유아의 무증상 폐결핵 (Asymptomatic Pulmonary Tuberculosis in Infant)

  • 영아에서의 결핵은 소아나 성인의 결핵에 비해 더 심한 증상과 합병증을 초래할 수 있다. 하지만 증상이 전형적이지 않거나 균 검출이 잘 되지 않는 경우가 많아 진단에 어려움이 있다. 본 증례는 폐결핵으로 진단된 3개월 환아에 대한 보고이다. 환아는 광범위한 양측 폐의 병변에도 불구하고 발열이나 호흡곤란, 기침 등의 증상을 보이지 않았으며 항결핵제 투약 6개월째 폐병변이 호전되었다. 일부 영아 결핵에서 증상이 나타나지 않는 것은 면역반응의 미성숙으로 부분적으로 설명될 수 있을 것이다. 따라서 영아에서 결핵 접촉자 검진의 중요성이 강조된다.

  • Extranodal marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue type (extranodal MZL) is a distinct subgroup of non-Hodgkin's lymphoma. Pulmonary extranodal MZL is a rare entity and accounts for less than 0.5% of primary pulmonary malignancies. Only a few cases of simultaneous occurrence of lung cancer and pulmonary extranodal MZL have been reported. A 60-year-old woman was referred to our hospital with a pulmonary nodule. She was diagnosed with lung adenocarcinoma by percutaneous needle biopsy. The protrusions into the left main bronchus were found by accident while performing bronchoscopy during lung cancer evaluation. The bronchial lesions were diagnosed as extranodal MZL. Although the patient underwent surgical resection for the lung adenocarcinoma, the pulmonary extranodal MZL was left untreated; it was monitored during follow-up visits. To our knowledge, this is the first report of synchronous lung adenocarcinoma and primary extranodal MZL of the main bronchus.

기흉을 동반한 폐 Aspergillosis 1예 (Pulmonary Aspergillosis with Staged Bilateral Pneumothorax - one case -)

  • We managed a 30 years old female pulmonary aspergillosis patient concomitant with staged pneumothorax bilaterally whose lung function decreased severely. Operative indication was fetal hemoptysis. She discharged at four weeks after left upper lobectomy in good general condition. And we .concluded that early resection should be considered in patients with pulmonary aspergilloma and coexistent pulmonary tuberculosis because those are at greater risk of fetal hemorrhage, particularly once severe hemoptysis has occurred although poor lung function in our country.

각혈을 동반한 폐결핵환자의 임상적 고찰 (Clinical Study of Hemoptysis in Patients with Pulmonary Tuberculosis)

  • We have reviewed 73 patients who experienced hemoptysis with pulmonary tuberculosis to analyze the factors contributing hemoptysis and its management The result are; 1. The age group between 30 * 50 years old was major prevalent group [59%]. 2. Sexual ratio was 3.9 to 1, as 58 men and 15 women. 3. In 58 patients [79.5 %], the number of hemoptysis in frequency was less than 5 times. 4. The quantity of hemoptysis was ranged from less than 50 ml to more than 600 ml and 20 patients [27.4%] experienced bleeding more than 600 ml at once. 5. There was no seasonal difference in hemoptysis. 6. The common prodromal symptoms of hemoptysis were coughing, fatigability, hot foreign sensation of throat in order of frequency. 7. In chest roentgenographic finding, the cavitary lesion was the most common finding, 67% [51 cases], and the multiple cavity was 19.2%[14 cases]. 8. 23 patients [31.37%] were operated for control of hemoptysis and pulmonary tuberculosis itself, 7[30.43 %] of them suffered from complication of empyema, respiratory failure, empyema with bronchopulmonary fistula. 9. One patient [4%] was died due to respiratory failure after left thoracoplasty.

  • Ankylosing spondylitis is a chronic inflammatory multisystem disease that primarily affects the axial joints. Pleuropulmonary involvement is an uncommon extra-articular manifestation of ankylosing spondylitis. There is a wide spectrum of pulmonary parenchymal changes in ankylosing spondylitis, beginning in the early stages of the disease and increasing over time. The lesions are usually asymptomatic, and not visible on chest radiographs in early stages. We reported a case of advanced ankylosing spondylitis in a 56-year-old man with progressive pulmonary bullous fibrocystic changes on both upper lobes that were misdiagnosed as tuberculosis in the early stages of the disease.

  • Tuberculosis (TB)-infected giant bullae are rare. A 55-year-old man was referred when an infected bulla did not respond to empirical treatment. Computed tomography showed a giant bulla in the right upper lobe with an air-fluid level and surrounding infiltrate. Sputum culture, acid-fast bacilli (AFB) stain, and polymerase chain reaction (PCR) for TB were negative. Percutaneous drainage of the bullous fluid was performed. AFB stain and PCR were positive in the drained fluid. The patient was given anti-TB drugs and later underwent obliteration of the pulmonary cavity using talc. To summarize, we report a patient with a TB-infected giant bulla that was treated successfully with anti-TB drugs and obliteration of the pulmonary cavity using talc.

  • 저자들은 객담, 기침, 발열과 호흡곤란을 보이고 단순 흉부촬영상 다발성 공동과 경변을 보이며 흉부 전산화 촬영과 폐동맥 혈관조영술상 폐 동맥류가 확인된 환자에서 폐 절제술후 만성 괴사성 침입성 폐 국균증으로 진단된 1예를 경험하였기에 이에 보고하는 바이다.

  • 성인에서 양성 원인에 의한 기관지식도루는 드물며 활동성 결핵이 원인 중 하나로 보고 되었지만 본 증례와 같이 결핵 후유증으로 장기간에 걸쳐서 발생할 수도 있고 만성 흡인과 기침 및 객혈의 원인이 될 수 있으며 활동성 폐결핵으로 오인될 수 있으므로 임상의사들의 주의가 필요하다.

Pneumocystis jiroveci Pneumonia Mimicking Miliary Tuberculosis in a Kidney Transplanted Patient

  • Bilateral interstitial infiltration in chest radiography, which may be fine granular, reticular or of ground glass opacity, is the typical radiographic findings of Pneumocystis jiroveci pneumonia. Recently, atypical radiographic features, including cystic lung disease, spontaneous pneumothorax or nodular opacity, have been reported intermittently in patients with P. jiroveci pneumonia. We report the case of a 29-year-old woman with a transplanted kidney whose simple chest radiography and HRCT scan showed numerous miliary nodules in both lungs, mimicking miliary tuberculosis (TB). Under the presumptive diagnosis of miliary TB, empirical anti-TB medication was started. However, Grocott methenamine silver nitrate staining of a transbronchial lung biopsy tissue revealed P. jiroveci infection without evidence of TB. These findings suggest that even in TB-endemic area other etiology such as P. jiroveci as well as M. tuberculosis should be considered as an etiology of miliary lung nodules in mmunocompromised patients.
