Neutrophil Gelatinase-Associated Lipocalin and Kidney Diseases

  • Neutrophil gelatinase-associated lipocalin (NGAL) has emerged as one of the most promising biomarkers of renal epithelial injury. Numerous studies have presented the diagnostic and prognostic utility of urinary and plasma NGAL in patients with acute kidney injury, chronic kidney disease, renal injury after kidney transplantation, and other renal diseases. NGAL is a member of the lipocalin family that is abundantly expressed in neutrophils and monocytes/macrophages and is a mediator of the innate immune response. The biological significance of NGAL to hamper bacterial growth by sequestering iron-binding siderophores has been studied in a knock-out mouse model. Besides neutrophils, NGAL is detectable in most tissues normally encountered by microorganisms, and its expression is upregulated in epithelial cells during inflammation. A growing number of studies have supported the clinical utility of NAGL for detecting invasive bacterial infections. Several investigators including our group have reported that measuring NGAL can be used to help predict and manage urinary tract infections and acute pyelonephritis. This article summarizes the biology and pathophysiology of NGAL and reviews studies on the implications of NGAL in various renal diseases from acute kidney injury to acute pyelonephritis.

A Case of Thromboembolic Complication in Right Common Iliac Artery in a Child with Minimal Change Nephrotic Syndrome (미세변화형 신증후군 환아에서 발생한 우측총장골동맥 혈전증 1례)

  • Thromboemolism is one of the severe complications of nephrotic syndrome. And arterial thromboembolism is rare than venous thromboembolism. Hypercoagulability is the main pathophysiologic factors of thromboembolism in nephrotic syndrome with severe hypoalbuminemia. We experienced one case of arterial thromboembolism which occured in right common iliac artery. It was seen in a 6 year-old male child that presented with generalized edema and rigth ankle joint pain. Emergency embolectomy and anticoagulant therapy (heparin and antithrombin III) was performed. He didn't have to be amputated and recovered to self ambulation. This is an uncommon case that successful recovery was possible by early diagnosis and invasive surgical management with proper anticoagulant therapy.

  • The change of plasma renin activity is one of the most important parameters explaining the pathological physiology of the hypertension. The relation between renal renin activity and plasma renin activity has not well been documented since last decades. In an attempt to clarify the relationship a series of experiments have been done in rats. The following results were observed. 1) Renal renin activity of clamped kidney increased after silver clipping and the increments were maintained until four to five weeks of operation. 2) Renal renin activity of the untouched contralateral kidney was vulnerable to be suppressed just after clamping, and the activity disappered almost below the limitation of the radioimmunoassay sensitivity up to four weeks. 3) Plasma lenin activity was changed by the renal renin activity, but the regression coefficient from the two kidney one clip Goldblatt hypertensive rats was different from the sham-operated, or age-matched control rats. 4) Plasma renin activity of all the groups tested has the exponential curve in terms of renal renin activity. These data suggest that the renal renin activity is important to control the plasma renin activity in certain experimental condition, especially in chronic status.

A Prediction Model Development on Quality of Life in Kidney Transplant Recipients (신장이식 수혜자의 삶의 질 예측모형 구축)

  • Purpose: The purpose of this study was to identify factors influencing quality of life in kidney transplant recipients and to understand the concrete pathway of influence and the power of each variable, so that integrated prediction model to promote the quality of life of kidney transplant recipients could be developed. Methods: The sample was composed of 218 patients in follow-up care after a kidney transplant in one of 4 university hospitals in the Honam area. A structured questionnaire was used and the collected data were analyzed for fitness, using the LISREL program. Results: This model was concise and extensive in predicting the quality of life of kidney transplant recipients. Conclusion: The research verified the factors influencing quality of life for kidney transplant recipients and it verified that direct factors such as perception of health state, compliance, self-efficacy, stress and indirect factors such as self-efficacy and social support can be important factors to predict the quality of life for recipients. Moreover, those variables represent 87% of variance in explaining quality of life in a prediction model so that the variables can be utilized to predict quality of life for kidney transplant recipients.

Psychosocial Adjustment after Kidney Transplantation (신장이식술 후의 사회심리적 적응)

  • The purpose of this phenomenological study was to understand and describe the essence and the structure of lived experience of people with kidney transplantation. Initially, nine individual interviews were conducted to gather data regarding their subjective experiences. And two focus group interviews were utilized to validate or discard the themes that were emerged from the analysis using Colaizzi's method. Among 17 participants, 13 had living related kidney donations, one living unrelated, and the remaining two cadavor donations. About 130 significant statements were extracted and these were clustered into 11 themes. All participants felt anxiety and fear toward the rejection of transplantation and the complication of immunosuppressive drugs. Although they were initially satisfied with their life after kidney transplantation, most of them lost a self-confidence and experienced loneliness, depression, and despair. Most of the participants also felt guilty for not being able to accomplish their appropriate roles in the family, They also had financial difficulties and social restrictions. However, they overcame these psychosocial distress by exercising, working and sharing love with others. They also could overcome it by living a religious life and by working to help others with kidney transplantations. Most of them felt gratitude toward the donor and did not have a psychological rejection toward the kidney transplanted. The results of the study might help nurses who work with people with kidney transplantations in establishing and implementing an effective nursing intervention by understanding their lived experience.

  • Objectives: The basic cause of developmental disability is congenital weakness, which is a disorder of the kidney according to the Oriental medicine definition. I suggest the oriental medicine music therapy, which can improve congenital weakness and recover the kidney dysfunction. Methods: This study focused autism and Asperger syndrome in terms of Oriental medicine, and also considered view points from the Western medicine. Conclusions 1. The kidney monitors vital elements which were produced from the bone marrow. Therefore, the growth and the development of a skeletal structure are related to the strength and weakness of kidney, which is measured in Qi score. 2. In a case of the deficiency of kidney, an essence due to congenital defect, it shows the symptoms of the developmental disability such as sluggish reaction and physical movements, falling of memory, hearing, and eyesight. 3. For the kidney disorder, "Eum music therapy", one of the oriental medicine music therapies, can promote development of the kidney and kidney-Qi score for the children with developmental disability.

  • Clear cell sarcoma of the kidney (CCSK) is a rare malignant tumor of the renal origin in childhood, distinguished from Wilms tumor by its pathologic and clinical features. Bone metastasis is one of the characteristic clinical features. The common site of metastasis of the clear cell Sarcoma of the kidney is axial skeleton including skull, spines, ribs and femur. A cases of clear cell Sarcoma of the kidney presented to us, which solely metastasized to the hand bones without metastasizing to any other tissues including axial skeleton. We report this case with review of literatures.

Severe Diarrhea-induced Acute Kidney Injury and Its Consequence in an Elderly

  • Methods: This study presents a comprehensive case study of an elderly male diagnosed with acute kidney injury (AKI) resulting from severe dehydration, supported by an extended follow-up with laboratory findings. Results: An 83-year-old male patient experienced severe diarrhea overnight, leading to hospitalization due to symptoms of dehydration and hypotension. His laboratory results displayed a typical AKI pattern, including a significant increase in creatinine levels (5.19 mg/dL) and the presence of hyperkalemia and hyponatremia. Following general treatments, including the administration of an herbal drug (Bulhwangeumjeonggi-san), the estimated glomerular filtration rate (eGFR) improved from 10 ml/min (Stage 5) to 34 ml/min (Stage 3) within five days when he was discharged. Although subsequent eGFR tests, conducted one and two months later as an outpatient, revealed an improvement of 42 ml/min, the patient still experienced mild chronic dysfunction as a consequence. Conclusion: This study presents a noteworthy case of acute kidney injury attributed to severe dehydration, emphasizing the importance of medical awareness regarding diarrhea-induced kidney function impairment, especially in the elderly population.

A Study on the Effect of Chitin, Chitosan and Dithiocarbamate Chitosan against Cadmium Accumulation in Liver and Kidney of Catfish (Chitin 및 Chitosan이 생체내 카드뮴 축적에 미치는 영향 -키틴, 키토산 및 Dithiocarbamate Chitosan이 메기 간장과 신장내 카드뮴 축적에 미치는 영향-)

  • This study was performed to investigate the effects of chitin, chitosan and dithiocarbamate chitosan on the cadmium accumulation in liver and kidney of catfish. The experimental groups were divided into four independent groups which were one control group and three experimental groups by cadmium alone treatment or chitin, chitosan and dithiocarbamate chitosan which cadmium. in order to investigate the effects of chitin, chitosan and dithiocarbamate chitosan on the cadmium accumulation in liver and kidney of catfish, the cadmium concentration, the metallothionein level were measured. The results obtained were as follows: 1. The cadmium concentrations in liver and kidney of catfish in cadmium alone treatment group were similar to that of the chitin treatment group, but chitosan and dithiocarbamate chitosan treatment groups were significantly decreased. 2. The metallothionein levels in liver and kidney of catfish alone treatment group were similar to that of the chitin treatment group, but chitosan and dithiocarbamate chitosan treatment groups were significantly increased. In conclusion, this study revealed the effect of chitin, chitosan and dithiocarbamate chitosan against cadmium accumulation in liver and kidney of catfish. It exhibited the highest effect at dithiocarbamate chitosan treatment group.

Interaction between Renin-Angiotensin and Endothelium-Derived Nitric Oxide Systems in Two-Kidney, One Clip Hypertensive Rats

  • The present study was aimed to investigate the role of endothelium-derived nitric oxide (NO) in the control of renin release and to examine if NO is implicated in the development of two-kidney, one clip (2K1C) hypertension. Male Sprague-Dawley rats $(150{\sim}200\; g)$ were constricted at the left renal artery. They were then supplemented with $N^{G}-nitro-L-arginine\;methyl\;ester\;(L-NAME,\; 5mg/100\;mL)$ or with L-arginine hydrochloride (400 mg/100 mL) in the drinking water. The control group was supplied with normal tap water. The sham-clipped rats were operated as in 2K1C rats except for that no clip was made. The kidneys were taken to examine in vitro release of renin at days 7 and 14 following clipping the renal artery. Northern blot analysis was also done to assess the expression of renin gene in the kidney. In sham-clipped rats, L-NAME caused a sustained increase of the blood pressure, whereas L-arginine was without effect. Neither L-NAME nor L-arginine-supplementation significantly affected the development of hypertension in 2K1C rats. Plasma renin concentration (PRC) measured on day 28 did not significantly differ among the L-NAME, L-arginine and control groups either in 2K1C or in sham-clipped rats. Renin contents (RRC) in the clipped kidney were increased, while those in the contralateral kidney were decreased. The release of renin in vitro from cortical slices was also enhanced in the clipped kidney, whereas it was attenuated in the contralateral. Comparing the RRC and in vitro release, the latter was more rapidly decreased than the former in the contralateral kidney. The renin mRNA levels in the contralateral kidney were almost at their nadir at days 7 and 14 in 2K1C rats. It is suggested that NO does not affect the development of 2K1C hypertension in which the renin-angiotensin system has been activated. The data also confirm that RRC and renin gene expression are increased in the clipped kidney and suppressed in the contralateral kidney in 2K1C rats.
