Combined application of roll flap and combination onlay-interpositional graft to enhance esthetics of maxillary anterior fixed partial denture: A case report

  • In the maxillary anterior region, reconstruction of the localized alveolar ridge defect is very important in enhancing the esthetics of fixed partial denture. A 40-year-old female patient presented with a chief complaint of the inconvenience and unesthetic problem of 3-unit maxillary anterior prosthesis due to alveolar ridge resorption. After removal of old prosthesis, intraoral examination revealed moderate (buccolingually 4 mm) ridge deficiency in missing tooth region, leading to the diagnosis of Class I alveolar ridge defect. One of the reconstruction techniques to overcome this problem might be a technique that combines two types of soft tissue augmentation techniques. The purpose of this paper was to demonstrate the new combined technique of roll flap and combination onlay-interpositional graft utilized to acquire sufficient dimension of recipient area by one time of operation and to present the esthetic improvement of fixed partial denture by using this procedure in case of maxillary anterior localized ridge defect.

  • In edentulous patients, the advantages of prosthodontic therapy using oral implants include both stability and comfort. Advantages suggested for this approach include the prevention of mandibular bone resorption and increased masticatory function. Implant hybrid prostheses place 4 to 6 implants between the mental foramens, and cantilevers are used to replace the posterior teeth at both ends. The 85 - year - old female patient visited our clinic with complete edentulism in the maxilla and mandible. This article reports a satisfactory clinical and esthetic outcome of full mouth rehabilitation using removable complete denture in the maxilla and implant hybrid prosthesis in the mandible.

  • Most patients having annuloaortic ectasia are associated with marked dilatation of the sinuses of Valsalva and the aortic annulus as well as the huge aneurysm of the ascending aorta. A 19 year old male patient complaining of tightness on left posterior chest wall underwent cardiac angiography in which demonstrated annuloaortic ectasia with ascending aortic aneurysm and aortic insufficiency. The patient had corrective operation replacing the ascending aorta and aortic valve with a composite graft[Dacron prosthesis containing a Bjork-Shiley aortic valve] within the aneurysmal sac. The coronary orifices were anastomosed to the tubular Dacron prosthesis [30 mm in diameter] by means of a second smaller Gore-Tex tube [8mm in diameter]. The aneurysmal sac was trimmed by removing the redundant wall and then wrapped outer wall of the Dacron prosthesis. Postoperatively, mediastinal bleeding was temporarily observed in the operative day and satisfactory blood pressure was maintained with small dose of dopamine. One week later, large amount of serous effusion was drained out of the retrosternal space making partial disruption of the skin which was healed well by daily local dressing. The patient discharged in good condition on postoperative 29th day with no residual complications and is doing very well on the 4 months follow-up.

Full mouth rehabilitation of an asthma patient with severe tooth wear and occlusion disharmony (심한 마모 및 교합 부조화를 가진 천식 환자의 전악 수복 증례)

  • Objectives: Patients with excessive tooth wear should first be diagnosed for the etiology of the tooth wear. Causes of tooth wear include bruxism, clenching, and taking medications for systemic diseases. After identifying the cause of tooth attrition, the final prosthesis should be restored with an appropriate vertical dimension. Methods: A 79-year-old man with worn out teeth desired a whole dental treatment. He was on medications for high blood pressure and asthma. The treatment proceeded with a consultation with a medical doctor. The medications for asthma evoked multiple teeth wear and a loss of the vertical dimension. After recovery of 3 mm of vertical dimension, 2 months of evaluation was followed by an interim prosthesis. Results: The increased vertical dimension caused no problem in function and esthetics, and the final restoration was performed with a full monolithic zirconia crown. Group function, adequate anterior guidance, and the occlusal plane were determined. Conclusions: After the final restoration, the patient was both esthetically and functionally satisfied, and a night guard splint was delivered to prevent prosthesis fracture. The patient was informed about the potential tooth wear associated with asthma drugs and educated to visit the clinic regularly.

  • A 58-year old woman with no molar teeth on her upper left side wanted these missing teeth to be replaced with a fixed implant prosthesis. Incision, from distal to 2nd premolar to hamulus, was performed and impression of canine fossa anteriorly, zygoma buccally, pterygoid hamulus posteriorly and palatal groove lingually was taken by Optosil (Bayer Dent.). The implant frame was made of Vitallium, and was inserted the next day after the impression had been taken. 12-days later, the 4-units fixed prosthesis was constructed. The patient was extremely comfortable, had no pain, and was able to chew food more efficiently.

  • Seven years old male lion in Everland Zoo has three fractured canine tooth. To avoid the risk of a possible fracture with pulp exposure after restroration of largest abration defects, endodontic therapy was performed. The pulp chamber was filled with calcium hydroxide, zinc oxide cement and gutta percha. We also installed the Crown prosthesis was installed in fractured canine tooth to protect the tooth and to improve the cosmetics and function of the tooth. The lion could eat in one day and there were no any signs of dental problem. It is the obvious prophylactic procedure for wild animal is the inclusion of an oral examination at every opportunity to handle the animal. It is considered that crown prosthesis after endodontic therapy was useful for maintaining normal physiological function and for provention of additional fracture or complication.

  • A 5-year-old intact female mixed dog was admitted to the Veterinary Teaching Hospital for reconstructive surgery of traumatic oronasal communication in the palatal region after suffering a facial gunshot injury. The surgical procedure involved making buccal mucosal flaps as well as a transposition flap of the hard palate mucoperiosteum from the tissue adjacent to the defect, followed by applying a prosthetic appliance to protect the surgical flaps. A satisfactory outcome was obtained using a palatal prosthesis.

  • Intravascular hemolysis occurs in the majority of patient with mechanical valve prosthesis. The primary cause is mechanical trauma to red cells from turbulent blood flow through the prosthesis. Degree of hemolysis is dependent upon the type, size and material of valve and aggravated by paravalvular leakage. Clinically important hemolytic anemia is required medical management or consideration of reoperation. In severe hemolysis, reoperation is recommended without delay when seems to be renal failure. In this case, postoperative severe mechanical hemolysis was developed immediately after aortic valve replacement with St. Jude medical valve in a 13 year-old male patient. Neither significant paravalvular leakage nor valvular dysfunction was found through redo, but the mechanical valve was strongly suspected the cause of severe hemolysis. The St. Jude Medical valve was changed with Ionescu-Shiley bioprosthesis and any significant clinical problems were not noted through the postoperative course.

Review of the Reasons in Cases Requiring Varus/Valgus Constrained Prosthesis in Primary Total Knee Arthroplasty (일차 슬관절 전치환술 시내·외반 구속형 치환물이 필요했던 사례들의 원인 분석)

  • Purpose: The least constrained prosthesis is generally recommended in primary total knee arthroplasty (TKA). Nevertheless, a varus/valgus constrained (VVC) prosthesis should be implanted when a semi-constrained prosthesis is not good for adequate stability, especially in the coronal plane. In domestic situations, however, the VVC prosthesis could not always be prepared for every primary TKA case. Therefore, it is sometimes impractical to use a VVC prosthesis for unsual unstable situations. This study provides information for preparing VVC prostheses in the preoperative planning of primary TKA through an analysis of primary VVC TKA cases. Materials and Methods: This study reviewed 1,797 primary TKAs, performed between May 2003 and February 2016. The reasons for requiring VVC prosthesis and the preoperative conditions in 29 TKAs that underwent primary TKA with a VVC prosthesis were analyzed retrospectively. Results: In primary TKA, 29 cases (1.6%) in 27 patients (6 male and 21 female) used VVC prosthesis. Two patients underwent a VVC prosthesis on both knees. The mean age of the patients was 63.4 years old (34-79 years). The mean flexion contracture was 16.2° (-20°-90°), and the mean angle of great flexion was 111.7° (35°-145°). The situations requiring a VVC prosthesis were severe valgus deformity in 10 knees, knee stiffness requiring extensive soft tissue release in 10 knees, previously injured collateral ligaments in five knees, and distal femoral bone defect due to avascular necrosis in four knees. The mean tibiofemoral angle was 25.7° (21°-43°) in 10 cases with a valgus deformity. The mean flexion contracture was 37.5° (20°-90°), and the mean range of motion was 48.5° (10°-70°) in 10 cases with knee stiffness. Conclusion: The preparation of VVC prosthesis is recommended, even for primary TKA in cases of severe valgus deformity (tibiofemoral angle>20°), stiff knee (the range of motion: less than 70° with more than 20° flexion contracture), and the cases with a previous collateral ligament injury. This information will help in the preparation of adequate TKA prostheses for unusual unstable situations.

  • Recently, digital implant systems are expanding its influence in dental area. Due to technical improvement, they jumped over their limits nowadays. We can use these newest systems to treat edentulous patient, from implant surgery to fabrication of prosthesis. In this case, The patient was a fifty years old female. She had a full edentulous ridge on mandible and wanted to reconstruct occlusion with using implants. We planned to use digital implant system with "all-on-4" concept on mandible and produced surgical guide for flapless implant surgery. After the surgery, we tried to fabricate full arch prosthesis just using a digital devices and confirmed satisfying result.
