자기효능증진 운동프로그램이 신장이식 환자의 체중, 체지방율, 운동시간 및 심폐기능에 미치는 효과 (The Effects of the Self Efficacy Promotion and Exercise Program on the weight, body fat rate, exercise time and cardiopulmonary function of kidney transplant recipients)

  • The purpose of this study is to develop and evaluate the Self Efficacy Promotion and Exercise Program to prevent of obesity and cardiovascular diseases for the kidney transplant recipients. The subjects were selected randomly among the patients who underwent kidney transplantations at one major transplantation hospital in Seoul, Korea. This study ws carried out between November 23, 1999 and February 15, 2000. The subjects in this study consisted of 16 patients who had kidney transplantations between 1 month and 12 months ago prior to this study. They were all on steroids therapy and did not take any physical exercise regularly. The subjects received the self efficacy promotion and exercise program for 12 weeks which contained self efficacy promotion education and exercise program. The weight, body fat rate, exercise time and cardiopulmonary function were evaluated before the experiment and after the experiment. The results were as follows: 1. After the experiment, the weight was significantly decreased than that of before the experiment(p=.0001). 2. After the experiment, the body fat rate was significantly decreased than that of before the experiment(p=.0010). 3. After the experiment, the exercise time was significantly increased than that of before the experiment(p=.0002). 4. After the experiment, the absolute VO2 was significantly increased than that of before the experiment(p=.0008). 5. After the experiment, the relative VO2 was significantly increased than that of before the experiment(p=.0002). This study showed that the self efficacy promotion and exercise program was effective. So it would be expected that this self efficacy promotion and exercise program could be applied as an effective independent nursing intervention widely in prevention of obesity and cardiovascular diseases for kidney transplant recipients.

신장이식 환자의 근력, 근지구력, 유연성 및 일상생활활동의 변화 (The Change of Muscle Strength, Muscle Endurance, Flexibility and Activities of Daily Living of the Kidney Transplant Recipients)

  • The purpose of this study was to explore the change of muscle strength, muscle endurance, flexibility and activities of daily living to develop a rehabilitational program. The subjects were selected randomly among the patients who underwent kidney transplantations at one major transplantation hospital in Seoul, Korea. This study was carried out between November 23, 1999 and February 15, 2000. The subjects in this study consisted of 16 patients who had kidney transplantations between 1 month and 12 months ago prior to this study. They were all on steroids and did not take any physical exercise regularly. The muscle strength, muscle endurance, flexibility and activities of daily living were evaluated at 1st week and after 12 weeks. The data were analyzed with numbers, percentiles, mean, standard deviation and t-test. The results were as follows: 1. After 12 weeks, the grip strength was significantly decreased than 1st week (p=.0002). 2. After 12 weeks, the back lift strength was significantly decreased than 1st week (p=.0002). 3. After 12 weeks, the muscle endurance was increased than 1st week, but it was not significant(p=.5487). 4. After 12 weeks, the flexibility was significantly decreased than 1st week (p=.0002). 5. After 12 weeks, the activities of daily living was significantly decreased than 1st week (p=.0006). Like the above result, the kidney transplant receipients' muscle strength, flexibility and activities of daily living were reduced. In order to solve this problem, the writer has found that program development to prevent the defects should be extremely required. Since now on the number of patients should expand. After the kidney transplantation, the periods of 4, 8, 12 and 16 weeks are extended. The writer proposes to examine the different phase of change in each periods.

소아 연령에서 발견된 medullary sponge kidney (Medullary Sponge Kidney Detected in the Pediatric Age)

  • Medullary sponge kidney disease (MSK)는 신장 수질 피라미드부위에서 야기되는 희귀질환으로 collecting precalyceal duct의 낭종성 확장(dilatation)과 ectasia을 특징으로 한다. MSK 환자의 발생 빈도에 대해서는 명확히 알려진 바가 없으며, 특히 소아 청소년 연령에서는 매우 드물게 발견된다. 연구자들은 국내 소아신장학회 회원들을 대상으로 MSK 환자의 전수 조사를 실시하였고 현재까지 문헌상으로 보고된 관련 유전자들인 GDNF, ATP6V1B1, ATP6V0A4 유전자에 대한 분석을 실시하였기에 이를 보고하는 바이다.

  • 다낭신장병은 신장실질에 다수의 낭포가 형성되는 것을 특징으로 하는 사람, 개, 고양이에서 흔히 있는 유전성 질환으로서, 사육상태의 꼬마하마(pygmy hippopotamus)에서도 몇 몇 증례가 보고되고 있다. 2013년 1월 15일 체중 198킬로그램, 33년령 암컷 꼬마하마의 부검과정 중에 양쪽 신장에서 다낭신장병이 관찰되었다. 한 쪽 신장은 약간 종대된 반면 다른 쪽 신장의 아랫부분은 옅은 황색의 수양성 액체로 채워진 한 개의 큰 낭포가 있었다. 양측 신장 모두 직경 2 mm에서 20 mm의 다양한 크기의 액체가 함유된 다수의 낭포들이 관찰되었다. 상당한 부분의 신장 피질과 수질부가 낭포들로 대체되어 있었다. 현미경 검사에서 낭포들의 안쪽은 낮은 입방세포에서부터 편평상피세포들로 구성되어 있었다. 육안적인 소견과 조직병리학적인 검사로 다낭신장병으로 진단하였다. 본 증례보고는 한국에서 최초로 꼬마하마에서 다낭신장병이 확인된 것이다.

금(今).고문상서(古文尙書) 오행배장(五行配藏) 논쟁(論爭)에 대한 고찰(考察) (A Discussion on the Controversy of Relation between the Five Changes theory(五行論) and the Five Viscera from "Geummun-Sangseo(今文尙書)" and "Gomun-Sangseo(古文尙書)")

  • The five viscera[五藏], one of the key concepts from Traditional Oriental Medicine in understanding the nature of human body, endows the characteristic of five phases[五行] on the five representative organs, based on the five phases theory[五行論]. Although the theory from "Geummun-Sangseo(今文尙書)" which states the order of 'liver-mok[肝木] heart-hwa[心火] pancreas-to[脾土] lung-geum[肺金] kidney-su[腎水]' is now widely approved by present-day academics concerning arrangement of five changes and the five viscera, there are some other less-known theories of arrangement. "Gomun-Sangseo[古文尙書]" had suggested the order of 'pancreas-mok[脾木] lung-hwa[肺火] heart-to[心土] liver-geum[肝金] kidney-su[腎水]', which was never practiced, and "Dong-uisusebowon(東醫壽世保元)" of Ijema(李濟馬) expressed the theory of 'lung-mok[肺木] pancreas-hwa[脾火] heart-to[心土] liver-geum[肝金] kidney-su[腎水]'.

Cynomolgus Monkey(Macaca fascicularis)에서의 복부 초음파에 관한 연구 (Ultrasonography of Abdominal Organs in Cynomolgus Monkey (Macaca fascicularis))

  • The purpose of this study is to construct fundamental information about the ultrasonographic diagnosis for extrinsic and intrinsic abdominal disease. Normal ultrasonography of liver, gall bladder, spleen, kidney, urinary bladder, stomach, pylorus, duodenum, and heart of 4 cynomolgus monkey(Macaca fascicularis) were determined by use of ultrasonography. One cynomolgus monkey was autopsied at the time of euthanasia which is performed 24 hours after ultrasonography, and above mentioned organs were measured actually. In ultrasonography of cynomolgus monkey, the gall bladder was 17.5 cm long, and 6.6 cm wide. The width of spleen was 8.8 mm. The right kidney was 35.5 mm long, 23.7 mm wide, and 15.2 mm deep. The ultrasonographic measurements of the left kidney in calves was similar. The urinary bladder was 27.7 mm long, and 20.5 mm wide.

신장이식환자의 스트레스, 강인성, 자가간호에 대한 연구 (A Study on Stress, Hardiness, and Self-care in Kidney Transplant Patients)

  • Purpose: This study was performed to identify relationships among stress, hardiness, and the level of self-care in kidney transplant patients and use the results of this study as baseline data for developing self-care promotion programs. Method: Subjects were 41 patients who undergone a kidney transplantation surgery at least one month before and were receiving regular follow-up care via OPD. Four researchers interviewed each subjects by filling out questionnaires. Results: The mean score of the level of stress was 2.32(SD=0.54) on a 4 point scale and the mean score of hardiness was 4.68(SD=0.62) on a 6 point scale. The mean score of self-care among the subjects was 3.83(SD=0.55). There was statistically a significant positive relationship between selfcare and hardiness(r=0.42). Especially, relationships between self-care and commitment (r=.51) and control(r=.36) as the subdimensions of hardiness were significant. The item suggested as most stressful was economic burden and the item suggested as least stressful was sexual activity. Conclusion: Although transplant patients have various stressors, the positive self-care activities of the patients could be increased if the commitment and control were enhanced by proper intervention programs.

Decreased Expression of Aquaporin-2 Water Channels in the Kidney in Rats Treated with Reserpine

  • Whether there exists a sympathetic neural mechanism regulating the expression of aquaporin (AQP) water channels in the kidney was investigated. Male Sprague-Dawley rats were treated with reserpine (1 mg/kg, IP), and the expression of AQP1-4 proteins was determined in the kidney one day thereafter. Following the treatment with reserpine, the systolic blood pressure measured in a conscious state was significantly decreased in the experimental group compared with that in the control $(83{\pm}8\;vs\;124{\pm}6\;mmHg;\;n=6\;each,\;P<0.05)$. The expression of AQP2 proteins was decreased in the cortex, outer medulla, and inner medulla. The decrease of AQP2 proteins was in parallel in the membrane and the cytoplasmic fractions, suggesting a preserved AQP2 targeting. No significant changes were observed in the expression of AQP1, AQP3, or AQP4. Neither basal nor AVP-stimulated formation of cAMP was significantly altered. These results suggest that the sympathetic nervous system has a tonic stimulatory effect specifically on the expression of AQP2 water channels in the kidney.

Level of Heavy Metals in the Onsan Bay in Korea and Involvement of Metal Binding Protein in the Accumulation of Cadmium in Littorina brevicula

  • The heavy metal concentrations in seawater and winkles (Littorina brevicula) collected from the Onsan bay area in southeast of Korea were analyzed. The heavy metal concentrations in the seawater obtained from the most polluted site showed approximately 189. 205. and 110 fold higher cadmium, copper. and zinc concentrations, respectively. than the uncontaminated control site. The contamination levels of these metals in winkles were 11.08 - 2.35, 334.5 - 212.5, and 426.0 - 499.2$\mu\textrm{g}$ per gram dry body weight. respectively. The concentrations of all three metals in both the seawater and winkles decreased gradually with increasing distance from Daejeong stream, suggesting the stream being the major source of heavy metal input into the bay. Among the four body parts of digestive gland and gonad. gill. kidney, and remaining tissue in contaminated winkles, kidney showed the highest accumulation level of cadmium: copper and zinc, however. were more or less distributed among the four body parts. Upon gel filtration chromatography of the cytosol from the kidney of cadmium induced winkles, one cadmium peak corresponded to the elution peak of horse kidney metallothionein.
