Benign symmetric lipomatosis, also known as Madelung's disease, Launois-Bensaude syndrome and multiple symmetric lipomatosis is a cutaneous condition characterized by extensive symmetric fat deposits in the neck, shoulders, and upper trunk. But symmetrical lipomatosis of the tongue is very rare and characterized by involvement exclusively of the tongue, invasiveness, and absence of encapsulation of the adipose tissue. A 75-year-old man presented with about a year history of painless tongue masses on both lateral border. Bilateral partial glossectomy was performed and subsequently was confirmed as symmetrical lipomatosis pathologically. We report this case with a review of the literatures.

Small cell carcinoma is mainly found in the lungs and extrapulmonary origins of small cell carcinoma in GI tract, head and neck, genitourinary system consists only 4% of the tumor. Thus, small cell carcinoma of the parotid gland is an extremely rare disease. Extrapulmonary small cell carcinoma is characterized by an aggressive clinical course with early metastasis. We report a case of small cell carcinoma of parotid gland in a 82-year-old woman with painless neck mass, who was managed with radiation therapy after surgery and review of relevant literatures.

The utilization of horticultural tools in Korea is historically viewed through limited materials. The garlic shown in the birth myth of the Korean nation appears to be the first record of horticultural foods. Only the fragmentary knowledge is available on the production and utilization of horticultural foods during the eras from Old Chosun to the Three States and the Unified Shilla. It seems that mutual exchange of horticultural foods between the Three States and neighbor countries was very active. Kinds of horticultural foods utilized were more variable in the era of Koryo dynasty. The situation on horticultural foods during Chosun dynasty is comparatively well known through published agricultural books and other literatures.

Multinucleated giant cells of osteoclast-like appearance can be seen in a type of anaplastic carcinoma of thyroid and only a few case reports for fine needle aspiration cytologic findings are found in the literatures. Recently, we experienced a case of anaplastic carcinoma of thyroid with many osteoclast-like giant cells in a 72-year-old woman. The cytologic features and immunohistochemical results are described with special emphasis on differential diagnosis.

Pulmonary embolism is a serious complication, which is well known in patients undergoing total hip or total knee arthroplasty or lower extremity fracture surgery. But, there are few literatures concerning pulmonary embolism after upper extremity surgery. Pulmonary embolism after minor upper extremity fracture surgery is extremely rare. We report a case of 66-year-old female patient that developed pulmonary embolism after percutaneous cannulated screw fixation for a greater tubercle fracture of the proximal humerus with literature review.

We had experienced a case of pericarditis associated with Mycoplasma and Salmonella co-infection. This 5-year old boy presented with high fever and generalized tonic-clonic seizure. The diagnosis was based on the markedly increased serial titer of Widal test and cold agglutinin with Mycoplasma indirect hemmagglution test during hospitalization. The electrocardiogram revealed P-R interval prolongation and an echocardiogram revealed moderate pericardial effusion. The patient treated with ceftriaxone and erythromycin for 2 weeks, and responded to the treatment well. We report this case with brief review of the related literatures.

Malignant tumors of the sinonasal tract constitute about 2% of those arising in the head and neck. Exposure to industrial fumes has been associated with an increased incidence of this malignant tumor. Early symptomatology of this cancer is identical to the symptomatology seen in benign conditions, such as swelling on forehead, headache, diplopia, and rhinorrhea. Thereby, delayed diagnosis leads to poor prognosis. Squamous cell carcinoma is the most common malignancy of the sinonasal tract, constituting about 80%, and primary carcinoma of the frontal sinus is extremely rare. Recently, authors experienced a case of primary carcinoma of the frontal sinus in a 60 year old male and performed transcranial resection. Now we report this case with brief review of literatures.

We experienced a case of congenital paraesophageal hiatal hernia[Type;IV in seventeen day-old female and treated through the right thoracotomy, reduction of the herniated viscera ,stomach and some part of transverse colon and omentum by gentle finger push, and narrowing the esopahgeal hiatus. Paraesophageal hiatal hernia accounts for only 5% per cent of all diaphragmatic defects but is a potentially dangerous lesion due to compressed lung by the herniated viscera. Symptoms are related to this, including exertional dyspnea, vomiting, cough, Tachypnea but noncyanotic, etc. Barium study shows that the stomach has herniated into the right pleural cavity. The speckled appearance in the herniated stomach in the herniated stomach was due to food material. It strongly suggests paraesophageal hiatal hernia. The operation was done. We report the case with the brief review of literatures.

Successful correction of the partial form of atrioventricular[AV] canal is now readily achievable. The most challenging technical features of this anomaly are the associated mitral valvular deformity and the frequent inability to obtain mitral competence. Residual mitral incompetence after repair of mitral cleft figures importantly in the causes of early and late failure. So, accurate and complete repair of the mitral valve cleft might be important. Determination of residual severe mitral regurgitation after repair is desirable to permit mitral valve replacement at the time. We have experienced one case of severe mitral regurgitation which was occurred 6 months after repair of partial form of A-V canal [ASD primum] in 22 years old virgin. Pre-op. left ventriculogram revealed severe mitral regurgitation in this patient. We preferred to perform mitral valve replacement with St. Jude valve in this patient at March 1984. Post-op. course was uneventful to now for 5 months and we report this case, review and discuss the literatures.

We have recently experienced a rare case of abdominal aortic injury caused by deceleration force during fall in upright position. The patient was 43-year-old brick-layer fell from 12 meter height and sustained compression fracture of the spine and both legs. The aortic injury was unnoticed at that time. About 2 years later, marked stenosis of the distal abdominal aorta was found together with clinical manifestations of ischemia of both legs. Aorto-femoral bypass on both sides has completely relieved the symptoms, Similar type of abdominal aortic injuries could not be found in the literatures.
