Case Report of Yukmijihwang-tang(Liuweidihuang-fang) on Diabetes Mellitus in Stroke Patients Diagnosed as Kidney-Yin Deficiency or Non Kidney-Yin Deficiency Syndrome (신음허(腎陰虛)로 변증(辨證)된 중풍환자(中風患者)와 신음허(腎陰虛)로 변증(辨證)되지 않은 중풍환자(中風患者)의 당뇨(糖尿)에 대한 육미지황탕(六味地黄湯)의 치험례(治驗例))

  • Because syndrome-differentiation is one of the major characteristics in oriental medicine, there would be a possibility that the same herbal medicine show different effects on the same disease, if syndrome differentiation is ignored. In this report, we observed different response of Yukmijihwang-tang in diabetic stroke patients according to their syndrome differentiation. One case diagnosed as 'Kidney-yin deficiency syndrome' showed remarkable improvement on diabetes mellitus, whereas the serum glucose levels of the other 4 cases diagnosed as 'non Kidney-yin deficiency syndrome' were lowered little. These results are in accordance with the oriental medical theory that Yukmijihwang-tang can treat 'Kidney-yin deficiency syndrome'. Therefore, we suggest that subsequent clinical trials on oriental medicine would have to include the concept of differentiation of syndromes in order to take out the merit of oriental medicine.

Effects of Herbal Medicine Injinchunggan-tang on Steroid-induced Symptoms in a Patient with Autoimmune Hepatitis-Primary Biliary Cirrhosis Overlap Syndrome: A Case Report (인진청간탕으로 호전된 스테로이드 부작용을 동반한 자가면역성 간염-원발성 담즙성 간경변증 중복증후군 1례)

  • Objectives: The medical records, including laboratory test results, of a patient with autoimmune hepatitis (AIH)-primary biliary cirrhosis (PBC) overlap syndrome were reviewed to observe changes in steroid-induced adverse reactions before and after herbal medicine treatment.Methods: We investigated the records of a 44-year-old female patient with AIH-PBC overlap syndrome treated at the Kyung Hee University Korean Medicine Hospital. Injinchunggan-tang and its modified decoctions were administered, and any changes inclinical manifestations as well as laboratory test results were monitored.Results and Conclusions: Significant decreases in ALP, GGT, ALT, and AST levels were observed after oral administration of the herbal medicines. Improvements in steroid-induced symptoms (hyperglycemia, fatigue, and moon face) were also observed. Injinchunggan-tang and its modified prescriptions are promising candidates for proper management of AIH-PBC overlap syndrome.

Case Report of Diabetic Neuropathy Treated with Palmijihwang-tang, Acupuncture, and Moxibustion (팔미지황탕가미 및 침, 뜸 치료를 이용한 당뇨병성 신경병증 치험 1례)

  • Objective: Diabetic neuropathy is a common complication of diabetes mellitus. Patients with this disease suffer from various peripheral sensory symptoms that are not easily alleviated. We report on a case of a patient with diabetic neuropathy whose symptoms were alleviated after traditional Korean medical treatment.Methods: A patient with hand and foot numbness and coldness diagnosed with diabetic neuropathy was treated with Palmijihwang-tang (Bai Wei Di Huang Wa, 八味地黃湯) three times a day. The progress was evaluated with the numerical rating scale (NRS), neuropathy disability score (NDS), and neuropathy symptom score (NSS).Results: The improvement of symptoms appeared to be faster and more effective than that of conventional treatment. The improvement was observed from the second day of the treatment.Conclusions: This case study shows that Palmijihwang-tang may be an effective treatment for diabetic neuropathy.

  • The pharmacological mechanisms of most Oriental medicines have not been clearly defined in spite of their effective use in treating many diseases throughout the world. Many Oriental medicines have been used against various allergic diseases for generations, and still occupy an important place in traditional medicine in far eastern countries including Korea. It is also still unclear how Oriental drugs prevent allergic disease in vivo or in vitro models. Some Korean folk medicine inhibited the mast cell-mediated allergic reaction. This review summarizes the effective folk medicine in experimental effect on systemic or local anaphylaxis reaction. Potential anti-anaphylactic folk medicines include: Poncirus trifoliata; Siegesbeckia glabrescence; Solanum lyratum; Aquilaria agallocha; Ulmi radicis; Polygonum tinctorium; Hwanglyun-Haedok-Tang; Rehmannia glutinosa; Kum-Hwag-San; Syzygium aromaticm; Spirulina platensis; Sosiho-Tang; Sinomenium acutum; Schizonepta tenuifolia; Shini-San; Magnoliae flos; Sochungryong-Tang; Oryza sativa; Cryptotympana atrata; Salviae radix; Rosa davurica; Asiasari radix; Chung-Dae-San; Cichorium intybus; Perilla frutescens; Vitex rotundifolia; Terminalia chebula; Siberian Ginseng; Solanum melongena; Gahmi-Shini-San; Alpinia oxyphylla; Acanthopanax senticosus root; Prunella vulgaris; Allergina; Ixeris dentate; Acanthopanax senticosus stem; Tongkyutang; Salvia plebeia; Rubus coreanus; Sinpo- Tang; Dodutang; Forsythia fructus; Xanthii fructus; and Purple bamboo slat. Ensuring the effects and understanding the mechanisms of action for these Oriental medicines can permit drug development and laying of the ground-work for evaluating potential synergistic effects by addition and subtraction of prescriptions.

Ikwi-tang for the treatment of allergic rhinitis as a traditional medicine

  • Ikwi-tang (IW) is an oriental medicine that has been used for the treatment of general symptoms due to a stomach yin deficiency. The aim of this study is to investigate precisely the effect of IW on allergic rhinitis (AR). We well show the effects and the mechanism of IW in the ovalbumin-sensitized AR model. IW significantly decreased the number of nasal/ear rubs and the increment of IgE levels in the AR mice. The levels of interferon-${\gamma}$ were enhanced while the levels of interleukin (IL)-4 were reduced in the spleen tissue of the IW-administered AR mice. Expressions of IL-$1{\beta}$ and cyclooxygenase-2 were inhibited by IW-administration in the nasal mucosa tissues. Infiltration of eosinophils and mast cells was decreased in the IW-administered AR mice. Our results indicate that IW may attenuate the development of AR by the inhibition of caspase-1 activity.

Case Study of a Cerebral Infarction Patient with Recurring Aspiration Pneumonia Treated with Korean Medicine, Including Shigyungbanha-tang-gami Combined with Antibiotics (시경반하탕가미를 포함한 한방치료를 항생제 치료와 병용하여 호전된 뇌경색 환자의 반복되는 흡인성 폐렴 치험 1례)

  • Objective: This study addressed the case of a cerebral infarction patient with recurring aspiration pneumonia. Methods: A patient diagnosed with cerebral infarction with recurring aspiration pneumonia was treated with Korean medicine therapies, including herbal medication (Shigyungbanha-tang-gami), acupuncture, electro-acupuncture, and moxibustion combined with antibiotics for 31 days. To evaluate the therapeutic effect, we checked chest X-rays, lab evaluations, the number of night suctions, and body temperature. Results: After treatment, chest X-rays and inflammation markers improved. In addition, the number of night suctions decreased, and the body temperature was normalized. Aspiration pneumonia recurred. However, the intensity of the symptoms gradually weakened, and the recovery speed increased. Conclusion: These results suggest that Korean medicine therapies, including Shigyungbanha-tang-gami combined with antibiotics, have a beneficial effect on recurring aspiration pneumonia, an additional disease related to cerebral infarction.

  • 1. Object: The primary purpose of this case study is to report that a Tae-Yin-In patients with sweaty scrotum and dysuria was treated with 'Chung sim youn ja tang' and then his symptom decreased 2. Subject: A male obese patient with sweaty scrotum and dysuria that visit Kirin oriental hospital to be treated for obesity 3. Method: The patient was to have a very low calory diet and to exercise for an hour daily and to take a Herb-medicine, 'Chung sim youn ja tang'. Change of his subjective symptom, sweaty scrotum and dysuria, was measured by VAS(Visual Analog Scale) 4. Result: A Tae-Yin-In patients with sweaty scrotum and dysuria was treated with 'Chung sim youn ja tang' and Change of his subjective symptom, sweaty scrotum and dysuria, was measured by VAS(Visual Analog Scale) then his symptom decreased. 5. Conclusion: 'Chung sim youn ja tang' may have an effect on Treatment of sweaty scrotum and dysuria. Further study is needed how to conclud 'disease pattern of constitution' accroding with co-symptom.

A Case Study of Parkinson's Disease Patient with Anxiety and Insomnia Treated with Korean-medicine Treatment, including Gyejigayonggolmolye-tang (계지가용골모려탕을 포함한 한방치료로 호전된 파킨슨병 환자의 불안 및 불면 치험 1례)

  • Objectives: The purpose of this study is to report the case of a patient with Parkinson's disease experiencing anxiety and insomnia treated with Korean medicine therapies. Methods: A patient diagnosed with Parkinson's disease with anxiety and insomnia was treated with Korean medicine therapies, including herbal medication (Gyejigayonggolmolye-tang), acupuncture, electro-acupuncture, and moxibustion, for 18 days. To evaluate the therapeutic effect, we checked the Beck Anxiety Inventory (BAI) and Insomnia Severity Index (ISI) scores to monitor anxiety and insomnia. Results: After treatment, the patient's BAI and ISI scores for anxiety and insomnia decreased. Further, the patient and her protector mentioned that their quality of life had improved. Conclusion: These results suggest that Korean medicine therapies with Gyejigayonggolmolye-tang have a beneficial effect on anxiety and insomnia in patients with Parkinson's disease.

The Effects of Chungganhaeju-tang(Qingganjiejiu-tang) on Alcoholic Liver Damages by Applying Proteomics (청간해주탕(淸肝解酒湯)이 알코올 유발 간섬유화와 단백질 발현에 미치는 영향)

  • Objectives : The purpose of this study was to investigate the effects of Chungganhaeju-tang(Qingganjiejiu-tang) on alcoholic liver damaged by applying proteomics. Materials and Methods : Sprague-Dawley rats were used in this experiment the rats were divided into the normal group, the control group(alcohol) and the sample group(CGHJT +alcohol). The ethanol was orally administered twice a day for 6 weeks in the control and sample groups. Water instead of ethanol was orally administered twice a day for 6 weeks in the normal group. CGHJT extract was orally administered once a day for 6 weeks in the sample group. The livers of each group were processed and assessed by histology, Western Blot, $Oxyblot^{TM}$, CBB and 2-dimensional electrophoresis. Results : In the histological findings of the liver, CGHJT inhibited hepatic fibrogenesis induced by alcohol. TIMP-1 decreased in the sample group assessed by western blot and statistical significance was noted by dot blotting(p<0.05). In the $Oxyblot^{TM}$, protein oxidation induced by alcohol treatment decreased with CGHJT. In the 2-dimensional electrophoresis finding, increased proteins alcohol such as HSP 60, 60kDa heat shock protein, 3-mercaptopyruvate sulfurtransferase were normalized by CGHJT. CGHJT was considered to normalize the anti-oxidation activity elevated by alcohol. In the 2-dimensional electrophoresis finding, increased oxidized proteins such as actin, prolyl 4-hydroxylase beta polypeptide, 94kDa glucose regulated protein(GRP94), heat shock protein 90-alpha(HSC86), calreticulin precursor(CRP55), ATP synthase beta chain mitochondrial precursor, caspase-8 precursor, and dihydrolipoamide succinyltransferase(E2) decreased with CGHJT. CGHJT was considered to reduce the oxidative stress of alcohol. Conclusion : Chungganhaeju-tang(Qingganjiejiu-tang) exerts an inhibitory effect against the fibrosis and protein oxidation induced by alcohol treatment of rat liver. CGHJT was considered to normalize the elevated anti-oxidation activity by alcohol and to reduce the level of oxidative stress due to alcohol.

An Experimental Study on the Component Variation of Naesowhajung-tang by the Three Types of Extraction Method and the Effects of Each Type on the Gastrointestinal Tract (추출방법(抽出方法)에 따른 내소화중탕(內消和中湯)의 효능(效能)에 대(對)한 연구(硏究))

  • When I evaluated the component variation of Naesowhajung-tang by the three types of extraction method, and each type's effects on the gastrointestinal tract, I got the following results. 1. The output ratio of extracts made out of Naesowhajung-tang were not significantly different among 13.8% of water extract(Sample-I), 13.5% of 50% ethanol extract(Sample-II), and 15.6% of water extract by spray dryer(Sample-III). 2. magnolol, honokiol, hesperidin, naringin, poncirin and glycyrrhizin Sample II had the largest amount of the following contents: magnolol, honokiol, hesperidin, naringin, poncirin and glycyrrhizin. 3. All the extracts of Naesowhajung-tang showed the inhibitory effect on the smooth muscle contraction of the isolated ileum in mice and fundus-strip in rats induced by acetylcholine chloride and barium chloride. 4. High concentration Sample-II was recognized to be effective in preventing gastric ulcers in Shay's rats. but not in the other rat group. 5. All the extracts of Naesowhajung-tang were recognized to be effective in preventing gastric ulcers induced by Ethanol-HCl in rats. 6. The increase of transport ability in the small intestine was recognized only when the concentration of all the samples was doubled, but not in the other concentrations. 7. The increase of transport ability in the large intestine was recognized only when the concentration of Sample-II was doubled, but not in other concentration. Using the results mentioned above, I suggest that Sample-II has more significant effects on the gastrointestinal tract than the others.
