On Efficient Processing of Multidimensional Temporal Aggregates In Temporal Databases (시간지원 데이타베이스에서 다차원 시간 집계 연산의 효율적인 처리 기법)

  • Temporal databases manage time-evolving data. They provide built-in supports for efficient recording and querying of temporal data. The temporal aggregate in temporal databases is an extension of the conventional aggregate to include time concept on the domain and range of aggregation. This paper focuses on multidimensional temporal aggregation. In a multidimensional temporal aggregate, we use one or more general attributes as well as a time attribute on the range of aggregation, thus it is a useful operation for historical data warehouse, Call Data Records(CDR), etc. In this paper, we propose a structure for multidimensional temporal aggregation, called PTA-tree, and an aggregate processing method based on the PTA-tree. Through analyses and performance experiments, we also compare the PTA-tree with the simple extension of SB-tree that was proposed for temporal aggregation.

Development of Real-time Closed-loop Neurostimulation System for Epileptic Seizure Suppression (뇌전증 경련 억제를 위한 실시간 폐루프 신경 자극 시스템 설계)

  • Epilepsy is a chronic neurological disease which produces repeated seizures. Over 30% of epileptic patients cannot be treated with anti-epileptic drugs, and surgical resection may cause loss of brain functions. Seizure suppression by electrical stimulation is currently being investigated as a new treatment method as clinical evidence has shown that electrical stimulation to brain could suppress seizure activity. In this paper, design of a real-time closed-loop neurostimulation system for epileptic seizure suppression is presented. The system records neural signals, detects seizures and delivers electrical stimulation. The system consists of a 6-channel electrode, front-end amplifiers, a data acquisition board by National Instruments, and a neurostimulator and Generic Osorio-Frei algorithm was applied for seizure detection. The algorithm was verified through simulation using electroencephalogram data, and the operation of whole system was verified through simulation and in- vivo test.

Type Ⅱ SLAP Lesion with the Rotator Cuff Tear (회전근개 파열과 동반한 TypeⅡ SLAP 병변)

  • Purpose: We reviewed the SLAP lesions and associated injuries, also evaluated a hypothesis that the type II posterior SLAP lesion is related with posterior rotator cuff tear and gives rise to the postero-superior instability. Materials and Methods: The patient recording papers, MRI, video and operation sheets were reviewed with the 28 SLAP lesions confirmed by the arthroscopy among 242 cases. Among these SLAP lesions, type II was 22 cases and classified to the anterior, posterior(16 cases), combined subtype(6 cases) based on the main anatomic location. There were 14 cases of the type II accompanying rotator cuff tear. The average follow-up(13 months) results were evaluated with the ASES and Rowe rating score after repair or debridement of the SLAP lesions. Results: In the type II lesions accompanying the rotator cuff tears(14 cases), the posterior(l0 cases) and combined type(4 cases), cuff lesions were all existed posteriorly. Also We could confirm the drive-through sign in the eleven cases, though did not check the disappearance of this sign after repair because of retrospective study. We could followed up the 22 cases, 18 cases(77%) were excellent or good, fair 3 cases(14%) and poor 1 case(4%). Also, type II lesions with the rotator cuff tear(14 cases) were showed better results in the repair(8 cases) than the debridement(6 cases) of the unstable type II with the cuff repair. Conclusion: The type II lesions were frequently associated with the cuff tear in the specific location. We could presume the possibility of postero-superior instability in the SLAP lesion with the cuff injuries. Also, satisfactory results could be experienced when the unstable SLAP lesions with the cuff tear were repaired at the same time.

A Trusted Sharing Model for Patient Records based on Permissioned Blockchain

  • As there has been growing interests in PHR-based personalized health management project, various institutions recently explore safe methods of recording personal medical and health information. In particular, innovative medical solution can be realized when medical researchers and medical service institutes can generally get access to patient data. As EMR data is extremely sensitive, there has been no progress in clinical information exchange. Moreover, patients cannot get access to their own health data and exchange it with researchers or service institutions. It can be operated in terms of technology, yet policy environment are affected by state laws as well as Privacy and Security Policy. Blockchain technology-independent, in transaction, and under test-is introduced in the medical industry in order to settle these problems. In other words, medical organizations can grant preliminary approval on patient information exchange by using the safely encrypted and distributed Blockchain ledger and can be managed independently and completely by individuals. More apparently, medical researchers can gain access to information, thereby contributing to the scientific advance in rare diseases or minor groups in the world. In this paper, we focused on how to manage personal medical information and its protective use and proposes medical treatment exchange system for patients based on a permissioned Blockchain network for the safe PHR operation. Trusted Model for Sharing Medical Data (TMSMD), that is proposed model, is based on exchanging information as patients rely on hospitals as well as among hospitals. And introduce medical treatment exchange system for patients based on a permissioned Blockchain network. This system is a model that encrypts and records patients' medical information by using this permissioned Blockchain and further enhances the security due to its restricted counterfeit. This provides service to share medical information uploaded on the permissioned Blockchain to approved users through role-based access control. In addition, this paper presents methods with smart contracts if medical institutions request patient information complying with domestic laws by using the distributed Blockchain ledger and eventually granting preliminary approval for sharing information. This service will provide an independent information transaction and the Blockchain technology under test will be adopted in the medical industry.

Analysis of Existing Guidelines and Randomized, Controlled, Clinical Trials for Development of [Guideline of Clinical Trial with Herbal Medicinal Product for gastric cancer] (위암 한약제제 임상시험 가이드라인 개발을 위한 한약제제 무작위배정 대조군 임상시험 고찰)

  • Objectives: This study aimed to learn what should be considered in [Guideline of Clinical Trial with Herbal Medicinal Product for Gastric Cancer)] by analyzing the existing guidelines and clinical trials. Methods: The development committee searched guidelines for herbal medicinal product or gastric cancer developed already. Then, clinical trials for gastric cancer using herbal medicine were searched. The searched trials were analyzed in terms of inclusion and exclusion of participants, intervention, comparator, outcomes and trial design. Then, we compared the results of analysis with the regulations and guidelines of Ministry of Food and Drug Safety to suggest the issue that we will have to consider when making the [Guideline of Clinical Trial with Herbal Medicinal Product for Gastric Cancer]. Results: As a result, few guidelines for anti-tumor agent and clinical trial with herbal medicinal product were searched in the national institution homepage. In addition, 10 articles were searched by using the combination following search term; 'stomach neoplasm', 'herbal medicine', 'Medicine, Korean traditional', 'Medicine, Chinese Traditional', 'TCM', 'TKM', 'trial'. Most trials included gastric cancer participants with medical history of operation. The type of intervention was various such as decoction, granules, and fluid of intravenous injection. Comparators were diverse such as placebo, conventional treatment including chemotherapy and nutritional supplement. The most frequently used outcome for efficacy was quality of life. Besides, the symptom score, tumor response, and survival rate were used. Safety was investigated by recording adverse events. Conclusion: We found out some issue by reviewing the existing guidelines and comparing it with clinical trials for gastric cancer and herbal medicinal products. These results will be utilized for developing [Guideline of Clinical Trial with Herbal Medicinal Product for Gastric Cancer].

The Effect of Laser Acupuncture at $HT_7$(Sinmun) for Mental Stress on Short-term Analysis of Heart Rate Variability (신문혈(神門穴) 레이저침 시술이 정신적 스트레스를 가한 성인의 심박변이도에 미치는 영향)

  • Objectives : The purpose of this study is to assess the effect of laser acupuncture at $HT_7$ for reducing mental stress using power spectrum analysis of the heart rate variability. Methods : 36 healthy volunteers(control group: 18, treatment group: 18) participated in this study. After instrumentation and 5-minute rest period, 5-minute metal stress by operation was provided. HRV was recorded before and after the mental stress(1st, 2nd HRV). After 2nd HRV recording, the control group rested for 15 minutes without any treatments. Participants in the treatment group received laser acupuncture to $HT_7$ unilaterally and rested for 15minutes. Points were irradiated for 60seconds, and the intensity was 1.8J(output 20%). And then 3rd HRV was recorded. Results : In simple rest group, LF norm showed a significant change after mental stress. In laser acupuncture group, LF norm, HF norm, LF, HF and LF/HF ratio showed a significant change after mental stress. But there were no significant difference between two groups(ANCOVA test, p>0.05). In laser acupuncture group, LF norm, HF norm, HF and LF/HF ratio showed a significant change after laser acupuncture treatment. And there were significant differences between two groups(p<0.05). Conclusions : This study suggests that laser acupuncture at $HT_7$ can regulate and prevent the alternation of autonomic nervous system due to mental stress.

  • The development of ICT technology has a huge impact on the existing closed CCTV security equipment market. With the importance of video data particularly highlighted in areas such as self-driving cars, unmanned vehicles and smart cities, various technologies using video are emerging. In this paper, we proposed a method to transmit videos and metadata as a part of smart city integration, and to solve the traffic, environment and security problems caused in urban life by utilizing the metadata instead of using CCTV videos for simple recording purposes, and reverse tunneling technique was designed and implemented as a method for accessing CCTV videos for vehicles from remote locations. Integrated management of CCTV videos and metadata for vehicles that have been used only for limited purposes in closed environments will enable efficient operation of integrated centers in real time required by smart cities, such as vehicle status check, road conditions and facility management.

A Study on the Supply obligations allotment rate of New Renewable Energy in Indoor Gymnasiums with the Application of a Daylighting System (집광채광시스템을 적용한 실내체육관의 신재생에너지 공급의무 분담률에 관한 연구)

  • Under the goal of analyzing the compulsory supply share of new renewable energy according to the application of a daylighting system to indoor gymnasiums, this study conducted analysis of energy consumption and operation schedule at three indoor gymnasiums in the nation through a survey. The investigator did an Energy Plus simulation on Building A based on the analysis results and analyzed the supply share of new renewable energy in the saving effects of lighting energy according to the application of a daylighting system. As a result, When 92 prism daylighting system were installed in the upper ceiling of a stadium, they were able to meet the criteria for the minimum illumination for official games(Min : 600㏓) and optimum illumination for general games and recreations, thus saving lighting energy during the daytime(09:00~17:00). The resulting saving effects of lighting energy amounted to 44.4% for official games, 57.6% for general games, and 66.7% for recreations. In addition, the daylighting systems had a compulsory supply share of new renewable energy at 2.04% for official games, 2.75% for general games, and 2.62% for recreations, recording an average compulsory supply share of 2.5%.

  • It is not easy to present a generalized operating model because the operating environment of a sport event is composed of a complex matrix structure. In this study, we propose 'standardization' which provides guidance of concept and process of sport event management to solve this sports event management problem. To purpose complete of this study, the frame was developed by the international standard ISO 9001: 2015 is applied. The standards framework for sports event process management system structure proposed in this study consists of scope of application, citing standard, Terms and definitions, organizational situation, leadership, planning, support, operation, performance evaluation, and improvement. This sport event process management system standard can be applied to any organization that wants to host, prepare, and operate sport events regardless of the form or scale of the sport event. In addition, this standard was focused on process management of life-cycle stages of sporting events, therefor it was possible to manage interrelationships and dependencies between processes and processes.

Reliability of the EOS Imaging System for Assessment of the Spinal and Pelvic Alignment in the Sagittal Plane

  • Background: The sagittal alignment of the spine and pelvis is not only closely related to the overall posture of the body but also to the evaluation and treatment of spine disease. In the last few years, the EOS imaging system, a new low-dose radiation X-ray device, became available for sagittal alignment assessment. However, there has been little research on the reliability of EOS. The purpose of this study was to evaluate the intrarater and interrater reliability of EOS for the sagittal alignment assessment of the spine and pelvis. Methods: Records of 46 patients were selected from the EOS recording system between November 2016 and April 2017. The exclusion criteria were congenital spinal anomaly and deformity, and previous history of spine and pelvis operation. Sagittal parameters of the spine and pelvis were measured by three examiners three times each using both manual and EOS methods. Means comparison t-test, Pearson bivariate correlation analysis, and reliability analysis by intraclass correlation coefficients (ICCs) for intrarater and interrater reliability were performed using R package "irr." Results: We found excellent intrarater and interrater reliability of EOS measurements. For intrarater reliability, the ICC ranged from 0.898 to 0.982. For interrater reliability, the ICC ranged from 0.794 to 0.837. We used a paired t-test to compare the values measured by manual and EOS methods: there was no statistically significant difference between the two methods. Correlation analysis also showed a statistically significant positive correlation. Conclusions: EOS showed excellent reliability for assessment of the sagittal alignment of the spine and pelvis.
