The purpose of this study is to analyze heating and cooling load variation due to envelope retrofits in an old school building. In a previous study, envelope retrofit of an old school building resulted in annual energy consumption reduction. However, cooling energy consumption increased with the envelope retrofit. This is because of high internal heat generation rates in school buildings and internal heat cannot escape through windows or walls when the envelope's thermal performance improves. To clarify this assumption, thermal performance changes due to envelope retrofits were analyzed by simulation. Results revealed indoor temperature and inner window surface temperature increased with high insulation level of windows. Indoor heat loss through windows by conduction, convection and radiation decreased and resulted in an increase of cooling load in an old school building. From results of this study, energy saving impact of envelope retrofits in an old school building may not be significant because of high internal heat gain level in school buildings. In case of replacing windows in school buildings, local climate and internal heat gain level should be considered.

Keeping the structure condition of a closed school maximum, the welfare center for the old who stay at home by remodeling a closed school ultimately proposed.
Background: We performed this meta-analysis to assess the effectiveness and safety of axillary dissection in old women. Methods: The Cochrane Library, PubMed, EMBASE and Chinese Biomedical Literature Database were searched and all randomized controlled trials of axillary dissection in old women (at least 60 years old) were considered. Meta-analyses were completed using RevMan5.1. Results: Three eligible randomized controlled trials (RCTs) including 5,337 patients were considered. There was weak evidence in favour of axillary dissection (AD) in old women. The meta-analysis showed that the overall survival (OS) after 1, 3, 5 and 7 years and the disease free survival (DFS) after 1, 3 and 5 year were not statistically significantly different between AD and no AD groups. However, there was a difference in the 7 year DFS. Conclusions: Axillary dissection did not provide survival benefit to the old women with breast cancer analysed. Therefore, axillary dissection is not well-indicated in old women with breast cancer.

The purpose of this study was to investigate the influences of household income, education needs, private education expenses and expenditures of preparation for old age for mothers of school-aged children and to discover the causal relationships among these variables. The subjects in this study were gathered from among 393 mothers living in Daegu with school-aged children from 8 to 19 years old and with wage-earner husbands. The research tool was a questionnaires which addressed the general characteristics of the subjects, household income, the scale of education needs, the scale of private education expenditures, the scale of perceived burden of private education, and the scale of expenditures of preparation for old age. In order to analyze the study data, factor analysis, and regression analysis using SPSS were used, and structural path analysis using AMOS was used. The main results of this study were as follow: First, the greater the household income, the greater the education needs. Second, household income and education needs had no significant influences on private education expenditures. Third, education needs and private education expenditures turned out to have significant effects on the perceived burden of private education expenditures. Fourth, expenditures of preparation for old age among mothers of school-aged children was influenced by the education needs, private education expenditures and perceived burden of private education expenditures. The household income of mothers of school-aged children had no significant influence on private education expenditures. Finally, education needs, private education expenditures and perceived burden of private education expenditures for mothers of school-aged children had a direct effect on the expenditures of preparations for old age. On the other hand, in mediating education needs, household income of these mothers had an indirect effect on expenditures of preparations for old age. The perceived burden of private education expenditures turned out to have the biggest total effect on the preparations for old age of mothers with school-aged children.

Purpose: The purpose of this study was to compare the clinical and radiographic features of temporomandibular joint (TMJ) osteoarthritis (OA) between adolescents and middle-old aged patients. Methods: The subjects were chosen among the patients who presented to the Department of Oral Medicine of Pusan National University Hospital and were diagnosed with TMJ OA by clinical exam, X-ray and cone-beam computed tomography (CBCT) from 2010 to 2011. We investigated 93 adolescent patients (12-19 years) and 53 middle-old aged (>45 years) patients who observed the erosive bony changes in TMJ. CBCT scans were retaken at intervals at an average of 8 months. Results: The adolescent patients showed unilateral degenerative changes more often, and the middle-old aged patients showed degenerative changes more frequently on both sides. The transition of bone changes to the improved group occurred most commonly in both the adolescent and middle-old aged patients. The adolescent patients were more likely to improve than middle-old aged patients. In the adolescent patients, loss of erosion and subjective symptoms occurred in shorter periods than in the middle-old aged patients. In the adolescent patients, the transition of erosion was distributed into proliferative, normal, and shortening in order. In the middle-old aged patients, the transition of erosion was distributed into shortening, proliferative, and normal in order. Conclusions: The clinical and radiographic features of TMJ OA are a significantly different between the adolescent and middle-old aged patients. Moreover, the difference by age of the adaptive and regenerative capacity of TMJ affects the prognosis of TMJ OA and adolescent patients have a better prognosis after treatment.

This study was to evaluate the performance status and associated factors with the health promoting activities. Two hundred three old aged over 65 years old sampled by quota sampling among 660 old aged in a rural area, Dae-Ma Myun, Young-Kwang Gun, Chollanam Do. The questionnaire survey was performed from June 1st to August 31st in 1997. The major findings were as follows ; 1. The performance rate of health promoting activities were higher in female, dwellers at community health practitioner post area, and old aged believes in a religion and high educated old aged. 2. A multiple regression analysis showed that physical health promoting activity was related to psychiatric wellbeing status, the disability of daily living, residental area and family numbers(P<0.05). 3. The disability of daily living and psychiatric wellbeing status were related to mental health promoting activities by a multiple regression analysis(P<0.05). In conclusion, psychiatric wellbeing status, the disability of daily living, the disability of daily living and residental area were associated the performance of health promoting activities for old aged in a rural area.

Enzyme replacement of therapy (ERT) is one of the most promising therapeutic strategies for the treatment of lysosomal storage disorders. ERT is available in three types of Mucopolysaccharidosis (MPS): for MPS I (Aludrazyme$^{(R)}$), MPS II (Elaprase$^{(R)}$) and MPS VI (Naglazyme$^{(R)}$) patients who are over 5 years old. But recently, early diagnosis can be done by expert clinicians and even in prenatal case. We describe the case of ERT under 5 years old MPS patients. Up to June, 2010 in Samsung Medical Center, there are 6patients who were diagnosed as MPS and started ERT under 5 years old. 3 patients were MPS I, 3 patients were MPS II. 2 patient who was diagnosed as MPS I was female and others were male. Their age at diagnosis were 4 to 37month-old (4, 13, 16, 25, 27, 37 month-old) and they are now 9 to 60 month-old (9, 39, 32, 81, 60 month-old). The youngest patient was started ERT at 4 month-old and others were started at their 13 to 49 month-old (13, 29, 27, 28, 49 month-old). First manifested symptoms of patients were macrocephaly, kyphosis and coarse face appearance. Especially, in 2 of them, one was MPS I and the other was MPS II had elder brother with same disease. And the youngest one was diagnosed by the iduronate-2-sulfatase (IDS) gene analysis from chorionic villi sampling. His mother knew that she was a heterozygous carrier of IDS gene mutation because her younger brother died from MPS II. All of them confirmed as MPS by the enzyme assay in leukocytes and fibroblast skin culture. We started ERT with ${\alpha}$-L-iduronidase(Aldurazyme$^{(R)}$) to MPS I and did recombinant human iduronate-2-sulfatase (Elaprase$^{(R)}$) to MPS II patients as recommended dose as over 5 years old. But for MPS II patient who was 4 month old, we started ERT by recombinant human IDS (Elaprase$^{(R)}$) with reduced dose 0.1 mg/kg and increased dose every 2 weeks by 0.1mg/kg up to 0.5mg/kg IV infusion. During ERT, all patients had no adverse effects and the excretion of GAGs were decreased. We have evaluated other clinical symptoms such as liver/ spleen volume, heart function and neurologic evaluation. We describe a successful ERT to MPS I and MPS II patient under 5 years old without any adverse event. It indicates that ERT in young children are well tolerated and that it has several effects which may confer clinical benefits with long-term therapy.

Due to the frequent occurrence of large-scale disasters such as recent earthquakes, the problem of the safety of old school buildings has emerged. The need to secure safety management technology through constant monitoring is increasing in an attempt to supplement old school buildings with weak disaster response capabilities. Traditional research is approaching the development of an existing sensor-based risk precursor information monitoring system. However, unlike this, in this study, we will focus on the development of a data analysis platform as part of the development of a continuous monitoring system that can be prepared for earthquakes, collapses, and fires, based on constantly measured data. For this reason, the development of a safety diagnostic algorithm based on the optimal sensor-attached points and sensor data reflecting the fragile characteristics of old school buildings was derived. Utilizing this, a message and action manual system for each management / use entity of school buildings after retirement was constructed.

Purpose: This study investigated the epidemiology of Kawasaki disease (KD) in infants ${\leq}3$-month-old. Methods: To study the epidemiology of KD in Korea, data for 27,851 KD patients were collected on a 3-year basis between 2000 and 2008 in a retrospective survey. From this, data for 609 KD patients ${\leq}3$-month-old were analyzed and compared with the data for KD patients >3-month-old. Results: The 609 KD patients ${\leq}3$-month-old (385 males and 224 females) constituted 2.2% of the KD patients. They included 25 infants <1-month-old, 198 infants ${\leq}1$- to 2-month-old, and 386 infants >2- and 3-months-old. The ratio of males to females was 1.72:1. The incidence of coronary artery (CA) dilatation (19.9% vs. 18.7%) and CA aneurysms (3.4% vs. 2.6%) detected by echocardiography did not differ significantly between patients with KD younger and older than 3-month-old. Conclusion: Compared with the data for the KD patients >3-month-old, the data for the 609 patients ${\leq}3$-month-old did not show a significantly higher incidence of CA dilatation or CA aneurysms.
The purpose of this study was to find out the effects of children's elementary school entrance age and sex on cognitive and social abilities. The subjects were 80 the same number was slected by elementary school entrance age(5-year and 6-year) and sex. Their cognitive and social abilities were measured by Intelligence Maturity Test and Revised Vineland Adaptive Behavior Scales. The data were statistically analyzed including descriptive statistics and multivariate analysis of variance. The results were as follows: 1) 6-year-old children had higher perceptive ability of sameness and calculation ability than 5-year-old children. Girls also had higher IQ and language concept than boys, 2) 6-year-old children's communication socialization and total social ability were higher than those of 5-year old children Girls' communication and total social ability were higher than those of boys. The implication was that it would be important to consider children's social ability if their entrance age were decided. In addition Early Entrance to Elementary School system should be reconsidered in the point of childer's developmental adaptation.
