노년기 연령집단에 따라 사회활동 유형이 행복감에 미치는 영향 (The Effects of Social Activity Types on the Happiness of Korean Older Adults by Age Groups)

  • 본 연구의 목적은 노년기 연령집단별로 사회활동 참여유형이 다른지와 연령집단별로 행복감에 미치는 사회활동 유형은 무엇인지를 조사하는 것이다. 한국고령화패널연구(KLoSA), 5차(2014) 자료를 활용하였으며 5차 시점에서 65세 이상이면서 자녀와 동거하지 않는 노인 3,273명을 대상으로 교차분석, 일원변량분석, 위계적 다중회귀분석(hierarchical multiple regression analysis)을 실시하였다. 연구결과, 경제활동 참여 비율은 연령대가 높아질수록 낮아졌으며, 여가활동은 연소노인이 중고령노인이나 초고령노인집단에 비해 참여수준이 높았다. 가족활동은 연소노인에 비해 중고령노인이나 초고령노인 집단에서 참여수준이 높았고, 그 외의 친목활동과 종교활동은 연령 집단 간 참여수준의 차이를 보이지 않았다. 연령집단별 행복감에 유의미한 영향을 주는 사회활동 유형을 회귀분석 한 결과, 연소노인 집단에서는 경제활동을 제외한 여가활동, 친목활동, 종교활동, 가족활동이 노인의 행복감에 정적으로 유의미한 영향을 미쳤으나, 중고령노인 집단에서는 친목활동과 가족활동이, 초고령집단에서는 가족활동만이 노인의 행복감에 정적으로 유의미한 영향을 미쳤다. 본 연구결과는 연령집단에 따라서 참여하는 사회활동 유형과 그 영향력이 달라질 수 있음을 입증하였다. 본 연구는 다양화되어가는 노인인구의 욕구를 경험적으로 검증하였다는데 그 의의가 있다.

배우자 사별노인의 생활만족도에 영향을 미치는 요인: 연령집단 비교를 중심으로 (Factors Influencing Life Satisfaction in Widowhood among Older Women: A Comparison of Three Age Groups)

  • The purpose of this study was to analyze the factors influencing life satisfaction in widowhood among older females. Using the sixth Korean Retirement and Income Study (KReIS) supplementary survey data, a total sample of 1,471 women aged 65 or older whose spouse had died were selected, divided into 508 young-old (65-74), 745 middle-old (75-84), and 218 oldest-old (85+). Then, factors influencing the life satisfaction of these three groups were analyzed, based on a combination of perceived health and interpersonal satisfaction levels. It was found that among the young-old group, a higher level of subjective health perception indicated by monthly income and material support and interpersonal satisfaction linked to more frequent participation in community gathering combined for an overall higher level of life satisfaction. For the middle-old group, higher life satisfaction was provided by monthly income, informational support, and material support for subjective health perception and no diseases for interpersonal satisfaction. The oldest-old group with higher life satisfaction referred to monthly income for subjective health perception and no experience in donation activities for interpersonal satisfaction. Based on these findings, this study provides implications for policy and practice to improve the life satisfaction of elderly women who have experienced their spouse's death.

Differences in youngest-old, middle-old, and oldest-old patients who visit the emergency department

  • Objective As aging progresses, clinical characteristics of elderly patients in the emergency department (ED) vary by age. We aimed to study differences among elderly patients in the ED by age group. Methods For 2 years, patients aged 65 and older were enrolled in the study and classified into three groups: youngest-old, ages 65 to 74 years; middle-old, 75 to 84 years; and oldest-old, ${\geq}85years$. Participants' sex, reason for ED visit, transfer from another hospital, results of treatment, type of admission, admission department and length of stay were recorded. Results During the study period, a total 64,287 patients visited the ED; 11,236 (17.5%) were aged 65 and older, of whom 14.4% were 85 and older. With increased age, the female ratio (51.5% vs. 54.9% vs. 69.1%, P<0.001), medical causes (79.5% vs. 81.3% vs. 81.7%, P=0.045), and admission rate (35.3% vs. 42.8% vs. 48.5%, P<0.001) increased. Admissions to internal medicine (57.5% vs. 59.3% vs. 64.7%, P<0.001) and orthopedic surgery (8.5% vs. 11.6% vs. 13.8%, P<0.001) also increased. The ratio of admission to intensive care unit showed no statistical significance (P=0.545). Patients over age 85 years had longer stays in the ED (330.9 vs. 378.9 vs. 407.2 minutes, P<0.001), were discharged home less (84.4% vs. 78.9% vs. 71.5%, P<0.001), and died more frequently (6.3% vs. 10.4% vs. 13.0%, P<0.001). Conclusion With increased age, the proportion of female patients and medical causes increased. Rates of admission and death increased with age and older patients had longer ED and hospital stays.

노인복지 수요예측을 위한 노인 인구의 공간적 분석 - GIS를 이용한 농촌인지적 접근 - (Geographical Distributions of the Aged population in Korea - Rural-specific approach with GIS -)

  • The aged population in Korea, especially in rural areas, has been growing rapidly. The welfare for the rural elderly has become a major concern, however we don't have enough information about that population; we don't even have detailed demographics of it. The present research is aimed at; 1) investigating the tendency and changing geographical distributions of the rural elderly, and 2) introducing GIS(Geographic Information Systems) as a useful tool in analyzing geographical distributions of the aged. The General Census Data from 1960 to 2000 was used to carry out this study. The major findings are; 1) There has been a gradual decrease in the rural population over the past 40 years, but this tendency has slowed down more recently; 2) It was observed that the 'oldest-old' group aged 85 years old and over has actuality increased in rural areas faster than any other age group; 3) The changing patterns of the elderly population were different in metropolitan areas and rural areas. In brief, there are far more aged people, especially of the 'oldest-old' group and females in rural areas than those in urban areas. These population, the 'oldest-old' or females, are the most vulnerable and have the greatest need for social welfare and social services of different kinds.

순창군 장수인의 식습관 및 식생활 특성 (Dietary Habits of the Oldest-old Population in Sunchang County)

  • The dietary patterns and characteristics of oldest-old subjects over 85 of Sunchang County (total of 171, 61 males and 110 females) were studied. The average age was $90.6{\pm}3.7$ years old (male; $89.1{\pm}2.8$, female; $91.5{\pm}3.8$), and 90.9% of them were reported to have had no schooling experience. Family types having a spouse were much greater in men (50%) than women (1.8%). The percentages of living alone showed the gender differences, which were comprised of 33% of females and 18.3% of males. In this study, 73.7% of subjects answered "very good" or "good" for their health status. These elderly subjects might maintain their health through a relatively lower rate of smoking (22.2%) and drinking (27.5%), having non-sedentary activities (84.2% of the subjects are physically active) and enough sleep. Their characteristic dietary patterns include high preference of rice (96.5%) and blanch & Seasoned vegetable dishes (Namal, 90.6%) frequent consumption of plant-based food groups containing plenty of anti-oxidants such as vegetables and legumes, and they showed high rates of meal regularity (77.2%) and no skipping of meals (94.2%). It seems to be developed and disseminated in the congregate meal program for improving nutritional status of the elderly, since the coming increment of single-elderly family and single dwellers of rural areas.

저소득 여성노인의 연령별 건강관련 삶의 질에 영향을 미치는 요인 (Factors Influencing the Health-Related Quality of Life by Age among Vulnerable Elderly Women)

  • 본 연구는 저소득 여성노인의 건강관련 삶의 질에 영향을 미치는 요인을 전기노인(65-69세), 중기노인(70-79세), 후기노인(80세 이상)으로 연령별로 구분하여 파악하고자 수행되었다. 자료는 B광역시의 5개 보건소의 맞춤형방문 보건사업에 등록된 기초생활수급자 여성 노인 383명으로부터 2010년 9월부터 11월까지 설문지를 통하여 수집되었으며 다중회귀분석으로 분석하였다. 저소득 여성노인의 건강관련 삶의 질, 생활 만족도, 인지기능, 허약수준은 연령별로 차이가 있었다. 저소득 여성노인의 건강관련 삶의 질에 영향을 미치는 요인은 연령별로 차이가 있었으며 생활 만족도, 인지기능, 허약수준, 주관적 건강수준, 만성질환개수를 포함한 모형은 전기노인의 건강관련 삶의 질을 42.8%, 중기노인의 28.9%, 후기노인의 31.5%를 설명하였다. 허약수준과 생활만족도는 저소득 여성노인의 전체 연령에 있어 건강관련 삶의 질에 영향을 미치는 요인으로 나타났다. 그러므로 저소득 노인여성의 건강관련 삶의 질 개선을 위한 건강증진 프로그램은 연령별 차이를 고려하여 개발되어 적용될 필요가 있다.

한국 남해에 분포하는 갯장어(Muraenesox cinereus)의 연령과 성장 (Age and Growth of Daggertooth Pike Conger Muraenesox cinereus in the South Sea of Korea)

  • The age and growth of Muraenesox cinereus collected in the South Sea of Korea by Danish seine and coastal Long-line fisheries were determined from right sagittal otoliths between May 2018 and April 2019. Otoliths of 353 fish were observed under reflected light, and the edges of the opaque zone were considered to be the otolith ring. Monthly changes in the marginal index indicated that rings (opaque zones) were formed once a year from June to July. The estimated von Bertalanffy growth equation with growth parameters estimated from non-linear regression were Lt=138.37 (1-e-0.18(t+0.79)) for females and Lt=99.00 (1-e-0.13(t+3.99)) for males. The oldest female M. cinereus was 12 years old, and the oldest male was 8 years old.

노인층에서 Clostridium difficile 감염 약물사용평가 및 비호전군에 대한 영향인자 (Drug Use Evaluation of Clostridium difficile Infection in Elderly Patients and Risk Factors of Non-improving Group)

  • Objective: Clostridium difficile Infection (CDI) is one of the common nosocomial infections. As elderly population increases, the proper treatment has been emphasized. We investigated the risk factors associated with CDI unimprovement in elderly patients. Furthermore, we performed drug use evaluation of old CDI patients and oldest-old CDI patients. Methods: It was a retrospective study using electronic medical record at Kangbuk Samsung Medical Center (KBSMC) from January 2016 to December 2017. Seventy three patients aged 65 years or older, diagnosed with CDI by Clostridium difficile Toxin B Gene [Xpert] were screened and they were assessed for risk factors regarding unimprovement status. We also evaluated drug use evaluation in old patients ($65{\leq}age$<80) and oldest-old patients ($80{\leq}age$) by assessing the use of initial therapy, severity, dose, route, treatment course, days of use, total days of use and treatment outcome of initial therapy. Results: Out of 73 patients aged over 65 years, four patients were excluded because they did not receive any treatment. There were 31 improved patients and 38 unimproved patients after initial therapy. We were able to find out patients with surgical comorbidity or endocrine comorbidity (especially, diabetes mellitus) had 2.885 more risk of becoming unimproved than those patients without surgical comorbidity or endocrine comorbidity. Drug use evaluation for CDI was generally fair, but vancomycin as initial therapy is more recommended than metronidazole. Conclusion: Although age, antibiotics exposure, use of antacids are all important risk factors for CDI, our result did not show statistical significance for these risk factors. However, the study is meaningful because the number of elderly population keeps increasing and recently updated guideline suggests the use of vancomycin as drug of choice for CDI.

지역사회 노인의 연령군에 따른 낙상예측요인 (Predictors of Accidental Falls in the Community-dwelling Elderly by Age)

  • Purpose: The purpose of this study is to identify risk factors of accidental falls by age in the Korean elderly. Methods: The study subjects included 10,242 community-dwelling elders aged 65 or over. The data were analyzed by logistic regression analysis. Results: The risk factors of accidental falls in the younger old group (aged 65~74) include fear of falling (OR=16.28, 95% CI: 12.03~22.03), depression (OR=1.24, 95% CI: 1.06~1.44), and hearing impairment (OR=1.20, 95% CI: 1.01~1.43). The risk factors of accidental falls in the older old group (aged 75~84) include fear of falling (OR=11.54, 95% CI: 7.04~18.93), muscle strength (OR=1.36, 95% CI: 1.15~1.60), number of chronic disease (OR=1.65, 95% CI: 1.09~2.50), and presence of spouse (OR=1.27, 95% CI: 1.06~1.51). The risk factors of accidental falls in the oldest old group (aged 85 years old or older) include fear of falling (OR=8.05, 95% CI: 1.85~34.98) and exercise (OR=2.91, 95% CI: 1.36~6.24). Fear of falling is defined as a common factor in the all age groups. Conclusion: Different strategies should be developed for age groups to prevent elderly falls with understanding of contributing factors of falls in each group.

한국 성인 여성에서 연령에 따른 혈중 염증 표지자와 심혈관계 질환 위험 요인에 대한 연구 (Age-related Circulating Inflammatory Markers and Cardiovascular Disease Risk Factors in Korean Women)

  • The purpose of this study was to investigate the age-related changes of cardiovascular disease risk factors and inflammatory markers in non-obese Korean women. Subjects were 112 women over 20 years old with body mass index (BMI) less than $30 kg/m^2$ and were divided into 3 groups (< 40 years, $40{\sim}59$ years, ${\ge}60$ years). Mean weight and BMI in the oldest group were significantly higher than those in the other 2 younger groups (p < 0.05). Mean total cholesterol, triglyceride, LDL-cholesterol and apolipoprotein B/apolipoprotein A1 ratio (BAR) in the oldest group were significantly higher than those in the youngest group (p < 0.05), and mean HDL-cholesterol of the oldest group was significantly lower than that of the youngest group (p < 0.05). The older-aged group showed significantly higher mean values of atherogenic index (AI) and LDL/HDL ratio (p < 0.05) than the respective younger-aged group, and AI was significantly correlated with age, nitric oxide and thiobarbituric acid reactive substances (p < 0.01). In addition, mean vascular cell adhesion molecule-l (VCAM-1) tended to be higher in the older-aged group than the younger group. Tumor necrosis factor-${\alpha}$, a proinflammatory maker, was significantly positively correlated with serum homocysteine, a cardiovascular disease risk factor (p < 0.01). In addition, a significantly positive correlation was observed between C-reactive protein and BAR (p < 0.01). Overall results suggested that the aging might affect the increase of cardiovascular disease risk factors including the serum lipid profiles, weight and BMI, and age-related increases of weight and BMI might play a role in changes in certain biomarkers of inflammation.
