Review on the Jaeumkangwha-tang in Hyungsang Medicine (자음항화탕의 형상의학적 고찰)

  • The following is the conclusion drawn from a review on the clinical cases cured by Jaeumkangwha-tang in perspective of Hyungsang medicine. Jaeumkangwha-tang originated from the modified Jibaiksamul-tang in 'Manbyunghuichun(萬病回春)' and applied to the diseases induced by flaming fire due to deficiency of Yin. In 'Donguibogam', there are two kinds of Jaeumkangwha-tang. One in the chapter of kidney is mainly prescribed to replenish the Jung of kidney and applicable to the cases with configuration and symptom of kidney along with fire. The other one in the chapter of fire is applied to the symptoms of flaming fire due to deficiency of Yin. The characteristics of the patients treated by Jaeumkangwha-tang from the viewpoint of Hyungsang medicine can be classified as follows : ① configuration: Dam type, Shin type, inverted triangular type(:天垂象), bird type, round eyes, thin lips, slender waist, thin leg, pretty face, sparkling eyes, prominent upper lip and upward eyes and nose. ② color : dark red complexion, red cheek bone, red lips and red glabella. ③ pulse : fine and fast pulse and pulsation on the Chuk(尺) in man and so forth. ④ symptoms: agility, talkativeness, pimpled face, crooked back, weakness to the heat of summer, light eating, timidity, blood-shot eyes, night sweat, cough, abundant phlegm, hemoptysis, bloody spittle, enervation, emaciation, back pain, flaccidity of lower limbs, involuntary emission, nocturnal emission, heat sensation in the chest palms and soles, anger, flush on cheek bone, red lips and dry mouth, reddish tongue, stiff excrement, scanty yellow urine, etc.

  • Acute lobar nephronia is a focal of acute bacterial nephritis, affecting one or more of the renal lobules. Nephronia is being diagnosed more frequently in accordance with the advancing imaging techniques of urinary tract. We report a case of acute nephronia in a 4 month old boy who presented persisting urinary tract infection after intial antibiotic treatment. Ultrasonography and computerized tomography showed round wedge-shaped, non-enhancing mass in right kidney. Gallium scan also revealed the lesion in the right kidney. This lesion resolved completely on one month follow up examinations. (J. Korean Soc Pediatr Nephrol 5 : 73- 7, 2001)

Parvovirus B19 Infection in Two Korean Pediatric Kidney Transplant Patients (소아에서 신장 이식 후 발생한 Parvovirus Bl9 감염 2례)

  • Parvovirus B19(PV B19) is a nonenveloped single-stranded DNA virus that causes a wide variety of diseases ranging from benign childhood infection such as slapped-cheek rash(fifth disease) to life threatening diseases such as hydrous fetalis in fetuses or aplastic anemic crises in patients with hemolytic anemia. In immunocompromised hosts including organ transplant recipients, this infection can cause chronic anemia. Recently, the reports of cases of PV B19 infection have been increasing in transplant recipients and most reported cases of PV B19 infection-associated anemia in renal transplant recipients were successfully treated with intravenous immunoglobulin infusion. We experienced two cases of aplastic anemia caused by PV B19 infection in pediatric renal transplant recipients. The patients were an 8 year-old boy and 12-year-old boy who received allograft kidneys from their mothers. Anemia developed 2 weeks after transplantation and their serum was positive for PV B19 PCR. They were treated with 400 mg/kg of intravenous immunoglobulin(IVIG) for 5 consecutive days. In one of the case, anemia was corrected promptly after the first 5-day course of IVIG therapy but in the other, anemia persisted but responded to the second course of IVIG therapy. One year later, the patients have normal hematocrit levels and stable renal function These are the first cases of PV B19 infection treated successfully with IVIG in pediatric renal transplant recipients in Korea. Effects of Unilateral Renal Arterial Infusion of Adenosine and Its Analogues on Renal Function in Two-Kidney One Clip Hypertensive Rabbits (신성 고혈압 가토에서 Adenosine 유사체가 신장기능에 미치는 영향)

  • Recently, it has been suggested that the endogenous adenosine may be the mediator for the intercellular communication in the regulation of tubuloglomerular feedback control and renin release. Even though two subclasses of adenosine receptors, A1 and A2, have been described, their functional roles are controversial. The present study was undertaken to clarify the role of adenosine receptors in hypertensive rabbit caused by clamping of renal artery. Experiments were done in two-kidney one clip Goldblatt hypertensive rabbits (2K1GHR) and sham-operated normotensive rabbits. Adenosine, N6-cyclohexyladenosine (CHA) and 5'-N-ethylcarboxamidoadenosine (NECA) were infused into a renal artery. The decreases in urine volume, renal blood flow, glomerular filtration rate and excreted amounts of electrolytes caused by adenosine and CHA were significantly attenuated in 2K1CHR. However, changes in renal function caused by A2 adenosine receptor agonist, NECA, tend to be accentuated in 2K1CHR. These results suggest that the attenuation of renal effect caused by adenosine and A1 adenosine receptor agonist may be due to the modification of adenosine receptor in the kidney in Goldblatt hypertensive rabbits.

Comparison of Usage Patterns and Outcomes by Dual Type Calcium Channel Blockers in Patients with Chronic Kidney Disease (만성 신장질환 환자에서 dual type calcium channel blocker의 사용 양상 및 결과 비교)

  • Background: Dual-type calcium channel blockers (CCBs), such as efonidipine and cilnidipine, are renoprotective drugs that reportedly reduce proteinuria by dilating afferent and efferent arterioles of the glomerulus. However, studies comparing the effect of dual-type CCB on proteinuria have not been conducted. Therefore, we aimed to compare the effect of dual-type CCB (efonidipine and cilnidipine) usage patterns in hypertensive patients with chronic kidney disease (CKD). Methods: This single-center, retrospective study included 53 patients with CKD who 1) initiated efonidipine or cilnidipine treatment while on a renin-angiotensin system inhibitor and 2) had received efonidipine or cilnidipine for at least one year. We compared usage patterns between the efonidipine and cilnidipine groups during the one-year period and analyzed the following outcomes: urinary protein-to-creatinine ratio, blood pressure, and serum creatinine. Results: The study included 25 patients in the efonidipine group and 28 patients in the cilnidipine group. In both groups, blood pressure and urinary protein-to-creatinine ratios tended to decrease; however, the change during each interval was not significant. Conclusions: In patients with CKD who were on renin-angiotensin system inhibitor therapy, the addition of a dual-type CCB (i.e., efonidipine or cilnidipine) tended to reduce proteinuria; however, the change during each interval was not significant.

Renal manifestations in tuberous sclerosis complex (결절성 경화증 환자에서의 신장 발현)

  • Purpose : The renal manifestations of tuberous sclerosis complex (TSC) are remarkably diverse, including polycystic kidney disease, simple renal cysts, renal cell carcinomas, and angiomyolipomas. All of these occur in children as well as adults in TSC. Angiomyolipomas, which can cause spontaneous life-threatening hemorrhages, are by far the most prevalent and the greatest source of morbidity. Here, we will address our experience, adding to the literature on pediatric patients with TSC requiring evaluation and treatment for renal manifestations. Methods : A retrospective analysis was made on 19 patients in whom TSC was diagnosed between May 2001 and Oct. 2005 at Severance Hospital. All patients had clinical diagnoses of TSC as defined by the 1998 tuberous sclerosis complex consensus conference. Results : The patients consisted of 13 boys and 6 girls with a mean age of 7.3 years (range 1 to 22). The renal disease associated with TSC included angiomyolipoma in nine patients (47.4 percent), renal simple cyst in one (5.3 percent), hydronephrosis in one (5.3 percent) patient. Eight patients (42.1 percent) presented with normal kidney contours at abdominal ultrasonography. One patient underwent renal replacement therapy due to chronic renal insufficiency after nephrectomy. Hemorrhage from angiomyolipoma was not detected. Conclusion : In our review of 19 cases of TSC, renal manifestations are reported in 57.9 percent of patients. Asymptomatic angiomyolipoma associated with TSC grow gradually, although severe hemorrhages are rare. So patients with TSC should be followed up with serial computerized tomography or abdominal ultrasonography. And also, renal function should be monitored conservatively.

A Study on the Sulfametazine residues in Swine Tissues (일부지역 돼지장기 및 근육내 잔류설파메타진에 대한 조사연구)

  • This study was carried out to determine the sulfametazine residues in swine tissues. For this experiment, 22 samples of porks were collected at meat markets in Seoul, 21 samples of swine muscles for export were collected at slaughterhouses in Gyunggi areas, and 18 samples of swine livers, kidneys, and muscles were collected from 6 swine at slaugterhouse in Seoul from the end of August to the early of October and were analyzed by High Performance Liquid Chromatography (HPLC). the results obtained were summarized as follows. 1. The sulfametazine residues were liver > kidney > muscle, and among liver, kidney and muscle were very significantly different(p<0.005). 2. The sulfametazine residue in liver was very significantly higher than that in muscle (p<0.005), the sulfametazine residue in kidney was significantly higher than that in muscle (p<0.005), but, the sulfametazine residue in liver was not significantly higher than that in kidney(p<0.05). 3. the sulfametazine residues in swine muscles for export wer exhibited a tendency to lower in small breeding size farms. 4. the sulfametazine fesidue in one of 22 samples of porks for domestic consumption was exceeded 0.1 ppm.

Effects of Squalene in Mouse Kidney with Contaminated Mercury (흰쥐의 신장에서 수은독성에 대한 스쿠알렌의 효과)

  • The mouse for identifying the histological changes of kidney were also divided into the two groups; treated with only $HgCl_2$ (4 mg/kg), the group treated with $HgCl_2$ and squalene (200 mg/kg). The $HgCl_2$ treated only one time at first day. The squalene treated two times a day (12 hours interval) for every day. Each groups were divided into the five groups; 6, 12, 24, 48 and 72 hours after treated $HgCl_2$ and squalene. Historical changes of the kidneys were investigated by electron microscope. The group with only $HgCl_2$ showed that the nuclear membrane was shrinked, the inner membrane (cristae) of the mitochondria were destructed, and ribosomes on the rough endoplasmic reticulum were lost. The group treated with $HgCl_2$ and Squalene showed that the nuclear membrane was more rounded, the cristae of the mitochondria were almost normal shape, and more ribosomes on the rough endoplasmic reticulum were attached. Therefore , we concluded that squalene has significantly protective effects in kidney to harmful $HgCl_2$.

Heavy Metal Accumulation in Some Korean Rare Bird Species (한국산 희귀조류 수 종에 대한 중금속 축적)

  • This paper reports on Pb and Cd concentrations in the tissues of 76 individuals of 16 rare bird species found dead across Korea. In most of species, Pb concentration was higher in kidney and bone than in liver. The range of the mean liver Pb was 0.47 (Cygnus cygnus)∼3.74 ㎍/wet g (Asio flammeus). Based on hepatic Pb concentrations, 39 (51.3%) of the 76 individuals were classifed as background level (〈2㎍/wet g), 35 (46.1%) were elevated level (〉2㎍/wet g) and 2 (2.6%) were poisoned level (〉10㎍/wet g). These results were indicated many rare birds were suffered from chronic Pb exposure. Cd concentration was generally higher in kidney than in liver and bone. The range of the mean kidney Cd was 0.06 (Bubo bubo)∼l.75㎍/wet g (falco subbuteo). Only one of the 76 individuals had elevated Cd concentration (〉2.7㎍/wet g) in kidney. These results indicated most of rare birds had low Cd accumulations.

Variation in clinical usefulness of biomarkers of acute kidney injury in young children undergoing cardiac surgery

  • Background: Acute kidney injury (AKI) is one of the most significant postoperative complications of pediatric cardiac surgery. Because serum creatinine has limitations as a diagnostic marker of AKI, new biomarkers including neutrophil gelatinase-associated lipocalin (NGAL), kidney injury molecule-1 (KIM-1), and interleukin-18 (IL-18) are being evaluated to overcome these limitations and detect AKI at an early stage after cardiac surgery. Purpose: This study aimed to investigate the clinical usefulness of these biomarkers in young children. Methods: Thirty patients with congenital heart diseases who underwent cardiac surgery using cardiopulmonary bypass (CPB) were selected, and their urine and blood samples were collected at baseline and 6, 24, and 48 hours after surgery. Serum creatinine and blood urea nitrogen levels as well as NGAL, KIM-1, and IL-18 levels in urine samples were measured, and clinical parameters were evaluated. Results: Of the 30 patients, 12 developed AKI within 48 hours after cardiac surgery. In the AKI group, 8 of 12 (66.6%) met AKI criteria after 24 hours, and urine KIM-1/creatinine (Cr) level (with adjustment of urine creatinine) peaked at 24 hours with significant difference from baseline level. Additionally, urine KIM-1/Cr level in the AKI group was significantly higher than in the non-AKI group at 6 hours. However, urine NGAL/Cr and IL-18/Cr levels showed no specific trend with time for 48 hours after cardiac surgery. Conclusion: It is suggested that urine KIM-1/Cr concentration could be considered a good biomarker for early AKI prediction after open cardiac surgery using CPB in young children with congenital heart diseases.
