radiation. The vertical profiles of temperature and moisture showed that cold bias and trapped moisture in the lower atmosphere produce favorable conditions for cloud formation over the Yellow Sea, which affected overestimation of cloud in downwind land. Sensitivity test was performed to reduce model bias, which was done by modulating moisture mixing parameter in the boundary layer scheme. Results indicated that the daytime temperature errors were reduced by increase in surface solar irradiance with enhanced cloud dissipation. This study suggested that not only the synoptic features but also the accuracy of low-level temperature and moisture condition played an important role in predicting the maximum temperature during the HWs in medium-range forecasts.

  • The Army is making efforts to increase combat power by incorporating technologies related to the Fourth Industrial Revolution into the field of defense. In order to utilize these technologies, it is necessary to develop a military tactical communication system that enables transmission and reception of data between command and control system and weapon systems. Therefore, in this paper, we analyze the tactical communication systems of the other countries, derive the limitations of the tactical communication system currently operating in the military. And, a multi-layered integrated operation structure centered on satellites and plans to provide communication on the move to small units are reviewed. Then, we present the necessity of a large-capacity transmission speed by predicting the amount of data that will be generated from weapon systems of the future, and a plan to efficiently manage the network using intelligent network technology.

A Study on Establishment of Essential Performance Evaluation Criteria for C-arm Computed Tomography (C-arm CT의 필수 성능평가 기준 마련을 위한 연구)

  • In order to overcome the image quality limitations of the conventional C-arm, a flat panel detector (FPD) is used to enhance spatial resolution, detective quantum efficiency, frame rate, and dynamic range. Three-dimensional (3D) visualized information can be obtained from C-arm computed tomography (CT) equipped with an FPD, which can reduce patient discomfort and provide various medical information to health care providers by conducting procedures in the interventional procedure room without moving the patient to the CT scan room. Unlike a conventional C-arm device, a C-arm CT requires different basic safety and essential performance evaluation criteria; therefore, in this study, basic safety and essential performance evaluation criteria to protect patients, medical staff, and radiologists were derived based on International Electrotechnical Commission (IEC) standards, the Ministry of Food and Drug Safety (MFDS) standards in Korea, and the rules on the installation and operation of special medical equipment in Korea. As a result of the study, six basic safety evaluation criteria related to electrical and mechanical radiation safety (leakage current, collision protection, emergency stopping device, overheating, recovery management, and ingress of water or particulate matter into medical electrical (ME) equipment and ME systems: footswitches) and 14 essential performance evaluation criteria (accuracy of tube voltage, accuracy of tube current, accuracy of loading time, accuracy of current time product, reproducibility of radiation output, linearity and consistency in radiography, half layer value in X-ray equipment, focal size and collimator, relationship between X-ray field and image reception area, consistency of light irradiation versus X-ray irradiation, performance of the mechanical device, focal spot to skin distance accuracy, image quality evaluation, and technical characteristic of cone-beam computed tomography) were selected for a total of 20 criteria.

Melting and draining tests on glass waste form for the immobilization of Cs, Sr, and rare-earth nuclides using a cold-crucible induction melting system

  • Cold-crucible induction melting (CCIM) technology has been intensively studied as an advanced vitrification process for the immobilization of highly radioactive waste. This technology uses high-frequency induction to melt a glass matrix and waste, while the outer surface of the crucible is water-cooled, resulting in the formation of a frozen glass layer (skull). In this study, for the fabrication of borosilicate glass waste form, CCIM operation test with 60 kg of glass per batch was conducted using surrogate wastes composed of Cs, Sr, and Nd as a representative of highly radioactive nuclides generated during spent nuclear fuel management. A 60 kg-scale glass waste form was successfully fabricated through melting and draining processes using a CCIM system, and its physicochemical properties were analyzed. In particular, to enhance the controllability and reliability of the draining process, an air-cooling drain control method that can control draining through air-cooling near drain holes was developed, and its validity for draining control was verified. The method can offer controllability on various draining processes, such as molten salt or molten metal draining processes, and can be applied to a process requiring high throughput draining.

Study of monolithic 3D integrated-circuit consisting of tunneling field-effect transistors (터널링 전계효과 트랜지스터로 구성된 3차원 적층형 집적회로에 대한 연구)

  • In this paper, the research results on monolithic three-dimensional integrated-circuit (M3DICs) stacked with tunneling field effect transistors (TFETs) are introduced. Unlike metal-oxide-semiconductor field-effect transistors (MOSFETs), TFETs are designed differently from the layout of symmetrical MOSFETs because the source and drain of TFET are asymmetrical. Various monolithic 3D inverter (M3D-INV) structures and layouts are possible due to the asymmetric structure, and among them, a simple inverter structure with the minimum metal layer is proposed. Using the proposed M3D-INV, this M3D logic gates such as NAND and NOR gates by sequentially stacking TFETs are proposed, respectively. The simulation results of voltage transfer characteristics of the proposed M3D logic gates are investigated using mixed-mode simulator of technology computer aided design (TCAD), and the operation of each logic circuit is verified. The cell area for each M3D logic gate is reduced by about 50% compared to one for the two-dimensional planar logic gates.

A multi-layer approach to DN 50 electric valve fault diagnosis using shallow-deep intelligent models

  • Timely fault identification is important for safe and reliable operation of the electric valve system. Many research works have utilized different data-driven approach for fault diagnosis in complex systems. However, they do not consider specific characteristics of critical control components such as electric valves. This work presents an integrated shallow-deep fault diagnostic model, developed based on signals extracted from DN50 electric valve. First, the local optimal issue of particle swarm optimization algorithm is solved by optimizing the weight search capability, the particle speed, and position update strategy. Then, to develop a shallow diagnostic model, the modified particle swarm algorithm is combined with support vector machine to form a hybrid improved particle swarm-support vector machine (IPs-SVM). To decouple the influence of the background noise, the wavelet packet transform method is used to reconstruct the vibration signal. Thereafter, the IPs-SVM is used to classify phase imbalance and damaged valve faults, and the performance was evaluated against other models developed using the conventional SVM and particle swarm optimized SVM. Secondly, three different deep belief network (DBN) models are developed, using different acoustic signal structures: raw signal, wavelet transformed signal and time-series (sequential) signal. The models are developed to estimate internal leakage sizes in the electric valve. The predictive performance of the DBN and the evaluation results of the proposed IPs-SVM are also presented in this paper.
