  • This study presents a case of diffuse large B cell lymphoma (DLBCL) in a 58-year-old man showing unusual manifestations mimicking chronic osteomyelitis. In this case review, we describe the imaging findings of DLBCL which mimics chronic osteomyelitis and review existing reports regarding the differential diagnosis of bone involvement of lymphoma and osteomyelitis through imaging and laboratory findings and diffusion-weighted magnetic resonance imaging (DWI) such as the advanced MRI sequence.

  • In this study, distribution of population with economic activities in ingress step to advanced-age society, the current status for the economic activities of old people and the necessity of economic activities through employment and inauguration for old people were investigated. The current status for the economic activities of old people and the changes of economic activities since IMF were also reviewed, and the activation of senior club in regional society and establishment of old man power bank were proposed. The speed of ingress to advanced-age society in Korea is so fast compared with other developed countries that the participation in economic activities of old people is urgent for international competitiveness and their economic independence. Up to now, due to the related regulations and institutional inertia, the practical support of employment and inauguration has not been achieved, but, the practical support should be achieved by solving the problems regarding the regulations and institution.

Traumatic Diaphragmatic Hernia: A Report of 3 Cases (외상성 횡격막 헤르니아: 3례 수술 보고)

  • Three cases of traumatic diaphragmatic hernia were repaired in this department from June 1967 to Nov.1968. The first case, a 14 year old girl, was diagnosed as diaphragmatic hernia during the operation of the diffuse peritonitis from jejunaI perforation 3 days after the traffic accident at local clinic and she was transfered to this hospital after the closure of the perforated jejunum. Herniated stomach, transverse colon, spleen and left lobe of the liver were repositioned and the diaphragmatic rupture at the posterolateral portion of the left diaphragm was repaired with two layer sutures by transthoracic approach. The second case. a 26 year old man. was diagnosed immediately after the traffic accident at local clinic and transfered to this hospital 24 hours later. Herniated and distended stomach, transverse colon and jejunum were repositioned and the large diaphragmatic rupture, about 9 cm in length, from the posterolateral portion to the base of the pericardium was directly repaired with two layer sutures. The third case, a 26 year old man, who had a history of stab wound at left lower lateral chest two years ago,was admitted with the sudden onset of abdominal pain and vomiting. The diaphragmatic hernia was confirmed with barium enema. The herniated stomach and transverse colon through the defect, about 3.5 cm in diameter, at anterolateral portion of the left diaphragm, were repositioned and the defect was repaired with two layer sutures. All of the cases recovered uneventfully.

  • Cor triatriatum is a rare anomaly in old age. This is a case report of a 66 year-old man who had been preoperatively diagnosed as coronary artery disease and cor triatriatum. The operative findings revealed that the left atrium had an intra-atrial septum with one small opening 10mm in diameter, the upper compartment received both pulmonary veins, and there were no other anomalies like anormalous pulmonary venous connection or atrial septal defect. The patient successfully underwent open heart surgery ; the anomalous septum was resected, the mitral valve was reconstructed using French technique with Carpentier-Edwards ring, and coronary artery bypass grafting was performed.

Is Axillary Dissection Necessary for Breast Cancer in Old Women? A Meta-analysis of Randomized Clinical Trials

  • Background: We performed this meta-analysis to assess the effectiveness and safety of axillary dissection in old women. Methods: The Cochrane Library, PubMed, EMBASE and Chinese Biomedical Literature Database were searched and all randomized controlled trials of axillary dissection in old women (at least 60 years old) were considered. Meta-analyses were completed using RevMan5.1. Results: Three eligible randomized controlled trials (RCTs) including 5,337 patients were considered. There was weak evidence in favour of axillary dissection (AD) in old women. The meta-analysis showed that the overall survival (OS) after 1, 3, 5 and 7 years and the disease free survival (DFS) after 1, 3 and 5 year were not statistically significantly different between AD and no AD groups. However, there was a difference in the 7 year DFS. Conclusions: Axillary dissection did not provide survival benefit to the old women with breast cancer analysed. Therefore, axillary dissection is not well-indicated in old women with breast cancer.

Surgical treatment of pulmonary hydatid cysts: two cases report (폐포충낭종의 수술치험 2)

  • Hydatid cystic disease of the lung is rare in Korea. Recently, the authors encountered 2 patients with this disease who were treated surgically with a successful outcome in both. The first patient, a 27 year old housewife never traveled outside Korea and the second, 25 year old man had a history of spending a year in Kuwait. These 2 cases are the first documented cases of pulmonary hydatoidosis among Koreans reported in the literature.

Role of Thymectomy for the Management of Myasthenia Gravis (근무력증의 외과적 치료)

  • The characteristic features of myasthenia gravis, which are weakness of voluntary muscle and easy fatigability, result from defective neuromuscular transmission caused by an autoimmune response to acetylcholine receptor. Recently, we performed two cases of thymectomy for the treatment of myasthenia gravis, one was 50 year old man who had malignant thymoma and the other was 19 year old girl who had benign thymic hyperplasia. The former had long duration of symptoms, and showed poor postoperative result. The latter had short duration of symptoms, and showed complete remission. Herewith, we suggest that early thymectomy can give the clinical assets for myasthenia gravis. So we report these two experiences with review of literatures.

Successful Removal of 15-year-old Pacemaker Leads by Weight and pulley method

  • Extraction of old pacemaker leads remains a complex procedure owing to fibrotic encapsulation and lead adhesions. We report a case of extraction of 15-year-old pacemaker leads by weight and pulley method. A 81-year-old man presented with exposed pacemaker leads out of body with purulent discharge from a pacemaker insertion site. He inserted DDD (dual chamber pacing, dual chamber sensing dual function) pacemaker implantation 15 years ago for SSS. Previously pacemaker battery was removed 3 years ago due to recurrent infection of pacemaker scar site. We extracted the pacemaker leads by weight and pulley method successfully without any complications.

  • In the vast extent of time, man and his ancestors have existed on earth about 2,000,000 years. For Near-Man, they knew how to make fire and protected themselves with hairs. Old-Man covered their bodies with the skins of wild animals. New Man improved the quality, the range and ingenuity, so they fabricated clothes with the animal skins. New Man is considered as ancestor of Modern Man. They had spread widely over the earth to take advantage of their surroundings. The next period, New Stone Age was chiefly distinguished by agricultural age. He was now about to make clothes from natural fibers. The two most obvious orgins of clothing are to protect the body against environment and to decorate the body. The forms of clothing were different from the various climates and regions they had settled, for example Yo-po Hyung (Apron), Kwae-po Hung (Sewn garment), Kwan-doo Hyung (Poncho), Chun-kae Hyung (Frontopend garment), Chai-hyung Hyung (Body-conforming shape). Our ancestor had moved from Baikal Lake, Central Asia toward northeast; Shun-tung in China, Manchuria and at last Han Panninsula. Considering our basic costume of ancient times were Yu(jacket), Ko(trousers), Sang(skirt), Po (coat) with the accessories of Kwan (crown), Mo (hat), Dai (belt), Hwa, Eie (footwear), here I conjecture our costume of prehistoric period was Chaihyung Hyung(Body-conforming shape) in the purpose of body covering.
