소아 일차성 신증후군의 치료반응과 재발빈도에 관련된 인자 (Predictive Clinical Factors for the Treatment Response and Relapse Rate in Childhood Idiopathic Nephrotic Syndrome)

  • Purpose : This study was aimed to determine the predictive risk factors for the treatment response and relapse rate in children diagnosed with idiopathic nephrotic syndrome. Methods : We analyzed the medical records of children who were diagnosed and treated for childhood idiopathic nephrotic syndrome from November 1991 to May 2005. Variables selected in this study were age at onset, sex, laboratory data, concomitant bacterial infections, days to remission, and interval to first relapse. Results : There were 46 males and 11 females, giving a male:female ratio of 4.2:1. The age($mean{\pm}SD$) of patients was $5.8{\pm}4.1$ years old. Of all patients who were initially given corticosteroids, complete remission(CR) was observed in 54(94.7%). Of the 54 patients who showed CR with initial treatment, 40(70.2%) showed CR within 2 weeks and 14(24.6%) showed CR after 2 weeks. The levels of serum IgG were lower in the latter group who showed CR after 2 weeks(P=0.036). Of the 54 patients who showed CR with initial treatment, 47(82.5%) relapsed. Of these patients, 35.1% were frequent relapsers and 43.9% were infrequent relapsers. There was no significant correlation between the frequency of relapse and the following variables : sex, days to remission, and laboratory data. However, age at onset and interval to first relapse had a negative correlation with the frequency of relapse(Pearson's coefficient=-0.337, -0.433, P<0.012, P<0.01). Conclusion : The age at onset and the interval to first relapse were found to be predictive clinical parameters for the relapse rate, while the levels of serum IgG at initial presentation were a predictive laboratory factor for treatment response in childhood idiopathic nephrotic syndrome.

  • Purpose: We conducted this study in order to determine clinical features and prognostic factors in adults with acute tetrodotoxin (TTX) poisoning caused by ingestion of puffer fish. Methods: In this retrospective study, 107 patients were diagnosed with TTX poisoning. The subjects were divided into two groups according to duration of treatment; Group I, patients were discharged within 48 hours (n=76, 71.0%), Group II patients were discharged after more than 48 hours (n=31, 29.0%). Group II was subsequently divided into two subgroups [IIa (n=12, 11.2%), IIb (n=19, 17.8%)] according to the need for mechanical ventilation support. Results: In multivariable logistic regression analysis, the predictors of the need for treatment over 48 hours were dizziness (odds ratio [OR], 4.72; 95% confidence intervals [CI], 1.59-12.83), time interval between onset of symptom and ingestion (OR, 0.56; 95% CI, 0.16-0.97), $PaCO_2$<35 mmHg (OR, 8.37; 95% CI, 2.37-23.59). In addition, predictors of the need for mechanical ventilation were a time interval between onset of symptoms and ingestion (OR, 0.54; 95% CI, 0.11-0.96) and $PaCO_2$<35 mmHg (OR, 5.65; 95% CI, 1.96-18.66). Conclusion: Overall, dizziness, time interval between onset of symptoms and ingestion, ${\Delta}DBP$ and $PaCO_2$<35 mmHg predict the need for treatment over 48 hours, time interval between onset of symptoms and ingestion and $PaCO_2$<35 mmHg predict the need for mechanical ventilation support after acute TTX poisoning.

  • Prolongation in QT corrected interval (QTc), measured in surface ECG, has been shown in the majority af patients to be marker of bad prognosis in postmyocardial infarction patients (PMIP). Hence it would seem logical that dynamic QTc interval measurement can be a very usefull indicator to stratify prognosis in PMIP. We present a new algorithm for QT as well as for QTP (distance value from Q wave onset to T wave peak) intervals measurement in 24 hour ECG Holter tapes. Validation of the algorithm by hand measurement has been done on first beats of 18 Holter tapes, resulting in a magnitude of deviations between 10 and 15 ms. Application on 24 hour Holter ECG signal has also been done.

만성불면증 환자에게 적용한 단기 인지행동중재의 효과 (Effects of the Abbreviated Cognitive Behavioral Therapy for Chronic Insomniacs)

  • Purpose: This study was conducted to exam the effects of the Abbreviated Cognitive Behavioral Therapy(ACBT) on chronic insomnia. Methods: Study was one-group interrupted time series study that involved 13 adults(mean age=51.7, aged 25-77 years) with chronic primary insomnia who visited sleep disorder clinic of S Hospital from November 2004 to October 2005. The subjects received 2-session individual ACBT with 2 week-interval($1^{st}$: 1.5- 2hrs, $2^{nd}$: 20-30min). To measure the subjective insomnia severity and sleep patterns, 3 times of insomnia severity index and sleep logs were completed(before ACBT, after ACBT, and 3-month after ACBT). The main outcomes were subjective insomnia severity and sleep patterns(sleep onset latency, waking after sleep onset, and total sleep time, sleep efficiency). The data were analyzed with SPSS 10.0 version program by Friedman test, Wilcoxon signed rank test with Bonferroni correction. Results: There were statistically significant decrease in insomnia severity index, sleep onset latency, and waking after sleep onset, and increase total sleep time and sleep efficiency. Conclusion: ACBT was effective in reducing subjective insomnia severity and improving sleep patterns. Sleep improvement was better sustained over time with ACBT.

Lengthening and shortening processes in Korean

  • This study examines the duration of Korean lax and tense stops in the prosodic word-medial position, their interactions with nearby segments, and the phonological implications of these interactions. It first examines the lengthening of consonants at the function of the short lax stop. Experiment 1 shows that the sonorant C1 is significantly longer before a short lax stop C2 than before a long tense stop. Experiment 2 shows that the short lax stop C1 cancels the contrast between the lax and tense obstruent at C2, making them appear as long tense obstruents (Post-Stop Tensing Rule). We suggest that such lengthening phenomena occur in Korean to robustly preserve the contrastive length difference between C and CC. Second, this study examines the vowel shortening, known as Closed-Syllable Vowel Shortening, before a long tense stop or before the consonant sequence. Experiment 3 suggests that it be interpreted as temporal adjustment to make the interval from the onset of a vowel to the onset of the following vowel of near-equal length. Conclusively, we suggest that Korean speech be planned and controlled with two specific intervals. One is the duration of contrastive consonant intervals between vowels, and the other is the duration from the onset of a vowel to the onset of the following vowel.

Effects of 20% Aluminum Chloride in Axillary Hyperhidrosis not Accompanying Osmidrosis

  • Objective: Surgical treatment of focal axillary hyperhidrosis is often unsatisfactory because of compensatory hyperhidrosis. The purpose of this study is to evalute the effect of decreased sweating production using 20% aluminum chloride on axillary hyperhidrosis. Methods: From February to December, 2002, 10 patients (mean age 25.2 male 2, female 8) with clinical diagnosis of axillary hyperhidrosis were treated by 20% aluminum chloride solution. Until the desired degree of symptom relief was obtained, they were educated to apply every day and thereafter, the agent would be applied as often as is necessary. We analyzed patient's satisfaction and application time at onset of desired dryness, application interval to maintain the relief of symptom and side effects. Results: Aluminum chloride solution was effective in treatment of axillary hyperhidrosis showing excellent result in 60% of patients and good in 40%. Application time at onset of desired dryness ranged from 1 to 6 days(mean 3 days). Application interval to maintain the relief of symptom ranged from 5 to 45 days(mean 12 days). There were no significant complications but just mild irritation and miliaria in seven patients. Conclusion: 20% aluminum chloride solution is the simple, safe and less expensive method for initial treatment for axillary hyperhidrosis not accompanying osmidrosis.

Analyzing Clustered and Interval-Censored Data based on the Semiparametric Frailty Model

  • We propose a semi-parametric model to analyze clustered and interval-censored data; in addition, we plugged-in a gamma frailty to the model to measure the association of members within the same cluster. We propose an estimation procedure based on EM algorithm. Simulation results showed that our estimation procedure may result in unbiased estimates. The standard error is smaller than expected and provides conservative results to estimate the coverage rate; however, this trend gradually disappeared as the number of members in the same cluster increased. In addition, our proposed method was illustrated with data taken from diabetic retinopathy studies to evaluate the effectiveness of laser photocoagulation in delaying or preventing the onset of blindness in individuals with diabetic retinopathy.

심근경색증 환자의 증상발현과 치료추구행위에 관한 연구 (Treatment-seeking Behavior among those with Signs and Symptoms of Acute Myocardial Infarction)

  • The main purposes of this study were to determine the time interval between the onset of symptoms of myocardial infarction and treatment-seeking time and to identify the factors related to the interval time. This study used a retrospective design. The sample consisted of 45 patients aged over 30 who were diagnosed with an acute myocardial infarction at two large university affiliated medical center from September 1, 1997 to June 30, 1998. Data was collected by using questionnaries, which included demographic data, permonitory clinical signs and symtoms of myocardial infarction, and a measure of the severity of the signs and symptoms. Also semi-structured interviews and chart reviews were used to obtain information related to treatment-seeking time. The results of this study are summarized as follows ; 1. The most frequent premonitory clinical symptom was chest pain(92.9%), the second, was perspiration(81.0%), and the next were nausea(40.5%) and dyspnea(38.1%). Thirty two patients reported having more than four premonitory signs and symtoms. Patients described the characteristics of chest pain as “somethings very heavy pressing down”(26.2%), “felt like my chest would burst”(24.4%), or “sharp pain”(16.7%), Over 95% of the sample reported having chest pain. 2. Twenty two (52.4%) patients reported to have “very severe” premonitory pain. 3. The mean time interval between the onset of signs and symptoms and the arrival at the medical center was 6.39$\pm$10.80 hours in 42 samples, the mean time from the onset to arrival at a local hospital was 3.27$\pm$5.39 hours and for transfer from a local hospital to the medical center was 4.75$\pm$9.87 hours in patients who had arrived at medical center via local hospital. 4. The severity of premonitory signs and symptoms did not differ significantly according to existence of premonitory signs and symptoms. 5. There was no significant relationship between treatment-seeking time and age, gender, marital status, economic status, occupation, or residence. But education had significant relationship(r=-0.51, p=0.01). Analysis of difference of the time interval according to the premonitory signs and symptoms showed that the time was shorter in patients who experienced nausea or dyspnea(U=115.50, p=0.01, U=132.00, p=0.04), however the severity of premonitory signs and symptoms did not have statistical significance.

Diagnostic Yield of Diffusion-Weighted Brain Magnetic Resonance Imaging in Patients with Transient Global Amnesia: A Systematic Review and Meta-Analysis

  • Objective: To investigate the diagnostic yield of diffusion-weighted imaging (DWI) in patients with transient global amnesia (TGA) and identify significant parameters affecting diagnostic yield. Materials and Methods: A systematic literature search of the MEDLINE and EMBASE databases was conducted to identify studies that assessed the diagnostic yield of DWI in patients with TGA. The pooled diagnostic yield of DWI in patients with TGA was calculated using the DerSimonian-Laird random-effects model. Subgroup analyses were also performed of slice thickness, magnetic field strength, and interval between symptom onset and DWI. Results: Twenty-two original articles (1732 patients) were included. The pooled incidence of right, left, and bilateral hippocampal lesions was 37% (95% confidence interval [CI], 30-44%), 42% (95% CI, 39-46%), and 25% (95% CI, 20-30%) of all lesions, respectively. The pooled diagnostic yield of DWI in patients with TGA was 39% (95% CI, 27-52%). The Higgins I2 statistic showed significant heterogeneity (I2 = 95%). DWI with a slice thickness ≤ 3 mm showed a higher diagnostic yield than DWI with a slice thickness > 3 mm (pooled diagnostic yield: 63% [95% CI, 53-72%] vs. 26% [95% CI, 16-40%], p < 0.01). DWI performed at an interval between 24 and 96 hours after symptom onset showed a higher diagnostic yield (68% [95% CI, 57-78%], p < 0.01) than DWI performed within 24 hours (16% [95% CI, 7-34%]) or later than 96 hours (15% [95% CI, 8-26%]). There was no difference in the diagnostic yield between DWI performed using 3T vs. 1.5T (pooled diagnostic yield, 31% [95% CI, 25-38%] vs. 24% [95% CI, 14-37%], p = 0.31). Conclusion: The pooled diagnostic yield of DWI in TGA patients was 39%. DWI obtained with a slice thickness ≤ 3 mm or an interval between symptom onset and DWI of > 24 to 96 hours could increase the diagnostic yield.

한국어 양순 파열음 발음시 구강내압과 폐쇄기, VOT에 대한 연구 (The Study on Intraoral Pressure, Closure Duration, and VOT During Phonation of Korean Bilabial Stop Consonants)

  • Acoustic analysis study was performed on 20 normal subjects by speaking nonsense syllables composed of Korean bilabial stops(/p, $p^{*}$/, ph/) and their Preceding and/or following vowel /a/(that is, [pa, $p^{*}a$, pha, apa, $ap^{*}a$, apha]) with an ultraminiature pressure sensor in their mouths. Speech materials were phonated twice, once with a moderate voice, another time with a loud voice. The acoustic signal and intraoral pressure were recorded simultaneously on computer. By these procedures, we were to measure the intraoral pressure, closure duration and VOT of Korean bilabial stops, and to compare the values one another according to the intensity of phonation and the position of the target consonants. Intraoral pressure was measured by the peak intraoral pressure value of its wave; closure duration by the time interval between the onset of intraoral pressure build-up and the burst meaning the release of closure; Voice onset time(VOT) by the time interval between the burst and the onset of glottal vibration. Heavily aspirated bilabial stop consonant /ph/ showed the highest intraoral pressure value, unaspirated /p$^{*}$/, the second, slightly aspirated /p/, the lowest. The syllable initial bilabial stops showed higher intraoral pressure than word initial stops, and the value of loudly phonated consonants were higher than moderate consonants. The longest closure duration period was that of /$p^{*}$/ and the shortest, /p/, and the duration was longer in word initial position and in the moderate voice. In VOT, the order of the longest to shortest was /ph/, /p/, /$p^{*}$/, and the value was shorter when the consonant was in intervocalic position and when it was phonated with a loud voice.
